1Impact of postoperative intravenous fluid administration on complications following elective hepato-pancreato-biliary surgery显示文摘Background: The impact of perioperative intravenous fluid administration on surgical outcomes has been documented in literature, but not specifically studied in the context of hepato-pancreato-biliary(HPB) surgery. This study aimed to investigate the impact of postoperative intravenous fluid administration on intensive care unit(ICU), in this subgroup of patients. Methods: A single-center retrospective cohort of 241 HPB patients was assessed, focusing on intravenous fluid administration in ICU, during the first 24 h. Intravenous fluid variables were compared to hospital stay and postoperative complications. Data were assessed using Spearman's correlation test for bivariate correlations and logistic regression for multivariate analysis. Results: The median volume of intravenous fluid administered in the first 24 h postoperatively was 4380 mL, of which 2200 mL was crystalloid, 1500 mL colloid and 680 mL 'other' fluid. Patients with one or more complications had a higher median total intravenous fluid input(4790 vs. 4300 mL), higher colloid volume(20 0 0 vs. 150 0 mL), lower urine output(1595 vs. 1900 mL) and greater overall fluid balance( + 3040 vs. + 2553 mL) than those without complications. There were correlations between total intravenous fluid volume administered( r = 0.278, P < 0.001), intravenous colloid input( r = 0.278, P < 0.001), urine output( r =-0.295, P < 0.001), positive fluid balance( r = 0.344, P < 0.001) and length of hospital stay. Logistic regression model was constructed to predict the occurrence of one or more complications; total intravenous fluid volume and overall fluid balance were both independent significant predictors(OR = 2.463, P = 0.007; OR = 1.001, P = 0.011; respectively). Conclusions: Administration of high volumes of intravenous fluids in the first 24 hours post-HPB surgery, along with higher positive fluid balance is associated with a higher rate of complications and longer hospital stay. Moreover, lower urine output is associated with longer hospital stay. 2Obesity and cardiometabolic disease risk factors among US adolescents with disabilities显示文摘AIM: To generate prevalence estimates of weight status and cardiometabolic disease risk factors among adolescents with and without disabilities.METHODS: Analysis of the 1999-2010 National Health and Nutrition Examination Survey data was conducted among 12-18 years old with(n = 256) and without disabilities(n = 5020). Mean values of waist circumference, fasting glucose, high-density-lipoprotein cholesterol, triglycerides, systolic and diastolic blood pressure and metabolic syndrome(Met S, ≥ 3 risk factors present) were examined by the following standardized body mass index(BMI) categories for those with and without disabilities; overweight(BMI ≥ 85th- < 95 th percentile for age and sex), obesity(BMI ≥ 95 th percentile) and severe obesity(BMI ≥35 kg/m2). Linear regression models were fit with each cardiometabolic disease risk factor independently as continuous outcomes to show relationships with disability status. RESULTS: Adolescents with disabilities were significantlymore likely to be overweight(49.3%), obese(27.6%) and severely obese(12%) vs their peers without disabilities(33.1%, 17.5% and 3.6%, respectively, P ≤ 0.01 for all). A higher proportion of overweight, obese and severely obese children with disabilities had abnormal SBP, fasting lipids and glucose as well as Met S(18.9% of overweight, 32.3% of obese, 55% of severely obese) vs their peers without disabilities(9.7%, 16.8%, 36.3%, respectively). US adolescents with disabilities are over three times as likely to have Met S(OR = 3.45, 95%CI: 1.08-10.99, P = 0.03) vs their peers with no disabilities.CONCLUSION: Results show that adolescents with disabilities are disproportionately affected by obesity and poor cardiometabolic health vs their peers with no disabilities. 7Development of a health education intervention strategy using an implementation research method to control taeniasis and cysticercosis in Burkina Faso显示文摘Background:Taeniasis and cysticercosis are two diseases caused by Taenia solium,a parasite transmitted between humans and pigs,leading to considerable economic loss and disabilities.Transmission of the parasite is linked to environmental and behavioural factors such as inadequate sanitation and hygiene,poor pig management,and consumption of infected pork.This study used implementation research method to design a health education intervention strategy for reducing T.solium infections in Burkina Faso,a country endemic for the parasite.Methods:Eighteen group discussions were conducted with 8-18 participants each in three villages.In addition,structured interviews were conducted among 4777 participants and 2244 pig owners,who were selected through cluster random sampling in 60 villages of three provinces of Burkina Faso.Both approaches assessed knowledge and practices related to T.solium.The information obtained was used to develop a community-adapted health education intervention strategy to control taeniasis and cysticercosis in Burkina Faso.Results:The group discussions revealed that participants had a poor quality of life due to the diseases as well as inadequate access to latrines,safe water,and healthcare services.In addition,it was found that pig production was an important economic activity,especially for women.Furthermore,financial and knowledge constraints were important limitations to improved pig management and latrine construction.The survey data also showed that open defecation and drinking unboiled water were common behaviours,enhanced by a lack of knowledge regarding the transmission of the parasite,perceived financial barriers to the implementation of control measures,lack of public sensitization,as well as a lack of self-efficacy towards control of the parasite.Nevertheless,the perceived financial benefits of controlling porcine cysticercosis could be emphasized by an education program that discourages open defecation and encourages drinking safe water.The final intervention strategy included a Participatory Hygiene and Sanitation Transformation(PHAST)approach,as well as a 52-min film and an accompanying comic booklet.Conclusions:The main problem in the study communities regarding the transmission of T.solium cysticercosis is the random disposal of human faeces,which can be contaminated with parasite eggs.Prevention of open defecation requires the building of latrines,which can be quite problematic in economically challenged settings.Providing the community with the skills to construct durable latrines using low-cost locally available materials would likely help to resolve this problem.Further studies are required to implement and evaluate the T.solium control strategy developed in this study.Helena Ngowi Ivan Ozbolt Athanase Millogo Veronique Dermauw Telesphore Some Paul Spicer Lori L.Jervis Rasmane Ganaba Sarah Gabriel Pierre Dorny Helene Carabin 2017Infectious Diseases of Poverty2017,6,1:1

13Exposure to cuticular bacteria can alter host behavior in a funnel-weaving spider显示文摘Contact with environmental microbes are arguably the most common species interaction in which any animal participates.Studies have noted diverse relationships between hosts and resident microbes,which can have strong consequences for host development,physiology,and behavior. 15Randomized controlled trials in frontotemporal dementia: cognitive and behavioral outcomes显示文摘Progress has been made in understanding the genetics and molecular biology of frontotemporal dementia(FTD).Targets for intervention have been identified,therapies are being developed,and clinical trials are advancing.A major challenge for FTD research is that multiple underlying pathologies can be associated with heterogeneous phenotypes.The neuropsychological profiles associated with FTD spectrum disorders often include executive dysfunction,language impairments and behavioral disturbance.Behavioral variant FTD is characterized by an initial presentation of changes in personality,behavior and/or emotion,which are often difficult to objectively capture using traditional neuropsychological measures.The two principal language variants of FTD are Progressive Nonfluent Aphasia(PNFA)with predominant agrammatic/non-fluent impairments and Semantic Dementia(SD)with semantic impairments and visual agnosia.Selection of appropriate endpoints for clinical trials is critical to ensure that the measures are adequately sensitive to detect change,yet specific enough to isolate signal from noise,and acceptable to regulatory agencies.Given the anticipated potential for small effect sizes,measures must be able to identify small incremental changes over time.It is also imperative that the measures provide adequate coverage of the constructs or behaviors of interest.Selected outcome measures should be suitable for repeat administration,yet relatively robust to practice effects to ensure that observed changes reflect true signal variance and not residual effects due to repeated measurement or poor reliability.To facilitate widespread adoption as an endpoint,measures should be readily accessible.We provide several examples of potential global,composite,and individual cognitive measures,as well as behavioral measures promising for FTD trials.Development and application of appropriate trial outcomes is critically important to success in advancing new treatments for FTD patients.Justin B Miller Sarah J Banks Gabriel C Léger Jeffrey L Cummings 2014Translational Neurodegeneration2014,3,1:0
