1Chronic Liver Failure-Sequential Organ Failure Assessment is better than the Asia-Pacific Association for the Study of Liver criteria for defining acute-on-chronic liver failure and predicting outcome显示文摘AIM:To compare the utility of the Chronic Liver FailureSequential Organ Failure Assessment(CLIF-SOFA)and Asia-Pacific Association for the Study of Liver(APASL)definitions of acute-on-chronic liver failure(ACLF)in predicting short-term prognosis of patients with ACLF.METHODS:Consecutive patients of cirrhosis with acute decompensation were prospectively included.They were grouped into ACLF and no ACLF groups as per CLIF-SOFA and APASL criteria.Patients were followed up for 3 mo from inclusion or mortality whichever was earlier.Mortality at 28-d and 90-d was compared between no ACLF and ACLF groups as per both criteria.Mortality was also compared between different grades of ACLF as per CLIF-SOFA criteria.Prognostic scores like CLIF-SOFA,Acute Physiology and ChronicHealth Evaluation(APACHE)-Ⅱ,Child-Pugh and Model for End-Stage Liver Disease(MELD)scores were evaluated for their ability to predict 28-d mortality using area under receiver operating curves(AUROC).RESULTS:Of 50 patients,38 had ACLF as per CLIFSOFA and 19 as per APASL criteria.Males(86%)were predominant,alcoholic liver disease(68%)was the most common etiology of cirrhosis,sepsis(66%)was the most common cause of acute decompensation while infection(66%)was the most common precipitant of acute decompensation.The 28-d mortality in no ACLF and ACLF groups was 8.3%and 47.4%(P=0.018)as per CLIF-SOFA and 39%and 37%(P=0.895)as per APASL criteria.The 28-d mortality in patients with no ACLF(n=12),ACLF grade 1(n=11),ACLF grade 2(n=14)and ACLF grade 3(n=13)as per CLIF-SOFA criteria was 8.3%,18.2%,42.9%and76.9%(χ2 for trend,P=0.002)and 90-d mortality was 16.7%,27.3%,78.6%and 100%(χ2 for trend,P<0.0001)respectively.Patients with prior decompensation had similar 28-d and 90-d mortality(39.3%and 53.6%)as patients without prior decompensation(36.4%and 63.6%)(P=NS).AUROCs for 28-d mortality were 0.795,0.787,0.739 and 0.710 for CLIF-SOFA,APACHE-Ⅱ,Child-Pugh and MELD scores respectively.On multivariate analysis of these scores,CLIF-SOFA was the only significant independent predictor of mortality with an odds ratio 1.538(95%CI:1.078-2.194).CONCLUSION:CLIF-SOFA criteria is better than APASL criteria to classify patients into ACLF based on their prognosis.CLIF-SOFA score is the best predictor of short-term mortality.Radha K Dhiman Swastik Agrawal Tarana Gupta Ajay Duseja Yogesh Chawla 2014World Journal of Gastroenterology2014,20,40:26
2Functional properties, phenolic constituents and antioxidant potential of industrial apple pomace for utilization as active food ingredient显示文摘Apple pomace is a waste biomass generated after apple fruit processing.In present investigation,efforts were made to comprehend influence of differently dried pomace on cell wall properties and phenolic profile.Different drying techniques were employed to remove moisture content from fresh apple pomace.Total dietary fiber yield(74%)and array of functional properties such as density,water and oil holding capacity,swelling capacity and glucose dialysis retardation index(36.91%)was found better in freeze dried fraction.The higher total phenolics(5.78±0.08 mg GAE/g dry weight)content was also recorded in freeze dried fraction followed by oven and sun drying.The 50%aqueous acetone was found as more efficient solvent for extraction of phenolic constituents.RP-HPLC analysis has revealed presence of quercetin,phloridzin and phloretin as major phenolics.Thus,it is evident from the results that pomace generated at industrial scale can be utilized as a source of dietary food ingredient.Shalika Ranaa Sakshi Gupta Ajay Rana Shashi Bhushan 2015Food Science and Human Wellness2015,4,4:8

4Simultaneous quantification of prodrug oseltamivir and its metabolite oseltamivir carboxylate in human plasma by LC-MS/MS to support a bioequivalence study显示文摘A simple,precise and rapid liquid chromatography-tandem mass spectrometry(LC-MS/MS) method has been developed and validated for the simultaneous determination of oseltamivir and oseltamivir carboxylate,a neuraminidase inhibitor,using their deuterated analogs as internal standards(ISs).The method involved solid phase extraction of the analytes and ISs from 200 μL human plasma with no reconstitution and drying steps.The chromatographic separation was achieved on a Symmetry C18(100 mm 4.6 mm,5 μm) column using 10 mM ammonium formate and acetonitrile(30:70,v/v) as the mobile phase in a run time of 2.0 min.Quantitation of analytes and ISs were done by multiple reaction monitoring on a triple quadrupole mass spectrometer in the positive ionization mode.The linearity of the method was established in the concentration range of 0.5-200 ng/mL and 2.0-800 ng/mL for oseltamivir and oseltamivir carboxylate respectively.The mean extraction recovery for oseltamivir(94.4%) and oseltamivir carboxylate(92.7%) from spiked plasma samples was consistent and reproducible.The application of this method was demonstratedby a bioequivalence study in 42 healthy Indian subjects with 75 mg oseltamivir phosphate capsules.The assay reproducibility was established by reanalysis of 151 incurred subject samples.Ajay Gupta Swati Guttikar Pranav S. 5Discovery and preclinical development of ⅢM-160, a Bergenia ciliatabased anti-inflammatory and anti-arthritic botanical drug candidate显示文摘Objective: Bergenia ciliata(Haw.) Sternb. is used in the Indian traditional system of medicine to treat various ailments including rheumatism and to heal wounds. The objective of the present study was to perform a preclinical characterization of the B. ciliata-based botanical extract IIIM-160.Methods: ⅢM-160 was chemically standardized and analyzed for heavy metal content, aflatoxins,pesticides and microbial load. The in vitro and in vivo efficacies were determined in suitable models of inflammation, arthritis and nociception. An acute oral toxicity study was performed in Swiss albino mice.A suitable oral formulation was developed and characterized.Results: Bergenin was found to be the major component(9.1% w/w) of ⅢM-160. The botanical lead displayed inhibition of lipopolysaccharide-induced production of proinflammatory cytokines in THP-1 cells, with selectivity toward interleukin-6(IL-6) and had an excellent safety-window. It showed anti-inflammatory, anti-arthritic and antinociceptive activity in animal models and was not toxic at oral doses up to 2 g/kg in Swiss-albino mice. The gastroretentive, sustained-release capsule formulation showed sustained-release of the bergenin over the period of 24 h, resulting in improved plasma-exposure of bergenin in Sprague–Dawley rats.Conclusion: The dual-activity of IL-6 inhibition and antinociception marks the suitability of ⅢM-160 for treating rheumatoid arthritis. 8Self-referenced measurements of the dielectric properties of metals using terahertz time-domain spectroscopy via the excitation of surface plasmon-polaritons显示文摘We present experimental measurements that show direct determination of the dielectric properties of various metals relevant to plasmonics.In contrast to traditional measurements that typically rely on transmission and reflectance measurements,we launch surface plasmon-polaritons on a variety of different substrates and measure the propagation properties using terahertz time-domain spectroscopy.Surprisingly,we find that the extracted values for the dielectric constant for these metals differ by orders of magnitude from published data.In order to validate the obtained results,we separately measure the 1∕e decay length,both along the propagation direction and normal to the metal surface,and show that the results are fully self-consistent with experimental data.The generality of the measurement technique makes it a useful tool to estimate the properties not only of planar conducting substrates but also a wide variety of more complex plasmonic structures.Shashank Pandey Shuchang Liu Barun Gupta Ajay Nahata 2013Photonics Research2013,1,4:2
9Gallbladder perforation:A single-center experience in north India and a step-up approach for management显示文摘Background: Spontaneous gallbladder perforation(GBP) is an uncommon diagnosis. This study presented the experience of managing spontaneous GBP over nine years at a large, tertiary care university hospital in north India and investigated the outcomes and treatment strategies. Methods: A retrospective review of prospectively maintained digital database of consecutive patients was performed. All patients received medical and/or surgical treatment for spontaneous GBP in our department between January 2010 and June 2018. Results: We identifed 151 patients(81 females and 70 males) with mean age of 53 years. Most common presenting features were pain(96.7%), fever(54.3%) and jaundice(31.1%). Most common cause was gallbladder stones(84.8%) followed by common bile duct stones(30.5%), xanthogranulomatous cholecystitis(17.9%) and malignancy(11.9%). As per Niemeier classifcation, 8.6% had type 1 GBP(free perforation in peritoneal cavity), 76.2% had type 2 GBP(localized perforation) and 13.2% had type 3 GBP(cholecystoenteric fstula). About 60% of the perforations were diagnosed preoperatively. Type 1 was more common in patients with diabetes and also had the worst prognosis. Surgery was performed in 109 patients(72.2%). Seven patients(4.6%) had a postoperative morbidity of Clavien-Dindo Ⅲ or higher. There were three mortalities in patients who underwent surgery. Conclusions: High index of suspicion is required for preoperative diagnosis of GBP, especially in types 2 and 3. Laparoscopic cholecystectomy can be difficult in these patients and patients may require open or partial cholecystectomy.
