1Diagnosis and management of bacterial infections in decompensated cirrhosis显示文摘Bacterial infections are one of the most frequent complications in cirrhosis and result in high mortality rates.Patients with cirrhosis have altered and impaired immunity,which favours bacterial translocation.Episodes of infections are more frequent in patients with decompensated cirrhosis than those with compensated liver disease.The most common and life-threatening infection in cirrhosis is spontaneous bacterial peritonitis followed by urinary tract infections,pneumonia,endocarditis and skin and soft-tissue infections.Patients with decompensated cirrhosis have increased risk of developing sepsis,multiple organ failure and death.Risk factors associated with the development of infections are severe liver failure,variceal bleeding,low ascitic protein level and prior episodes of spontaneous bacterial peritonitis (SBP).The prognosis of these patients is closely related to a prompt and accurate diagnosis.An appropriate treatment decreases the mortality rates.Preventive strategies are the mainstay of the management of these patients.Empirical antibiotics should be started immediately following the diagnosis of SBP and the first-line antibiotic treatment is third-generation cephalosporins.However,the efficacy of currently recommended empirical antibiotic therapy is very low in nosocomial infections including SBP,compared to community-acquired episodes.This may be associated with the emergence of infections caused by Enterococcus faecium and extended-spectrum β-lactamaseproducing Enterobacteriaceae,which are resistant to the first line antimicrobial agents used for treatment.The emergence of resistant bacteria,underlines the need to restrict the use of prophylactic antibiotics to patients with the greatest risk of infections.Nosocomial infections should be treated with wide spectrum antibiotics.Further studies of early diagnosis,prevention and treatment are needed to improve the outcomes in patients with decompensated cirrhosis.Maria Pleguezuelo Jose Manuel Benitez Juan Jurado Jose Luis Montero Manuel De la Mata 2013World Journal of Hepatology2013,5,1:44

4Strategies to improve outcome of patients with hepatocellular carcinoma receiving a liver transplantation显示文摘Liver transplantation is the only therapeutic option which allows to treat both, the hepatocellular carcinoma and the underlying liver disease. Indeed, liver transplantation is considered the standard of care for a subset of patients with cirrhosis and hepatocellular carcinoma.However, tumour recurrence rates are as high as 20%, and once the recurrence is established the therapeutic options are scarce and with little impact on prognosis. Strategies to minimize tumour recurrence and thus to improve outcome may be classified into 3 groups:(1) An adequate selection of candidates for liver transplantation by using the Milan criteria;(2) An optimized management within waiting list including prioritization of patients at high risk of tumour progression, and the implementation of bridging therapies, particularly when the expected length within the waiting list is longer than 6 mo; and(3) Tailored immunosuppression comprising reduced exposure to calcineurin inhibitors, particularly early after liver transplantation, and the addition of mammalian target of rapamycin inhibitors. 9Endoscopic ultrasonography-guided celiac plexus neurolysis in patients with unresectable pancreatic cancer: An update显示文摘Pancreatic cancer produces disabling abdominal pain,and the pain medical management for pancreatic cancer is often challenging because it mainly relies on the use of narcotics(major opioids).However,opioids often provide suboptimal pain relief,and the use of opioids can lead to patient tolerance and several side effects that considerably reduce the quality of life of pancreatic cancer patients.Endosonography-guided celiac plexus neurolysis(EUS-CPN)is an alternative for pain control in patients with nonsurgical pancreatic cancer;EUS-CPN consists of the injection of alcohol and a local anesthetic into the area of the celiac plexus to achieve chemical ablation of the nerve tissue.EUS-CPN via the transgastric approach is a safer and more accessible technique than the percutaneous approach.We have reviewed most of the studies that evaluate the efficacy of EUSCPN and that have compared the different approaches that have been performed by endosonographers.The efficacy of EUS-CPN varies from 50%to 94%in the different studies,and EUS-CPN has a pain relief duration of 4–8 wk.Several factors are involved in its efficacy,such as the onset of pain,previous use of chemotherapy,presence of metastatic disease,EUS-CPN technique,type of needle or neurolytic agent used,etc.According to this review,injection into the ganglia may be the best technique,and a good visualization of the ganglia is the best predictor for a good EUS-CPN response,although more studies are needed.However,any of the 4 different techniques could be used to perform EUS-CPN effectively with no differences in terms of complications between the techniques,but more studies are needed.The effect of EUS-CPN on pain improvement,patient survival and patient quality of life should be evaluated in well-designed randomized clinical trials.Further research also needs to be performed to clarify the best time frame in performing a EUS-CPN.Guillermo Pérez-Aguado Diego Martinez-Acitores de la Mata Carlos Marra-López Valenciano Ignacio Fernandez-Urien Sainz 2021World Journal of Gastrointestinal Endoscopy2021,13,10:4
10Tools for the diagnosis of hepatitis C virus infection and hepatic fibrosis staging显示文摘Hepatitis C virus(HCV)infection represents a major public health issue.Hepatitis C can be cured bytherapy,but many infected individuals are unaware of their status.Effective HCV screening,fast diagnosis and characterization,and hepatic fibrosis staging are highly relevant for controlling transmission,treating infected patients and,consequently,avoiding end-stage liver disease.Exposure to HCV can be determined with high sensitivity and specificity with currently available third generation serology assays.Additionally,the use of point-of-care tests can increase HCV screening opportunities.However,active HCV infection must be confirmed by direct diagnosis methods.Additionally,HCV genotyping is required prior to starting any treatment.Increasingly,high-volume clinical laboratories use different types of automated platforms,which have simplified sample processing,reduced hands-on-time,minimized contamination risks and human error and ensured full traceability of results.Significant advances have also been made in the field of fibrosis stage assessment with the development of non-invasive methods,such as imaging techniques and serum-based tests.However,no single test is currently available that is able to completely replace liver biopsy.This review focuses on approved commercial tools used to diagnose HCV infection and the recommended hepatic fibrosis staging tests.Verónica Saludes Victoria González Ramon Planas Lurdes Matas Vicente Ausina Elisa Martró 2014World Journal of Gastroenterology2014,20,13:4

12Proteomic analysis for developing new biomarkers of hepatocellular carcinoma显示文摘AIM: To identify new markers of hepatocellular carcinoma (HCC) using a proteomic analysis. METHODS: Patients with liver cirrhosis of the three most frequent etiologies: hepatitis C virus, hepatitis B virus and alcoholic liver disease, were included in the study. The samples were analysed by 2D-electrophoresis in order to determine the differential protein expression. The proteins were separated according to the charge in immobilized pH 3-10 gradient strips and then by sodium dodecyl sulfate polyacrylamide gel electrophoresis. Proteins of interest were excised, digested with trypsin and the resulting peptides were separated and identified. RESULTS: Three differentially expressed apolipoproteins (Apo) were identified based on the protein profile using proteomic techniques: Apo-A1, Apo-A4 and Apo-E. Apo-A4 levels were significantly lower in HCC than in non-HCC patients regardless of etiology (P < 0.01). Multivariate logistic regression showed that Apo-A4 and Apo-A1 were the only independent factors related to HCC diagnosis (P < 0.05). The receiver operating characteristic (ROC) curve including both Apo-A4 and Apo-A1 showed an area under the ROC of 0.944 (P < 0.001), a sensitivity of 0.89 and a specificity of 0.81 for diagnosis of HCC. CONCLUSION: Apo-A4 and Apo-A1 may be used clinically as biomarkers of HCC with a high sensibility and specificity. 14Complex causes and consequences of rangeland greening in South America -multiple interacting natural and anthropogenic drivers and simultaneous ecosystem degradation and recovery trends显示文摘Land-surface greening has been reported globally over the past decades.While often seen to represent ecosystem recovery,the impacts on biodiversity and society can also be negative.Greening has been widely reported from rangelands,where drivers and processes are complex due to its high environmental heterogeneity and societal dynamics.Here,we assess the complexity behind greening and assess its links to various drivers in an iconic,heterogeneous rangeland area,the IberáWetlands and surroundings,in Argentina.Time-series satellite imagery over the past 19 years showed overall net greening,but also substantial local browning both in protected and unprotected areas,linking to land use,temporal changes in surface water,fire,and weather.We found substantial woody expansion mainly in the unprotected land,with 37%contributed by tree plantations and the remaining 63%by spontaneous woody expansion,along with widespread transitions from terrestrial land to seasonal surface water.Fire occurrences tended to reduce greening with unprotected areas experiencing widespread and frequent fire.However,protected areas had more browning in unburnt areas than burned areas.Temporal variation in annual precipitation and temperature tended to nonlinearly influence fire occurrences with an interplay of human fire management,further shaping the vegetation greening,pointing to high complexity behind the observed rangeland greening involving interactions among local drivers.Our findings highlight that the observed overall greening is an outcome of multiple trends with clear negative impacts on biodiversity and the local livestock-oriented culture(notably expanding tree plantations)and spontaneous vegetation dynamics,partly involving spontaneous woody expansion.The latter has positive potential for biodiversity and ecosystem services in terms of woodland recovery,but can become negative in such a natural savanna region if expansions develop on a too broad scale,highlighting the importance of ensuring recovery of natural fire and herbivory regimes in protected areas along with sustainable rangeland management elsewhere.Wang Li Robert Buitenwerf Renata Nicora Chequín Javier Elias Florentín Roberto Manuel Salas Julia Carolina Mata Li Wang Zheng Niu Jens-Christian Svenning 2020Geography and Sustainability2020,1,4:2

16Deciphering the superior thermoelectric property of posttreatment-free PEDOT:PSS/IL hybrid by X-ray and neutron scattering characterization显示文摘In this work,a polymer and ionic liquid(IL)hybrid with superior thermoelectric performance is prepared via a system design of the chemical composition,molar ratio of the constituent molecules and manipulating the structure in solution and dried films.The solution-casted hybrid film,consisting of poly(3,4-ethylenedioxythiophene):poly(styrenesulfonate)(PEDOT:PSS)and 1-ethyl-3-methylimidazolium tricyanomethanide(EMIM:TCM),shows the highest power factor of 175μW m^(−1) K^(−2) in the polymer hybrid prepared by a post-treatment-free method.With a set of complementary structure characterization methods,it is found that EMIM:TCM can induce the structure reorganization of PEDOT:PSS in solution from a core-shell model to a rod-like model,during which PEDOT partially separates from PSS that eases the conductive network formation.In addition,the oxidation level of PEDOT:PSS is reduced by adding EMIM:TCM.Based on which,the PEDOT:PSS/IL hybrid shows the best performance in optimizing the conductivity(1163 S cm^(−1))and Seebeck coefficient(38.8μV K^(−1))simultaneously.Xin Li Ruike Zou Zhen Liu Jitendra Mata Ben Storer Yu Chen Weiheng Qi Zekun Zhou Peng Zhang 2022npj Flexible Electronics2022,6,1:2

18Bioengineering bacterial encapsulin nanocompartments as targeted drug delivery system显示文摘The development of Drug Delivery Systems(DDS)has led to increasingly efficient therapies for the treatment and detection of various diseases.DDS use a range of nanoscale delivery platforms produced from polymeric of inorganic materials,such as micelles,and metal and polymeric nanoparticles,but their variant chemical composition make alterations to their size,shape,or structures inherently complex.Genetically encoded protein nanocages are highly promising DDS candidates because of their modular composition,ease of recombinant production in a range of hosts,control over assembly and loading of cargo molecules and biodegradability.One example of naturally occurring nanocompartments are encapsulins,recently discovered bacterial organelles that have been shown to be reprogrammable as nanobioreactors and vaccine candidates.Here we report the design and application of a targeted DDS platform based on the Thermotoga maritima encapsulin reprogrammed to display an antibody mimic protein called Designed Ankyrin repeat protein(DARPin)on the outer surface and to encapsulate a cytotoxic payload.The DARPin9.29 chosen in this study specifically binds to human epidermal growth factor receptor 2(HER2)on breast cancer cells,as demonstrated in an in vitro cell culture model.The encapsulin-based DDS is assembled in one step in vivo by co-expressing the encapsulin-DARPin9.29 fusion protein with an engineered flavin-binding protein mini-singlet oxygen generator(MiniSOG),from a single plasmid in Escherichia coli.Purified encapsulin-DARPin_miniSOG nanocompartments bind specifically to HER2 positive breast cancer cells and trigger apoptosis,indicating that the system is functional and specific.The DDS is modular and has the potential to form the basis of a multi-receptor targeted system by utilising the DARPin screening libraries,allowing use of new DARPins of known specificities,and through the proven flexibility of the encapsulin cargo loading mechanism,allowing selection of cargo proteins of choice.Alexander Van de Steen Rana Khalife Noelle Colant Hasan Mustafa Khan Matas Deveikis Saverio Charalambous Clare M.Robinson Rupali Dabas Sofia Esteban Serna Diana A.Catana Konstantin Pildish Vladimir Kalinovskiy Kenth Gustafsson Stefanie Frank 2021Synthetic and Systems Biotechnology2021,6,3:2
