| 1 | Liver stiffness in the hepatitis B virus carrier:A non-invasive marker of liver disease influenced by the pattern of transaminases显示文摘AIM: To investigate the usefulness of transient elastography by Fibroscan (FS),a rapid non-invasive technique to evaluate liver fibrosis,in the management of chronic hepatitis B virus (HBV) carriers.METHODS: In 297 consecutive HBV carriers,we studied the correlation between liver stiffness (LS),stage of liver disease and other factors potentially influencing FS measurements.In 87 chronic hepatitis B (CHB) patients,we monitored the FS variations according to the spontaneous or treatment-induced variations of biochemical activity during follow-up.RESULTS: FS values were 12.3 ± 3.3 kPa in acute hepatitis,10.3 ± 8.8 kPa in chronic hepatitis,4.3 ± 1.0 kPa in inactive carriers and 4.6 ± 1.2 kPa in blood donors.We identified the cut-offs of 7.5 and 11.8 kPa for the diagnosis of fibrosis ≥ S3 and cirrhosis respectively,showing 93.9% and 86.5% sensitivity,88.5% and 96.3% specificity,76.7% and 86.7% positive predictive value (PPV),97.3% and 96.3% negative predictive value (NPV) and 90.1% and 94.2% diagnostic accuracy.At multivariate analysis in 171 untreated carriers,fibrosis stage (t = 13.187,P < 0.001),active vs inactive HBV infection (t = 6.437,P < 0.001),alanine aminotransferase (ALT) (t = 4.740,P < 0.001) and HBV-DNA levels (t = -2.046,P = 0.042) were independently associated with FS.Necroinflammation score (t = 2.158,> 10/18 vs ≤ 10/18,P = 0.035) and ALT levels (t = 3.566,P = 0.001) were independently associated with LS in 83 untreated patients without cirrhosis and long-term biochemical remission (t = 4.662,P < 0.001) in 80 treated patients.During FS monitoring (mean follow-up 19.9 ± 7.1 mo) FS values paralleled those of ALT in patients with hepatitis exacerbation (with 1.2 to 4.4-fold increases in CHB patients) and showed a progressive decrease during antiviral therapy.CONCLUSION: FS is a non-invasive tool to monitor liver disease in chronic HBV carriers,provided that the pattern of biochemical activity is taken into account.In the inactive carrier,it identifies non-HBV-related causes of liver damage and transient reactivations.In CHB patients,it may warrant a more appropriate timing of control liver biopsies. | Filippo Oliveri Barbara Coco Pietro Ciccorossi Piero Colombatto Veronica Romagnoli Beatrice Cherubini Ferruccio Bonino Maurizia Rossana Brunetto | 2008 | World Journal of Gastroenterology2008,14,40: | 43 |

| 3 | Device-associated infection rates, mortality, length of stay and bacterial resistance in intensive care units in Ecuador: International Nosocomial Infection Control Consortium's findings显示文摘AIM To report the results of the International Nosocomial Infection Control Consortium(INICC) study conducted in Quito, Ecuador.METHODS A device-associated healthcare-acquired infection(DAHAI) prospective surveillance study conducted from October 2013 to January 2015 in 2 adult intensive care units(ICUs) from 2 hospitals using the United States Centers for Disease Control/National Healthcare Safety Network(CDC/NHSN) definitions and INICC methods. RESULTS We followed 776 ICU patients for 4818 bed-days. The central line-associated bloodstream infection(CLABSI) rate was 6.5 per 1000 central line(CL)-days, the ventilator-associated pneumonia(VAP) rate was 44.3 per 1000 mechanical ventilator(MV)-days, and the catheterassociated urinary tract infection(CAUTI) rate was 5.7 per 1000 urinary catheter(UC)-days. CLABSI and CAUTI rates in our ICUs were similar to INICC rates [4.9(CLABSI) and 5.3(CAUTI)] and higher than NHSN rates [0.8(CLABSI) and 1.3(CAUTI)]- although device use ratios for CL and UC were higher than INICC and CDC/NSHN's ratios. By contrast, despite the VAP rate was higher than INICC(16.5) and NHSN's rates(1.1), MV DUR was lower in our ICUs. Resistance of A. baumannii to imipenem and meropenem was 75.0%, and of Pseudomonas aeruginosa to ciprofloxacin and piperacillin-tazobactam was higher than 72.7%, all them higher than CDC/NHSN rates. Excess length of stay was 7.4 d for patients with CLABSI, 4.8 for patients with VAP and 9.2 for patients CAUTI. Excess crude mortality in ICUs was 30.9% for CLABSI, 14.5% for VAP and 17.6% for CAUTI. | 6 | Frequency dependent polarization analysis of ambient seismic noise recorded at a broadband seismometer in the central United States显示文摘We present a new approach to polarization analysis of seismic noise recorded by three-component seismometers.It is based on statistical analysis of frequency-dependent particle motion properties determined from a large number of time windows via eigenanalysis of the 3-by-3,Hermitian,spectral covariance matrix.We applied the algorithm to continuous data recorded in 2009 by the seismic station SLM,located in central North America.A rich variety of noise sources was observed.At low frequencies (<0.05 Hz) we observed a tilt-related signal that showed some elliptical motion in the horizontal plane.In the microseism band of 0.05-0.25 Hz,we observed Rayleigh energy arriving from the northeast,but with three distinct peaks instead of the classic single and double frequency peaks.At intermediate frequencies of 0.5-2.0 Hz,the noise was dominated by non-fundamental-mode Rayleigh energy,most likely P and Lg waves.At the highest frequencies (>3 Hz),Rayleigh-type energy was again dominant in the form of Rg waves created by nearby cultural activities.Analysis of the time dependence of noise power shows that a frequency range of at least 0.02-1.0 Hz (much larger than the microseism band) is sensitive to annual,meteorologically induced sources of noise. | Keith D. | 11 | Cardiovascular dysfunction following spinal cord injury显示文摘Both sensorimotor and autonomic dysfunctions often occur after spinal cord injury(SCI). Particularly, a high thoracic or cervical SCI interrupts supraspinal vasomotor pathways and results in disordered hemodynamics due to deregulated sympathetic outflow. As a result of the reduced sympathetic activity, patients with SCI may experience hypotension, cardiac dysrhythmias, and hypothermia post-injury. In the chronic phase, changes within the CNS and blood vessels lead to orthostatic hypotension and life-threatening autonomic dysreflexia(AD). AD is characterized by an episodic, massive sympathetic discharge that causes severe hypertension associated with bradycardia. The syndrome is often triggered by unpleasant visceral or sensory stimuli below the injury level. Currently the only treatments are palliative – once a stimulus elicits AD, pharmacological vasodilators are administered to help reduce the spike in arterial blood pressure. However, a more effective means would be to mitigate AD development by attenuating contributing mechanisms, such as the reorganization of intraspinal circuits below the level of injury. | 13 | Role of antiviral therapy in the natural history of hepatitis B virus-related chronic liver disease显示文摘Hepatitis B virus(HBV) infection is a dynamic state ofinteractions among HBV, hepatocytes, and the host immune system. Natural history studies of chronic hepatitis B(CHB) infection have shown an association between active viral replication and adverse clinical outcomes such as cirrhosis and hepatocellular carcinoma. The goal of therapy for CHB is to improve quality of life and survival by preventing progression of the disease to cirrhosis, decompensation, end-stage liver disease, hepatocellular carcinoma(HCC) and death. This goal can be achieved if HBV replication is suppressed in a sustained manner. The accompanying reduction in histological activity of CHB lessens the risk of cirrhosis and of HCC, particularly in non-cirrhotic patients. However, CHB infection cannot be completely eradicated, due to the persistence of covalently closed circular DNA in the nucleus of infected hepatocytes, which may explain HBV reactivation. | 20 | Debris flow hazard assessment by means of numerical simulations:implications for the Rotolon creek valley(Northern Italy)显示文摘On 4^(th) November 2010, a debris flow detached from a large debris cover accumulated above the lowermost portion of the Rotolon landslide(Vicentine Pre-Alps, NE Italy) and channelized in the valley below within the Rotolon Creek riverbed. Such event evolved into a highly mobile and sudden debris flow, damaging some hydraulic works and putting at high risk four villages located along the creek banks. A monitoring campaign was carried out by means of a ground based radar interferometer(GB-In SAR) to evaluate any residual displacement risk in the affected area and in the undisturbed neighbouring materials.Moreover, starting from the current slope condition, a landslide runout numerical modelling was performed by means of DAN-3D code to assess the impacted areas, flow velocity, and deposit distribution of the simulated events. The rheological parameters necessary for an accurate modelling were obtained through the back analysis of the 2010 debris flow event. Back analysis was calibrated with all of the available terrain data coming from field surveys and ancillary documents, such as topographic,geomorphological and geological maps, with pre-and post-event Li DAR derived DTMs, and with orthophotos. Finally, to identify new possible future debris flow source areas as input data for the new modelling, all the obtained terrain data were reanalysed and integrated with the GB-In SAR displacement maps; consequently, new simulations were made to forecast future events.
