| 1 | Quadruple therapy with moxifloxacin and bismuth for first-line treatment ofHelicobacter pylori显示文摘AIM:To compare triple therapy vs quadruple therapy for 10 d as first-line treatment ofHelicobacter pylori(H.pylori) infection.METHODS:Consecutive H.pylori positive patients never treated in the past for this infection were randomly treated with triple therapy of pantoprazole(PAN) 20 mg bid,amoxicillin(AMO) 1 g bid and moxifloxacin(MOX) 400 mg bid for 10 d(PAM) or with quadruple therapy of PAN 20 mg bid,AMO 1 g bid,MOX 400 mg bid and bismuth subcitrate 240 mg bid for 10 d(PAMB).All patients were found positive at 13 C-Urea breath test(UBT) performed within ten days prior to the start of the study.A successful outcome was confirmed with an UBT performed 8 wk after the end of treatment.χ 2 analysis was used for statistical comparison.Per protocol(PP) and intention-to-treat(ITT) values were also calculated.RESULTS:Fifty-seven patients were enrolled in the PAM group and 50 in the PAMB group.One patient in each group did not return for further assessment.Eradication was higher in the PAMB group(negative:46 and positive:3) vs the PAM group(negative:44 and positive:12).The H.pylori eradication rate was statistically significantly higher in the PAMB group vs the PAM group,both with the PP and ITT analyses(PP:PAMB 93.8%,PAM 78.5%,P < 0.02;ITT:PAMB 92%,PAM 77.1 %,P <0.03).CONCLUSION:The addition of bismuth subcitrate can be considered a valuable adjuvant to triple therapy in those areas where H.pylori shows a high resistance to fluoroquinolones. | Antonio Francesco Ciccaglione Luigina Cellini Laurino Grossi Leonardo Marzio | 2012 | World Journal of Gastroenterology2012,18,32: | 5 |
| 2 | Osmoprotectants and Antioxidative Enzymes as Screening Tools for Salinity Tolerance in Radish (Raphanus sativus)显示文摘The research addresses the identification of a screening methodology for salt stress tolerance in radish cultivars. In the first experiment, two different radish cultivars(long white and round red) were compared in their morphological and physiological responses to different salinity levels. Round red radish showed better morphological and physiological responses to incremental salinity in terms of yield and better adaptation of overall water relations. In the second experiment, the most tolerant genotype from the first experiment was used as a control against other seven round red radish genotypes ranked by their salinity tolerance according to morphological, physiological and biochemical indices. Salt stress did not significantly affect malondialdehyde(MDA) and hydrogen peroxide(H2O2) content, and ascorbate peroxidase(APX) activity in the studied cultivars. Nonetheless, the relatively salt tolerant cultivar SAXA2 showed higher ability to accumulate compatible solutes(e.g.proline and proteins) and maintain osmotic adjustment. In addition, cultivar SAXA2 also showed considerable increase in glutathione reductase(GR) activity. | 4 | Pathways of flower infection and pollen-mediated dispersion of Pseudomonas syringae pv. actinidiae, the causal agent of kiwifruit bacterial canker显示文摘Flowers can provide a protected and nutrient-rich environment to the epiphytic microflora,thus representing a sensible entry point for pathogens such as Pseudomonas syringae pv.actinidiae(Psa).This bacterium can colonize both male and female Actinidia flowers,causing flower browning and fall,and systemic invasion of the host plant,eventually leading to its death.However,the process of flower colonization and penetration into the host tissues has not yet been fully elucidated.In addition,the presence of Psa in the pollen from infected flowers,and the role of pollination in the spread of Psa requires confirmation.The present study employed a Psa strain constitutively expressing the fluorescent GFPuv protein,to visualize in vivo flower colonization.Microscopy observations were performed by means of confocal laser scanning and wide-field fluorescent microscopy,and were coupled with the study of Psa population dynamics by quantitative PCR(q-PCR).The pathogen was shown to colonize stigmata,move along the stylar furrow,and penetrate the receptacles via the style or nectarhodes.Once the receptacle was invaded,the pathogen migrated along the flower pedicel and became systemic.Psa was also able to colonize the anthers epiphytically and endophytically.Infected male flowers produced contaminated pollen,which could transmit Psa to healthy plants.Finally,pollinators(Apis mellifera and Bombus terrestris)were studied in natural conditions,showing that,although they can be contaminated with Psa,the pathogen’s transmission via pollinators is contrasted by its short survival in the hive. | Irene Donati Antonio Cellini Giampaolo Buriani Sofia Mauri Callum Kay Gianni Tacconi Francesco Spinelli | 2018 | Horticulture Research2018,5,1: | 3 |

| 13 | MGUS:Proposal for outpatient management显示文摘The term monoclonal gammopathy of undetermined significance(MGUS) indicates the presence of a monoclonal protein(M-protein) without features of multiple myeloma, Waldenstrom's macroglobulinemia, primary amyloidosis or malignant lymphoproliferative disorders(LPD). While several guidelines on the treatment of LPD exist, many doubts and perplexities still exist on who should treat a MGUS, when and how. Even where MGUS does not require any therapy, the risk of progression to a LPD is 1% per year. This risk does not diminish over time and persists even in patients(pts) whose condition has remained stable for decades, and a prolonged follow up is, therefore, recommended. We met primary care doctors to share and agree on criteria for the management of outpatients with MGUS. Our aim is to draw up guidelines or, at least, suggestions that may help to determine which MGUS pts could be cared for by the primary care doctor and which should be followed by the hematologist. We suggest that once a MGUS is diagnosed, the primary care physician will attend patients with M-protein < 15 g/L if Ig G and pts with M-protein< 10 g/L if Ig A or Ig M, without end-organ damage and without signs and symptoms of LPD. | 14 | Robotic radiosurgery in pancreatic cancer: A systematic review显示文摘AIM: To present a systematic review of techniques and clinical results.METHODS: A systematic review of published literature was performed. Only studies reporting patient outcome after radiosurgery(single fraction) delivered with robotic devices [i.e.,robotic radiosurgery(RRS)] have been analyzed.RESULTS: A total of 96 patients from 5 studies were included. The studies are characterized by small series and different methods in terms of dose,target definition,combination with chemotherapy and/or standard fractionated radiotherapy and evaluation modalities. Preliminary results are positive in terms of tumor response(ORR = 56%) and local control of the tumor(crude rate of local progressions: 19.5%).Results for median overall survival(11.4 mo) seem comparable with the ones of prolonged chemoradiation(range: 8.6-13.0 mo). However,gastrointestinal toxicity seems to be the main limitation of RRS,especially at the duodenal level.CONCLUSION: RRS allows for local treatment in a shortened time(1 fraction) compared to traditional treatments(about 1 mo),providing the possibility for an easy integration with systemic therapies. Preliminary results did not show any outcome differences compared to standard chemoradiation.
