| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 4 | Synergistic effect of a novel oxymatrine-baicalin combination against hepatitis B virus replication, a smooth muscle actin expression and typeⅠcollagen synthesis in vitro显示文摘AIM: To study the effect of oxymatrine-baicalin combination (OB) against HBV replication in 2.2.15 cells and a smooth muscle actin (αSMA) expression, typeⅠ, collagen synthesis in HSC-T6 cells. METHODS: The 2.2.15 cells and HSC-T6 cells were cultured and treated respectively. HBsAg and HBeAg in the culture supernatants were detected by ELISA and HBV DNA levels were determined by fluorescence quantitative PCR. Total RNA was extracted from HSC-T6 cells and reverse transcribed into cDNA. The cDNAs were amplified by PCR and the quantities were expressed in proportion to p actin. The total cellular proteins extracted from HSC-T6 cells were separated by electrophoresis. Resolved proteins were electrophoretically transferred to nitrocellulose membrane. Protein bands were revealed and the quantities were corrected by p actin. RESULTS: In the 2.2.15 cell culture system, the inhibitory rate against secretion of HBsAg and HBeAg in the OB group was significantly stronger than that in the oxymatrine group (HBsAg, P=0.043; HBeAg, P=0.026; respectively); HBV DNA level in the OB group was significantly lower than that in the oxymatrine group (P = 0.041). In HSC-T6 cells the mRNA and protein expression levels of a SMA in the OB group were significantly lower as compared with those in the oxymatrine group (mRNA, P = 0.013; protein, P-0.042; respectively); The mRNA and protein expression levels of typeⅠcollagen in the OB group were significantly lower as compared with those in the oxymatrine group (mRNA, P<0.01; protein, P<0.01; respectively). | 5 | Extending the CONSORT Statement to moxibustion显示文摘The STandards for Reporting Interventions in Clinical Trials Of Moxibustion (STRICTOM), in the form of a checklist and descriptions of checklist items, were designed to improve reporting of moxibustion trials, and thereby facilitating their interpretation and replication. The STRICTOM checklist included 7 items and 16 sub-items. These set out reporting guidelines for the moxibustion rationale, details of moxibustion, treatment regimen, other components of treatment, treatment provider background, control and comparator interventions, and precaution measures. In addition, there were descriptions of each item and examples of good reporting. It is intended that the STRICTOM can be used in conjunction with the main CONSORT Statement, extensions for nonpharmacologic treatment and pragmatic trials, and thereby raise the quality of reporting of clinical trials of moxibustion. | 9 | Oxidative Stress before and after Operation in Patients with Chronic Cholecystitis Containing Gallstone显示文摘To study the relationship of oxidative, antioxidative constituents and antioxidases in blood with chronic cholecystitis containing gallstone, levels of lipoperoxides (LPO), nitric oxide (NO), vitamin C(VC), vitamin E (VE) and β carotene (β CAR) in plasma as well as level of LPO, activities of superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GSH Px) in erythrocytes were investigated by spectrophotometric assay in 107 patients with this condition (PCg) and 100 healthy volunteers (HVs). Compared with HVs group, the average value of LPO and NO in plasma and that of LPO in erythrocytes of PCg group were significantly increased ( P <0.0001), while that of VC, VE and β CAR in plasma and the average activities of SOD, CAT and GSH Px in erythrocytes were significantly decreased ( P <0.0001). Linear regression and correlation analysis for 107 preoperative PCg showed that the value of LPO and NO in plasma and that of LPO in erythrocytes of PCg gradually increased ( P <0.0001), representing a significant linear positive correlation. The value of VC, VE and β CAR in plasma and that of SOD, CAT and GSH Px in erythrocytes of PCg gradually decreased ( P <0.0001), representing a significant linear negative correlation. Stepwise regression and correlation analysis for 107 preoperative PCg suggested that the closest correlation was observed between the course of disease and the value of NO and VC in plasma and that of SOD, GSH Px and LPO in erythrocytes, r =0.7306, F =32.1408, P <0.0001. Compared with the preoperative PCg group, the average value of LPO and NO in plasma and that of LPO in erythrocytes of the postoperative PCg group were significantly decreased ( P <0.0001). Furthermore, the average value of VC in plasma and that of SOD, CAT and GSH Px in erythrocytes of the postoperative PCg group were significantly increased ( P <0.0001), whereas no significant difference was found between their average value of VE and β CAR in plasma. These findings suggested that oxidative stress was an aggravating pathological condition in PCg group. | 12 | Transjugular intrahepatic portosystemic shunt for pyrrolizidine alkaloid-related hepatic sinusoidal obstruction syndrome显示文摘BACKGROUND Treatments for hepatic sinusoidal obstruction syndrome(HSOS)are limited.AIM To evaluate transjugular intrahepatic portosystemic shunting(TIPS)as a treatment for pyrrolidine alkaloid-related HSOS(PA-HSOS).METHODS This retrospective analysis included patients with PA-HSOS admitted to the First Affiliated Hospital of the University of Science and Technology of China(June 2015 to January 2019).Baseline clinical characteristics and follow-up data were extracted from the medical records.All patients included in this study experienced failure of initial therapy.Patients were divided into the TIPS and conservative treatment groups according to the therapy they received.Liver function,maximal ascites depth,imaging characteristics,pathology findings,and survival were compared between groups.RESULTS The TIPS group included 37 patients(28 males),and the conservative treatment group included 17 patients(11 males).Baseline characteristics were similar between groups.There were two deaths in the TIPS group and seven deaths in the conservative treatment group during follow-up(3-48 mo).The 3-,6-,12-and 24-mo survival rates were 94.6%,94.6%,94.6%and 94.6%,respectively,in the TIPS group and 70.6%,57.8%,57.8%and 57.8%,respectively,in the conservative treatment group.Kaplan-Meier analysis revealed significantly longer survival for the TIPS group than for the conservative treatment group(P=0.001).Compared with the pre-treatment value,maximal ascites depth was significantly lower at 1 wk,2 wk,1 mo,and 3 mo for the TIPS group(all P<0.05)but not in the conservative treatment group.Contrast-enhanced computed tomography demonstrated the disappearance of patchy liver enhancement after TIPS.Pathology showed that liver congestion and hepatocyte swelling improved with time after TIPS placement.CONCLUSION TIPS may achieve better outcomes than conventional symptomatic treatment in patients with PA-HSOS. | Chun-Ze Zhou Rui-Feng Wang Wei-Fu Lv Yu-Qin Fu De-Lei Cheng Yi-Jiang Zhu Chang-Long Hou Xian-Jun Ye | 2020 | World Journal of Gastroenterology2020,26,24: | 7 |
| 13 | Analyzing Influencing Factors of Rural Poverty in Typical Poverty Areas of Hainan Province: A Case Study of Lingao County显示文摘Rural poverty and poverty reduction are not only the focal issues that have attracted worldwide attention, but also the vital issues on people's livelihood that has attached great importance and aimed to be solved by the central and local governments of China. Based on the survey data of 354 farming households, this paper, taking the national poverty county of Lingao County, Hainan Province for an example, examined the characteristics of rural poverty of the county. Moreover, this paper established the spatial lag model(SLM) from five dimensions, namely, status of the household head, household structure, health status, income composition and traffic accessibility, to analyze the main influencing factors of rural poverty according to the values of Moran's I and the diagnosis of spatial dependence of the OLS model. It is found that the poor farming households gathered mainly in five towns in the north and southwest of the county, and the rural poverty have the characteristics of low educational level of the heads, more minor children, high population of farming peasants, high incidence of disease and low proportion of household wage-equivalent income. Whole cell patch-clamp techniques wereused in our experiment.RESULTS: The peak amplitude.of ICRAC was -508±115 pA(n=15), its reversal potential of ICRAC was about 0 mV. At thepotential of -100 mV, Tet inhibited the peak amplitude ofICRAC from -521±95 pA to -338±85 pA (P<0.01 vs control,n=5), with the inhibitory rate of 35 % at 10 μmol/L andfrom -504±87 pA to -247±82 pA (P<0.01 vscontrol, n=5),with the inhibitory rate of 49 % at 100 μmol/L, withoutaffecting its reversal potential. The amplitude of ICRAC wasdependent on extracellular Ca2+ concentration. The peakamplitude of ICRAC was -205±105 pA (n=3) in tyrode's solutionwith Ca2+ 1.8 mmol/L (P<0.01 vs the peak amplitude ofICRAC in external solution with Ca2+ 10 mmol/L). | 16 | Dark matter direct search sensitivity of the PandaX-4T experiment显示文摘The Panda X-4T experiment, a 4-ton scale dark matter direct detection experiment, is being planned at the China Jinping Underground Laboratory. In this paper we present a simulation study of the expected background in this experiment. In a 2.8-ton fiducial mass and the signal region between 1-10 keV electron equivalent energy, the total electron recoil background is found to be 4.9 × 10^(-5) kg^(-1) d^(-1) keV^(-1). The nuclear recoil background in the same region is 2.8 × 10^(-7) kg^(-1) d^(-1) keV^(-1). | 18 | The heterogeneity of islet autoantibodies and the progression of islet failure in type 1 diabetic patients显示文摘Type 1 diabetes mellitus is heterogeneous in many facets. The patients suffered from type 1 diabetes present several levels of islet function as well as variable number and type of islet-specific autoantibodies. This study was to investigate prevalence and heterogeneity of the islet autoantibodies and clinical phenotypes of type 1 diabetes mellitus; and also discussed the process of islet failure and its risk factors in Chinese type 1 diabetic patients. A total of 1,291 type 1 diabetic patients were enrolled in this study. Demographic information was collected. Laboratory tests including mixed-meal tolerance test, human leukocyte antigen alleles, hemoglobin A1 c, lipids, thyroid function and islet autoantibodies were conducted. The frequency of islet-specific autoantibody in newly diagnosed T1 DM patients(duration shorter than half year) was 73% in East China. According to binary logistic regressions, autoantibody positivity, longer duration and lower Body Mass Index were the risk factors of islet failure. As the disease developed, autoantibodies against glutamic acid decarboxylase declined as well as the other two autoantibodies against zinc transporter 8 and islet antigen 2. The decrease of autoantibodies was positively correlated with aggressive beta cell destruction. Autoantibodies can facilitate the identification of classic T1 DM from other subtypes and predict the progression of islet failure. | 19 | Comparing the intestinal bacterial communies of sympatric wintering Hooded Crane (Grus monacha) and Domestic Goose(Anser anser domesticus)显示文摘Background:Gut microbiota play crucial roles in host health.Wild birds and domestic poultry often occupy sympatric habitats,which facilitate the mutual transmission of intestinal microbes.However,the distinct intestinal microbial communities between sympatric wild birds and poultry remain unknown.At present,the risk of interspecies transmission of pathogenic bacteria between wild and domestic host birds is also a research hotspot.Methods:This study compared the intestinal bacterial communities of the overwintering Hooded Crane(Grus monacha)and the Domestic Goose(Anser anser domesticus)at Shengjin Lake,China,using Illumina high-throughput sequencing technology(Mi-Seq platform).Results:Our results revealed that Firmicutes,Actinobacteria,Proteobacteria,Bacteroidetes and Chloroflexi were the dominant bacterial phyla in both hosts.The gut bacterial community composition differed significantly between sympatric Hooded Cranes and Domestic Geese.However,the hosts exhibited little variation in gut bacterial alphadiversity.The relative abundance of Firmicutes was significantly higher in the guts of the Hooded Cranes,while the relative abundances of Actinobacteria,Proteobacteria,Bacteroidete and Chloroflexi were significantly higher in guts of Domestic Geese.Moreover,a total of 132 potential pathogenic operational taxonomic units(OTUs)were detected in guts of Hooded Cranes and Domestic Geese,and 13 pathogenic OTUs(9.8%)were found in both host guts.Pathogenic bacterial community composition and diversity differed significantly between hosts.Conclusions:The results showed that the gut bacterial community composition differs significantly between sympatric Hooded Cranes and Domestic Geese.In addition,potential pathogens were detected in the guts of both Hooded Cranes and Domestic Geese,with 13 pathogenic OTUs overlapping between the two hosts,suggesting that more attention should be paid to wild birds and poultry that might increase the risk of disease transmission in conspecifics and other mixed species. | Rong Fu Xingjia Xiang Yuanqiu Dong Lei Cheng Lizhi Zhou | 2020 | Avian Research2020,11,2: | 5 |
| 20 | Effect of traditional Chinese medicine compounds on rumen fermentation, methanogenesis and microbial flora in vitro显示文摘This study was conducted to investigate the effects of traditional Chinese medicine compounds(TCMC)on rumen fermentation, methane emission and populations of ruminal microbes using an in vitro gas production technique. Cablin patchouli herb(CPH), Atractylodes rhizome(AR), Amur Cork-tree(AC) and Cypsum were mixed with the weight ratios of 1:1:1:0.5 and 1:1:1:1 to make up TCMC1 and TCMC2,respectively. Both TCMC were added at level of 25 g/kg of substrate dry matter. In vitro gas production was recorded and methane concentration was determined at 12 and 24 h of incubation. After 24 h, the incubation was terminated and the inoculants were measured for pH, ammonia nitrogen, volatile fatty acids(VFA). Total deoxyribonucleic acid of ruminal microbes was extracted from the inocula, and populations were determined by a real-time quantitative polymerase chain reaction. Populations of total rumen methanogens, protozoa, total fungi, Ruminococcus albus, Fibrobacter succinogenes and Ruminococcus flavefaciens were expressed as a proportion of total rumen bacterial 16 S ribosomal deoxyribonucleic acid. Compared with the control, the 2 TCMC decreased(P < 0.05) total VFA concentration,acetate molar proportion, acetate to propionate ratio, gas and methane productions at 12 and 24 h,hydrogen(H) produced and consumed, and methanogens and total fungi populations, while the 2 TCMC increased(P < 0.05) propionate molar proportion. Traditional Chinese medicine compound 1 also decreased(P ≤ 0.05) R. flavefaciens population. From the present study, it is inferred that there is an effect of the TCMC in suppressing methanogenesis, probably mediated via indirect mode by channeling H2 utilized for methanogenesis to synthesis of propionate and direct action against the rumen microbes involved in methane formation.
