1Maintenance for healed erosive esophagitis:PhaseⅢcomparison of vonoprazan with lansoprazole显示文摘AIM To compare vonoprazan 10 and 20 mg vs lansoprazole 15 mg as maintenance therapy in healed erosive esophagitis(EE).METHODS A total of 607 patients aged ≥ 20 years, with endoscopically-confirmed healed EE following 8 wk of treatment with vonoprazan 20 mg once daily, were randomized 1:1:1 to receive lansoprazole 15 mg(n = 201), vonoprazan 10 mg(n = 202), or vonoprazan 20 mg(n = 204), once daily. The primary endpoint of the study was the rate of endoscopically-confirmed EE recurrence during a 24-wk maintenance period. The secondary endpoint was the EE recurrence rate at Week 12 during maintenance treatment. Additional efficacy endpoints included the incidence of heartburn and acid reflux, and the EE healing rate 4 wk after the initiation of maintenance treatment. Safety endpoints comprised adverse events(AEs), vital signs, electrocardiogram findings, clinical laboratory results, serum gastrin and pepsinogen Ⅰ/Ⅱ levels, and gastric mucosa histopathology results.RESULTS Rates of EE recurrence during the 24-wk maintenance period were 16.8%, 5.1%, and 2.0% with lansoprazole 15 mg, vonoprazan 10 mg, and vonoprazan 20 mg, respectively. Vonoprazan was shown to be non-inferior to lansoprazole 15 mg(P < 0.0001 for both doses). In a post-hoc analysis, EE recurrence at Week 24 was significantly reduced with vonoprazan at both the 10 mg and the 20 mg dose vs lansoprazole 15 mg(5.1% vs 16.8%, P = 0.0002, and 2.0% vs 16.8%, P < 0.0001, respectively); by contrast, the EE recurrence rate did not differ significantly between the two doses of vonoprazan(P = 0.1090). The safety profiles of vonoprazan 10 and 20 mg were similar to that of lansoprazole 15 mg in patients with healed EE. 2Prognostic value of KRAS and BRAF mutations in curatively resected colorectal cancer显示文摘AIM: To investigate the prognostic role of KRAS and BRAF mutations after adjustment for microsatellite instability(MSI) status in Japanese colorectal cancer(CRC) population.METHODS: We assessed KRAS and BRAF mutations and MSI status in 813 Japanese patients with curatively resected, stage Ⅰ-Ⅲ CRC and examined associations of these mutations with disease-free survival(DFS) and overall survival(OS) using uni- and multivariate Cox proportional hazards models.RESULTS: KRAS and BRAF mutations were detected in 312(38%) of 812 and 40(5%) of 811 tumors, respectively. KRAS mutations occurred more frequently in females than in males(P = 0.02), while the presence of BRAF mutations was significantly associated with the female gender(P = 0.006), proximal tumor location(P < 0.001), mucinous or poorly differentiated histology(P < 0.001), and MSI-high tumors(P < 0.001). After adjusting for relevant variables, including MSI status, KRAS mutations were associated with poorer DFS(HR = 1.35; 95%CI: 1.03-1.75) and OS(HR = 1.46; 95%CI: 1.09-1.97). BRAF mutations were poor prognostic factors for DFS(HR = 2.20; 95%CI: 1.19-4.06) and OS(HR = 2.30; 95%CI: 1.15-4.71). 8Impact of Thickness of Polymer Electrolyte Membrane and Gas Diffusion Layer on Temperature Distributions in Polymer Electrolyte Fuel Cell Operated at Temperature around 90℃显示文摘Polymer Electrolyte Fuel Cell(PEFC)is desired to be operated at temperature around 90℃ for stationary applications during the period from 2020 to 2025 in Japan.It can be expected thinner polymer electrolyte membrane(PEM)and gas diffusion layer(GDL)would promote the power generation performance of PEFC at this temperature.The aim of this study is to understand the impact of thickness of PEM and GDL on the temperature profile of interface between PEM and catalyst layer at the cathode(i.e.,the reaction surface)in a single PEFC with an initial operation temperature(Tini).An 1D multi-plate heat transfer model based on temperature data of separator measured using thermograph in power generation process was developed to evaluate temperature of the reaction surface(Treact).This study investigated the effect of Tini,flow rate and relative humidity of supply gas on Treact distribution.The study finds that when using thin GDL,the even distribution of Treact – Tini is obtained irrespective of thickness of PEM,Tini and relative humidity conditions.Treact – Tini using Nafion 115 is higher than the other thin PEMs irrespective of Tini and relative humidity conditions.It can be concluded that the even temperature distribution could be achieved by using thin PEM and GDL.Akira Nishimura Yusuke Sato Satoru Kamiya Tatsuya Okado Kouhei Yamamoto Masafumi Hirota Eric Hu 2019Journal of Energy and Power Engineering2019,13,3:2
