| 2 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 3 | Detection of nighttime sea fog/stratus over the Huang-hai Sea using MTSAT-1R IR data显示文摘A dual channel difference (DCD) method is applied to detect nighttime sea fog/stratus over the Huanghai Sea using the infrared (IR) data of shortwave (3.5–4.0 μm) and longwave (10.3–11.3 μm) channels from the Multi-functional Transport Satellite (MTSAT)-1R, i.e., shortwave minus longwave brightness temperature difference (SLTD). Twenty-four sea fog events over the Huanghai Sea during March to July of 2006 and 2007 are chosen to determine a suitable value of SLTD for nighttime sea fog/stratus detection, and it is found that the value of –5.5––2.5°C can be taken as a criterion. Two case examples of sea fog events are especially demonstrated in detail utilizing the criterion, and the results show that the derived sea fog/stratus coverage is quite reasonable. This coverage information is very helpful to analyze the formation and evolution of sea fog/stratus during night and can provide sea fog researchers with observational evidences for model results verification. | 6 | ZIKA virus isolated from mosquitoes:a field and laboratory investigation in China,2016显示文摘A field investigation of arboviruses was conducted in Dejiang, Guizhou Province in the summer of 2016. A total of 8,795 mosquitoes, belonging to four species of three genera, and 1,300 midges were collected. The mosquito samples were identified on site according to their morphology, and the pooled samples were ground and centrifuged in the laboratory. The supernatant was incubated with mosquito tissue culture cells(C6/36) and mammalian cells(BHK-21) for virus isolation. The results indicated that 40%(3,540/8,795) were Anopheles sinensis, 30%(2,700/8,795) were Culex pipiens quinquefasciatus, and 29%(2,530/8,795)were Armigeres subbalbeatus. Furthermore, a total of eight virus isolates were obtained, and genome sequencing revealed two Zika viruses(ZIKVs) isolated from Culex pipiens quinquefasciatus and Armigeres subbalbeatus, respectively; three Japanese encephalitis viruses(JEVs) isolated from Culex pipiens quinquefasciatus; two Banna viruses(BAVs) isolated from Culex pipiens quinquefasciatus and Anopheles sinensis, respectively; and one densovirus(DNV) isolated from Culex pipiens quinquefasciatus.The ZIKVs isolated from the Culex pipiens quinquefasciatus and Armigeres subbalbeatus mosquitoes represent the first ZIKV isolates in China's Mainland. | 7 | Effect of local wound infiltration with ropivacaine on postoperative pain relief and stress response reduction after open hepatectomy显示文摘AIM To prospectively evaluate the effect of local wound infiltration with ropivacaine on postoperative pain relief and stress response reduction after open hepatectomy.METHODS A total of 56 patients undergoing open hepatectomy were randomly divided into two groups:a ropivacaine group(wound infiltration with ropivacaine solution)and a control group(infiltration with isotonic saline solution).A visual analog scale(VAS)at rest and on movement was used to measure postoperative pain for the first 48 h after surgery.Mean arterial pressure(MAP),heart rate(HR),time to bowel recovery,length of hospitalization after surgery,cumulative sufentanil consumption,and incidence of nausea and vomiting were compared between the two groups.Surgical stress hormones(epinephrine,norepinephrine,and cortisol)were detected using enzyme-linked immunosorbent assay,and the results were compared. RESULTS VAS scores both at rest and on movement at 24 h and48 h were similar between the two groups.Significantly lower VAS scores were detected at 0,6,and 12 h in the ropivacaine group compared with the control group(P<0.05 for all).MAP was significantly lower at 6,12,and 24 h(P<0.05 for all);HR was significantly lower at 0,6,12,and 24 h(P<0.05 for all);time to bowel recovery and length of hospitalization after surgery(P<0.05 for both)were significantly shortened;and cumulative sufentanil consumption was significantly lower at 6,12,24,and 36 h(P<0.05 for all)in the ropivacaine group than in the control group,although the incidence of nausea and vomiting showed no significant difference between the two groups.The levels of epinephrine,norepinephrine,and cortisol were significantly lower in the ropivacaine group than in the control group at 24 and 48 h(P<0.01 for all). | 18 | Hyperpolarization activated cation current (If) in cardiac myocytes from pulmonary vein sleeves in the canine with atrial fibrillation显示文摘Objective To investigate the characteristics of ectopic automaticity and cation current (If) of cardiac myocytes from pulmonary vein sleeves (PVs) in canines with atrial fibrillation. Methods The canines (8–10 years old) were subjected to long-term, rapid atrial pacing (RAP) for 10 weeks, which induced the atrial fibrillation model. Disassociation of PVs of canines yielded single cardiac myocytes from a Landengorff column. Action potential, If and hyperpolarisation activated cyclic nucleotide-gated (HCN) currents were measured with the patch-clamp technique. Results Compared with the control group, cardiac myocytes from the RAP canine PVs had spontaneous diastolic depolarization, shorter action potential duration, and larger If densities. In the group of RAP cells, the half maximal activation potential (V1/2) was found to be less negative (��105.5 ±5.2 mV) compared to control cells (��87.3 ±4.9 mV). Current densities of If were increased significantly by β-adrenergic receptor stimulation with isoproterenol and caused an acceleration of current activation. In contrast, If currents in the RAP were reduced by carvedilol, a selective beta-adrenergic receptor. Another important finding is that HCN4-based channels may make a significant contribution to If in PVs cells, but not HCN2. Meanwhile, HCN4 current significantly increases in canine PVs cardiac myocytes with RAP. | 19 | Large field of view computed laminography with the asymmetric rotational scanning geometry显示文摘X-ray computed laminography(CL)is used in the fields of industrial inspection and medical imaging.It can provide the internal structure three-dimensional(3D)information of a region of the objects nondestructively.It is important to the clinical diagnosis and the quality control of flat objects like printed circuit boards,aircraft wings and satellite solar panels.With the restriction that the imaging region must be within the X-ray beam formed by the X-ray source and the detector,the imaging field of view of CL is limited by the size of detector.A new CL method with an asymmetric rotational cone-beam scanning geometry,called large field of view CL,is presented to overcome the existing disadvantage.It can extend the imaging region when the imaging spatial resolution keeps the same as that of the conventional CL.It can also improve the imaging spatial resolution when the imaging region keeps the same as that of the conventional CL.The asymmetric configuration can be achieved by offsetting the detector from the conventional symmetric configuration.It does not,however,require new detectors and X-ray source nor alter the scanning mechanical system.The filtered back-projection(FBP)reconstruction algorithm and the data truncation smoothing functions have been deduced to reconstruct the images directly from the data acquired with this asymmetric configuration.We performed numerical studies and experiments to demonstrate and validate the proposed approach. | 20 | Insight-HXMT observations of the first binary neutron star merger GW170817显示文摘Finding the electromagnetic(EM) counterpart of binary compact star merger, especially the binary neutron star(BNS) merger,is critically important for gravitational wave(GW) astronomy, cosmology and fundamental physics. On Aug. 17, 2017,Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart,GRB 170817 A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope(HE) onboard Insight-HXMT(Hard X-ray Modulation Telescope) is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart(SSS17 a/AT2017 gfo) with very large collection area(~1000 cm^2) and microsecond time resolution in 0.2-5 MeV. In addition,Insight-HXMT quickly implemented a Target of Opportunity(ToO) observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy(0.2-5 MeV) radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817 A. Meanwhile,Insight-HXMT/HE provides one of the most stringent constraints(~10^(-7) to 10^(-6) erg/cm^2/s) for both GRB170817 A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger.
