| 1 | Physical exercise in the prevention and treatment of Alzheimer’s disease显示文摘Dementia is one of the greatest global challenges for health and social care in the 21st century.Alzheimer’s disease(AD),the most common type of dementia,is by no means an inevitable consequence of growing old.Several lifestyle factors may increase,or reduce,an individual’s risk of developing AD.Much has been written over the ages about the benefits of exercise and physical activity.Among the risk factors associated with AD is a low level of physical activity.The relationship between physical and mental health was established several years ago.In this review,we discuss the role of exercise(aerobic and resistance)training as a therapeutic strategy for the treatment and prevention of AD.Older adults who exercise are more likely to maintain cognition.We address the main protective mechanism on brain function modulated by physical exercise by examining both human and animal studies.We will pay especial attention to the potential role of exercise in the modulation of amyloid b turnover,inflammation,synthesis and release of neurotrophins,and improvements in cerebral blood flow.Promoting changes in lifestyle in presymptomatic and predementia disease stages may have the potential for delaying one-third of dementias worldwide.Multimodal interventions that include the adoption of an active lifestyle should be recommended for older populations. | Adrian De la Rosa Gloria Olaso-Gonzalez Coralie Arc-Chagnaud Fernando Millan Andrea Salvador-Pascual Consolacion Garcıa-Lucerga Cristina Blasco-Lafarga Esther Garcia-Dominguez Aitor Carretero Angela G.Correas Jose Vina Mari Carmen Gomez-Cabrera | 2020 | Journal of Sport and Health Science2020,9,5: | 32 |
| 2 | Optimizing proton pump inhibitors in Helicobacter pylori treatment:Old and new tricks to improve effectiveness显示文摘The survival and replication cycle of Helicobacter pylori(H.pylori)is strictly dependant on intragastric pH,since H.pylori enters replicative phase at an almost neutral pH(6-7),while at acid pH(3-6)it turns into its coccoid form,which is resistant to antibiotics.On these bases,it is crucial to increase intragastric pH by proton pump inhibitors(PPIs)when an antibiotic-based eradicating therapy needs to be administered.Therefore,several tricks need to be used to optimize eradication rate of different regimens.The administration of the highest dose as possible of PPI,by doubling or increasing the number of pills/day,has shown to be able to improve therapeutic outcome and has often proposed in rescue therapies,even if specific trials have not been performed.A pre-treatment with PPI before starting antibiotics does not seem to be effective,therefore it is discouraged.However,the choice of PPI molecule could have a certain weight,since second-generation substances(esomeprazole,rabeprazole)are likely more effective than those of first generation(omeprazole,lansoprazole).A possible explanation is due to their metabolism,which has been proven to be less dependent on cytochrome P450(CYP)2C19 genetic variables.Finally,vonoprazan,a competitive inhibitor of H+/K+-ATPase present on luminal membrane of gastric parietal cells has shown the highest efficacy,due to both its highest acid inhibition power and rapid pharmacologic effect.However current data come only from Eastern Asia,therefore its strong power needs to be confirmed outside this geographic area in Western countries as well as related to the local different antibiotic resistance rates. | Enzo Ierardi Giuseppe Losurdo Rosa Federica La Fortezza Mariabeatrice Principi Michele Barone Alfredo Di Leo | 2019 | World Journal of Gastroenterology2019,25,34: | 15 |

| 6 | Validation of a new scoring system: Rapid assessment faecal incontinence score显示文摘AIM: To implement a quick and simple test- rapid assessment faecal incontinence score(RAFIS) and show its reliability and validity.METHODS: From March 2008 through March 2010, we evaluated a total of 261 consecutive patients, including 53 patients with faecal incontinence. Demographic and comorbidity information was collected. In a single visit, patients were administered the RAFIS. The results obtained with the new score were compared with those of both Wexner score and faecal incontinence quality of life scale(FIQL) questionnaire. The patient withoutinfluence of the surgeon completed the test. The role of surgeon was explaining the meaning of each section and how he had to fill. Reliability of the RAFIS score was measured using intra-observer agreement and Cronbach's alpha(internal consistency) coefficient. Multivariate analysis of the main components within the different scores was performed in order to determine whether all the scores measured the same factor and to conclude whether the information could be encompassed in a single factor. A sample size of 50 patients with faecal incontinence was estimated to be enough to detect a correlation of 0.55 or better at 5% level of significance with 80% power.RESULTS: We analysed the results obtained by 53 consecutive patients with faecal incontinence(median age 61.55 ± 12.49 years) in the three scoring systems. A total of 208 healthy volunteers(median age 58.41 ± 18.41 years) without faecal incontinence were included in the study as negative controls. Pearson's correlation coefficient between 'state' and 'leaks' was excellent(r = 0.92, P < 0.005). Internal consistency in the comparison of 'state' and 'leaks' yielded also excellent correlation(Cronbach's α = 0.93). Results in each score were compared using regression analysis and a correlation value of r = 0.98 was obtained with Wexner score. As regards FIQL questionnaire, the values of 'r ' for the different subscales of the questionnaire were: 'lifestyle' r =-0.87, 'coping/behaviour' r =-0.91, 'depression' r =-0.36 and 'embarrassment' r =-0.90,(P < 0.01). A multivariate analysis showed that all the scoring systems measured the same factor. A single factor may explain 80.84% of the variability of FI, so all the scoring systems measure the same factor. | 9 | Radio Electric Asymmetric Conveyer Technology Modulates Neuroinflammation in a Mouse Model of Neurodegeneration显示文摘In this study, the effects of Radio Electric Asymmetric Conveyer(REAC), a non-invasive physical treatment, on neuroinflammatory responses in a mouse model of parkinsonism induced by intoxication with1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP),were investigated in vivo. We found that the REAC tissue optimization treatment specific for neuro-regenerative purposes(REAC TO-RGN-N) attenuated the inflammatory picture evoked by MPTP-induced nigro-striatal damage inmice, decreasing the levels of pro-inflammatory molecules and increasing anti-inflammatory mediators. Besides, there was a significant reduction of both astrocyte and microglial activation in MPTP-treated mice exposed to REAC TORGN-N. | 12 | Intramuscular vs intradermal route for hepatitis B booster vaccine in celiac children显示文摘AIM:To compare intradermal(ID) and intramuscular(IM) booster doses,which have been used in healthy and high risk subjects,such as healthcare workers,haemodialysis patients,human immunodeficiency virus patients,and renal transplant recipients unresponsive to initial hepatitis B vaccination,in celiac individuals.METHODS:We conducted our study on 58 celiac patients,vaccinated in the first year of life,whose blood analysis had showed the absence of protective hepatitis B virus(HBV) antibodies.All patients had received the last vaccine injection at least one year before study enrolment and they had been on a gluten free diet for at least 1 year.In all patients we randomly performed an HBV vaccine booster dose by ID or IM route.Thirty celiac patients were revaccinated with recombinant hepatitis B vaccine(Engerix B) 2 g by the ID route,while 28 celiac patients were revaccinated with Engerix B 10 g by the IM route.Four weeks after every booster dose,the anti-hepatitis B surface(HBs) antibody titer was measured by an enzyme-linked immuneadsorbent assay.We performed a maximum of three booster doses in patients with no anti-HBs antibodies after the first or the second vaccine dose.The cut off value for a negative anti-HBs antibody titer was 10 IU/L.Patients with values between 10 and 100 IU/L were considered 'low responders' while patients with an antibody titer higher than 1000 IU/L were considered 'high responders'.RESULTS:No significant difference in age,gender,duration of illness,and years of gluten intake was found between the two groups.We found a high percentage of 'responders' after the first booster dose(ID = 76.7%,IM = 78.6%) and a greater increase after the third dose(ID = 90%,IM = 96.4%) of vaccine in both groups.Moreover we found a significantly higher number of high responders(with an anti-HBs antibody titer > 1000 IU/L) in the ID(40%) than in the IM(7.1%) group,and this difference was evident after the first booster dose of vaccination(P < 0.01).No side effects were recorded in performing delivery of the vaccine by either the ID or IM route.CONCLUSION:Our study suggests that both ID and IM routes are effective and safe options to administer a booster dose of HBV vaccine in celiac patients.However the ID route seems to achieve a greater number of high responders and to have a better cost/benefit ratio. | Salvatore Leonardi Andrea Domenico Praticò Elena Lionetti Massimo Spina Giovanna Vitaliti Mario La Rosa | 2012 | World Journal of Gastroenterology2012,18,40: | 2 |

| 16 | Effects of Tillage and Crop Residue Management on Maize Yields and Net Returns in the Central Mexican Highlands Under Drought Conditions显示文摘In the subtropical highlands of Central Mexico,where the main crop is maize(Zea mays),the conventional practice(CP) involves tillage,monoculture and residue removal,leading to soil degradation and unsustainable use of natural resources and agricultural inputs.Conservation agriculture(CA) has been proposed as a viable alternative in the region,based on reduction in tillage,retention of adequate levels of crop residues and soil surface cover and use of crop rotation.This study began in 2009 when the highlands of Central Mexico suffered from a prolonged drought during vegetative maize growth in July-August,providing an opportunity for the on-farm comparison of CA with CP under severe drought conditions which 21 climate change models projected to become more frequent.Under dry conditions,CA resulted in higher yields and net returns per hectare as early as the first and second years after adoption by farmers.As an average of 27 plots under farmers' management in 2009,the maize yields were 26%higher under CA(6.3t ha^(-1)) than under CP(5.0 t ha^(-1)).2010 was close to a normal year in terms of rainfall so yields were higher than in 2009 for both practices;in addition,the yield difference between the practices was reduced to 19%(6.8 t ha^(-1) for CA vs.5.7 t ha^(-1) for CP).When all the 2009 and 2010 observations were analyzed in a modified stability analysis,CA had an overall positive effect of 3 838 Mexican Pesos ha^(-1)(320 $US ha^(-1)) on net return and 1.3 t ha^(-1) on yield.After only one to two years of adoption by farmers on their fields,CA had higher yields and net returns under dry conditions that were even drier than those predicted by the analyzed 21 climate change models under a climate change scenario,emission scenario A2. | R.ROMERO-PEREZGROVAS N.VERHULST D.DE LA ROSA V.HERNáNDEZ M.MAERTENS J.DECKERS B.GOVAERTS | 2014 | Pedosphere2014,24,4: | 2 |
