1附睾炎:基于临床和基础研究的总结启示显示文摘急性附睾炎在泌尿外科门诊的病人中有共同的临床表现。大多数情况下,性传播疾病或者泌尿系的感染都会引起病原菌的感染,并通过泌尿生殖道逆行引发急性附睾炎。虽然保守治疗可能对大多数患者有效并能根除病原菌,但有研究表明这类患者中少精子症甚至无精子症患者升高至40%。为了探讨引起这种现象和随后对精子功能损伤的潜在原因,我们建立了附睾炎的动物模型。研究数据表明附睾管里聚集着异常表达的免疫细胞、免疫调节基因以及病原菌敏感分子。我们推测在附睾头部存在一个耐药微环境,而炎症反应常发生在附睾尾部。这与附睾在不需要降低自身抵抗上行感染能力的前提下,当精子从睾丸中射出后,保护其不受新产生的抗精子抗体损害的观点相一致。然而,严重的炎症反应,特别是在附睾尾部,也可能迁延引发附睾结构和功能的损害。通过对动物模型临床观察研究,我们更好地理解附睾的免疫微环境,附睾炎的易感因素以及合理的治疗,在此基础上减轻附睾炎对附睾的损害,降低这类人群不育的发生率。Vera Michel Adrian Pilatz Mark P Hedger Andreas Meinhardt 2015Asian Journal of Andrology2015,17,5:16

3Refining pathological evaluation of neoadjuvant therapy for adenocarcinoma of the esophagus显示文摘AIM:To assess tumour regression grade(TRG)and lymph node downstaging to help define patients who benefit from neoadjuvant chemotherapy.METHODS:Two hundred and eighteen consecutive patients with adenocarcinoma of the esophagus or gastro-esophageal junction treated with surgery alone or neoadjuvant chemotherapy and surgery between 2005and 2011 at a single institution were reviewed.Triplet neoadjuvant chemotherapy consisting of platinum,fluoropyrimidine and anthracycline was considered for operable patients(World Health Organization performance status≤2)with clinical stage T2-4 N0-1.Response to neoadjuvant chemotherapy(NAC)was assessed using TRG,as described by Mandard et al.In addition lymph node downstaging was also assessed.Lymph node downstaging was defined by cN1 at diagnosis:assessed radiologically(computed tomography,positron emission tomography,endoscopic ultrasonography),then pathologically recorded as N0 after surgery;ypN0 if NAC given prior to surgery,or pN0if surgery alone.Patients were followed up for 5 years post surgery.Recurrence was defined radiologically,with or without pathological confirmation.An association was examined between t TRG and lymph node downstaging with disease free survival(DFS)and a comprehensive range of clinicopathological characteristics.RESULTS:Two hundred and eighteen patients underwent esophageal resection during the study interval with a mean follow up of 3 years(median follow up:2.552,95%CI:2.022-3.081).There was a 1.8%(n=4)inpatient mortality rate.One hundred and thirty-six(62.4%)patients received NAC,with 74.3%(n=101)of patients demonstrating some signs of pathological tumour regression(TRG 1-4)and 5.9%(n=8)having a complete pathological response.Forty four point one percent(n=60)had downstaging of their nodal disease(cN1 to ypN0),compared to only 15.9%(n=13)that underwent surgery alone(pre-operatively overstaged:cN1 to pN0),(P<0.0001).Response to NAC was associated with significantly increased DFS(mean DFS;TRG 1-2:5.1years,95%CI:4.6-5.6 vs TRG 3-5:2.8 years,95%CI:2.2-3.3,P<0.0001).Nodal down-staging conferred a significant DFS advantage for those patients with a poor primary tumour response to NAC(median DFS;TRG 3-5 and nodal down-staging:5.533 years,95%CI:3.558-7.531 vs TRG 3-5 and no nodal down-staging:1.114 years,95%CI:0.961-1.267,P<0.0001).CONCLUSION:Response to NAC in the primary tumour and in the lymph nodes are both independently associated with improved DFS.Fergus Noble Luke Nolan Adrian C Bateman James P Byrne Jamie J Kelly Ian S Bailey Donna M Sharland Charlotte N Rees Timothy J Iveson Tim J Underwood Andrew R Bateman 2013World Journal of Gastroenterology2013,19,48:4
4Impact of Helicobacter pylori on the healing process of the gastric barrier显示文摘AIM To determine the impact of selected well defined Helicobacter pylori(H. pylori) antigens on gastric barrier cell turnover.METHODS In this study,using two cellular models of gastric epithelial cells and fibroblasts,we have focused on exploring the effects of well defined H. pylori soluble components such as glycine acid extract antigenic complex(GE),subunit A of urease(Ure A),cytotoxin associated gene A protein(Cag A) and lipopolysaccharide(LPS) on cell turnover by comparing the wound healing capacity of the cells in terms of their proliferative and metabolic activity as well as cell cycle distribution. Toxic effects of H. pylori components have been assessed in an association with damage to cell nuclei and inhibition of signal transducer and activator of transcription 3(STAT3) phosphorylation. RESULTS We showed that H. pylori GE,Cag A and Ure A promoted regeneration of epithelial cells and fibroblasts,which is necessary for effective tissue healing. However,in vivo increased proliferative activity of these cells may constitute an increased risk of gastric neoplasia. In contrast,H. pylori LPS showed a dose-dependent influence on the process of wound healing. At a low concentration(1 ng/m L) H. pylori LPS accelerated of healing epithelial cells,which was linked to significantly enhanced cell proliferation and MTT reduction as well as lack of alterations in cell cycle and downregulation of epidermal growth factor(EGF) production as well as cell nuclei destruction. By comparison,H. pylori LPS at a high concentration(25 ng/m L) inhibited the process of wound repair,which was related to diminished proliferative activity of the cells,cell cycle arrest,destruction of cell nuclei and downregulation of the EGF/STAT3 signalling pathway.CONCLUSION In vivo H. pylori LPS driven effects might lead to the maintenance of chronic inflammatory response and pathological disorders on the level of the gastric mucosal barrier.Eliza Mnich Magdalena Kowalewicz-Kulbat Paulina Sicińska Krzysztof Hinc Michal Obuchowski Adrian Gajewski Anthony P Moran Magdalena Chmiela 2016World Journal of Gastroenterology2016,22,33:4
