| 3 | WONCA研究论文摘要汇编--临床指南在全科医学中的应用:潜在困境的定性研究显示文摘背景 针对单一疾病的临床指南在共病患者的全科医疗实践中常出现问题。然而,很少有研究关注遵循多个临床指南对全科医师的影响。本文探讨了挪威全科医生对全科医学实践中应用多个指南后果的体会和反思。方法采用立意抽样选取25位全科医生进行定性焦点小组研究。录制访谈音频,通过系统文本凝结(如现象学的方法)进行文字整理和分析。结果 全科医生的反馈集中在以下两大主题:(1)全科医生应用多个指南的困境;(2)全科医生应用多个指南为患者带来困境。对于全科医生而言,全科医生被迫遵循多数情况下并不适用其患者的多个指南,由此带来一些列问题。全科医生也有对自己的医疗行为产生不安全感的经历,他们承认其行为导致了'防御式医疗'增加的趋势。对患者而言,全科医生体会到应用多个指南导致多重用药、过多的非药物治疗建议,以及医疗化的倾向,甚至导致某些患者生活质量下降。结论 全科医生应用多个针对单一疾病的临床指南处理共病患者造成了消极后果,如增加多重用药和过度治疗的风险。以患者为中心的护理,并鼓励全科医生必要时不遵守指南有助于降低上述风险。医疗卫生部门和指南的制定者需要意识到在患病率较高的共病患者中应用针对单一疾病指南的潜在负面影响。 | AUSTAD B HETLEVIK I MJ?LSTAD B P 本刊编辑部 | 2016 | 中国全科医学2016，19，26： | 2 |
