| 1 | Vascular endothelial dysfunction, cardiomyopathy a major mediator in diabetic cardiomyopathy显示文摘Diabetes mellitus is currently a major public health problem.A common complication of diabetes is cardiac dysfunction,which is recognized as a microvascular disease that leads to morbidity and mortality in diabetic patients.While ischemic events are commonly observed in diabetic patients,the risk for developing heart failure is also increased,independent of the severity of coronary artery disease and hypertension.This diabetes-associated clinical entity is considered a distinct disease process referred to as 'diabetic cardiomyopathy'.However,it is not clear how diabetes promotes cardiac dysfunction.Vascular endothelial dysfunction is thought to be one of the key risk factors.The impact of diabetes on the endothelium involves several alterations,including hyperglycemia,fatty acid oxidation,reduced nitric oxide (NO),oxidative stress,inflammatory activation,and altered barrier function. | 4 | Role of endoscopic ultrasonography in the loco-regional staging of patients with rectal cancer显示文摘The prognosis of rectal cancer(RC) is strictly related to both T and N stage of the disease at the time of diagnosis. RC staging is crucial for choosing the best multimodal therapy: patients with high risk locally advanced RC(LARC) undergo surgery after neoadjuvant chemotherapy and radiotherapy(NAT); those with low risk LARC are operated on after a preoperative short-course radiation therapy; finally, surgery alone is recommended only for early RC. Several imaging methods are used for staging patients with RC: computerized tomography, magnetic resonance imaging, positron emission tomography, and endoscopic ultrasound(EUS). EUS is highly accurate for the loco-regional staging of RC, since it is capable to evaluate precisely the mural infiltration of the tumor(T), especially in early RC. On the other hand, EUS is less accurate in restaging RC after NAT and before surgery. Finally, EUS is indicated for follow-up of patients operated on for RC, where there is a need for the surveillance of the anastomosis. | 7 | Hydrogen storage in a rare-earth perovskite-type oxide La_(0.6)Sr_(0.4)Co_(0.2)Fe_(0.8)O_3 for battery applications显示文摘Recently, rare-earth perovskite-type oxides with the general formula ABO_3(A rare earth element, B transition metal, O oxygen) are regarded as promising materials for Ni/oxide batteries due to their hydrogen storage ability. In the present study, the hydrogen storage properties of the rare-earth perovskite-type oxide La_(0.6)Sr_(0.4)Co_(0.2)Fe_(0.8)O_3 were evaluated in alkaline solution and at various temperatures. The hydrogen storage properties were investigated electrochemically by applying galvanostatic charge/discharge currents. Electrochemical pressurecomposition-temperature isotherms were constructed using the Nernst equation. The exchange current density and proton hydrogen diffusion coefficient were calculated using linear polarization measurements and the potential-step method, respectively. Interestingly, the maximum hydrogen absorption capacity, re-calculated from the electrochemical capacity, is 1.72 wt% at 333 K and 6 mol·L^(-1)KOH. | 8 | Mobilization of hematopoietic progenitor cells in patients with liver cirrhosis显示文摘AIM:To test the hypothesis that liver cirrhosis is associated with mobilization of hematopoietic progenitor cells. METHODS:Peripheral blood samples from 72 patients with liver cirrhosis of varying etiology were analyzed by flow cytometry.Identified progenitor cell subsets were immunoselected and used for functional assays in vitro. | 18 | Successful treatment of psychosis induced by interferon alpha and ribavirin with paliperidone:first case reported显示文摘Several clinical studies have shown a large number of mental symptoms by immunomodulatory treatment with interferon (IFN). The most frequently described symptoms are depression, suicidal behaviour, manic symptoms, anxiety, psychosis and delirium, associated with other non-specific symptoms such as fatigue, irritability, psychomotor retardation, decreased libido, insomnia, difficulty in concentration and attention. Having a history of mental disorder contraindicates the use of IFN-alpha. These adverse effects that affect the mental state appear usually at the beginning of the treatment (most after 3 weeks of treatment). The incidence of psychotic episodes is low and the episodes usually remit when treatment is interrupted;only some cases require antipsychotic treatment. We present the case of a patient affected with hepatitis C who began to present self-referential delirious symptoms after receiving the treatment with IFN and who was successfully treated with paliperidone. This patient could be classified within the group of high-risk psychiatric patients given the family history of schizophrenia and his personal history of illegal drug consumption. The pharmacological actions of paliperidone are similar to other high potency atypical antipsychotics.丁he receptorbinding profile of paliperidone most closely resembles that of risperidone and ziprasidone. Paliperidone differs from risperidone and most other antipsychotics by its relatively low extent of enzymatic hepatic metabolism.
