1Ileocecal valve dysfunction in small intestinal bacterial overgrowth: A pilot study显示文摘AIM: To explore whether patients with a defective ileocecal valve (ICV)/cecal distension reflex have small intestinal bacterial overgrowth. METHODS: Using a colonoscope, under conscious sedation, the ICV was intubated and the colonoscope was placed within the terminal ileum (TI). A manometry catheter with 4 pressure channels, spaced 1 cm apart, was passed through the biopsy channel of the colonoscope into the TI. The colonoscope was slowly withdrawn from the TI while the manometry catheter was advanced. The catheter was placed across the ICV so that at least one pressure port was within the TI, ICV and the cecum respectively. Pressures were continuously measured during air insufflation into the cecum, under direct endoscopic visualization, in 19 volunteers. Air was insufflated to a maximum of 40 mmHg to prevent barotrauma. All subjects underwent lactulose breath testing one month after the colonoscopy. The results of the breath tests were compared with the results of the pressures within the ICV during air insufflation. RESULTS: Nineteen subjects underwent colonoscopy with measurements of the ICV pressures after intubation of the ICV with a colonoscope. Initial baseline readings showed no statistical difference in the pressures of the TI and ICV, between subjects with positive lactulose breath tests and normal lactulose breath tests. The average peak ICV pressure during air insufflation into the cecum in subjects with normal lactu-lose breath tests was significantly higher than cecal pressures during air insufflation (49.33 ± 7.99 mmHg vs 16.40 ± 2.14 mmHg, P = 0.0011). The average percentage difference of the area under the pressure curve of the ICV from the cecum during air insufflations in subjects with normal lactulose breath tests was significantly higher (280.72% ± 43.29% vs 100% ± 0%, P = 0.0006). The average peak ICV pressure during air insufflation into the cecum in subjects with positive lactulose breath tests was not significantly different than cecal pressures during air insufflation 21.23 ± 3.52 mmHg vs 16.10 ± 3.39 mmHg. The average percentage difference of the area under the pressure curve of the ICV from the cecum during air insufflation was not significantly different 101.08% ± 7.96% vs 100% ± 0%. The total symptom score for subjects with normal lactulose breath tests and subjects with positive lactu-lose breath tests was not statistically different (13.30 ± 4.09 vs 24.14 ± 6.58). The ICV peak pressures during air insufflations were significantly higher in subjects with normal lactulose breath tests than in subjects with positive lactulose breath tests (P = 0.005). The average percent difference of the area under the pressure curve in the ICV from cecum was significantly higher in subjects with normal lactulose breath tests than in subjects with positive lactulose breath tests (P = 0.0012). Individuals with positive lactulose breath tests demonstrated symptom scores which were significantly higher for the following symptoms: not able to finish normal sized meal, feeling excessively full after meals, loss of appetite and bloating. CONCLUSION: Compared to normal, subjects with a positive lactulose breath test have a defective ICV cecal distension reflex. 2Correlation of periodontal inflamed surface area with glycemic status in controlled and uncontrolled type 2 diabetes mellitus显示文摘BACKGROUND The bidirectional link between periodontitis and diabetes mellitus(DM)has been established.Periodontitis causes systemic inflammatory burden through inflammatory mediators.The currently utilized tools[clinical attachment loss(CAL)and probing pocket depth(PPD)]are linear measurements,that do not exactly quantify the inflammatory burden of periodontitis.Periodontal inflamed surface area(PISA)quantifies the surface area of bleeding pocket epithelium and estimates the inflammatory burden.Studies relating to the periodontal status of diabetic patients with and without microvascular complications are scarce.This study assessed the proportion of periodontitis and correlation of PISA with glycemic status in controlled,uncontrolled type 2 DM(T2DM)with and without microvascular complications.AIM To assess the proportion of periodontitis and correlation of PISA with glycemic status in controlled,and uncontrolled T2DM with and without microvascular complications.METHODS This study comprised 180 T2DM patients.Based on glycated hemoglobin(HbA1c)levels,they were grouped into:(1)Controlled T2DMgroup:(HbA1c≤7%);(2)Uncontrolled T2DM group:(HbA1c>7%)without microvascular complications;and(3)Uncontrolled T2DM group:(HbA1c>7%)with microvascular complications.Each group comprised 60 patients.All patients were assessed for periodontal parameters(Bleeding on Probing,PPD,CAL,Oral hygiene index simplified and PISA),and systemic parameters(HbA1c,fasting plasma glucose and post prandial plasma glucose).RESULTS The proportion of periodontitis among controlled T2DM group,uncontrolled T2DM group without microvascular complications,uncontrolled T2DM group with micro-vascular complications was 75%,93.4%and 96.6%respectively.Extent and severity of periodontitis were high in the uncontrolled T2DM group.A significant positive correlation was found between PISA and HbA1c among all patients(r=0.393,P<0.001).The dose–response relationship between PISA and HbA1c was observed.An increase of PISA with 168 mm^(2) was associated with a 1.0%increase of HbA1c.CONCLUSION High proportion and severity of periodontitis,and increased inflamed surface area in uncontrolled T2DM may have contributed to the poor glycemic control and microvascular complications.Krishna Anil Rosamma Joseph Vadakkekuttical Chandni Radhakrishnan Fairoz Cheriyalingal Parambath 2021World Journal of Clinical Cases2021,9,36:2

12IoT Task Offloading in Edge Computing Using Non-Cooperative Game Theory for Healthcare Systems显示文摘In this paper,we present a comprehensive system model for Industrial Internet of Things(IIoT)networks empowered by Non-Orthogonal Multiple Access(NOMA)and Mobile Edge Computing(MEC)technologies.The network comprises essential components such as base stations,edge servers,and numerous IIoT devices characterized by limited energy and computing capacities.The central challenge addressed is the optimization of resource allocation and task distribution while adhering to stringent queueing delay constraints and minimizing overall energy consumption.The system operates in discrete time slots and employs a quasi-static approach,with a specific focus on the complexities of task partitioning and the management of constrained resources within the IIoT context.This study makes valuable contributions to the field by enhancing the understanding of resourceefficient management and task allocation,particularly relevant in real-time industrial applications.Experimental results indicate that our proposed algorithmsignificantly outperforms existing approaches,reducing queue backlog by 45.32% and 17.25% compared to SMRA and ACRA while achieving a 27.31% and 74.12% improvement in Qn O.Moreover,the algorithmeffectively balances complexity and network performance,as demonstratedwhen reducing the number of devices in each group(Ng)from 200 to 50,resulting in a 97.21% reduction in complexity with only a 7.35% increase in energy consumption.This research offers a practical solution for optimizing IIoT networks in real-time industrial settings.Dinesh Mavaluru Chettupally Anil Carie Ahmed I.Alutaibi Satish Anamalamudi Bayapa Reddy Narapureddy Murali Krishna Enduri Md Ezaz Ahmed 2024Computer Modeling in Engineering & Sciences2024,139,5:0
13Redundant Transmission Control Algorithm for Information-Centric Vehicular IoT Networks显示文摘Vehicular Adhoc Networks(VANETs)enable vehicles to act as mobile nodes that can fetch,share,and disseminate information about vehicle safety,emergency events,warning messages,and passenger infotainment.However,the continuous dissemination of information fromvehicles and their one-hop neighbor nodes,Road Side Units(RSUs),and VANET infrastructures can lead to performance degradation of VANETs in the existing hostcentric IP-based network.Therefore,Information Centric Networks(ICN)are being explored as an alternative architecture for vehicular communication to achieve robust content distribution in highly mobile,dynamic,and errorprone domains.In ICN-based Vehicular-IoT networks,consumer mobility is implicitly supported,but producer mobility may result in redundant data transmission and caching inefficiency at intermediate vehicular nodes.This paper proposes an efficient redundant transmission control algorithm based on network coding to reduce data redundancy and accelerate the efficiency of information dissemination.The proposed protocol,called Network Cording Multiple Solutions Scheduling(NCMSS),is receiver-driven collaborative scheduling between requesters and information sources that uses a global parameter expectation deadline to effectively manage the transmission of encoded data packets and control the selection of information sources.Experimental results for the proposed NCMSS protocol is demonstrated to analyze the performance of ICN-vehicular-IoT networks in terms of caching,data retrieval delay,and end-to-end application throughput.The end-to-end throughput in proposed NCMSS is 22%higher(for 1024 byte data)than existing solutions whereas delay in NCMSS is reduced by 5%in comparison with existing solutions.Abdur Rashid Sangi Satish Anamalamudi Mohammed SAlkatheiri Murali Krishna Enduri Anil Carie Mohammed AAlqarni 2023Computers, Materials & Continua2023,76,8:0
14Genome-Wide Analysis of von Willebrand Factor A Gene Family in Rice for Its Role in Imparting Biotic Stress Resistance with Emphasis on Rice Blast Disease显示文摘von Willebrand factor A(vWA)genes are well characterized in humans except for few BONZAI genes,but the vWA genes are least explored in plants.Considering the novelty and vital role of vWA genes,this study aimed at characterization of vWA superfamily in rice.Rice genome was found to have 40 vWA genes distributed across all the 12 chromosomes,and 20 of the 40 vWA genes were unique while the remaining shared large fragment similarities with each other,indicating gene duplication.In addition to vWA domain,vWA proteins possess other different motifs or domains,such as ubiquitin interacting motif in protein degradation pathway,and RING finger in protein-protein interaction.Expression analysis of vWA genes in available expression data suggested that they probably function in biotic and abiotic stress responses including hormonal response and signaling.The frequency of transposon elements in the entire 3K rice germplasm was negligible except for 9 vWA genes,indicating the importance of these genes in rice.Structural and functional diversities showed that the vWA genes in a blast-resistant rice variety Tetep had huge variations compared to blast-susceptible rice varieties HP2216 and Nipponbare.qRT-PCR analysis of vWA genes in Magnaporthe oryzae infected rice tissues indicated OsvWA9,OsvWA36,OsvWA37 and OsvWA18 as the optimal candidate genes for disease resistance.This is the first attempt to characterize vWA gene family in plant species.Suhas Gorakh KARKUTE Vishesh KUMAR Mohd TASLEEM Dwijesh Chandra MISHRA Krishna Kumar CHATURVEDI Anil RAI Amitha Mithra SEVANTHI Kishor GAIKWAD Tilak Raj SHARMA Amolkumar U.SOLANKE 2022Rice science2022,29,4:0
15Posterior tibial nerve stimulation for fecal incontinence:Where are we?显示文摘Neurostimulation remains the mainstay of treatment for patients with faecal incontinence who fails to respond to available conservative measures.Sacral nerve stimulation(SNS)is the main form of neurostimulation that is in use today.Posterior tibial nerve stimulation(PTNS)-both the percutaneous and the transcutaneous routes-remains a relatively new entry in neurostimulation.Though in its infancy,PTNS holds promise to be an effective,patient friendly,safe and cheap treatment.However,presently PTNS only appears to have a minor role with SNS having the limelight in treating patients with faecal incontinence.This seems to have arisen as the strong,uniform and evidence based data on SNS remains to have been unchallenged yet by the weak,disjointed and unsupported evidence for both percutaneous and transcutaneous PTNS.The use of PTNS is slowly gaining acceptance.However,several questions remain unanswered in the delivery of PTNS.These have raised dilemmas which as long as they remain unsolved can considerably weaken the argument that PTNS could offer a viable alternative to SNS.This paper reviews available information on PTNS and focuses on these dilemmas in the light of existing evidence.Anil Thomas George Rudra Krishna Maitra Charles Maxwell-Armstrong 2013World Journal of Gastroenterology2013,19,48:0
