| 2 | 新生儿复苏培训降低新生儿死亡率的效能分析:文献综述,meta分析与Delphi法的应用显示文摘[背景]全球每年13600万新生儿中,约1000万需要外界的帮助才能建立自主呼吸,每年有约814000例新生儿死亡是由分娩相关足月儿死亡(以往定义为'出生窒息')所导致的。目前尚缺乏关于新生儿复苏培训降低新生儿死亡率效能的系统性评价。[目的]对在医院和社区层面开展的快速评估及初步刺激、基本的复苏措施这两种新生儿复苏培训干预方法在降低足月新生儿分娩相关死亡率('出生窒息')、早产相关并发症死亡率这两个生命指标方面的效能进行评价。[方法]采用系统综述的方法,检索并筛选符合标准的新生儿复苏培训研究文献。采用GRADE方法对文献的质量进行评价。对符合要求的文件进行Meta分析。对数据质量较低的文献采用专家咨询法估计效能大小。[结果]通过文献检索筛选了24篇新生儿复苏培训干预研究(20篇观察性研究,2篇半试验研究,2篇随机对照研究)。对其中3篇文献进行Meta分析结果显示,以医院为基础的新生儿复苏培训可使分娩相关足月儿死亡降低30%(RR=0.70,95%CI0.59~0.84);文献中涉及早产儿死亡率效能的数据质量不高,因此采用专家咨询法进行估计。以社区为基础的新生儿复苏培训干预研究具有较高的异质性,复苏培训通常是一系列培训包的一部分,且对早产儿和足月儿的定义标准不一致。专家咨询法估计结果显示,新生儿快速评估和初步刺激可使分娩相关新生儿死亡、早产儿死亡均降低10%;以医院为基础的培训可使早产儿死亡进一步降低10%,以社区为基础的培训可使分娩相关足月儿死亡率进一步降低20%,早产儿死亡率进一步降低5%。[结论]新生儿复苏培训在医院层面可以使足月新生儿分娩相关死亡率降低30%,然而中低收入水平的国家培训覆盖率仍较低。专家咨询法估计新生儿复苏培训在社区层面降低足月儿及早产儿死亡率的效能较小。需要对新生儿复苏培训的效果、成本、实施策略等进行更深入的研究与评价。 | Anne CC Lee Simon Cousens Stephen N Wall 徐韬 | 2012 | 中国妇幼卫生杂志2012,3,2: | 11 |

| 4 | Liver pathology of hepatitis C,beyond grading and stagingof the disease显示文摘Liver biopsy evaluation plays a critical role in management of patients with viral hepatitis C. In patients with acute viral hepatitis, a liver biopsy, though uncommonly performed, helps to rule out other nonviral causes of deranged liver function. In chronic viral hepatitis C, it is considered the gold standard in assessment of the degree of necroinflammation and the stage of fibrosis, to help guide treatment and determine prognosis. It also helps rule out any concomitant diseases such as steatohepatitis, hemochromatosis or others. In patients with chronic progressive liver disease with cirrhosis and dominant nodules, a targeted liver biopsy is helpful in differentiating a regenerative nodule from dysplastic nodule or hepatocellular carcinoma. In the setting of transplantation, the liver biopsy helps distinguish recurrent hepatitis C from acute rejection and also is invaluable in the diagnosis of fibrosing cholestatic hepatitis, a rare variant of recurrent hepatitis C. | 6 | Interprofessional, multiple step simulation course improves pediatric resident and nursing staff management of pediatric patients with diabetic ketoacidosis显示文摘AIM To investigate the use of a multidisciplinary, longitudinal simulation to educate pediatric residents and nurses on management of pediatric diabetic ketoacidosis.METHODS A multidisciplinary, multiple step simulation course was developed by faculty and staff using a modified Delphi method from the Pediatric Simulation Center and pediatric endocrinology department. Effectiveness of the simulation for the residents was measured with a pre- and post-test and a reference group not exposed to simulation. A follow up post-test was completed 3-6 mo after the simulation. Nurses completed a survey regarding the education activity. RESULTS Pediatric and medicine-pediatric residents(n = 20) and pediatric nurses(n = 25) completed the simulation course. Graduating residents(n = 16) were used as reference group. Pretest results were similar in the control and intervention group(74% ± 10% vs 76% ± 15%, P = 0.658). After completing the intervention, participants improved in the immediate post-test in comparison to themselves and the control group(84% ± 12% post study; P < 0.05). The 3-6 mo follow up post-test results demonstrated knowledge decay when compared to their immediate post-test results(78% ± 14%, P = 0.761). | 10 | Alcoholic hepatitis: The pivotal role of Kupffer cells显示文摘Kupffer cells play a central role in the pathogenesis of alcoholic hepatitis(AH). It is believed that alcohol increases the gut permeability that results in raised levels of serum endotoxins containing lipopolysaccharides(LPS). LPS binds to LPS-binding proteins and presents it to a membrane glycoprotein called CD14, which then activates Kupffer cells via a receptor called tolllike receptor 4. This endotoxin mediated activation of Kupffer cells plays an important role in the inflammatory process resulting in alcoholic hepatitis. There is no effective treatment for AH, although notable progress has been made over the last decade in understanding the underlying mechanism of alcoholic hepatitis. We specifically review the current research on the role of Kupffer cells in the pathogenesis of AH and the treatment strategies. | 11 | Hepatitis B infection among adults in the philippines:A national seroprevalence study显示文摘AIM:To determine the prevalence of hepatitis B surface antigen(HBsAg)seropositivity among adult Filipinos.METHODS:Testing for HBsAg was performed on serum samples from persons aged ≥ 20 years old who participated in the National Nutrition and Health Survey(NNHeS)conducted in 2003.Information on age,sex,marital status,educational attainment,employment status,and income were collected.For this study,marital status was classified as never married or otherwise(i.e.,married,divorced,separated,widowed);educational attainment was classified as high school graduate or below or at least some tertiary education;and employment status was classified as currently employed or currently unemployed.Annual income was divided into 4 quartiles in Philippine pesos(PhP):Q1,≤ PhP 53064;Q2,PhP 53065-92192;Q3,PhP 92193-173387;and Q4,≥ PhP 173388.Prevalence estimates were weighted so that they represented the general population.Social and demographic factors were correlated with HBsAg seropositivity.Multivariate analysis was used to determine independent predictors of HBsAg seropositivity.RESULTS:A total of 2150 randomly selected adults,20 years and over,out of the 4753 adult participants of NNHeS were tested for HBsAg.The HBsAg seroprevalence was 16.7%(95%CI:14.3%-19.1%),which corresponded to an estimated 7278968 persons infected with hepatitis B.There was no significant difference between males and females(17.5% vs 16.0%;P = 0.555).This corresponded to an estimated 3721775 men and 3557193 women infected with hepatitis B.The HBsAg seroprevalence peaked at age 20-39 years old,with declining prevalence in the older age groups.The only independent predictor of HBsAg seropositivity was the annual income,with persons in the highest income quartile being less likely to be HBsAg positive(age-adjusted OR = 0.51;95%CI:0.30-0.86)compared to subjects in the lowest income quartile.Sex,marital status,educational attainment,and employment status were not found to be independent predictors of HBsAg seropositivity.CONCLUSION:The high HBsAg seroprevalence among adults in the Philippines classifies the country as hyperendemic for HBV infection and appears unchanged over the last few decades. | Stephen N Wong Janus P Ong Madalinee Eternity D Labio Oscar T Cabahug Maria Lourdes O Daez Erlinda V Valdellon Jose D Sollano Jr Marilyn O Arguillas | 2013 | World Journal of Hepatology2013,5,4: | 3 |
| 12 | Physico-chemical characteristics and heavy metal concentrations of copper mine wastes in Zambia:implications for pollution risk and restoration显示文摘Soil characterization is a vital activity to develop appropriate and effective restoration protocols for mine wastelands while insights into the total content of heavy metals in the soil is an important step in estimating the hazards that the metals may pose to the vital roles of soil in the ecosystem.This study addressed the following research questions:(1)To what extent do the physico-chemical characteristics vary between mine waste sediments and the nearby forest soil?(2)Are the concentrations of heavy metals high enough to be considered as toxic?and(3)Are heavy metals present in mine waste sediments potential sources of pollution?We hypothesized that the physicochemical characteristics of mine waste sediments are less favorably for plant establishment and growth while the concentrations of heavy metals are very high,thus restricting the success of revegetation of mine waste lands.Mine waste sediments were sampled following a diagonal transect across tailings dams,overburden dump sites and the local forest soil from the top layer(0-20 cm)using a closed auger.Samples were analyzed for arsenic,barium,lead,cadmium,cobalt,copper,chromium,nickel,vanadium,and zinc as well as for soil physico-chemical properties.The mine waste sediments were dominated by silt whilst the forest soil by sand particles,with significantly high bulk density in the former.Both the forest soil and overburden sediments were acidic than the alkaline tailings dam sediment.Total organic carbon and nitrogen contents were significantly low in mine wasteland substrates but the concentration of Ca and Mg were significantly higher in tailings dam substrate than the forest soil.The concentrations of available P,K and Na were similar across sites.The mean concentrations of heavy metals were significantly(p<0.01)higher in mine waste sediments than the forest soil;except for cadmium(p=0.213).The order of contamination by heavy metals on the tailings was Cu>Co>Ba>Ni>As>Zn>Pb>Cr>V>Cd,and that on the overburdens was Cu>Co>Ba>Ni>Zn>Cr>Pb>V>As>Cd.The pollution load index(PLI)was nearly twice higher for the tailings dam(8.97)than the overburden(5.84).The findings show that the copper mine wastes(the tailings dams and overburden waste rock sites)are highly contaminated by heavy metals;which,in turn,might pose serious hazards to human health and agricultural productivity.In addition,poor macro-nutrient availability,substrate compaction and soil acidity(particularly on overburden sites)coupled with toxic level of heavy metals would be the main challenges for successful phytostabilization of copper mine wastelands. | Mutale N.Chileshe Stephen Syampungani Emma Sandell Festin Mulualem Tigabu Abolfazl Daneshvar Per Christer Odén | 2020 | Journal of Forestry Research2020,31,4: | 2 |

| 15 | Demographic trends and climate over 35 years in the Barro Colorado 50 ha plot显示文摘Background:The first three censuses of the 50-ha plot at Barro Colorado Island spanned an unusually harsh dry season during the 1983 El Nino.By the early 1990 s,we had documented increases in tree mortality,tree growth,and large population fluctuations of many species during the 1982-1985 census interval.At the time,we asserted that increasing drought frequency would greatly affect the forest.With the benefit of five more censuses at Barro Colorado from 1995-2015,we can now put the 1980 conditions in a longer perspective and test the hypothesis that increasing droughtiness has continued to change the forest.Methods:A 50-ha forest plot on Barro Colorado Island was censused eight times,in 1982 and every five years since1985.All free-standing woody stems were measured,mapped,and identified In each census.Results:1) The period 1982-1992 included several extreme dry seasons,not just 1983,but since then there have been few such droughts.2) Dbh growth declined from a peak in the early 1980 s to its lowest in the early 1990 s.From 1995-2015 it increased slightly,but not returning to the initial peak.Nearly every species and all dbh categories followed the same pattern.3) The elevated stand-wide mortality rate of large trees during the 1982-1985 drought has not returned,and most individual species showed the same pattern of elevated mortality in the 1980 s followed by low and fairly stable mortality after 1990.4) Sapling mortality declined after 1985,but rose again in the late-90 s,so the 1980 s drought period no longer looks unusual.Mortality of individual species' saplings fluctuated erratically,including cases where mortality during the drought was lower than after.5) Population sizes of individual species fluctuated in all possible directions.Some species declined precipitously during the drought,then recovered,but others did not recover.Other species increased in abundance during the drought.Conclusions:Droughts of the 1980 s elevated tree growth and mortality at Barro Colorado,but since 1990,demographic rates have remained lower,paralleling a moderate climate with few severe droughts after 1990.Moisture-demanding species suffered during the drought,but many have since recovered.We do not know how often such drought periods recur.Moreover,many species' abundances fluctuated over 35 years with no known cause. | Richard Condit Rolando Pérez Suzanne Lao Salomón Aguilar Stephen P.
