1Diagnostic value of elevated serum carbohydrate antigen 199 level in acute cholangitis secondary to choledocholithiasis显示文摘AIM To investigate the diagnostic value of abnormal serum carbohydrate antigen 199(CA199) level in acute cholangitis secondary to choledocholithiasis.METHODS In this retrospective cohort study, the clinical data of 727 patients with choledocholithiasis admitted to the Third Affiliated Hospital of Zunyi Medical College from June 2011 to June 2017 were collected. Among these patients, 258 patients had secondary acute cholangitis and served as observation group, and the remaining 569 choledocholithiasis patients served as the control group. Serum liver function indexes and tumor markers were detected in both groups, and the receiver operating characteristic(ROC) curves were constructed for markers showing statistical significances. The cutoff value, sensitivity, and specificity of each marker were calculated according to the ROC curves. RESULTS The results of liver function tests showed no significant differences between the two groups(P > 0.05). Tumor markers including serum CA125, CA153, carcinoembryonic antigen, and alpha fetoprotein levels were also not significantly different(P > 0.05); however, the serum CA199 level was significantly higher in the observation group than in the control group(P < 0.05). The ROC curve analysis showed that the area under the curve was 0.885(95%CI: 0.841-0.929) for CA199, and the cutoff value of 52.5 kU/L had the highest diagnostic accuracy, with a sensitivity of 86.8% and a specificity of 81.6%.CONCLUSION Abnormally elevated serum CA199 level has an important value in the diagnosis of acute cholangitis secondary to choledocholithiasis. 2Combination of serum gamma-glutamyltransferase and alkaline phosphatase in predicting the diagnosis of asymptomatic choledocholithiasis secondary to cholecystolithiasis显示文摘BACKGROUND Gamma-glutamyltransferase(GGT) is one of the most important laboratory tests for the evaluation of liver damage. Through a long-term clinical observation of patients with secondary asymptomatic choledocholithiasis, we found that most patients had abnormal GGT serum levels.AIM To investigate the combination of serum GGT and alkaline phosphatase(ALP) in predicting the diagnosis of asymptomatic choledocholithiasis secondary to cholecystolithiasis.METHODS In this retrospective cohort study, the clinical data of 829 patients with cholecystolithiasis admitted to the Third Affiliated Hospital of Zunyi Medical College from August 2014 to August 2017 were collected. Among these patients,151 patients had secondary asymptomatic choledocholithiasis and served as the observation group, and the remaining 678 cholecystolithiasis patients served as the control group. Serum liver function indexes were detected in both groups,and the receiver operating characteristic(commonly known as ROC) curves were constructed for markers showing statistical significances. The cutoff value,sensitivity, and specificity of each marker were calculated according to the ROC curves.RESULTS The overall incidence of asymptomatic choledocholithiasis secondary to cholecystolithiasis was 18.2%. The results of liver function indexes including serum aspartate aminotransferase, alanine aminotransferase, direct bilirubin and total bilirubin levels showed no significant differences between the two groups(P> 0.05). However, the serum GGT and ALP levels were significantly higher in the observation group than in the control group(P < 0.05). The ROC curve analysis showed that the area under the curve was 0.881(95%CI: 0.830-0.932), 0.647(95%CI: 0.583-0.711) and 0.923(0.892-0.953) for GGT, ALP, and GGT + ALP,respectively. The corresponding cut-off values of GGT and ALP were 95.5 U/L and 151.5 U/L, sensitivity were 90.8% and 65.1%, and specificity were 83.6% and59.8%, respectively. The sensitivity and specificity of GGT + ALP were 93.5% and85.1%, respectively.CONCLUSION An abnormally elevated serum GGT level has an important value in the diagnosis of asymptomatic choledocholithiasis secondary to cholecystolithiasis.The combination of serum GGT and ALP has better diagnostic performance. 6Experimental study on permanent deformation characteristics of coarse-grained soil under repeated dynamic loading显示文摘Practical assessment of subgrade settlement induced by train operation requires developing suitable models capable of describing permanent deformation characteristics of subgrade filling under repeated dynamic loading.In this paper,repeated load triaxial tests were performed on coarse-grained soil(CGS),and the axial permanent strain of CGS under different confining pressures and dynamic stress amplitudes was analysed.Permanent deformation behaviors of CGS were categorized based on the variation trend of permanent strain rate with accumulated permanent strain and the shakedown theory.A prediction model of permanent deformation considering stress state and number of load cycles was established,and the ranges of parameters for different types of dynamic behaviors were also divided.The results indicated that the variational trend of permanent strain rate with accumulated permanent strain can be used as a basis for classifying dynamic behaviors of CGS.The stress state(confining pressure and dynamic stress amplitude)has significant effects on the permanent strain rate.The accumulative characteristics of permanent deformation of CGS with the number of load cycles can be described by a power function,and the model parameters can reflect the influence of confining pressure and dynamic stress amplitude.The study’s results could help deepen understanding of the permanent deformation characteristics of CGS.Huihao Mei Sajjad Satvati Wuming Leng 2021Railway Engineering Science2021,29,1:2
7Risk factors for liver disease among adults of Mexican descent in the United States and Mexico显示文摘AIM To compare the prevalence of chronic liver disease(CLD) risk factors in a representative sample of MexicanAmericans born in the United States(US) or Mexico, to a sample of adults in Mexico.METHODS Data for Mexican-Americans in the US were obtained from the 1999-2014 National Health and Nutrition Examination Survey(NHANES), which includes persons of Mexican origin living in the US(n = 4274). The NHANES sample was restricted to Mexican-American participants who were 20 years and older, born in the US or Mexico, not pregnant or breastfeeding, and with medical insurance.The data in Mexico were obtained from the 2004-2013 Health Worker Cohort Study in Cuernavaca, Mexico(n =9485). The following known risk factors for liver disease/cancer were evaluated: elevated aminotransferase levels(elevated alanine aminotransferase was defined as > 40 IU/L for males and females; elevated aspartate aminotransferase was defined as > 40 IU/L for males and females), infection with hepatitis B or hepatitis C,metabolic syndrome, high total cholesterol, diabetes,obesity, abdominal obesity, and heavy alcohol use. The main independent variables for this study classified individuals by country of residence(i.e., Mexico vs the US) and place of birth(i.e., US-born vs Mexico-born).Regression analyses were used to investigate CLD risk factors.RESULTS After adjusting for socio-demographic characteristics,Mexican-American males were more likely to be obese,diabetic, heavy/binge drinkers or have abdominal obesity than males in Mexico. The adjusted multivariate results for females also indicate that Mexican-American females were significantly more likely to be obese, diabetic, be heavy/binge drinkers or have abdominal obesity than Mexican females. The prevalence ratios and prevalence differences mirror the multivariate analysis findings for the aforementioned risk factors, showing a greater risk among US-born as compared to Mexico-born MexicanAmericans. 15Alanine aminotransferase is an inadequate surrogate marker for detecting lamivudine resistance显示文摘AIM: To investigate the accuracy of serum alanine aminotransferase (ALT) in diagnosing lamivudine resistance and factors that contributed to abnormal serum ALT.METHODS: This was a retrospective study of chronic hepatitis B patients on lamivudine therapy who were followed for 3-mo with liver function tests and hepatitis B virus (HBV) DNA measurement. Lamivudine resistance was defined as HBV DNA ≥ 1 log from nadir on at least 2 occasions, confirmed by genotyping. Serum ALT levels in patients with lamivudine resistance were compared to serum ALT levels in those without lamivudine resistance. RESULTS: There were 111 patients with and 117 without lamivudine resistance. The area under the receiver operating characteristic of serum ALT to diagnose lamivudine resistance was 0.645 ± 0.037. Serum ALT > 42.5 U/L gave the best diagnostic accuracy with sensitivity = 61%, specificity = 60%, positive predictive value = 60%, negative predictive value = 61%, positive likelihood ratio = 1.53 and negative likelihood ratio = 0.65 for predicting lamivudine resistance, missing 39% of resistant patients. Using other serum ALT cutoffs, diagnostic accuracy was lower. By multivariate analysis, baseline abnormal serum ALT was associated with abnormal ALT during resistance (OR = 5.98, P = 0.003), and males were associated with serum ALT flares during resistance (OR = 8.9, P = 0.016). 20Spectroscopic/Microscopic Elucid显示文摘The Arundo donax is a typical fast-growing species from the family Gramineae,which is widely cultivated in China.With a huge yield of A.donax in China,this plant offers great potential for biofuels production.The different types of organization of cell and the tissue in the A.donax could influence the efficiency of enzymatic hydrolysis.In this study,A.donax was subjected to 0.5%(w/w)sulfuric acid(H2SO4)for pretreatment at 140℃for 10 min,20 min,40 min,and 60 min,respectively.The changes in microstructure,chemical composition,topochemical properties were comprehensively analyzed.Using a series of spectroscopic and microscopic techniques including Fourier transform infrared spectroscopy(FT-IR),X-Ray diffraction(XRD),polarized light microscopy(PLM),and confocal Raman microscopy(CRM)to obtain the correlative structural and chemical information.Analysis results of chemical composition,FT-IR spectra and XRD indicated that with increasing reaction time,more hemicellulose and lignin would be removed.Correspondingly,there was an obvious increase of the cellulose relative crystallinity via extending reaction time.Results of the PLM observations showed that the birefringence gradually dimmed due to the diminishing of the cellulose component.Furthermore,the CRM mapping images showed the lignin component in compound middle lamellar(CML)was difficult to remove relatively as compared with that in secondary walls.These results indicated that the combination of spectroscopic and microscopic elucidation could give an insightful understanding of chemical changes in cellular level during pretreatment.Bingwei CHEN Xinzhou WANG Weiqi LENG Yu’na KAN Changtong MEI Shengcheng ZHAI 2019Journal of Bioresources and Bioproducts2019,4,3:0
