| 1 | 新农合对灾难性卫生支出影响及公平性显示文摘目的了解新医改后新农合对因住院导致的灾难性卫生支出的影响及公平性。方法采用分层抽样方法抽取山东省济南市的章丘市、长清区、平阴县共2 029户家庭进行入户面访问卷调查,以其中263户参保住院家庭为研究对象,了解2011年新农合补偿前后灾难性卫生支出发生的频率和强度,并采用集中指数及其分解法分析其公平性及导致不公平的原因。结果 40%阈值下,2011年新农合补偿前后灾难性卫生支出发生率分别为44.11%和23.19%,下降率为47.43%,平均差距分别为10.50%和4.07%,下降率为61.24%;补偿后发生率集中指数为0.064,平均差距集中指数为-0.006。住院费用实际补偿比>0.4、住院≥15 d和住院机构县级以上的集中指数分别为-0.035、0.235、0.239。结论新农合对降低灾难性卫生支出的作用虽有提高,但仍有限;高收入人群发生频率较高,但是低收入人群所受影响相对较大。 | 郭娜 朱大伟 Tor Iversen 王健 | 2013 | 中国公共卫生2013,29,11: | 10 |
| 2 | Modeling Pain Using fMRI:From Regions to Biomarkers显示文摘Pain is a subjective and complex phenomenon.Its complexity is related to its heterogeneity: multiple component processes, including sensation, affect, and cognition, contribute to pain experience and reporting.These components are likely to be encoded in distributed brain networks that interact to create pain experience and pain-related decision-making. Therefore, to understand pain, we must identify these networks and build models of these interactions that yield testable predictions about pain-related outcomes. We have developed several such models or `signatures' of pain, by(1) integrating activity across multiple systems, and(2) using pattern-recognition to identify processes related to pain experience. One model, the Neurologic Pain Signature, is sensitive and specific to pain in individuals, involves brain regions that receive nociceptive afferents, and shows little effect of expectation or self-regulation in tests to date. Another, the'Stimulus Intensity-Independent Pain Signature',explains substantial additional variation in trial-to-trial pain reports.It involves many brain regions that do not show increased activity in proportion to noxious stimulus intensity, including medial and lateral prefrontal cortex, nucleus accumbens, and hippocampus. Responses in this system mediate expectancy and perceived control effects in several studies.Overall, this approach provides a pathway to understanding pain by identifying multiple systems that track different aspects of pain. AUD is associated with greater exposure to HCV infection, increased HCV infection persistence, and more extensive liver damage due to interactions between AUD and HCV on immune responses, cytotoxicity, and oxidative stress. Although AUD and HCV infection are associated with increased morbidity and mortality, HCV antiviral therapy is less commonly prescribed in individuals with both conditions. AUD is also common in human immunodeficiency virus(HIV) infection, which negatively impacts proper HIV care and adherence to antiretroviral therapy, and liver disease. In addition, AUD and HCV infection are also frequent within a proportion of patients with HIV infection, which negatively impacts liver disease. | 10 | Linoleic acid metabolic pathway allows for an efficient increase of intramuscular fat content in pigs显示文摘Background: Intramuscular fat(IMF) content is a relevant trait for high-quality meat products such as dry-cured ham,but increasing IMF has the undesirable correlated effect of decreasing lean growth.Thus,there is a need to find selection criteria for IMF independent from lean growth.In pigs,the proportion of linoleic(C18:2) and arachidonic(C20:4) acids decline with fat deposition and therefore they can be considered as indicators of fatness.The aim of this research was to estimate the genetic variation for C18:2 and C20:4 in IMF and their genetic correlations with IMF and lean growth traits,with the objective to assess their potential as specific biomarkers of IMF.The analysis was conducted using a full-pedigreed Duroc resource line with 91,448 records of body weight and backfat thickness(BT) at 180 days of age and 1371 records of fatty acid composition in the muscle gluteus medius.Results: The heritability estimates for C18:2 and C20:4 in IMF,whether expressed in absolute(mg/g of muscle) or in relative(mg/g of fatty acid) terms,as well as for their ratio(C20:4/C18:2),were high(> 0.40),revealing that the C18:2 to C20:4 pathway is subjected to substantial genetic influence.Litter effects were not negligible,with values ranging from8% to 15% of the phenotypic variance.The genetic correlations of C18:2 and C20:4 with IMF and BT were negative(-0.75 to-0.66,for IMF,and-0.64 to-0.36,for BT),if expressed in relative values,but almost null(-0.04 to 0.07),if expressed in absolute values,except for C18:2 with IMF,which was highly positive(0.88).The ratio of C20:4 to C18:2 also displayed a stronger genetic correlation with IMF(-0.59) than with BT(-0.10).Conclusions: The amount of C18:2 in muscle can be used as an IMF-specific biomarker.Selection for the absolute amount of C18:2 is expected to deliver a similar response outcome as selection for IMF at restrained BT.Further genetic analysis of the C18:2 metabolic pathway may provide new insights into differential fat deposition among adipose tissues and on candidate genes for molecular markers targeting specifically for one of them. | Sofia Gol Rayner Gonzalez-Prendes Lluis Bosch Marc Tor Josep Reixach Ramona N.Pena Joan Estany | 2019 | Journal of Animal Science and Biotechnology2019,10,3: | 3 |

In the present century, the availability of digital elevation models(DEMs) incorporated into geographic information systems(GIS) has allowed the definition of mountain areas based on topographic and other criteria. This paper presents the various delineations of mountains that have been prepared at three scales – global, regional(Europe), and national – and explores the reasons and processes leading to these delineations, and how they have been used. A detailed case study is then presented for Norway. Overall, two types of approaches to mapping mountains have been taken: first, considering mountains per se, based on elevation and/or topography; second, considering them among other categories, e.g., landforms or biogeographical, environmental or landscape zones. | 18 | A Fault Tolerant Control Scheme Using the Feasible Constrained Control Allocation Strategy显示文摘This paper investigates the necessity of feasibility considerations in a fault tolerant control system using the constrained control allocation methodology where both static and dynamic actuator constraints are considered. In the proposed feasible control allocation scheme, the constrained model predictive control(MPC) is employed as the main controller. This considers the admissible region of the control allocation problem as its constraints. Using the feasibility notion in the control allocation problem provides the main controller with information regarding the actuator′s status, which leads to closed loop system performance improvement. Several simulation examples under normal and faulty conditions are employed to illustrate the effectiveness of the proposed methodology. The main results clearly indicate that closed loop performance and stability characteristics can be significantly degraded by neglecting the actuator constraints in the main controller.
