1Portal vein thrombosis in liver cirrhosis显示文摘Portal vein thrombosis(PVT) is considered to be a frequent complication of liver cirrhosis. However, unlike PVT in patients without cirrhosis, very few data are available on the natural history and management of PVT in cirrhosis, despite its association with potentially life-threatening conditions, such as gastroesophageal bleeding and acute intestinal ischemia. Moreover, no consensus regarding PVT in cirrhosis exists. Suggested causes of PVT in cirrhosis include reduced portal blood flow velocity, multiple congenital or acquired thrombophilic factors, inherited or acquired conditions, and derangement of liver architecture. However, the understanding of PVT in cirrhosis is incomplete. In addition, information on the management of PVT in cirrhosis is inadequate. 2Diagnosis and Treatment of Keloids and Hypertrophic Scars—Japan Scar Workshop Consensus Document 2018显示文摘There has been a long-standing need for guidelines on the diagnosis and treatment of keloids and hypertrophic scars that are based on an understanding of the pathomechanisms that underlie these skin fibrotic diseases.This is particularly true for clinicians who deal with Asian and African patients because these ethnicities are highly prone to these diseases.By contrast,Caucasians are less likely to develop keloids and hypertrophic scars,and if they do,the scars tend not to be severe.This ethnic disparity also means that countries vary in terms of their differential diagnostic algorithms.The lack of clear treatment guidelines also means that primary care physicians are currently applying a hotchpotch of treatments,with uneven outcomes.To overcome these issues,the Japan Scar Workshop(JSW)has created a tool that allows clinicians to objectively diagnose and distinguish between keloids,hypertrophic scars,and mature scars.This tool is called the JSW Scar Scale(JSS)and it involves scoring the risk factors of the individual patients and the affected areas.The tool is simple and easy to use.As a result,even physicians who are not accustomed to keloids and hypertrophic scars can easily diagnose them and judge their severity.The JSW has also established a committee that,in cooperation with outside experts in various fields,has prepared a Consensus Document on keloid and hypertrophic scar treatment guidelines.These guidelines are simple and will allow even inexperienced clinicians to choose the most appropriate treatment strategy.The Consensus Document is provided in this article.It describes(1)the diagnostic algorithm for pathological scars and how to differentiate them from clinically similar benign and malignant tumors,(2)the general treatment algorithms for keloids and hypertrophic scars at different medical facilities,(3)the rationale behind each treatment for keloids and hypertrophic scars,and(4)the body site-specific treatment protocols for these scars.We believe that this Consensus Document will be helpful for physicians from all over the world who treat keloids and hypertrophic scars.Rei Ogawa Sadanori Akita Satoshi Akaishi Noriko Aramaki-Hattori Teruyuki Dohi Toshihiko Hayashi Kazuo Kishi Taro Kono Hajime Matsumura Gan Muneuchi Naoki Murao Munetomo Nagao Keisuke Okabe Fumiaki Shimizu Mamiko Tosa Yasuyoshi Tosa Satoko Yamawaki Shinichi Ansai Norihisa Inazu Toshiko Kamo Reiko Kazki Shigehiko Kuribayashi 2019Burns & Trauma2019,7,1:31

4Diagnostic efficacy of gadoxetic acid-enhanced MRI for hepatocellular carcinoma and dysplastic nodule显示文摘AIM:To evaluate the relationship between the signal intensity of hepatobiliary phase images on gadoxetic acid-enhanced magnetic resonance imaging(MRI)and histological grade.METHODS:Fifty-nine patients with 82 hepatocellular lesions were evaluated retrospectively.Hepatobiliary phase images on gadoxetic acid-enhanced MRI were classified into 3 groups:low,iso or high.Angiographyassisted computed tomography(CT)findings were also classified into 3 groups:CT during arterial portography,and CT hepatic arteriography:A:iso,iso or low;B:slightly low,iso or low;;and C:low,high.We correlated angiography-assisted CT,hepatobiliary phase findings during gadoxetic acid-enhanced MRI and histological grades.Furthermore,correlations between MRI findings and histological grade for each hemodynamic pattern were performed.Correlations among radiologicaland pathological findings were statistically evaluated using the chi-square test and Fisher's exact test.RESULTS:There was a significant correlation between histological grade and hemodynamic pattern(P<0.05).There was a significant correlation between histological grade and signal intensity in the hepatobiliary phase(P<0.05)in group A lesions.There was no significant correlation between histological grade and signal intensity in the hepatobiliary phase in group B or C lesions(P>0.05).CONCLUSION:Signal intensity in the hepatobiliary phase correlated with histological grade in the lesions that maintained portal blood flow,but did not correlate in lesions that showed decreased or defective portal blood flow.Kazuhiro Saito Ryota Nishio Toru Saguchi Soichi Akata Koichi Tokuuye Fuminori Moriyasu Katsutoshi Sugimoto Junichi Taira Toshitaka Nagao 2011World Journal of Gastroenterology2011,17,30:12
5Silver Nanowire Electrodes: Conductivity Improvement Without Post-treatment and Application in Capacitive Pressure Sensors显示文摘Transparent electrode based on silver nanowires(Ag NWs) emerges as an outstanding alternative of indium tin oxide film especially for flexible electronics. However, the conductivity of Ag NWs transparent electrode is still dramatically limited by the contact resistance between nanowires at high transmittance. Polyvinylpyrrolidone(PVP) layer adsorbed on the nanowire surface acts as an electrically insulating barrier at wire–wire junctions, and some devastating post-treatment methods are proposed to reduce or eliminate PVP layer, which usually limit the application of the substrates susceptible to heat or pressure and burden the fabrication with high-cost, time-consuming, or inefficient processes. In this work, a simple and rapid pre-treatment washing method was proposed to reduce the thickness of PVP layer from 13.19 to0.96 nm and improve the contact between wires. Ag NW electrodes with sheet resistances of 15.6 and 204 X sq-1have been achieved at transmittances of 90 and 97.5 %, respectively. This method avoided any post-treatments and popularized the application of high-performance Ag NW transparent electrode on more substrates. 6Ore-forming mechanism for the Xiaoxinancha Au-rich Cu deposit in Yanbian,Jilin Province,China:Evidence from noble gas isotope geochemistry of fluid inclusions in minerals显示文摘The Xiaoxinancha Au-rich copper deposit is one of important Au-Cu deposits along the continental margin in Eastern China. The deposit consists of two sections: the Beishan mine (North), composed of altered rocks with veinlet-dissemination sulfides and melnicovite-dominated sulfide-quartz veins, and the Nanshan mine (South), composed of pyrrhotite-dominated sulfide-quartz veins and pure sulfide veins. The isotope compositions of noble gases extracted from fluid inclusions in ore minerals, i.e. ratios of 3He/4He, 20Ne/22Ne and40Ar/36Ar are in the ranges of 4.45―0.08 Ra, 10.2―8.8 and 306―430, respectively. Fluid inclusions in minerals from the Nanshan mine have higher 3He/4He and 20Ne/22Ne ratios whereas those from the Beishan mine have lower 3He/4He ratios. The analysis of origin, and evolution of the ore fluids and its relations with the ore-forming stages and the ages of mineralization suggests that the initial hydrothermal fluids probably come from the melts generated by partial melting of oceanic crust with the participation of fluids from the mantle (mantle-plume type)/aesthenosphere. This also corresponds to the continental margin settings during the subduction of Izanagi ocaneic plate towards the palaeo-Asian continent (123―102 Ma). The veinlet-dissemination ore bodies of the Beishan mine were formed through replacement and crystallization of the mixed fluids generated by mixing of the ascending high-temperature boiling fluid with young crustal fluid whereas the melnicovite-dominated sulfide-quartz veins were formed subsequently by filling of the high-temperature ore fluid in fissures. Pyrrhotite-dominated sulfide-quartz veins in the Nanshan mine were formed by filling-deposition-crystallization of the moderate-temperature ore fluids and the pure sulfide veins were formed later by filling-deposition-crystallization of ore substance-rich fluids after boiling of the moderate-temperature ore fluids. The metallogenic dynamic processes can be summarized as: (1) formation of fluidand ore substance-bearing Adakitic magma by degassing, dewatering and partial melting during subduction of the Izanagi plate; (2) separation and formation of ore fluids from the Adakitic magma; and (3) success-sive ascending of the ore fluids and final formation of the Au-rich Cu deposit of veinlet-dissemination and vein types by secondary boiling.KEISUKE Nagao HIROCHIKA Sumino 2008Science China Earth Sciences2008,51,2:9

11Electrodeposition and growth mechanism of preferentially orientated nanotwinned Cu on silicon wafer substrate显示文摘Homogeneous columnar Cu film with fully embedded nanotwins was successfully fabricated on Ti/Cu seed layer on silicon wafer. The nanotwins with thickness of tens of nanometers are generally parallel to the silicon surface, showing a strong(111) preferred orientation. The acid concentration was found to be important in influencing the formation of nanoscale twins. By adjusting the acid concentration,the nanotwins can be induced from the top columnar grain to middle columnar grain and reach the bottom equiaxed grain, and a microstructural transformation model was given. A theory focusing on the cathode overpotential was proposed to reveal the effect of acid concentration on the growth mechanism of nanoscale twins. 12Real-world efficacy of daclatasvir and asunaprevir with respect to resistance-associated substitutions显示文摘AIM To investigate daclatasvir(DCV) and asunaprevir(ASV) efficacy in hepatitis C(HCV) patients, with respect to resistance-associated substitutions(RASs).METHODS A total of 392 HCV-infected patients from multiple centers were included in this study. We evaluated their clinical courses and sustained virologic responses(SVR) according to pretreatment factors(gender, age, history of interferon-based regimens, platelet counts, level of viremia, pretreatment NA5A:L31, and Y93 substitutions). We also analyzed the pretreatment and post-treatment major RASs of NS3:D168, NS5A:L31 and Y93 substitutions using a direct-sequencing method in 17 patients who were unable to achieve SVR at 12 wk after treatment completion(SVR12).RESULTS The overall SVR12 rate was 88.3%. Thirty-one patients discontinued treatment before 24 wk because of adverse events, 23 of whom achieved SVR12. There were no significant differences in SVR12 rates with respect to gender, age, history of interferon-based regimens, and platelet counts. The SVR12 rate in patients with viral loads of ≥ 6.0 log IU/m L was significantly lower than those with viral loads of < 6.0 log IU/m L(P < 0.001). The SVR12 rate in patients with Y93 substitution-positive was significantly lower than those with Y93 substitution-negative(P < 0.001). The L31 substitution-positive group showed a lower SVR12 rate than the L31 substitution-negative group, but the difference was not statistically significant. Seventeen patients who did not achieve SVR12 and had available pretreatment and post-treatment sera had additional RASs in NS3:D168, NS5:L31, and Y93 substitution at treatment failure.CONCLUSION Combination of DCV and ASV is associated with a high SVR rate. 15Laparoscopic-assisted catheter insertion for continuous ambulatory peritoneal dialysis:A case report of simple technique for optimal placement显示文摘A 40-year-old male underwent tube placement surgery for continuous ambulatory peritoneal dialysis(CAPD).A2-cm skin incision was made,and the peritoneum was reflected enough to perform secure fixation.A swannecked,double-felted silicone CAPD catheter was inserted,and the felt cuff was sutured to the peritoneum to avoid postoperative leakage.An adequate gradient for tube fixation to the abdominal wall was confirmed.The CAPD tube was passed through a subcutaneous tunnel.Aeroperitoneum was induced to confirm that there was no air leakage from the sites of CAPD insertion.Two trocars were placed,and we confirmed that the CAPD tube led to the rectovesical pouch.Tip position was reliably observed laparoscopically.Optimal patency of the CAPD tube was confirmed during surgery.Placement of CAPD catheters by laparoscopic-assisted surgery has clear advantages in simplicity,safety,flexibility,and certainty.Laparoscopic technique should be considered the first choice for CAPD tube insertion.Tomohide Hori Masaya Nakauchi Kazuhiro Nagao Fumitaka Oike Takahiro Tanaka Daigo Gunji Noriyuki Okada 2013World Journal of Gastrointestinal Surgery2013,5,10:2

20Diffusion-weighted imaging might be useful for reactive lymphoid hyperplasia diagnosis of the liver: A case report显示文摘BACKGROUND Reactive lymphoid hyperplasia(RLH)of the liver is a rare liver lesion.It is considered difficult to differentiate radiologically from hepatocellular carcinoma,metastatic liver tumor and other pathologies.CASE SUMMARY A 54-year-old woman presented to our hospital with RLH of the liver.The patient had a diagnosis of metastatic carcinoma of the liver from an unknown origin and subsequently underwent partial hepatectomy.However,histopathological analysis revealed RLH.The lesion showed perinodular enhancement in the arterial phase on contrast-enhanced computed tomography and magnetic resonance imaging.On diffusion-weighted imaging(DWI),we encountered linear hyperintensity along the portal tract consecutive to the liver lesion,which is a new characteristic radiologic finding.This finding corresponded to the lymphoid cell infiltration of the portal tract.Furthermore,there was strongly restricted diffusion on the apparent diffusion coefficient map.We used these characteristic radiologic findings to diagnose the lesion as a lymphoproliferative disease.CONCLUSION The linear hyperintensity consecutive to the liver lesion on DWI provided additional valuable diagnostic information.Taro Tanaka Kazuhiro Saito Daisuke Yunaiyama Jun Matsubayashi Yuichi Nagakawa Maki Tanigawa Toshitaka Nagao 2020World Journal of Clinical Cases2020,8,21:2
