| 1 | Impact of preoperative chemoradiotherapy on survival in patients with resectable pancreatic cancer显示文摘AIM:To explore whether preoperative chemoradiation therapy improves survival of patients with pancreatic cancer undergoing resectional surgery. METHODS:Forty-seven patients with a malignant pancreatic tumor localized in the head or uncinate process of the pancreas underwent radical pancreaticoduodenectomy. Twenty-two received chemoradiation therapy (gemcitabine and radiation dose 50.4 Gy) before surgery (CRR) and 25 patients underwent surgery only (RO). The study was non-randomised. Patients were identified from a prospective database. RESULTS:The median survival time was 30.2 mo in the CRR group and 35.9 mo in the RO group. No statistically significant differences were found in subclasses according to lymph node involvement,TNM stages,tumor size,or perineural invasion. The one,three and five year survival rates were 81%,33% and 33%,respectively,in the CRR group and 72%,47% and 23%,respectively,in the RO group. In ductal adenocarcinoma,the median survival time was 27 mo in the CRR group and 20 mo in the RO group. No statistically significant differences were found in the above subclasses. The one,three and five year survival rates were 79%,21% and 21%,respectively,in the CRR group and 64%,50% and 14%,respectively,in the RO group. The overall hospital mortality rate was 2%. The morbidity rate was 45% in the CRR group and 32% (NS) in the RO group. | 2 | Aerobic bacteria associated with chronic suppurative otitis media in Angola显示文摘Background:Chronic suppurative otitis media(CSOM)is an important cause of hearing loss in children and constitutes a serious health problem globally with a strong association to resource-limited living conditions.Topical antibiotics combined with aural toilet is the first-hand treatment for CSOM but antimicrobial resistance and limited availability to antibiotics are obstacles in some areas.The goal of this study was to define aerobic pathogens associated with CSOM in Angola with the overall aim to provide a background for local treatment recommendations.Methods:Samples from ear discharge and the nasopharynx were collected and cultured from 152 patients with ear discharge and perforation of the tympanic membrane.Identification of bacterial species was performed with matrix-assisted laser desorption/ionization-time of flight mass spectrometry and pneumococci were serotyped using multiplex polymerase chain reactions.Antimicrobial susceptibility testing was done according to EUCAST.Results:One hundred eighty-four samples from ear discharge and 151 nasopharyngeal swabs were collected and yielded 534 and 289 individual isolates,respectively.In all patients,correspondence rate of isolates from 2 ears in patients with bilateral disease was 27.3%and 9.3%comparing isolates from the nasopharynx and ear discharge,respectively.Proteus spp.(14.7%),Pseudomonas aeruginosa(13.2%)and Enterococcus spp.(8.8%)were dominating pathogens isolated from ear discharge.A large part of the remaining species belonged to Enterobacteriaceae(23.5%).Pneumococci and Staphylococcus aureus were detected in approximately 10%of nasopharyngeal samples.Resistance rates to quinolones exceeded 10%among Enterobacteriaceae and was 30.8%in S.aureus,whereas 6.3%of P.aeruginosa were resistant.Conclusions:The infection of the middle ear in CSOM is highly polymicrobial,and isolates found in nasopharynx do not correspond well with those found in ear discharge.Pathogens associated with CSOM in Angola are dominated by gram-negatives including Enterobacteriaceae and P.aeruginosa,while gram-positive enterococci also are common.Based on the results of antimicrobial susceptibility testing topical quinolones would be the preferred antibiotic therapy of CSOM in Angola.Topical antiseptics such as aluminium acetate,acetic acid or boric acid,however,may be more feasible options due to a possibly emerging antimicrobial resistance. | Fabian Udden Matuba Filipe Ake Reimer Maria Paul Erika Matuschek John Thegerstrom Sven Hammerschmidt Tuula Pelkonen Kristian Riesbeck | 2018 | Infectious Diseases of Poverty2018,7,1: | 6 |
| 3 | Mission impossible completed:unlocking the nomenclature of the largest and most complicated subgenus of Cortinarius,Telamonia显示文摘So far approximately 144,000 species of fungi have been named but sequences of the majority of them do not exist in the public databases.Therefore,the quality and coverage of public barcode databases is a bottleneck that hinders the study of fungi.Cortinarius is the largest genus of Agaricales with thousands of species world-wide.The most diverse subgenus in Cortinarius is Telamonia and its species have been considered one of the most taxonomically challenging in the Agaricales.Its high diversity combined with convergent,similar appearing taxa have earned it a reputation of being an impossible group to study.In this study a total of 746 specimens,including 482 type specimens representing 184 species were sequenced.Also,a significant number of old types were successfully sequenced,105 type specimens were over 50 years old and 18 type specimens over 100 years old.Altogether,20 epi-or neotypes are proposed for recently commonly used older names.Our study doubles the number of reliable DNA-barcodes of species of C.subgenus Telamonia in the public sequence data-bases.This is also the first extensive phylogenetic study of the subgenus.A majority of the sections and species are shown in a phylogenetic context for the first time.Our study shows that nomenclatural problems,even in difficult groups like C.subgenus Telamonia,can be solved and consequently identification of species based on ITS barcodes becomes an easy task even for non-experts of the genus. | Kare Liimatainen Tuula Niskanen Bálint Dima Joseph F.Ammirati Paul M.Kirk Ilkka Kytövuori | 2020 | Fungal Diversity2020,,5: | 4 |
| 4 | Taming the beast:a revised classification of Cortinariaceae based on genomic data显示文摘Family Cortinariaceae currently includes only one genus,Cortinarius,which is the largest Agaricales genus,with thousands of species worldwide.The species are important ectomycorrhizal fungi and form associations with many vascular plant gen-era from tropicals to arctic regions.Genus Cortinarius contains a lot of morphological variation,and its complexity has led many taxonomists to specialize in particular on infrageneric groups.The previous attempts to divide Cortinarius have been shown to be unnatural and the phylogenetic studies done to date have not been able to resolve the higher-level classification of the group above section level.Genomic approaches have revolutionized our view on fungal relationships and provide a way to tackle difficult groups.We used both targeted capture sequencing and shallow whole genome sequencing to produce data and to perform phylogenomic analyses of 75 single-copy genes from 19 species.In addition,a wider 5-locus analysis of 245 species,from the Northern and Southern Hemispheres,was also done.Based on our results,a classification of the family Cortinariaceae into ten genera-Cortinarius,Phlegmacium,Thaxterogaster,Calonarius,Aureonarius,Cystinarius,Volvanarius,Hygronarius,Mystinarius,and Austrocortinarius-is proposed.Seven genera,10 subgenera,and four sec-tions are described as new to science and five subgenera are introduced as new combinations in a new rank.In addition,41 section names and 514 species names are combined in new genera and four lecto-and epitypes designated.The position of Stephanopus in suborder Agaricineae remains to be studied.Targeted capture sequencing is used for the first time in fungal taxonomy in Basidiomycetes.It provides a cost-efficient way to produce-omics data in species-rich groups.The-omics data was produced from fungarium specimens up to 21 years old,demonstrating the value of museum specimens in the study of the fungal tree of life.This study is the first family revision in Agaricales based on genomics data and hopefully many others will soon follow. | Kare Liimatainen Jan T.Kim Lisa Pokorny Paul M.Kirk Bryn Dentinger Tuula Niskanen | 2022 | Fungal Diversity2022,,1: | 4 |

combinations are proposed where necessary to improve our traditional species concepts and provide updates on their classification.In addition to extensive morphological descriptions and appropriate asexual and sexual connections,DNA sequence data are also analysed from concatenated datasets(rDNA,TEF-a,RBP2 and b-Tubulin)to infer phylogenetic relationships and substantiate systematic position of taxa within appropriate ranks.Wherever new species or combinations are being proposed,we apply an integrative approach(morphological and molecular data as well as ecological features wherever applicable).Notes on 125 fungal taxa are compiled in this paper,including eight new genera,101 new species,two new combinations,one neotype,four reference specimens,new host or distribution records | 13 | Trade-offs between wood production and forest grouse habitats in two regions with distinctive landscapes显示文摘Background:Forest management affects the viability of forest grouse populations,causing alterations to habitat quality.At the regional level,common targets for wood harvesting and safeguarding of specific habitats are negotiated between various stakeholders.Analysing potential trade-offs between forest grouse habitats and wood production in the region could support resource-smart decision making.Methods:In this paper,we compiled trade-off curves represented as production possibility frontiers demonstrating the relationship between forest grouse habitat area and wood removal,using a Finnish forestry dynamics model and the Finnish Multi-Source National Forest Inventory.For the modelling of forest grouse habitats,a landscape-level occurrence model based on nationwide wildlife triangle census data was used.Five alternative forest scenarios in terms of wood removal were compiled for two study areas in Finland representing two different landscape structures.Results and conclusions:Results showed that impacts on forest grouse habitats were case-specific.In the southern study area,increasing the roundwood harvesting rate affected grouse habitats more strongly as forests were already fragmented for other land uses and became more spruce-dominated over time.If the maximum sustainable removal rate was implemented,predicted grouse habitat area was 55%less than in a no-removal scenario.In the eastern study area,a more heavily forested region,the decrease was far lower at 22%.Scenario results were also compared to levels of recorded(business as usual)wood removal and that envisaged by valid regional forestry programmes,and their sustainability in terms of grouse habitat area was discussed.The production possibility frontiers calculated in this study support the evaluation of the loss of suitable habitat caused by different wood harvesting rates,or vice versa,the economic cost of increasing habitat areas. | Helena Haakana Esa Huhta Hannu Hirvelä Tuula Packalen | 2020 | Forest Ecosystems2020,7,2: | 1 |
