1Induction of antiproliferative effect by diosgenin through activation of p53, release of apoptosis-inducing factor (AIF) and modulation of caspase-3 activity in different human cancer cells显示文摘Previously, we demonstrated that a plant steroid, diosgenin, altered cell cycle distribution and induced apoptosis in the human osteosarcoma 1547 cell line. The objective of this study was to investigate if the antiproliferative effect of diosgenin was similar for different human cancer cell lines such as laryngocarcinoma HEp-2 and melanoma M4Beu cells. Moreover, this work essentially focused on the mitochondrial pathway. We found that diosgenin had an important and similar antiproliferative effect on different types of cancer cells. In addition, our new results show that diosgenininduced apoptosis is caspase-3 dependent with a fall of mitochondrial membrane potential, nuclear localization of AIF and poly (ADP-ribose) polymerase cleavage. 4Cytoreductive surgery and intraperitoneal chemotherapy for colorectal peritoneal metastases显示文摘AIM:To systematically review the available evidence regarding cytoreductive surgery(CRS) and intraperitoneal chemotherapy(IPC) for colorectal peritoneal metastases(CPM).METHODS:An electronic literature search was carried out to identify publications reporting oncological outcome data(overall survival and/or disease free survival and/or recurrence rates)following CRS and IPC for treatment of CPM.Studies reporting outcomes following CRS and IPC for cancer subtypes other than colorectal were only included if data were reported independently for colorectal cancer-associated cases;in addition studies reporting outcomes for peritoneal carcinomatosis of appendiceal origin were excluded.RESULTS:Twenty seven studies,published between1999 and 2013 with a combined population of 2838patients met the predefined inclusion criteria.Included studies comprised 21 case series,5 case-control studies and 1 randomised controlled trial.Four studies provided comparative oncological outcome data for patients undergoing CRS in combination with IPC vs systemic chemotherapy alone.The primary indication for treatment was CPM in 96%of cases(2714/2838)and recurrent CPM(rCPM)in the remaining 4%(124/2838).In the majority of included studies(20/27)CRS was combined with hyperthermic intraperitoneal chemotherapy(HIPEC).In 3 studies HIPEC was used in combination with early post-operative intraperitoneal chemotherapy(EPIC),and 2 studies used EPIC only,following CRS.Two studies evaluated comparative outcomes with CRS+HIPEC vs CRS+EPIC for treatment of CPM.The delivery of IPC was performed using an'open'or'closed'abdomen approach in the included studies.CONCLUSION:The available evidence presented in this review indicates that enhanced survival times can be achieved for CPM after combined treatment with CRS and IPC.Reza Mirnezami Brendan J Moran Kate Harvey Tom Cecil Kandiah Chandrakumaran Norman Carr Faheez Mohamed Alexander H Mirnezami 2014World Journal of Gastroenterology2014,20,38:6
5不同针头设计的预灌封注射器用于预防接种的对比研究显示文摘目的探讨5切面27G与3切面25G两种针头设计的预灌封注射器在受试者的疼痛感、接种护士的穿刺阻力以及注射部位不良事件等预防接种实践中的差异。方法采用随机双盲平行对照临床试验。将604例受试者随机分为2组,试验组299例使用5切面27G预灌封注射器,对照组305例使用3切面25G预灌封注射器接种甲肝疫苗,同时2组均使用3切面25G预灌封注射器接种乙肝疫苗作为自身对照。比较受试者疼痛与穿刺阻力,并比较受试者注射部位皮肤不良反应情况。结果采用视觉模拟量表(VAS)和疼痛程度语言等级量表中文版(C-PIVRS)评分法对甲肝疫苗穿刺时的疼痛感进行分级评价,试验组中等及以上疼痛比例分别为7.02%、16.55%;对照组分别为15.73%、20.32%,组间差异有统计学意义(均P≤0.001)。对照组注射甲肝时穿刺皮肤阻力VAS评分显著高于试验组(P<0.001)。试验组注射部位皮肤不良反应发生率低于对照组,分别为47.49%和65.25%(P<0.001)。结论5切面27G预灌封注射器用于疫苗接种时,受试者的疼痛度、针头的穿刺阻力以及注射部位不良事件发生率显著低于3切面25G针头设计的预灌封注射器。吕敏 白霜 高培 王怀 富继业 史如晶 董琳 Florence Schwarzenbach Cecile Berteau 吴疆 2019中华全科医学2019,17,11:6

7Outcomes in randomized controlled trials of exercise interventions in solid organ transplant显示文摘AIM To identify the outcome measures that have been used in randomized controlled trials(RCTs) of exercise training in solid organ transplant(SOT) recipients and to link these outcomes to the International Classification of Functioning, Disability and Health(ICF) framework.METHODS Electronic literature searches of MEDLINE, EMBASE, CINAHL, Cochrane, Scopus, and Web of Science were performed. We sought RCTs that investigated the effect of exercise training in SOT recipients. Reference lists of all eligible publications were searched for other appropriate studies not identified by the electronic search. A complete list of outcome measures used in the RCTs was generated and each of these was linked to an ICF category.RESULTS Four hundred and thirteen articles were retrieved, of which 35 met our inclusion criteria. The studies included were designed to compare the effects of exercise training programs to usual care or to another exercise training program and reported on recipients of heart(n = 21), kidney(n = 9), lung(n = 3) or liver(n = 2) transplant. Of the 126 outcome measures identified, 62 were used as primary outcome measures. The most commonly occurring primary outcomes were aerobic capacity using the peak VO2(n = 13), quality of life using the shortform-36(n = 8), and muscle strength(n = 7). Theseoutcome measures were linked to 113 ICF categories and the majority of outcomes fall into the body function domain(n = 93). CONCLUSION There is little standardization in outcome measures used in RCTs of exercise interventions in SOT recipients. 8Is it growing exponentially fast? -Impact of assuming exponential growth for characterizing and forecasting epidemics with initial near-exponential growth dynamics显示文摘The increasing use of mathematical models for epidemic forecasting has highlighted the importance of designing models that capture the baseline transmission characteristics in order to generate reliable epidemic forecasts.Improved models for epidemic forecasting could be achieved by identifying signature features of epidemic growth,which could inform the design of models of disease spread and reveal important characteristics of the transmission process.In particular,it is often taken for granted that the early growth phase of different growth processes in nature follow early exponential growth dynamics.In the context of infectious disease spread,this assumption is often convenient to describe a transmission process with mass action kinetics using differential equations and generate analytic expressions and estimates of the reproduction number.In this article,we carry out a simulation study to illustrate the impact of incorrectly assuming an exponential-growth model to characterize the early phase(e.g.,3e5 disease generation intervals)of an infectious disease outbreak that follows near-exponential growth dynamics.Specifically,we assess the impact on:1)goodness of fit,2)bias on the growth parameter,and 3)the impact on short-term epidemic forecasts.Our findings indicate that devising transmission models and statistical approaches that more flexibly capture the profile of epidemic growth could lead to enhanced model fit,improved estimates of key transmission parameters,and more realistic epidemic forecasts.Gerardo Chowell Cecile Viboud 2016Infectious Disease Modelling2016,1,1:4
9Diabetic nephropathy:Treatment with phosphodiesterase type 5 inhibitors显示文摘The importance of nitric oxide(NO) in vascular physiology is irrefutable;it stimulates the intracellular production of cyclic guanosine monophosphate(cGMP),initiating vascular smooth muscle relaxation.This biochemical process increases the diameter of small arteries,regulating blood flow distribution between arterioles and the microvasculature.The kidney is no exception,since NO predominantly dilates the glomerular afferent arterioles.It is now evident that the vascular production of cGMP can be augmented by inhibitors of phosphodiesterase type 5(PDE 5),the enzyme which breakdowns this cyclic nucleotide.This has clinical relevance,since diabetic nephropathy(DN) a major microvascular complication of diabetes mellitus and the most common cause of end-stage renal disease,increases intraglomerular capillary pressure,leading to glomerular hypertension.PDE 5 inhibitors may have,therefore,the potential to reduce glomerular hypertension.This review describes the use of PDE 5 inhibitors to improve the metabolic,haemodynamic and inflammatory pathways/responses,all of which are dysfunctional in DN.Cecil Stanley Thompson 2013World Journal of Diabetes2013,4,4:4

20A conceptual framework for ex ante valuation of ecosystem services of brownfield greening from a systematic perspective显示文摘Introduction:Although Brownfield greening(BG)can be a crucial solution to green space deficiency in dense urban areas,the potential benefits of different BG initiatives have rarely been quantitatively pre-evaluated.Here,the concept and main features of BG and its costs and benefits are firstly depicted.Next,a conceptual framework is presented which combines ecosystem service(ESS)valuation,economic cost-benefit analysis,and spatial pattern analysis.The framework is used to perform ex ante valuation of the ESS value of BG in Xuhui District,Shanghai.Outcomes:The scenario comparison results show that it can spatially reflect a closer-to-reality value of ESS at the urban scale instead of a theoretical value at the site scale.The ultimate values of ESS(After economic and landscape adjustments)in the composite scenario is more than 21%higher than those in the full-eco scenario,whereas the initial values of ESS(before the adjustments)in the former was 14%lower than those in the latter.These results suggest that a considerable part of brownfield greening projects need to be implemented in the more populated and economically vibrant areas instead of solely in the ecological construction zone,thereby correspondingly providing more cultural and regulation services and forming a better urban green space network.Conclusion:The framework provides a simple,science-based,and feasible tool for making BG decisions from a systematic perspective in dense urban areas.It can help meet the need for applying ESS knowledge to BG-based green space planning and policy making in the context of urban sustainable development.Qicheng Zhong Lang Zhang Yi Zhu Cecil Konijnendijk van den Bosch Jigang Han Guilian Zhang Yuezhong Li 2020Ecosystem Health and Sustainability2020,6,1:2
