1Clinical features and prognosis of patients with extrahepatic metastases from hepatocellular carcinoma显示文摘AIM: To assess the clinical features and prognosis of 151 patients with extrahepatic metastases from primary hepatocellular carcinoma (HCC), and describe the treatment strategy for such patients. METHODS: After the diagnosis of HCC, all 995 consecutive HCC patients were followed up at regular intervals and 151 (15.2%) patients were found to have extrahepatic metastases at the initial diagnosis of primary HCC or developed such tumors during the follow-up period. We assessed their clinical features, prognosis, and treatment strategies. RESULTS: The most frequent site of extrahepatic metastases was the lungs (47%), followed by lymph nodes (45%), bones (37%), and adrenal glands (12%). The cumulative survival rates after the initial diagnosis of extrahepatic metastases at 6, 12, 24, and 36 mo were 44.1%, 21.7%, 14.2%, 7.1%, respectively. The median survival time was 4.9 mo (range, 0-37 mo). Fourteen patients (11%) died of extrahepatic HCC, others died of primary HCC or liver failure. CONCLUSION: The prognosis of HCC patients with extrahepatic metastases is poor. With regard to the cause of death, many patients would die of intrahepatic HCC and few of extrahepatic metastases. 2Apoptosis and non-alcoholic fatty liver diseases显示文摘The number of patients with nonalcoholic fatty liver diseases(NAFLD) including nonalcoholic steatohepatitis(NASH), has been increasing. NASH causes cirrhosis and hepatocellular carcinoma(HCC) and is one of the most serious health problems in the world. The mechanism through which NASH progresses is still largely unknown. Activation of caspases, Bcl-2 family proteins, and c-Jun N-terminal kinase-induced hepatocyte apoptosis plays a role in the activation of NAFLD/NASH. Apoptotic hepatocytes stimulate immune cells and hepatic stellate cells toward the progression of fibrosis in the liver through the production of inflammasomes and cytokines. Abnormalities in glucose and lipid metabolism as well as microbiota accelerate these processes. The production of reactive oxygen species, oxidative stress, and endoplasmic reticulum stress is also involved. Cell death, including apoptosis, seems very important in the progression of NAFLD and NASH. Recently, inhibitors of apoptosis have been developed as drugs for the treatment of NASH and may prevent cirrhosis and HCC. Increased hepatocyte apoptosis may distinguish NASH from NAFLD, and the improvement of apoptosis could play a role in controlling the development of NASH. In this review, the association between apoptosis and NAFLD/NASH are discussed. 3The Plant Vascular System: Evolution, Development and Functions显示文摘The emergence of the tracheophyte-based vascular system of land plantshad major impacts on the evolution of terrestrial biology, in general,through its role in facilitating the development of plants with increasedstature, photosynthetic output, and ability to colonize a greatly expandedrange of environmental habitats. Recently, considerable progress hasbeen made in terms of our understanding of the developmental andphysiological programs involved in the formation and function of theplant vascular system. In this review, we first examine the evolutionaryevents that gave rise to the tracheophytes, followed by analysis of thegenetic and hormonal networks that cooperate to orchestrate vasculardevelopment in the gymnosperms and angiosperms. The two essentialfunctions performed by the vascular system, namely the delivery of resources (water, essential mineralnutrients, sugars and amino acids) to the various plant organs and provision of mechanical support arenext discussed. Here, we focus on critical questions relating to structural and physiological propertiescontrolling the delivery of material through the xylem and phloem. Recent discoveries into the role ofthe vascular system as an effective long-distance communication system are next assessed in termsof the coordination of developmental, physiological and defense-related processes, at the whole-plantlevel. A concerted effort has been made to integrate all these new findings into a comprehensive pictureof the state-of-the-art in the area of plant vascular biology. 4Irinotecan, a key chemotherapeutic drug for metastatic colorectal cancer显示文摘Irinotecan hydrochloride is a camptothecin derivative that exerts antitumor activity against a variety of tumors. SN-38 produced in the body by carboxylesterase is the active metabolite of irinotecan. After irinotecan was introduced for the treatment of metastatic colorectal cancer(CRC) at the end of the last century,survival has improved dramatically. Irinotecan is now combined with 5-fluorouracil,oxaliplatin and several molecularly-targeted anticancer drugs,resulting in the extension of overall survival to longer than 30 mo. Severe,occasionally life-threatening toxicity occurs sporadically,even in patients in relatively good condition who have a low risk of chemotherapyinduced toxicity,often causing the failure of irinotecanbased chemotherapy. Clinical pharmacological studies have revealed that such severe toxicity is related to exposure to SN-38 and genetic polymorphisms in UDPglucuronosyltransferase 1A1 gene. The large interand intra-patient variability in systemic exposure to SN-38 is determined not only by genetic factors but also by physiological and environmental factors. 5Arsenic Disulfide Induced Apoptosis and Concurrently Promoted Erythroid Differentiation in Cytokine-Dependent Myelodysplastic Syndrome-progressed Leukemia Cell Line F-36p with Complex Karyotype Including Monosomy 7显示文摘Objective:Acute myeloid leukemia progressed from myelodysplastic syndrome(MDS/AML)is generally incurable with poor prognosis for complex karyotype including monosomy 7(-7).Qinghuang Powder(青黄散,QHP),which includes Qing Dai(Indigo naturalis)and Xiong Huang(realgar)in the formula,is effective in treating MDS or MDS/AML even with the unfavorable karyotype,and its therapeutic efficacy could be enhanced by increasing the Xiong huang content in the formula,while Xiong huang contains>90%arsenic disulfide(As_2S_2).F-36p cell line was established from a MDS/AML patient with complex karyotype including-7,and was in cytokine-dependent.The present study was to investigate the effects of AS_2S_2 on F-36p cells.Methods:Cell proliferation was measured by an3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide(MTT)assay.Cell apoptosis was identified by Annexin V-staining.Cell viability was determined by a propidium iodide(PI)exclusion.Erythroid differentiation was evaluated by the expression of cell surface antigen CD235a(GpA).Results:After treatment with AS_2S_2,at concentrations of 0.5to 16μmol/L for 72 h,As_2S_2 inhibited the proliferation of F-36p cells.The 50%inhibitory concentrations(IC_(50))of As_2S_2against the proliferation of F-36p cells was 6μmol/L.The apoptotic cells significantly increased in a dose-dependent mannar(P<0.05).The cell viabilities were significantly inhibited by As_2S_2 dose-dependent in a dose-dependent manner(P<0.05).Significant increases of CD235a-positive cells were concurrently observed(P<0.05)also in a dosedependent manner.Conclusions:As_2S_2 could inhibit proliferation and viability,induce apoptosis,and concurrently promote erythroid differentiation dose-dependently in F-36p cells.AS_2S_2 can inhibit proliferation and viability,induce apoptosis,and concurrently promote erythroid differentiation in cytokine-dependent MDS-progressed human leukemia cell line F-36p with complex karyotype including-7.The data suggest that QHP and/or As_2S_2 could be a potential candidate in the treatment of MDS or MDS/AML even with unfavorable cytogenetics.胡晓梅 Sachiko Tanaka Kenji Onda 袁博 Hiroo Toyoda 麻柔 刘峰 Toshihiko Hirano 2014Chinese Journal of Integrative Medicine2014,20,5:20

7Regulation of Vascular Development by CLE Peptide-receptor Systems显示文摘Cell division and differentiation of stem cells are controlled by non-cell-autonomous signals in higher organisms. The plant vascular meristem is a stem-cell tissue comprising procambial cells that produce xylem cells on one side and phloem cells on the other side. Recent studies have revealed that TDIF (tracheary element differentiation inhibitory factor)/CLE41/CLE44 peptide signal controls the procambial cell fate in a non-cell-autonomous manner. TDIF produced in and secreted from phloem cells is perceived by TDR/PXY, a leucine-rich repeat receptor kinase located in the plasma membrane of procambial cells. This signal suppresses xylem cell differentiation of procambial cells and promotes their proliferation. In addition to TDIF, some other CLE peptides play roles in vascular development. 8Excessive portal flow causes graft failure in extremely small-for-size liver transplantation in pigs显示文摘AIM: To evaluate the effects of a portocaval shunt on the decrease of excessive portal flow for the prevention of sinusoidal microcirculatory injury in extremely smallfor-size liver transplantation in pigs.METHODS: The right lateral lobe of pigs, i.e. the 25%of the liver, was transplanted orthotopically. The pigs were divided into two groups: graft without portocaval shunt (n = 11) and graft with portocaval shunt (n=11).Survival rate, portal flow, hepatic arterial flow, and histological findings were investigated.RESULTS: In the group without portocaval shunt, all pigs except one died of liver dysfunction within 24 h after transplantation. In the group with portocaval shunt,eight pigs survived for more than 4 d. The portal flow volumes before and after transplantation in the group without portocaval shunt were 118.2±26.9 mL/min/100 g liver tissue and 270.5±72.9 mL/min/100 g liver tissue,respectively. On the other hand, in the group with portocaval shunt, those volumes were 124.2±27.8 mL/min/100 g liver tissue and 42.7±32.3 mL/min/100 g liver tissue, respectively (P<0.01). 9Inhibitory effects of carbon dioxide insufflation on pneumoperitoneum and bowel distension after percutaneous endoscopic gastrostomy显示文摘AIM:To evaluate the inhibitory effects of carbon dioxide (CO2) insufflation on pneumoperitoneum and bowel distension after percutaneous endoscopic gastrostomy (PEG).METHODS:A total of 73 consecutive patients who were undergoing PEG were enrolled in our study.After eliminating 13 patients who fitted our exclusion criteria,60 patients were randomly assigned to either CO2 (30 patients) or air insufflation (30 patients) groups.PEG was performed by pull-through technique after threepoint fixation of the gastric wall to the abdominal wall using a gastropexy device.Arterial blood gas analysis was performed immediately before and after the procedure.Abdominal X-ray was performed at 10 min and at 24 h after PEG to assess the extent of bowel distension.Abdominal computed tomography was performed at 24 h after the procedure to detect the presence of pneumoperitoneum.The outcomes of PEG for 7 d postprocedure were also investigated.RESULTS:Among 30 patients each for the air and the CO2 groups,PEG could not be conducted in 2 patients of the CO2 group,thus they were excluded.Analyses of the remaining 58 patients showed that the patients' backgrounds were not significantly different between the two groups.The elevation values of arterial partial pressure of CO2 in the air group and the CO2 group were 2.67 mmHg and 3.32 mmHg,respectively (P = 0.408).The evaluation of bowel distension on abdominal X ray revealed a significant decrease of small bowel distension in the CO2 group compared to the air group (P < 0.001) at 10 min and 24 h after PEG,whereas there was no significant difference in large bowel distension between the two groups.Pneumoperitoneum was observed only in the air group but not in the CO2 group (P = 0.003).There were no obvious differences in the laboratory data and clinical outcomes after PEG between the two groups.CONCLUSION:There was no adverse event associated with CO2 insufflation.CO2 insufflation is considered to be safer and more comfortable for PEG patients because of the lower incidence of pneumoperitoneum and less distension of the small bowel.Shinji Nishiwaki Hiroshi Araki Motoshi Hayashi Jun Takada Masahide Iwashita Atsushi Tagami Hiroo Hatakeyama Takao Hayashi Teruo Maeda Koshiro Saito 2012World Journal of Gastroenterology2012,18,27:7
10Orthogonal-Gradient Measurement Matrix Construction Algorithm显示文摘An orthogonal-gradient measurement matrix construction algorithm is proposed for reducing the maximum and average mutual-coherence of sensing matrix. It shrinks Gram matrix based on equiangular tight frame theory. An orthogonal-gradient factor matrix is deduced. It obtains an optimized measurement matrix with the orthogonal-gradient factor matrix. The results of experiments show that the proposed algorithm effectively reduces the maximum and average mutual-coherence of sensing matrix. 11The Relationships between Rheological Properties and Structural Changes of Chilled Abalone Meat显示文摘The quantitative correlation between theological properties and structural characteristic values of chilled abalone meat was studied. Structural changes were observed, and these values were enumerated using image processing and analysis technique. Structural changes in the myofibrils and collagen fibrils were the greatest in chilling for 24 h. After chilling for 48 h, similar structures of vertical and cross sections were observed. For chilling from 0h to 72h, the instantaneous modulus E0 of the both section meat decreases gradually with time, but no significant differences were observed after chilling for 48 h.The relaxation time and viscosity of both sections attained the same values for the same chilling time, but increased gradually with increasing chilling time. Meanwhile, a negative correlation between the structural characteristic values (Dm, Am,Rvm), and rheological properties (E1,τi,η1) clearly exists. Some logarithmic expressions have been obtained for these negative correlations. These results suggest that the difference in rheological properties between the cross and vertical sections was mainly due to the structural changes of myofibrils and collagen fibrils, and rheological properties are influenced quantitatively by the structural characteristic values for chilling from 0 h to 72 h.TASHIRO Yuri OGAWA Hiroo 2003Journal of Ocean University of Qingdao2003,2,2:4

15Effects of a 24-week course of interferon-αtherapy after curative treatment of hepatitis C virus-associated hepatocellular carcinoma显示文摘AIM:To assess whether a 24-wk course of interferon (IFN)could prevent hepatocellular carcinoma(HCC) recurrence and worsening of liver function in patients with hepatitis C virus(HCV)-infected patients after receiving curative treatment for primary HCC. METHODS:Outcomes in 42 patients with HCV infection treated with IFN-α,after curative treatment for primary HCC(IFN group),were compared with 42 matched curatively treated historical controls not given IFN(non- IFN group). RESULTS:Although the rate of initial recurrence did not differ significantly between IFN group and non-IFN group (0%,44%,61%,and 67% vs 4.8%,53%,81%,and 87% at 1,3,5,and 7 years,P=0.153,respectively), IFN group showed a lower rate than the non-IFN group for second recurrence(0%,10.4%,28%,and 35% vs 0%,30%,59%,and 66% at 1,3,5 and 7 years, P=0.022,respectively).Among the IFN group,patients with sustained virologic response(SVR)were less likely to have a second HCC recurrence than IFN patients without an SVR,or non-IFN patients.Multivariate analysis identified the lack of SVR as the only independent risk factor for a second recurrence,while SVR and Child-Pugh class A independently favored overall survival. CONCLUSION:Most intrahepatic recurrences of HCV- related HCC occurred during persistent viral infection. 17Laparoscopic Kasai portoenterostomy can be a standard surgical procedure for treatment of biliary atresia显示文摘BACKGROUND Biliary atresia(BA)is a rare pediatric disease.AIM To compare the outcomes of laparoscopic portoenterostomy(Lap-PE)with those of laparotomy(Open-PE)at a single institution.METHODS The surgical outcomes of PE were retrospectively analyzed for patients with a non-correctable type of BA from 2003 to 2020.RESULTS Throughout the assessment period,119 patients received PE for BA treatment,including 66 Open-PE and 53 Lap-PE cases.Although the operation duration was longer(medians:for Open-PE,242 min;for Lap-PE,341 min;P<0.001),blood loss was considerably less(medians:for Open-PE,52 mL;for Lap-PE,24 mL;P<0.001)in the Lap-PE group than in the Open-PE group.The postoperative recovery of the Lap-PE group was more favorable;specifically,both times to resume oral intake and drain removal were significantly shorter in the Lap-PE group.Complete resolution of jaundice was observed in 45 Open-PE cases and 42 Lap-PE cases,with no statistically significant difference(P=0.176).Native liver survival rates were>80%for both groups for the first half year post surgery,followed by a gradual decrease with time;there were no statistically significant differences in the native liver survival rates for any durations assessed.CONCLUSION Lap-PE could be a standard therapy for BA.Chiyoe Shirota Akinari Hinoki Takahisa Tainaka Wataru Sumida Fumie Kinoshita Kazuki Yokota Satoshi Makita Hizuru Amano Yoichi Nakagawa Hiroo Uchida 2022World Journal of Gastrointestinal Surgery2022,14,1:2
