| 1 | Recent developments and innovations in gastric cancer显示文摘Gastric cancer has an important place in the worldwide incidence of cancer and cancer-related deaths. It can metastasize to the lymph nodes in the early stages, and lymph node metastasis is an important prognostic factor. Surgery is a very important part of gastric cancer treatment. A D2 lymphadenectomy is the standard surgical treatment for c T1N+ and T2-T4 cancers, which are potentially curable. Recently, the TNM classification system was reorganized, and the margins for gastrectomy and lymphadenectomy were revised. Endoscopic, laparoscopic and robotic treatments of gastric cancer have progressed rapidly with development of surgical instruments and techniques, especially in Eastern countries. Different endoscopic resection techniques have been identified, and these can be divided into two main categories: endoscopic mucosal resection and endoscopic submucosal dissection. Minimally invasive surgery has been reported to be safe and effective for early gastric cancer, and it can be successfully applied to advanced gastric cancer with increasing experience. Cytoreductive surgery and hypertherm?c intraper?toneal chemotherapy were developed as a combined treatment modality from the results of experimental and clinical studies. Also, hyperthermia increases the antitumor activity and penetration of chemotherapeutics. Trastuzumab which is a monoclonal antibody interacts with human epidermal growth factor(HER) 2 and is related to gastric carcinoma. The anti-tumor mechanism of trastuzumab is not clearly known, but mechanisms such as interruption of the HER2-mediated cell signaling pathways and cell cycle progression have been reported previously. H. pylori is involved in 90% of all gastric malignancies and Japanese guidelines strongly recommend that all H. pylori infections should be eradicated regardless of the associated disease. | 2 | Efficacy of ankaferd blood stopper application on nonvariceal upper gastrointestinal bleeding显示文摘AIM:To prospectively assess the hemostatic efficacy of the endoscopic topical use of ankaferd blood stopper(ABS) in active non-variceal upper gastrointestinal system(GIS) bleeding.METHODS:Endoscopy was performed on 220 patients under suspiciency of GIS bleeding.Patients with active non-variceal upper gastrointestinal bleeding(NVUGIB) with a spurting or oozing type were included.Firstly,8-10 cc of isotonic saline was sprayed to bleeding lesions.Then,8 cc of ABS was applied on lesions in which bleeding continued after isotonic saline application.The other endoscopic therapeutic methods were applied on the lesions in which the bleeding did not stop after ABS.RESULTS:Twenty-seven patients had an active NVUGIB with a spurting or oozing type and 193 patients were excluded from the study since they did not have non-variceal active bleeding.8 cc of ABS was sprayed on to the lesions of 26 patients whose bleeding continued after isotonic saline and in 19 of them,bleeding stopped after ABS.Other endoscopic treatment methods were applied to the remaining patients and the bleeding was stopped with these interventions in 6 of 7 patients.CONCLUSION:ABS is an effective method on NVUGIB,particularly on young patients with no coagulopathy.ABS may be considered as part of a combination treatment with other endoscopic methods. | Gokhan Gungor M Hakan Goktepe Murat Biyik Ilker Polat Tuncer Tuna Huseyin Ataseven Ali Demir | 2012 | World Journal of Gastrointestinal Endoscopy2012，4，12： | 5 |

| 4 | Colorectal stenting for palliation and as a bridge to surgery:A 5-year follow-up study显示文摘AIM: To evaluate the long-term effectiveness of colonic stents in colorectal tumors causing large bowel obstruction.METHODS: We retrospectively analyzed data from 49 patients with colorectal cancer who had undergone colorectal stent placement between January 2008 and January 2013. Patients' symptoms,characteristics and clinicopathological data were obtained by reviewing medical records. The obstruction was diagnosed clinically and radiologically. Histopathological diagnosis was achieved endoscopically. Technical success rate(TSR)was defined as the ratio of patients with correctly placed SEMS upon stent deployment across the entire stricture length to total number of patients. Clinical success rate(CSR) was defined as the ratio of patients with technical success and successful maintenance of stent function before elective surgery(regardless of number of SEMS deployed) to total number of patients. The surgical success rate(SSR) of colorectal stent as a bridge to surgery was defined as the ratio of patients with successful surgical procedures. Unsuccessful surgical outcomes were defined as being due to insufficient colonic decompression. The technical,clinical,surgical success rates and complications after stenting were assessed.RESULTS: The median age of patients was 64(36 to 89). 44.9% of patients were male and 55.1% were female. Eighteen patients had the obstruction located in the rectum,15 patients in the rectosigmoid region,10 patients in the sigmoid region,and 6 patients had a tumor causing obstruction in the proximal colon. Each patient was categorized pathologically as stage 2(32.7%,16 patients) or stage 3(42.9%,21 patients) and 12 patients(24.4%) had metastatic disease. None of the patients received chemotherapy before stenting. Stenting was undertaken in 37 patients as a bridge to surgery,and in 12 patients stents were used for palliation. Median time to surgery after stenting was 30 ± 91.9 d. All surgery was completed in one single operation and thus no colostomy with stoma was needed. The median overall survival rate of patients with stage 2-3 colorectal cancer was 53.1 mo and stage 4 was 37.1 mo(P = 0.04). Metastatic colorectal patients who were treated palliatively with stents had backbone chemotherapy with oxaliplatin and/or irinotecan-based regimens plus antiangiogenic therapies,especially bevacizumab. Resolution of the obstruction and clinical improvement was achieved in all patients. The technical,clinical and surgical success rates were 95.9%,100% and 94.6%,respectively.CONCLUSION: The efficacy and safety of colonic stents was demonstrated both as a bridge to surgery and for palliative decompression. | 6 | Evaluation of mixed mode-Ⅰ/Ⅱ criteria for fatigue crack propagation using experiments and modeling显示文摘In this study, in-plane mixed mode-Ⅰ/Ⅱ fatigue crack growth simulations and experiments are performed for the Al 7075-T651 aluminum alloy which is widely used in the aerospace industry. Tests are carried out under different mode mixity ratios to evaluate the applicability of a fracture criterion developed in a previous study to mixed mode-Ⅰ/Ⅱ fatigue crack growth tests.Results obtained from the analyses and experiments are compared with existing and developed criteria in terms of crack growth lives. Compact Tension Shear(CTS) specimens, which enable mixed mode loading with loading devices under different loading angles, are used in the simulations and experiments. In an effort to model and simulate the actual conditions in the experiments, crack surfaces of fractured specimens are scanned, crack paths are modeled exactly, and contacts are defined between the contact surfaces of a specimen and the loading device for each crack propagation step in the analyses.
