13体外扩张增大皮下组织厚度、促进细胞增殖和血管重建的小鼠模型显示文摘背景与目的 脂肪移植是一种有效的软组织重建方式;然而,关于移植受床处理的科学事实并未被完全理解.脂肪移植前利用吸力进行的体外扩张已应用于临床,以改善移植物成活的可行性和稳定性.笔者通过小鼠模型来探讨体外扩张的潜在机制.方法 笔者制作了一个连于25mmHg真空负压的软硅胶碗状外扩张装置,将其置于小鼠背部,并将其反应的效果与封闭包扎治疗的效果进行对比.利用MRI成像监测治疗区域.处理后7d采用血管灌注的方法研究微血管重塑情况.处理后28d分析组织厚度、脂肪细胞数量、细胞分化和血管密度的改变情况.结果 宏观分析(大体观察)显示,外扩张部位处理后21d局部组织肿胀.28d,外扩张使皮下脂肪层厚度倍增,MRI下也可观察到同样结果.扩张部位皮下层次的细胞增殖率倍增,脂肪细胞数增加2.2倍;血流灌注研究显示血管网重塑,表现为血管的重定位和管径增大,同时血管密度增大1.9倍.结论 体外扩张的小鼠模型显示,扩张可诱导皮下组织高度增殖和血管化.利用外扩张装置进行受区预处理可能成为提高细胞和组织移植效果的重要步骤.Yvonne I.
