| 1 | Diagnostic and therapeutic single-operator cholangiopancreatoscopy in biliopancreatic diseases:Prospective multicenter study in Japan显示文摘AIM: To assess the utility and safety of single-operator cholangiopancreatoscopy(SOCPS) using the Spy Glass system in widespread clinical application for biliary and pancreatic diseases.METHODS: This study was a prospective case series conducted in 20 referral centers in Japan. There were 148 patients who underwent SOCPS; 124 for biliary diseases and 24 for pancreatic diseases. The attempted interventions were SOCPS examination, SOCPS-directed tissue sampling, and therapy for stone removal, among others. The main outcomes were related to the procedure success rate in terms of visualizing the target lesions, SOCPS-directed adequate tissue sampling, and complete stone removal. RESULTS: A total of 148 patients were enrolled for the diagnosis of indeterminate biliary and pancreatic lesions or treatment of biliary and pancreatic disease. The overall procedure success rate of visualizing the target lesions was 91.2%(135/148). The overall procedural success rates of visualizing the target lesions of diagnostic SOCPS in the bile duct and pancreatic duct were 95.5%(84/89) and 88.2%(15/17), respectively. Diagnosis: the overall adequate tissue for histologic examination was secured in 81.4% of the 86 patients who underwent biopsy under SOCPS(bile duct, 60/75, 80.0%; pancreatic duct, 10/11, 90.9%). The accuracy of histologic diagnosis using SOCPS-directed biopsies in indeterminate bile duct lesions was 70.7%(53/75). In the pancreatic duct, the accuracy of SOCPS visual impression of intraductal papillary mucinous neoplasm was 87.5%(14/16). Stone therapy: complete biliary and pancreatic stone clearance combined with SOCPS-directed stone therapy using electrohydraulic lithotripsy or laser lithotripsy was achieved in 74.2%(23/31) and 42.9%(3/7) of the patients, respectively. Others: SOCPS using the Spy Glass system was used in cannulation of the cystic duct in two patients and for passing across the obstructed self-expandable metallic stent for a malignant biliary stricture in two patients. All procedures were successful in both SOCPS-guided therapies. The incidence of procedure-related adverse events was 5.4%(8/148). | 3 | Applications of same-beam VLBI in the orbit determination of multi-spacecrafts in a lunar sample-return mission显示文摘Same-beam VLBI means that two spacecrafts with small separation angles that transmit multi-frequency signals specially designed are observed simultaneously through the main beam of receiving antennas. In same-beam VLBI,the differential phase delay between the two spacecrafts and the two receiving antennas can be obtained within a small error of several picoseconds. As a successful application,the short-arc orbit determination of several hours for Rstar and Vstar,which are two small sub-spacecrafts of SELENE,has been much improved by using the same-beam VLBI data together with the Doppler and range data. The long-arc orbit determination of several days has also been accomplished within an error of about 10 m with the same-beam VLBI data incorporated. These results show the value of the same-beam VLBI for the orbit determination of multi-spacecrafts. This paper introduces the same-beam VLBI and Doppler observations of SELENE and the orbit determination results. In addition,this paper introduces how to use the same-beam VLBI for a lunar sample-return mission,which usually consists of an orbiter,a lander and a return unit. | 5 | High-accuracy same-beam VLBI observations using Shanghai and Urumqi telescopes显示文摘The same-beam VLBI observations of Rstar and Vstar,which were two small satellites of Japanese lunar mission,SELENE,were successfully performed by using Shanghai and Urumqi 25-m telescopes. When the separation angle between Rstar and Vstar was less than 0.1 deg,the differential phase delay of the X-band signals between Rstar and Vstar on Shanghai-Urumqi baseline was obtained with a very small error of 0.15 mm rms,which was reduced by 1-2 order compared with the former VLBI results. When the separation angle was less than 0.56 deg,the differential phase delay of the S-band signals was also obtained with a very small error of several mm rms. The orbit determination for Rstar and Vstar was performed,and the accuracy was improved to a level of several meters by using VLBI and Doppler data. | 8 | Familial pancreatic cancer: Concept, management and issues显示文摘Familial pancreatic cancer (FPC) is broadly defined as two first-degree-relatives with pancreatic cancer (PC) and accounts for 4%-10% of PC. Several genetic syndromes, including Peutz-Jeghers syndrome, hereditary pancreatitis, hereditary breast-ovarian cancer syndrome(HBOC), Lynch syndrome, and familial adenomatous polyposis (FAP), also have increased risks of PC, but the narrowest definition of FPC excludes these known syndromes. When compared with other familial tumors, proven genetic alterations are limited to a small proportion (<20%) and the familial aggregation is usually modest. However, an ethnic deviation (Ashkenazi Jewish>Caucasian) and a younger onset are common also in FPC. In European countries, 'anticipation' is reported in FPC families, as with other hereditary syndromes; a trend toward younger age and worse prognosis is recognized in the late years. The resected pancreases of FPC kindred often show multiple pancreatic intraepithelial neoplasia (Pan IN) foci, with various K-ras mutations, similar to colorectal polyposis seen in the FAP patients. As with HBOC patients, a patient who is a BRCA mutation carrier with unresectable pancreatic cancer (accounting for 0%-19% of FPC patients) demonstrated better outcome following platinum and Poly (ADP-ribose) polymerase inhibitor treatment. Western countries have established FPC registries since the 1990 s and several surveillance projects for highrisk individuals are now ongoing to detect early PCs. Improvement in lifestyle habits, including non-smoking, is recommended for individuals at risk. | 15 | Determination of the free lunar libration modes from ephemeris DE430显示文摘The Moon's physical librations have been extensively studied, and elaborate researches have been developed for the purpose of deriving accurate modes of free librations. Our motivation comes from the Planetary and Lunar Ephemeris DE430 by JPL/NASA, which was created in April 2013,and is reported to be the most accurate lunar ephemeris today using the data from Gravity Recovery and Interior Laboratory(GRAIL). Therefore, the residuals after fitting the model have reduced owing to improvement in the libration models, and the free librations embedded in the Euler angles have also improved. We use Fourier analysis to extract the approximate frequencies from DE430 and then a quadratic interpolation method is used to determine higher accuracy frequencies. With the frequencies,the linear least-squares fitting method is employed to fit the lunar physical librations to DE430. From this analysis we identified the three modes of free physical librations, and estimated the amplitudes as 1.471′′in longitude, 0.025′′in latitude and 8.19′′× 3.31′′for the wobble, with the respective periods of1056.16, 8806.9 and 27262.99 d. | 18 | AIF-regulated oxidative phosphorylation supports lung cancer development显示文摘Cancer is a major and still increasing cause of death in humans. Most cancer cells have a fundamentally different metabolic profile from that of normal tissue. This shift away from mitochondrial ATP synthesis via oxidative phosphorylation towards a high rate of glycolysis, termed Warburg effect, has long been recognized as a paradigmatic hallmark of cancer, supporting the increased biosynthetic demands of tumor cells. Here we show that deletion of apoptosis-inducing factor (AIF) in a Kras^G12D-driven mouse lung cancer model resulted in a marked survival advantage, with delayed tumor onset and decreased malignant progression. Mechanistically, Aif deletion leads to oxidative phosphorylation (OXPHOS) deficiency and a switch in cellular metabolism towards glycolysis in non-transformed pneumocytes and at early stages of tumor development. Paradoxically, although Aif-deficient cells exhibited a metabolic Warburg profile, this bioenergetic change resulted in a growth disadvantage of Kras^G12D-driven as well as Kras wild-type lung cancer cells. Cell-autonomous re-expression of both wild-type and mutant AIF (displaying an intact mitochondrial, but abrogated apoptotic function) in Aif-knockout Kras^G12D mice restored OXPHOS and reduced animal survival to the same level as AIF wild-type mice. In patients with non-small cell lung cancer, high AIF expression was associated with poor prognosis.
