1Percutaneous endoscopic gastrostomy:Indications,technique,complications and management显示文摘Percutaneous endoscopic gastrostomy(PEG)is the preferred route of feeding and nutritional support in patients with a functional gastrointestinal system who require long-term enteral nutrition.Besides its wellknown advantages over parenteral nutrition,PEG offers superior access to the gastrointestinal system over surgical methods.Considering that nowadays PEG tube placement is one of the most common endoscopic procedures performed worldwide,knowing its indications and contraindications is of paramount importance in current medicine.PEG tubes are sometimes placed inappropriately in patients unable to tolerate adequate oral intake because of incorrect and unrealistic understanding of their indications and what they can accomplish.Broadly,the two main indications of PEG tube placement are enteral feeding and stomach decompression.On the other hand,distal enteral obstruction,severe uncorrectable coagulopathy and hemodynamic instability constitute the main absolute contraindications for PEG tube placement in hospitalized patients.Although generally considered to be a safe procedure,there is the potential for both minor and major complications.Awareness of these potential complications,as well as understanding routine aftercare of the catheter,can improve the quality of care for patients with a PEG tube.These complications can generally be classified into three major categories:endoscopic technical difficulties,PEG procedure-related complications and late complications associated with PEG tube use and wound care.In this review we describe a variety of minor and major tube-related complications as well as strategies for their management and avoidance.Different methods of percutaneous PEG tube placement into the stomach have been described in the literature with the'pull'technique being the most common method.In the last section of this review,the reader is presented with a brief discussion of these procedures,techniques and related issues.Despite the mentioned PEG tube placement complications,this procedure has gained worldwide popularity as a safe enteral access for nutrition in patients with a functional gastrointestinal system.Ata A Rahnemai-Azar Amir A Rahnemaiazar Rozhin Naghshizadian Amparo Kurtz Daniel T Farkas 2014World Journal of Gastroenterology2014,20,24:38

3Spheroidizing Kinetics and Optimization of Heat Treatment Parameters in CK60 Steel Using Taguchi Robust Design显示文摘For processing parts made from medium carbon steel,toughness and flexibility are of importance.Therefore,to achieve these properties,the cementite in the steels is spheroidized through heat treatment.Different parameters such as the time and temperature of spheroidizing and the initial microstructure of the steel affect the amount of spheroidized cementite.In the present work,the percent of contribution of two parameters,i.e.initial microstructure and spheroidizing time,to the percent of spheroidization in CK60 steel was investigated using Taguchi robust design.The initial microstructures consisted of martensite,coarse pearlite,fine pearlite and bainite and the chosen spheroidization times were 4,8,12,and 16h.Spheroidizing was done at the constant temperature 700℃.After spheroidizing was completed,the samples were prepared in order to observe their microstructure under an optical microscope and to determine the spheroidized percent using MIPTM(metallographic image processing)software.It was found that the spheroidizing time had the most influence(58.5%)on spheroidized percent and the initial microstructure only had a 31.1% contribution.Finally,the instantaneous growth rate of the carbide was also deduced.Ata Kamyabi-Gol Meisam Sheikh-Amiri 2010Journal of Iron and Steel Research(International)2010,17,4:9
4EXPERIMENTAL AND FIELD STUDY ON MINING-PIT MIGRATION显示文摘Rivers are one of the most essential sources of sand and gravel supply for civil works. However, undesirable effects of irregular in-stream mining have been reported on natural sources, environment and infrastructures close to rivers. Therefore, it is necessary to find the effects of mining on rivers in more details. This research concentrates on mining-pit migration phenomenon and its effects on the channel bed. This paper reports an experimental study on the migration of rectangular mining pits and variation of longitudinal profile in the channel bed composed of rather uniform sediments. Different values of widths and lengths were used for pit while pit depths and flow variables were kept constant. The results show that the migration speed changes with the length/width ratio of the pit. The migration speed in convection period is higher than that in diffusion period. In addition, by increasing the length or width, filling rate of pit increases, where the effect of width is more important than the effect of the length. Also is reported in this paper a field study on the changes of three pits excavated at different locations of a river. 6Comparison of corneal endothelial changes following phacoemulsification with transversal and torsional phacoemulsification machines显示文摘AIM:To compare and evaluate the phacoemulsification parameters and postoperative endothelial cell changes of two different phacoemulsification machines, each with different modes, but also to assess the relationship between postoperative endothelial cell loss and the phacoemulsification parameters, as well as the other factors in both groups.METHODS:This prospective observational study was comprised of consecutive eligible cataract patients operated with phacoemulsification technique performed by the same surgeon using either a WHITESTAR Signature Ellips FX(transversal, group 1) or Infiniti OZil IP(torsional, group 2) machine.RESULTS:The study included 86 patients. Baseline characteristics in the groups were similar. The median nuclear sclerosis grade was 3(2-4) in the first group and2(2-4) in the second group(P =0.265). Both groups had similar phacoemulsification needle times(group 1: 60.63±36 s; group 2: 55.98±30 s; P =0.789). The percentage of endothelial cell loss 30 d after surgery ranged from 3% to15% with a median of 7% in group 1, and from 2% to13% with a median of 6% in group 2; however, there was no statistically significant difference between the groups(P =0.407). Hexagonality(P =0.794) and the coefficient of variation(CV; P =0.142) did not differ significantly between the groups before and 30 d after surgery. Asignificant positive correlation was found between the endothelial cell loss and nuclear sclerosis grade(group1: P <0.001; group 2: P <0.001) and between the endothelial cell loss and average phacoemulsification power(group 1: P =0.007; group 2: P =0.008).CONCLUSION:Both of these machines were efficient,with similar endothelial cell loss. 7Measurement of choroidal thickness and macular thickness during and after pregnancy显示文摘AIM: To investigate the effect of pregnancy on subfoveal choroidal thickness(SFCT) and macular thickness in both pregnant and not pregnant healthy women.METHODS: Twenty-nine healthy pregnant women in their third trimester and 36 age-matched healthy women were enrolled in a prospective, cross-sectional study.Foveal and parafoveal thickness in the four quadrants and SFCT were measured by optical coherence tomography(OCT) in the healthy pregnant women(i.e.study group) and healthy women(i.e. control group).OCT measurements were again measured 3mo after delivery in the study group.RESULTS: Mean SFCT measurements in the control group, pregnant women of the study group, and after delivery of the study group were 320.86 ±59.18 μm,387.97 ±59.91 μm, and 332.40 ±26.03 μm, respectively.There was a statistically significant difference in the mean SFCT values between pregnant women of the study group and the control group(P =0.000). Foveal and parafoveal thickness values were not statistically significant in either the study or control group.CONCLUSION: SFCT increases during pregnancy and returns to normal range in the three months after delivery. 8Evaluation of early results of quick-chop phacoemulsification in the patients with high myopic cataract显示文摘AIM:To assess the early surgical outcomes of quickchop phacoemulsification technique in patients with high myopia.METHODS:The data of patients with high myopia who underwent quick-chop phacoemulsification were reviewed retrospectively. There were 42 eyes of 31 patients. The axial length was more than 26 mm in all eyes. All eyes underwent quick-chop phacoemulsification surgery with the placement of an intraocular lens(IOL) in the capsular bag. Postoperative visits were performed at 1, 3d; 2wk,1mo. Early postoperative best corrected visual acuity(BCVA), preoperative and postoperative corneal endothelial cell density(ECD), central corneal thickness(CCT) and postoperative complications were assessed.Paired sample t-test or Wilcoxon tests were used to compare data between preoperative and postoperative data.· RESULTS:There was no statistically significant difference between preoperative and postoperative ECD and CCT. Retinal detachment was developed in one eye at postoperative first day. There was an iris prolapsus from side port insicion.· CONCLUSION:Quick-chop phacoemulsification technique is a safe surgical technique. However we can encounter some complications in high myopic eyes due to histopathological differences. Both side port and clear corneal tunnel insicion size is crucial for preventing postoperative complications. 9Simultaneous operation of dibenzothiophene hydrodesulfurization and methanol reforming reactions over Pd promoted alumina based catalysts显示文摘In the current study simultaneous reactions of hydrodesulfurization(HDS) of dibenzothiophene(DBT) and reforming of methanol in a micro-autoclave reactor were studied over bi-metallic(Co-Mo/Al2O3 and Ni-Mo/Al2O3) and tri-metallic(Pd-Co-Mo/Al2O3 and Pd-Ni-Mo/Al2O3) catalyst systems which were prepared by incipient impregnation method.In situ hydrogen utilization and low Pd loadings were the major targets of this study.For comparison purpose,catalytic activity was separately determined for both the methanol reforming and HDS of DBT reactions as well.Ni based catalysts were confirmed with better activity than Co ones for both the reactions with Pd promoted ones ranking at the top i.e.Pd-Ni-Mo/Al2O3 > Ni-Mo/Al2O3 > Pd-Co-Mo/Al2O3 > Co-Mo/Al2O3 where Pd-Ni-Mo/Al2O3 showed 91% DBT conversion at 380 ℃ and 12 h reaction time.Some of the selected organic additives on catalytic activity were tested for their effect toward HDS reaction which was unique with close relation to their chemical nature.Reaction products were quantitatively and qualitatively analyzed via HPLC and GC-MS techniques respectively which helped in elucidating reaction mechanism.Muhammad Yaseen Muhammad Shakimllah Imtiaz Ahmad Ata Ur Rahman Faiz Ur Rahman Muhammad Usman Rauf Razzaq 2012燃料化学学报2012,40,6:3
10Comparison of 45- degree Kelman and 45- degree balanced phaco tip designs in torsional microcoaxial phacoemulsification显示文摘AIM: To compare the intraoperative performance and postoperative outcome after microcoaxial torsional phacoemulsification using either a Kelman or balanced phaco tip.· METHODS: Cataracts were treated using 2.2 mm microcoaxial torsional phacoemulsification using either a45-degree mini-flared Kelman or a 45-degree IntrepidBalanced phaco tip. Intraoperative measurements included total ultrasound(US) time, cumulative dissipated energy(CDE), torsional US time, and balanced salt solution(BSS) use. The central endothelial cell density(ECD) and central corneal thickness(CCT) were evaluated preoperatively and postoperatively 1, 7, and30 d after surgery using noncontact specular microscopy.·RESULTS: The 116 enrolled eyes(116 patients) were divided equally between the Kelman and balanced tip groups. Intraoperative measurements showed significantly less total US time, torsional US time, CDE, and BSS use in the balanced group than in Kelman group(P <0.05).The total US time, torsional US time, CDE, and BSS use were 17.45 ±14.53 s, 16.63 ±13.97 s, 6.38 ±5.26, and 48.21 ±17.21 m L in the Kelman group and 11.39 ±9.60 s, 10.90 ±9.25 s, 4.04 ±3.42, and 41.36 ±12.70 m L in the balanced group, respectively.·CONCLUSION: Torsional phacoemulsification performed with a balanced tip provided more effective lens removal with less total US time, torsional time, CDE, and BSS use, as well as similar changes in ECD with a Kelman tip in all cataract grades. 11Oxidative desulfurization of dibenzothiophene over Fe promoted Co–Mo/Al_2O_3 and Ni–Mo/Al_2O_3 catalysts using hydrogen peroxide and formic acid as oxidants显示文摘This work reports the enhancing effect of a highly cost effective and efficient metal, Fe, incorporation to Co or Ni based Mo/Al_2O_3 catalysts in the oxidative desulfurization(ODS) of dibenzothiophene(DBT) using H_2O_2 and formic acid as oxidants. The influence of operating parameters i.e. reaction time, catalyst dose, reaction temperature and oxidant amount on oxidation process was investigated. Results revealed that 99% DBT conversion was achieved at 60 °C and 150 min reaction time over Fe–Ni–Mo/Al_2O_3. Fe tremendously enhanced the ODS activity of Co or Ni based Mo/Al_2O_3 catalysts following the activity order: Fe–Ni–Mo/Al_2O_3 NFe–Co–Mo/Al_2O_3 NNi–Mo/Al_2O_3 NCo–Mo/Al_2O_3, while H_2O_2 exhibited higher oxidation activity than formic acid over all catalyst systems. Insight about the surface morphology and textural properties of fresh and spent catalysts were achieved using scanning electron microscopy(SEM), X-ray diffraction(XRD), energy dispersive X-ray(EDX)analysis, Atomic Absorption Spectroscopy(AAS) and BET surface area analysis, which helped in the interpretation of experimental data. 12Effect of torsional mode phacoemulsification on cornea in eyes with/without pseudoexfoliation显示文摘AIM: To evaluate the effect of torsional mode phacoemulsification on central corneal thickness, corneal endothelial cell density, and morphology in eyes with/without pseudoexfoliation(PEX) syndrome.METHODS: Fourty-two consecutive patients with and42 patients without PEX as a control group scheduled for cataract surgery was studied. Phacoemulsification, using OZi L IP system, was performed with quick chop technique. Using noncontact specular microscopy, the central endothelial cell density(ECD), coefficient of variation, percentage of hexagonal cells, and the central corneal thickness(CCT) were evaluated preoperatively and postoperatively at 1, 7 and 30 d.RESULTS: The ECD in PEX syndrome was statistically significantly lower than that in the control group preoperatively and postoperatively(P ≤0.001).Percentage change in ECD was statistically significantly higher in PEX than that in control group after surgery follow up(P ≤0.04).There was no statistically significant difference between both groups comparing percentage of hexagonal cells and coefficient of variation in the cell size before and after the surgery. At 1 and 7d after surgery, percentage change in CCT was statistically significantly higher in PEX group than that in the control group(P ≤0.041).CONCLUSION: Although torsional mode phacoemulsification and intraocular lens(IOL) implantation provided a safe and favorable surgical outcome in patients with/without PEX, torsional phacoemulsification led to significantly higher ECD loss in the PEX group than that in the control group during the whole follow up period.In addition, more corneal swelling in the PEX group than that in the control group during the early postoperative period has indicated that the corneal endothelium, in presence of PEX endotheliopathy, seems to be more susceptible to the effects of phacoemulsification surgeryin eyes with PEX. 14Pentacam Scheimpflug成像测量晶状体密度与torsional超声乳化参数间的关系(英文)显示文摘目的:评价运用Pentacam Scheimpflug成像测量晶状体核密度与torsinal超声乳化动力学参数(如:运用于老年核性白内障患者的超声能量水平、液体留存时间及液体量)间的关系。方法:此研究为前瞻性双盲研究。瞳孔扩大后运用Pentacam Scheimpflug 成像进行测量。白内障的等级由pentacam核密度测定法[ pentacam nucleus densitometry, PND;又名Pentacam核分段( Pentacam nucleus staging, PNS)软件]自主分为1到5个等级。超声乳化术后,自动在Infiniti Ozil IP超声乳化系统监视器上计算并显示总超声( U/S)时间、累计耗散能、Torsional U/S时间以及估算液体的使用情况。组间差异运用单因素方差分析( ANOVA )进行评定,多组分析运用Tamhane测试法,PND测量的晶状体密度与torsinal超声乳化动力学参数间的关系运用Spearman相关分析进行评定。界定P〈0.05时具有统计学意义。结果:此项研究包括125例患者(125眼),平均年龄为69.7±9.4岁(范围:48~88岁),其中61例男性、64例女性。 U/S总时间、torsional U/S时间、CDE及估算液体使用情况的范围分别为0.70~158.9s,0.70~158.5s,0.11~42.65和21~98mL.
