1Charlson comorbidity index helps predict the risk of mortality for patients with type 2 diabetic nephropathy显示文摘研究目的:探讨Charlson合并症指数(CCI)对2型糖尿病肾病死亡率的预测作用。研究方法:建立2型糖尿病肾病研究队列,根据CCI评分将患者合并症的严重程度分为三度:轻度(CCI1–2分)、中度(CCI 3–4分)、重度(CCI≥5分)。将影响死亡率的因素及按CCI分层后的组间差异指标进行Logistic回归分析及方差分析(ANOVA)。Kaplan-Meier生存曲线分析CCI指数对生存时间及死亡率的影响。重要结论:533例2型糖尿病肾病患者纳入研究。所有患者的CCI评分均大于1,44.7%(238/533)的患者死亡。患者的死亡率随CCI评分增加而增加,CCI 1–2分患者的死亡率为21.0%(50/238),CCI 3–4分患者的死亡率为56.7%(135/238),CCI≥5分患者的死亡率为22.3%(53/238)。Logistic回归分析显示CCI评分、血红蛋白和血浆白蛋白水平是患者死亡率的预测因子(P<0.05)。方差分析结果显示,与CCI评分相对较低的患者相比,CCI评分越高的患者其血红蛋白水平较低,而血清肌酐较高,死亡率也更高。Kaplan-Meier生存曲线显示CCI评分越高的患者生存时间越短。CCI评分是一种简单易行且实用的评估疾病合并症的方法,可用于预测2型糖尿病肾病患者的死亡率。关注2型糖尿病肾病患者的合并症将有利于早期、有效治疗及改善预后。You-qun HUANG Rong GOU Yong-shu DIAO Qing-hua YIN Wen-xing FAN Ya-ping LIANG Yi CHEN Min WU Li ZANG Ling LI Jing ZANG Lu CHENG Ping FU Fang LIU 2014Journal of Zhejiang University-Science B(Biomedicine & Biotechnology)2014,15,1:23

5Establishment of a transgenic mouse model with liver-specific expression of secretory immunoglobulin D显示文摘Mutation of mevalonate kinase(MVK) is thought to account for most cases of hyperimmunoglobulinemia D syndrome(HIDS) with recurrent fever.However,its mechanism and the relationship between elevated serum immunoglobulin D(IgD) and the clinical features of HIDS are unclear.In this study,we generated by fusion PCR a vector to express high levels of chimeric secretory IgD(csIgD) specifically in the liver.We then generated seven founder lines of transgenic mice by co-microinjection,and verified them using genomic PCR and Southern blotting.We detected the expression of csIgD by reverse transcription PCR,quantitative PCR,western blotting,and enzyme-linked immunosorbent assays.We demonstrated that csIgD could be specifically and stably expressed in the liver.We used flow cytometry to show that overexpression of csIgD in the bone marrow and spleen cells had no effect on B cell development.Morphologic and anatomical observation of the transgenic mice revealed skin damage,hepatosplenomegaly,and nephromegaly in some transgenic mice;in these mice,pathological sections showed high levels of cell necrosis and protein-like sediments in the liver,spleen,and kidney.We demonstrated that the genomic insertion sites of the transgenes did not disrupt the MVK gene on mouse chromosome 5.This transgenic mouse will be useful to explore the pathogenesis of HIDS.WANG Ping WEI ZhiGuo YAN BoWen HUANG Tan GOU KeMian DAI YunPing ZHENG Min WANG MeiLi CHENG XueQian WANG XiFeng XU Chen SUN Yi 2012Science China(Life Sciences)2012,55,3:6
6Mechanical and fatigue properties of self-piercing riveted joints in high-strength steel and aluminium alloy显示文摘Static tensile and fatigue tests were performed on shear and tensile self-piercing riveted aluminium-steel structures to evaluate their mechanical and fatigue properties. The influences of the thickness and the strength of the highstrength steel on mechanical and fatigue performances were investigated based on the tensile and F-N curves of the joints. The results show that mechanical and fatigue properties of the shear self-piercing riveted joints are much better than those of the tensile self-piercing riveted joints. Mechanical and fatigue performances of the two joints were significantly influenced by the thickness and strength of the steel sheet,and were markedly improved when the thickness of steel sheet increased. The steel strength showed significantly different effects on shear and tensile riveted structures,i. e.,when the steel strength increased,the strength of the shear structure greatly increased while the tensile structure just had a slight increase in the strength. 7Combinatorial effects of conception and governor vessel electroacu-puncture and human umbilical cord blood-derived mesenchymal stem cells on pathomorphologic lesion and cellular apoptosis in rats with cerebral ischemia/reperfusion显示文摘OBJECTIVE: To investigate the combinatorial effects of conception and governor vessel electroacupuncture(EA) and human umbilical cord blood-derived mesenchymal stem cells(HUCB-MSCs) on pathomorphologic lesion and cellular apoptosis in rats with cerebral ischemia/reperfusion. METHODS: With the HUCB-MSCs isolated, cultured and identified and the models of cerebral ischemia-reperfusion established, the HUCB-MSCs of passage three were intracranially transplanted and the EA at conception and governor vessels was applied. The pathomorphologic lesion by hematoxylin-eosin staining and the cellular apoptosis by terminal deoxynucleotidyl transferase-mediated nick-end labeling method around the ischemic focus were observed. RESULTS: The cultured adherent HUCB-MSCs exhibited a spindle shape and expressed MSC-specific markers, with the cell purity and proliferation rate significantly increasing after the primary passage. HE staining showed that there were no pathological changes observed in the sham surgery group. However, in the PBS transplantation group, degeneration and necrosis of a great number of nerve cells were seen. In both the HUCB-MSCs transplantation group and the HUCB-MSCs transplantation + EA group, reparative changes of the pathomorphism of the tissue were found. Both combination treatment and simple MSCs treatment were able to improve the pathomorphorlogic lesion following cerebral ischemia and reduce the abnormal TUNEL-positive numbers, with former better than latter. CONCLUSION: HUCB-MSCs improve pathological lesions and inhibit the cellular apoptosis around the cerebral ischemic area. 8Total Energy Expenditure of 16 Chinese Young Men Measured by the Doubly Labeled Water Method显示文摘Objective Doubly labeled water (DLW) method is the gold standard for measuring total energy expenditure (TEE). We used this method to measure TEE in Chinese young men. Methods Sixteen healthy young men age 23±1 years with body mass index 22.0±1.4 kg/m 2 were recruited. TEE was measured by the DLW method, and basal energy expenditure (BEE) was determined by indirect calorimetry. We also conducted 24-h activity, energy balance and factorial approach to estimate energy requirements of the subjects. Results TEE of subjects by DLW method was 9.45±0.57 MJ/day (2258±180 kcal/day). The 24-h activity was 10.80±0.33 MJ/day (2582±136 kcal/day). The energy requirement, derived from energy balance observations, was 9.93±1.32 MJ/day (2373±315 kcal/day). The BEE of 6.65±0.28 MJ/day (1589±67 kcal/day), calculated by the adjusted Schofield equation, was significantly higher (P<0.001) than that measured by indirect calorimetry, 5.99±0.66 MJ/day (1433±158 kcal/day). The TEE derived from the factorial approach was 10.31±0.43 MJ/day (2463±104 kcal/day). Conclusion The TEE of Chinese young men measured by the DLW method was about 10% lower than the current recommended nutrient intake (RNI), suggesting that the RNI for Chinese men maybe overestimated.
