| 1 | A combination treatment of entecavir and early-phase corticosteroid in severe exacerbation of chronic hepatitis B显示文摘Of patients with severe exacerbation of chronic hepatitis B accompanied by jaundice and coagulopathy,20%-30%have a fatal outcome.In this report,we describe 2 cases of severe exacerbation of chronic hepatitis B with jaundice and coagulopathy who were successfully treated with a combination of entecavir and corticosteroid.In both cases,rapid reductions in serum hepatitis B virus(HBV)-DNA levels were observed,and corticosteroid was stopped after serum HBV-DNA levels became undetectable.Entecavir treatment was continued.Generally,entecavir treatment reduced serum HBV-DNA levels rapidly,although the improvement in liver function was delayed by a few weeks.During this time lag,liver cell injury continued and the disease progressed.Corticosteroid suppressed the excessive host immune response and was useful for stopping progressive deterioration.A combination of entecavir and early-phase corticosteroid may be a useful treatment in severe exacerbation of chronic hepatitis B. | Kazuyuki Matsumoto Yasuhiro Miyake Hirokazu Miyatake Masahiro Takahara Takayuki Imada Satoru Yagi Tatsuya Toyokawa Morihito Nakatsu Masaharu Ando Mamoru Hirohata | 2009 | World Journal of Gastroenterology2009，15，13： | 16 |
| 2 | Chromoendoscopy of gastric adenoma using an acetic acid indigocarmine mixture显示文摘AIM:To investigate the usefulness of chromoendoscopy,using an acetic acid indigocarmine mixture(AIM),for gastric adenoma diagnosed by forceps biopsy.METHODS:A total of 54 lesions in 45 patients diagnosed as gastric adenoma by forceps biopsy were prospectively enrolled in this study and treated by endoscopic submucosal dissection(ESD)between January 2011 and January 2012.AIM-chromoendoscopy(AIMCE)was performed followed by ESD.AIM solution was sprinkled and images were recorded every 30 s for 3min.Clinical characteristics such as tumor size(<2cm,≥2 cm),surface color in white light endoscopy(WLE)(whitish,normochromic or reddish),macroscopic appearance(flat or elevated,depressed),and reddish change in AIM-CE were selected as valuables.RESULTS:En bloc resection was achieved in all 54 cases,with curative resection of fifty two lesions(96.3%).Twenty three lesions(42.6%)were diagnosed as welldifferentiated adenocarcinoma and the remaining 31lesions(57.4%)were gastric adenoma.All adenocarcinoma lesions were well-differentiated tubular adenocarcinomas and were restricted within the mucosal layer.The sensitivity of reddish color change in AIM-CE is significantly higher than that in WLE(vs tumor size≥2 cm,P=0.016,vs normochromic or reddish surface color,P=0.046,vs depressed macroscopic type,P=0.0030).On the other hand,no significant differences were found in the specificity and accuracy.In univariate analysis,normochromic or reddish surface color in WLE(OR=3.7,95%CI:1.2-12,P=0.022)and reddish change in AIM-CE(OR=14,95%CI:3.8-70,P<0.001)were significantly related to diagnosis of early gastric cancer(EGC).In multivariate analysis,only reddish change in AIM-CE(OR=11,95%CI:2.3-66,P=0.0022)was a significant factor associated with diagnosis of EGC.CONCLUSION:AIM-CE may have potential for screening EGC in patients initially diagnosed as gastric adenoma by forceps biopsy. | Yoshiyasu Kono Ryuta Takenaka Yoshiro Kawahara Hiroyuki Okada Keisuke Hori Seiji Kawano Yasushi Yamasaki Koji Takemoto Takayoshi Miyake Shigeatsu Fujiki Kazuhide Yamamoto | 2014 | World Journal of Gastroenterology2014，20，17： | 12 |
| 3 | A case of hepatic angiomyolipoma difficult to distinguish from hepatocellular carcinoma显示文摘We report a case of hepatic angiomyolipoma with uncommon clinical features. A 56-year-old man presented with a hepatic tumor in the caudate lobe. The tumor was hypoechoic on ultrasonography, showed early-phase hyperattenuation on enhanced computed tomography and did not absorb iron on superparamagnetic iron oxide-enhanced magnetic resonance imaging. Hepatocellular carcinoma was highly suspected, and the patient underwent hepatic resection. Histologically, the tumor was mainly composed of smooth muscle cells and contained small amounts of adipose cells and blood vessels. On immunohistochemical staining, the smooth muscle cells were positive for a melanocytic cell-specific monoclonal antibody. | 4 | A rare case of primary choriocarcinoma in the sigmoid colon显示文摘Primary colorectal choriocarcinoma is an extremely rare neoplasm and is usually associated with a poor prognosis.Only 13 cases of colorectal choriocarcinoma have previously been reported.There is no standard chemotherapeutic regimen for this tumor type.A 68-year-old man presented with melena and was diagnosed with sigmoid colonic adenocarcinoma with multiple liver metastases.He underwent a laparoscopic sigmoidectomy.Pathology revealed choriocarcinoma with a focal component of moderately differentiated adenocarcinoma of colon origin.Based on the collagen gel droplet-embedded culture drug sensitivity test(CD-DST)results,mFOLFOX6 and bevacizumab were administered,which suppressed aggressive tumor growth for 4 mo.The patient died 9 mo after the initial diagnosis.Our study results suggest that the standard chemotherapy regimen for colorectal cancer might have suppressive effects against primary colorectal choriocarcinoma.Moreover,CD-DST may provide,at least in part,therapeutic insight for the selection of appropriate antitumor agents for such patients. | Hiromitsu Maehira Tomoharu Shimizu Hiromichi Sonoda Eiji Mekata Tomoharo Yamaguchi Tohru Miyake Mitsuaki Ishida Tohru Tani | 2013 | World Journal of Gastroenterology2013，19，39： | 6 |

| 7 | Pedunculated Brunner's gland hamartoma of the duodenum causing upper gastrointestinal hemorrhage显示文摘A case of pedunculated Brunner's gland hamartoma(BGH) of the duodenum causing upper gastrointestinal(GI) hemorrhage is reported.The patient was a 47-year-old man who visited our hospital for further evaluation of tarry stools and shortness of breath.Endoscopic examination of the upper digestive tract revealed a large peduncular polyp with bleeding,about 30 mm in diameter,arising from the wall of the second portion of the duodenum.GI bleeding occurred from the base of the stalk of the polyp.Endoscopic polypectomy was performed.Histological examination of the specimen revealed that the main body of the polyp contained several lobules of mature Brunner's gland with areas of cystic dilatation.The surface epithelium consisted of normal duodenal mucosa with areas of focal ulceration.This polyp was diagnosed as a BGH.The symptom of tarry stools resolved after endoscopic resection.Our case shows that treatment is necessary for duodenal BGH if GI bleeding occurs. | Shoji Hirasaki Motoharu Kubo Atsushi Inoue Yasuyuki Miyake Hisako Oshiro | 2009 | World Journal of Gastroenterology2009，15，3： | 3 |
| 8 | Jejunal small ectopic pancreas developing into jejunojejunal intussusception:A rare cause of ileus显示文摘Intussusception is rare in adults. We describe a 62-year-old man with jejunal ectopic pancreas that led to jejunojejunal intussusception and ileus. The patient was admitted to our hospital because of intermittent abdominal pain. Plain abdominal radiography showed some intestinal gas and fluid levels. Abdominal CT scan demonstrated a target sign suggesting bowel intussusception. Jejunography using a naso-jejunal tube showed an oval-shaped mass about 15 mm in diameter with a smooth surface in the jejunum, which suggested a submucosal tumor (SMT), and edematous mucosa around the mass. Partial jejunal resection was carried out and the resected oval-shaped tumor, 14 mm×11 mm in size, was found to be covered with normal jejunal mucosa. | 9 | Azathioprine is essential following cyclosporine for patients with steroid-refractory ulcerative colitis显示文摘AIM: To evaluate long-term prognosis following cyclosporine treatment by examining the rate of surgery avoidance among cyclosporine responders.METHODS: We retrospectively reviewed clinical records for 29 patients diagnosed with severe steroid-refractory ulcerative colitis in our hospital from August 1997 to August 2008 and treated with cyclosporine by continuous intravenous infusion.All patients were treated with intravenous corticosteroids for more than 5 d prior to cyclosporine therapy.Administration was continued for up to 21 d under serum monitoring to maintain cyclosporine levels between 400 and 600 ng/mL.Clinical activity was assessed before and after cyclosporine therapy using the clinical activity index score,with a reduction of ≥ 5 considered to indicate a response.Among responders,we defined cases not requiring surgery for more than 5 years as exhibiting long-term efficacy of cyclosporine.Factors considered to be possibly predictive of long-term efficacy of cyclosporine were sex,age,disease duration,clinical activity index score,C-reactive protein level,hemoglobin level,disease extent,endoscopic findings,and clinical course.RESULTS: Cyclosporine was not discontinued due to side effects in any patient.Nineteen(65.5%) of 29 patients were considered responders.A statistically significant(P = 0.004) inverseas sociation wa s observed between an endoscopic finding of 'mucosal bleeding' and responsive cases.Fifteen(9 males,6 females) of these 19 patients were followed for 5 years or more,of whom 9(60%) exhibited long-termefficacy of cyclosporine.Of the 10 non-responders,9(90%) underwent surgery within 6 mo of cyclosporine therapy.None of the following factors had a significant impact on the long-term efficacy of cyclosporine: sex,age,duration of disease,clinical activity index score,C-reactive protein level,hemoglobin level,extent of disease,endoscopic findings,or clinical course.In contrast,a significant association was observed for maintenance therapy with azathioprine after cyclosporine therapy(P = 0.0014).CONCLUSION: Maintenance therapy with azathioprine might improve the long-term efficacy of continuously infused cyclosporine for severe steroid-refractory ulcerative colitis patients. | Nobuyuki Miyake Takafumi Ando Kazuhiro Ishiguro Osamu Maeda Osamu Watanabe Yutaka Hirayama Keiko Maeda Kazuhiro Morise Masanobu Matsushita Kazuhiro Furukawa Kohei Funasaka Masanao Nakamura Ryoji Miyahara Naoki Ohmiya Hidemi Goto | 2015 | World Journal of Gastroenterology2015，21，1： | 3 |

| 13 | A review of isolated muscular ventricular septal defect显示文摘Background Color Doppler echocardiography greatly facilitates the diagnosis of isolated muscular ventricular septal defect with a small shunt.Data sources Original research articles were collected from database,including PubMed and Google scholar.Relevant articles about muscular ventricular septal defect were included.Results The frequency of isolated muscular ventricular septal defect is 5.7%in preterm infants and 1.1-5.3%in term infants.Spontaneous closure in muscular ventricular septal defect occurs with higher frequency and earlier than in perimembranous ventricular septal defect.Approximately 80-90%of isolated muscular ventricular septal defect closes spontaneously by 12 months of age.Midventricular muscular ventricular septal defect is spontaneously closed earlier in the short term,but no site difference is found in the long term.The spontaneous closure mechanism is regarded as aposition of the muscle tissueor fibrous tissue formation in the right ventricular side,but in rare cases involves aneurysm formation of the fibrous tissue.Regarding spontaneous closure of isolated muscular ventricular septal defect diagnosed for the fetus,further studies are needed.Chromosomal microarray analysis of fetuses with isolated muscular ventricular septal defect has revealed that it is not a severe risk factor of chromosomal abnormalities.Conclusions This paper presents a review of the history of the diagnosis and frequency of ventricular septal defect,with discussion of its natural history from the fetal period to after birth in patients with isolated muscular ventricular septal defect. | Toshiharu Miyake | 2020 | World Journal of Pediatrics2020，16，2： | 2 |

Here, we discuss state-of-the-art RSPs in the crude-oil refining field and present examples that illustrate how smart scheduling can impact operations in the high-performing chemical process industry. | 19 | Roles of rare earth oxide additives in millimeter-wave sintering of AlN显示文摘Roles of rare earth oxide (RE2O3) additives in millimeter-wave(MM) sintering of AlN were investigated from the standpoints of phase diagram, heating characteristics of rare earth oxides, and morphology of intergranular oxide phase. In the millimeter-wave sintering of AlN, densification temperature decreased with the decrease of the ionic radius of rare earth ion and was closely related with the eutectic temperature in the RE2O3-Al2O3 binary system. The lowest densification temperature in the millimeter-wave sintering of AlN with Yb2O3 additive was attributed to the largest heating rate of Yb2O3·Al2O3 binary oxide under millimeter-wave radiation. Furthermore, the lowest densification temperature could be attained while selecting the Yb2O3 content so as to form the intergranular phase with the eutectic composition in the Yb2O3-Al2O3 binary system. The result showed good agreement with the above mentioned during the sintering of Si3N4 with Yb2O3-Al2O3 additive.
