| 1 | Foveal avascular zone area and parafoveal vessel density measurements in different stages of diabetic retinopathy by optical coherence tomography angiography显示文摘AIM:To investigate foveal avascular zone(FAZ) and parafoveal vessel densities(PRVD) by means of optical coherence tomography angiography(OCTA) in diabetic patients with or without diabetic retinopathy(DR) and to assess the reproducibility of FAZ and PRVD measurements.METHODS:Sixty diabetic patients(60 eyes) with different stage of DR(graded according to the International Clinical Severity Scale for DR) and 20 healthy subjects underwent FAZ area and PRVD measurements using OCTA by two experienced examiners.FAZ area in all patients was also assessed using fluorescein angiography(FA).RESULTS:In subject with proliferative DR and with moderate-severe non proliferative DR,FAZ area was significantly increased compared to healthy controls(P=0.025 and P=0.050 respectively measured with OCTA and P=0.025 and P=0.048 respectively measured with FA).OCTA showed significantly less inter-observer variability compared to FA.Concordance correlation coefficient(CCC) for FAZ area measurements was 0.829(95%CI:0.736-0.891) P<0.001 with FA and 1.000(95%CI:0.999-1.000) P<0.001 with OCTA.CCC was 0.834(95%CI:0.746-0.893) P<0.001 and 0.890(95%CI:0.828-0.930) P<0.001 for parafoveal superficial and deep vessel density measurements,respectively.CONCLUSION:OCTA shows progressive increase of FAZ area and reduction of PRVD in both superficial and deep plexus at increasing DR severity.FAZ area and PRVD measurements using OCTA are highly reproducible. | Rodolfo Mastropasqua Lisa Toto Alessandra Mastropasqua Raffaella Aloia Chiara De Nicola Peter A Mattei Guido Di Marzio Marta Di Nicola Luca Di Antonio | 2017 | International Journal of Ophthalmology(English edition)2017，10，10： | 26 |

| 7 | Clinical outcomes of tricuspid valve repair accompanying left-sided heart disease显示文摘AIM To determine whether the need for additional tricuspid valve repair is an independent risk factor when surgery is required for a left-sided heart disease.METHODS One hundred and eighty patients(68 ± 12 years,79 males) underwent tricuspid annuoplasty.Cox proportionalhazards regression model for multivariate analysis was performed for variables found significant in univariate analyses.RESULTS Tricuspid regurgitation etiology was functional in 154 cases(86%),organic in 16 cases(9%),and mixed in10 cases(6%),respectively.Postoperative mortality at 30 days was 11.7%.Mean follow-up was 51.7 mo with survival at 5 years of 73.5%.Risk factors for mortality were acute endocarditis [hazard ratio(HR) = 9.22(95%CI:2.87-29.62),P < 0.001],ischemic heart disease requiring myocardial revascularization [HR = 2.79(1.26-6.20),P = 0.012],and aortic valve stenosis [HR = 2.6(1.15-5.85),P = 0.021].Significant predictive factors from univariate analyses were double-valve replacement combined with tricuspid annuloplasty [HR = 2.21(1.11-4.39),P = 0.003] and preoperatively impaired ejection fraction [HR = 1.98(1.04-3.92),P = 0.044].However,successful mitral valve repair showed a protective effect [HR = 0.32(0.10-0.98),P = 0.046].Additionally,in instances where tricuspid regurgitation required the need for concomitant tricuspid valve repair,mortality predictor scores such as Euroscore 2 could be shortened to a simple Euroscore-tricuspid comprised of only 7 inputs.The explanation may lie in the fact that significant tricuspid regurgitation following leftsided heart disease represents an independent risk factor encompassing several other factors such as pulmonary arterial hypertension and dyspnea.CONCLUSION Tricuspid annuloplasty should be used more often as a concomitant procedure in the presence of relevant tricuspid regurgitation,although it usually reveals an overly delayed correction of a left-sided heart disease. | Kasra Azarnoush Ahmad S Nadeemy Bruno Pereira Massoud A Leesar Céline Lambert Alaa Azhari Vedat Eljezi Nicolas Dauphin Etienne Geoffroy Lionel Camilleri | 2017 | World Journal of Cardiology2017，9，10： | 4 |
| 8 | Immunohistochemical expression of intrarenal renin angiotensin system components in response to tempol in rats fed a high salt diet显示文摘AIM To determine the effect of tempol in normal rats fed high salt on arterial pressure and the balance between antagonist components of the renal renin-angiotensin system.METHODS Sprague-Dawley rats were fed with 8% Na Cl highsalt(HS) or 0.4% Na Cl(normal-salt,NS) diet for 3 wk,with or without tempol(T)(1 mmol/L,administered in drinking water). Mean arterial pressure(MAP),glomerularfiltration rate(GFR),and urinary sodium excretion(UVNa) were measured. We evaluated angiotensin Ⅱ(Ang Ⅱ),angiotensin 1-7(Ang 1-7),angiotensin converting enzyme 2(ACE2),mas receptor(Mas R),angiotensin type 1 receptor(AT1R) and angiotensin type 2 receptor(AT2R) in renal tissues by immunohistochemistry.RESULTS The intake of high sodium produced a slight but significant increase in MAP and differentially regulated components of the renal renin-angiotensin system(RAS). This included an increase in Ang Ⅱ and AT1 R,and decrease in ACE-2 staining intensity using immunohistochemistry. Antioxidant supplementation with tempol increased natriuresis and GFR,prevented changes in blood pressure and reversed the imbalance of renal RAS components. This includes a decrease in Ang Ⅱ and AT1 R,as increase in AT2,ACE2,Ang(1-7) and Mas R staining intensity using immunohistochemistry. In addition,the natriuretic effects of tempol were observed in NS-T group,which showed an increased staining intensity of AT2,ACE2,Ang(1-7) and Mas R.CONCLUSION These findings suggest that a high salt diet leads to changes in the homeostasis and balance between opposing components of the renal RAS in hypertension to favour an increase in Ang Ⅱ. Transanal total mesorectal excision(Ta TME) might offer technical advantages over laparoscopic or open approaches particularly for tumors in the distal third of the rectum. The aim of this article is to review the current experience with Ta TME. The limits and future developments are also explored. Although the experience with Ta TME is still limited, it might be a promising alternative to laparoscopic TME, especially for difficult cases where laparoscopy is too demanding. The preliminary data on complications and short-term oncological outcomes are good, but also emphasize the importance of careful patient selection.
