| 1 | From fatty liver to fibrosis:A tale of “second hit”显示文摘Although much is known about how fat accumulates in the liver,much remains unknown about how this causes sustained hepatocellular injury.The consequences of injury are recognized as nonalcoholic steatohepatitis(NASH) and progressive fibrosis.The accumulation of fat within the hepatocytes sensitizes the liver to injury from a variety of causes and the regenerative capacity of a fatty liver is impaired.An additional stressor is sometimes referred to as a 'second hit' in a paradigm that identifies the accumulation of fat as the 'first hit'.Possible candidates for the second hit include increased oxidative stress,lipid peroxidation and release of toxic products such as malondialdehyde and 4-hydroxynonenal,decreased antioxidants,adipocytokines,transforming growth factor(TGF)-β,Fas ligand,mitochondrial dysfunction,fatty acid oxidation by CYPs(CYP 2E1,4A10 and 4A14),and peroxisomes,excess iron,small intestinal bacterial overgrowth,and the generation of gut-derived toxins such as lipopolysaccharide and ethanol.Oxidative stress is one of the most popular proposed mechanisms of hepatocellular injury.Previous studies have specifically observed increased plasma and tissue levels of oxidative stress markers and lipid peroxidation products,with reduced hepatic and plasma levels of antioxidants.There is also some indirect evidence of the benefit of antioxidants such as vitamin E,S-adenosylmethionine,betaine,phlebotomy to remove iron,and N-acetylcysteine in NASH.However,a causal relationship or a pathogenic link between NASH and oxidative stress has not been established so far.A number of sources of increased reactive oxygen species production have been established in NASH that include proinflammatory cytokines such as tumor necrosis factor(TNF)-α,iron overload,overburdened and dysfunctional mitochondria,CYPs,and peroxisomes.Briefly,the pathogenesis of NASH is multifactorial and excess intracellular fatty acids,oxidant stress,ATP depletion,and mitochondrial dysfunction are important causes of hepatocellular injury in the steatotic liver. | Metin Basaranoglu Gkcen Basaranoglu Hakan Sentürk | 2013 | World Journal of Gastroenterology2013,19,8: | 28 |

Climbing robots could perform many useful tasks such as surveillance, inspection, repair, cleaning, and exploration. This paper presents and discusses the design, fabrication, and evaluation of two climbing robots which mimic the gait of the gecko. The first robot is designed considering macro-scale operations on Earth and in space. The second robot, whose motion is controlled using shape memory alloy actuators, is designed to be easily scaled down for micro-scale applications. | 4 | Minimally invasive surgery for gastric cancer: A comparison between robotic, laparoscopic and open surgery显示文摘AIM To investigate the role of minimally invasive surgery for gastric cancer and determine surgical, clinical, and oncological outcomes.METHODS This is a propensity score-matched case-control study, comparing three treatment arms: robotic gastrectomy(RG), laparoscopic gastrectomy(LG), open gastrectomy(OG). Data collection started after sharing a specific study protocol. Data were recorded through a tailored and protected web-based system. Primary outcomes: harvested lymph nodes, estimated blood loss, hospital stay, complications rate. Among the secondary outcomes, there are: operative time, R0 resections, POD of mobilization, POD of starting liquid diet and soft solid diet. The analysis includes the evaluation of type and grade of postoperative complications. Detailed information of anastomotic leakages is also provided.RESULTS The present analysis was carried out of 1026 gastrectomies. To guarantee homogenous distribution of cases, patients in the RG, LG and OG groups were 1:1:2 matched using a propensity score analysis with a caliper = 0.2. The successful matching resulted in a total sample of 604 patients(RG = 151; LG = 151; OG = 302). The three groups showed no differences in all baseline patients characteristics, type of surgery(P = 0.42) and stage of the disease(P = 0.16). Intraoperative blood loss was significantly lower in the LG(95.93 ± 119.22) and RG(117.91 ± 68.11) groups compared to the OG(127.26 ± 79.50, P = 0.002). The mean number of retrieved lymph nodes was similar between the RG(27.78 ± 11.45), LG(24.58 ± 13.56) and OG(25.82 ± 12.07) approach. A benefit in favor of the minimally invasive approaches was found in the length of hospital stay(P < 0.0001). A similar complications rate was found(P = 0.13). The leakage rate was not different(P = 0.78) between groups.CONCLUSION Laparoscopic and robotic surgery can be safely performed and proposed as possible alternative to open surgery. | 7 | Long-term vision-threatening complications of phakic intraocular lens implantation for high myopia显示文摘AIM:To report the long-term vision-threatening complications in patients who underwent phakic intraocular lens(pIOLs)implantation for high myopia.METHODS:Thisstudywasdesignedfromaconsecutive series of phakic intraocular lens complication and corrective surgeries.Sixteen eyes of 13 patients had implantation of phakic intraocular lens for correction high myopia and developed serious complications have been included in this study.The mean age of patients was 38.6±6.35y(range 32-50y)and the mean time of history of pIOL implantation for high myopia was 6±2y(range 2-10y).Before corrective surgery,best spectaclecorrective visual acuity(BSCVA)ranged from perception to 20/200 in the eyes in which severe complications occurred.RESULTS:Corneal decompensation occurred in 12eyes of 9 high myopic patients after anterior chamber pIOL implantation.Rhegmatogenous retinal detachment(RRD)occurred in 4 eyes of 4 high myopic patients following anterior chamber and posterior chamber pIOL implantation.Patients with corneal decompensation,had combined procedures consisting of pIOL removal and penetrating keratoplasty(PKP).Removals of pIOL,phacoemulsification and pars plana vitrectomy(PPV)with silicone oil tamponade were performed in patients with RRD.After corrective surgeries,all patients but one(P+,patient 2,right eye)achieved moderate BSCVA ranged from 20/200 to 20/50 at the last visit.CONCLUSION:Phakic IOLs may be effective for thecorrection of high myopia.Although these IOLs may have severe complications and it affects safety and efficacy of this surgery.As seen here,corneal decompensation and rhegmatogenous retinal detachment are possible postoperative vision-threatening complications of phakic IOLs.Patients must be carefully examined before and after surgery for possible endothelial cell loss and vitreoretinal problems. | Isil Bahar Sayman Muslubas Baran Kandemir Ayse Yesim Aydin Oral Suleyman Kugu Metin Dastan | 2014 | International Journal of Ophthalmology(English edition)2014,7,2: | 14 |
| 8 | Fructose as a key player in the development of fatty liver disease显示文摘We aimed to investigate whether increased consumption of fructose is linked to the increased prevalence of fatty liver.The prevalence of nonalcoholic steatohepatitis(NASH) is 3% and 20% in nonobese and obese subjects,respectively.Obesity is a low-grade chronic inflam-m-atory condition and obesity-related cytokines such as interleukin-6,adiponectin,leptin,and tumor necrosis factor-α may play important roles in the developm-ent of nonalcoholic fatty liver disease(NAFLD).Additionally,the prevalence of NASH associated with both cirrhosis and hepatocellular carcinom-a was reported to be high am-ong patients with type 2 diabetes with or without obesity.Our research group previously showed that consumption of fructose is associated with adverse alterations of plasma lipid profiles and metabolic changes in m-ice,the Am-erican Lifestyle-Induced Obesity Syndrom-e m-odel,which included consum-ption of a high-fructose corn syrup in amounts relevant to that consum-ed by som-e Am-ericans.The observation reinforces the concerns about the role of fructose in the obesity epidem-ic.Increased availability of fructose(e.g.,high-fructose corn syrup) increases not only abnorm-al glucose flux but also fructose m-etabolism-in the hepatocyte.Thus,the anatomic position of the liver places it in a strategic buffering position for absorbed carbohydrates and am-ino acids.Fructose was previously accepted as a beneficial dietary com-ponent because it does not stim-ulate insulin secretion.However,since insulin signaling plays an important role in central m-echanism-s of NAFLD,this property of fructose m-ay be undesirable.Fructose has a selective hepatic m-etabolism,and provokes a hepatic stress response involving activation of c-Jun N-term-inal kinases and subsequent reduced hepatic insulin signaling.As high fat diet alone produces obesity,insulin resistance,and som-e degree of fatty liver with m-inim-al inflam-m-ation and no fibrosis,the fast food diet which includes fructose and fats produces a gene expression signature of increased hepatic fibrosis,inflam-m-ation,endoplasm-ic reticulumstress and lipoapoptosis.Hepatic de novo lipogenesis(fatty acid and triglyceride synthesis) is increased in patients with NAFLD.Stable-isotope studies showed that increased de novo lipogenesis(DNL) in patients with NAFLD contributed to fat accum-ulation in the liver and the developm-ent of NAFLD.Specifically,DNL was responsible for 26% of accum-ulated hepatic triglycerides and 15%-23% of secreted very low-density lipoprotein triglycerides in patients with NAFLD com-pared to an estim-ated less than 5% DNL in healthy subjects and 10% DNL in obese people with hyperinsulinem-ia.In conclusion,understanding the underlying causes of NAFLD form-s the basis for rational preventive and treatm-ent strategies of this m-ajor form-of chronic liver disease. | Metin Basaranoglu Gokcen Basaranoglu Tevfik Sabuncu Hakan Sentürk | 2013 | World Journal of Gastroenterology2013,19,8: | 14 |
| 9 | Increased levels of homocysteine in patients with ulcerative colitis显示文摘AIM: To investigate serum levels of homocysteine (Hcys) and the risk that altered levels carry for thrombosis development in ulcerative colitis (UC) patients. METHODS: 55 UC patients and 45 healthy adults were included. Hcys, vitamin B12 and folic acid levels were measured in both groups. Clinical history and thrombo- embolic events were investigated. RESULTS: The average Hcys level in the UC patients was 13.3 ± 1.93 μmmol/L (range 4.60-87) and was higher than the average Hcys level of the control group which was 11.2 ± 3.58 μmmol/L (range 4.00-20.8) (P < 0.001). Vitamin B12 and folic acid average values were also lower in the UC group (P < 0.001). Whenmultivariate regression analysis was performed, it was seen that folic acid deficiency was the only risk factor for hyperhomocysteinemia. Frequencies of thromboembolic complications were not statistically significantly different in UC and control groups. When those with and without a thrombosis history in the UC group were compared according to Hcys levels, it was seen that there were no statistically significant differences. A negative linear relationship was found between folic acid levels and Hcys. | 10 | Prognostic efficacy of inflammation-based markers in patients with curative colorectal cancer resection显示文摘AIM To evaluate the prognostic significance of neutrophilto-lymphocyte ratio(NLR), platelet-to-lymphocyte ratio(PLR), lymphocyte-to-monocyte ratio(LMR), and prognostic nutritional index(PNI) and other clinicopathological factors in patients undergoing curative resection of colon cancer.METHODS183 patients with histologically proven colorectal cancer who had undergone potentially curative resection between 2010 and 2016 at Ankara Numune Training and Research Hospital were retrospectively analyzed and clinicopathological characteristics included age, sex, tumor type, grade, size and localization, the number of metastatic and total number of lymph nodes removed, vascular and perineural invasion of the tumor, TNM stages, tumor marker levels(CEA, CA19-9, AFP, CA-125, CA15-3), complete blood counts, albumin levels, overall survival(months), NLR, PLR, LMR and PNI ratios were retrospectively reviewed and analyzed from the electronic database. The primary outcome measure was overall survival.RESULTS Regarding overall survival, on univariate analysis the following variables were significantly associated with poor outcome following resection: T-stage(P = 0.037), lymph node invasion(P = 0.037), cancer stage(P = 0.034), CEA(P = 0.042), CA19-9(P = 0.004), and PNI(P = 0.001). To evaluate the independent prognostic value, multivariate Cox proportional hazard analysis to control for other prognostic factors was used. Using cancer-specific death as an end point for NLR, PLR, LMR, PNI and CA19-9 the optimal cut off values were calculated by ROC analysis. Regarding overall survival, on multivariate analysis high CA19-9(HR = 1.001, 95%CI: 1.00-1.002, P = 0.012) and low PNI(HR = 0.938, 95%CI: 0.891-0.987, P = 0.014) were the only variables independently associated with shortened overall survival. Patients with a PNI < 35 had a median OS of 52.25 mo. In contrast, patients with an PNI > 35 had a median OS of 66 mo. | 11 | Synergistic effect of multiple predisposing risk factors on the development of bezoars显示文摘AIM:To describe the clinical characteristics of patients with gastric or intestinal bezoars recently treated in our hospital.METHODS:In this study,a retrospective chart review of consecutive patients with gastrointestinal bezoars,who were treated at the Samsun Education and Research Hospital between January 2006 and March 2011,was conducted.Data on demographic characteristics,clinical presentation,history of risk factors,diagnostic procedures,localization of bezoars,treatment interventions,and postoperative morbidity and mortality rates were collected and evaluated.RESULTS:Forty-two patients [26 (61.9%) males and 16 (31.1%) females] with a mean±SD (range) age of 55.8±10.5 (37-74) years were enrolled in this study.Thirty-six patients (85.7%) had one or more predisposing risk factors for gastrointestinal bezoars.The most common predisposing risk factor was a history of previous gastric surgery which was identified in 18 patients (42.8%).Twenty three patients (54.8%) had multiple predisposing risk factors.Phytobezoars were identified in all patients except one who had a trichobezoar in the stomach.Non-operative endoscopic fragmentation was performed either initially or after unsuccessful medical treatment in 14 patients with gastric bezoars and was completely successful in 10 patients (71.5%).Surgery was the most frequent treatment method in our study,which was required in 28 patients (66.7%).Intestinal obstruction secondary to bezoars was the most common complication (n=18,42.8%) in our study.CONCLUSION:The presence of multiple predisposing factors may create a synergistic effect in the development of bezoars. | Metin Kement Nuraydin Ozlem Elif Colak Sadik Kesmer Cem Gezen Selahattin Vural | 2012 | World Journal of Gastroenterology2012,18,9: | 11 |
| 12 | Monodisperse Nickel Nanoparticles Supported on SiO_(2) as an Effective Catalyst for the Hydrolysis of Ammonia-Borane显示文摘Monodisperse Ni nanoparticles(NPs)have been synthesized by the reduction of nickel(Ⅱ)acetylacetonate with the borane–tributylamine complex in a mixture of oleylamine and oleic acid.These Ni NPs are an active catalyst for the hydrolysis of the ammonia–borane(AB,H3N⋅BH3)complex under ambient conditions and their activities are dependent on the chemical nature of the oxide support that they were deposited on.Among various oxides(SiO_(2),Al_(2)O_(3),and CeO_(2))tested,SiO_(2)was found to enhance Ni NP catalytic activity due to the etching of the 3.2 nm Ni NPs giving Ni(Ⅱ)ions and the subsequent reduction of Ni(Ⅱ)that led to the formation of 1.6 nm Ni NPs on the SiO_(2)surface.The kinetics of the hydrolysis of AB catalyzed by Ni/SiO_(2)was shown to be dependent on catalyst and substrate concentration as well as temperature.The Ni/SiO_(2)catalyst has a turnover frequency(TOF)of 13.2 mol H_(2)⋅(mol Ni)^(–1)⋅min^(–1)—the best ever reported for the hydrolysis of AB using a nickel catalyst,an activation energy of 34 kJ/mol±2 kJ/mol and a total turnover number of 15,400 in the hydrolysis of AB.It is a promising candidate to replace noble metals for catalyzing AB hydrolysis and for hydrogen generation under ambient conditions. | Önder Metin SaimÖzkar Shouheng Sun | 2010 | Nano Research2010,3,9: | 9 |

| 14 | Pseudolymphoma (reactive lymphoid hyperplasia) of the liver:A clinical challenge显示文摘Reactive lymphoid hyperplasia(RLH), also known as pseudolymphoma or nodular lymphoid lesion of the liver is an extremely rare condition, and only 51 hepatic RLH cases have been described in the literature since the first case was described in 1981. The majority of these cases were asymptomatic and incidentally found through radiological imaging. The precise etiology of hepatic RLH is still unknown, but relative high prevalence of autoimmune disorder in these cases suggests an immune-based liver disorder. Imaging features of hepatic RLH often suggest malignant lesions such as hepatocellular carcinoma and cholangiocarcinoma. In this report, we discuss two cases of hepatic RLH in patients with autoimmune hepatitis. We also present pathologic and magnetic resonance imaging findings, including one case utilizing a hepatocellular contrast agent, Eovist. | 17 | Mallory-Denk Bodies in chronic hepatitis显示文摘Mallory-Denk Bodies(MDB) are important as investigators,suggesting MDB as an indicator of the histologic severity of chronic hepatitis,causes of which include hepatitis C,primary biliary cirrhosis(PBC),and nonalcoholic fatty liver disease(NAFLD).Matteoni et al scored MDB in patients with NAFLD as none,rare and many,and reported that MDB plays a prominent role in this classification scheme in an earlier classification system.In this study,we evaluated 258 patients with chronic hepatitis due to metabolic,autoimmune and viral etiologies.Liver biopsy samples were evaluated with hematoxylin and eosin,periodic acid-Schiff-diastase,Gordon and Sweet's reticulin,Masson's trichrome,and iron stains.Both staging and grading were performed.Additionally,MDB were evaluated and discussed for each disease.We examined patients with nonalcoholic steatohepatitis(NASH;50 patients),alcoholic hepatitis(10 patients),PBC(50 patients),Wilson disease(WD;20 patients),hepatitis B(50 patients),hepatitis C(50 pati-ents) and hepatocellular carcinoma(HCC;30 patients).Frequency of MDB was as follows;NASH:10 patients with mild in 60% and moderate in 40% and observed in every stage of the disease and frequently seen in zone 3.PBC:11 patients with mild in 10%,moderate in 70%,and cirrhosis in 20%,and frequently seen in zone 1.WD:16 patients with moderate and severe in 60% and cirrhosis in 40% and frequently seen in zone 1.Hep B:3 patients with mild in 66% and severe in 34%.Hep C:7 patients with mild in 40% and moderate in 60% and observed in every stage.HCC:3 patients with hep B in 2 patients.We found that there is no relationship between MDB and any form of chronic hepatitis regarding histologic severity such as alcoholic steatohepatitis and NAFLD and variable zone distribution by etiology. | Metin Basaranoglu Nesrin Turhan Abdullah Sonsuz Gkcen Basaranoglu | 2011 | World Journal of Gastroenterology2011,17,17: | 6 |

It is a flattened oval organism covered with cilia, and a gullet at the anterior end. It is infrequently pathogenic for man,although epidemic buds in tropical zones have been described. The infection fundamentally affects the colon and causes variable clinic pictures, from asymptomatic to serious dysenteric forms. | 20 | Re-adhesion control strategy based on the optimal slip velocity seeking method显示文摘In the railway industry, re-adhesion control plays an important role in attenuating the slip occurrence due to the low adhesion condition in the wheel–rail interaction. Braking and traction forces depend on the normal force and adhesion coefficient at the wheel–rail contact area. Due to the restrictions on controlling normal force,the only way to increase the tractive or braking effect is to maximize the adhesion coefficient. Through efficient utilization of adhesion, it is also possible to avoid wheel–rail wear and minimize the energy consumption. The adhesion between wheel and rail is a highly nonlinear function of many parameters like environmental conditions, railway vehicle speed and slip velocity. To estimate these unknown parameters accurately is a very hard and competitive challenge. The robust adaptive control strategy presented in this paper is not only able to suppress the wheel slip in time, but also maximize the adhesion utilization performance after re-adhesion process even if the wheel–rail contact mechanism exhibits significant adhesion uncertainties and/or nonlinearities. Using an optimal slip velocity seeking algorithm, the proposed strategy providesa satisfactory slip velocity tracking ability, which was demonstrated able to realize the desired slip velocity without experiencing any instability problem. The control torque of the traction motor was regulated continuously to drive the railway vehicle in the neighborhood of the optimal adhesion point and guarantee the best traction capacity after re-adhesion process by making the railway vehicle operate away from the unstable region.
