| 1 | Gastroenterology in developing countries:Issues and advances显示文摘Developing countries shoulder a considerable burden of gastroenterological disease. Infectious diseases in particular cause enormous morbidity and mortality. Diseases which afflict both western and developing countries are often seen in more florid forms in poorer countries. Innovative techniques continuously improve and update gastroenterological practice. However, advances in diagnosis and treatment which are commonplace in the West, have yet to reach many developing countries. Clinical guidelines, based on these advances and collated in resource-rich environments,lose their relevance outside these settings. In this two-part review, we first highlight the global burden of gastroenterological disease in three major areas: diarrhoeal diseases, hepatitis B, and Helicobacter pylori . Recent progress in their management is explored, with consideration of future solutions. The second part of the review focuses on the delivery of clinical services in developing countries. Inadequate numbers of healthcare workers hamper efforts to combat gastroenterological disease. Reasons for this shortage are examined, along with possibilities for increased specialist training. Endoscopy services, the mainstay of gastroenterology in the West, are in their infancy in many developing countries. The challenges faced by those setting up a service are illustrated by the example of a Nigerian endoscopy unit. Finally, we highlight the limited scope of many clinical guidelines produced in western countries. Guidelines which take account of resource limitations in the form of 'cascades' are advocated in order to make these guidelines truly global. | 2 | PEComa of the colon resistant to sirolimus but responsive to doxorubicin/ifosfamide显示文摘A 23-year-old male presented with a three-week-history of crampy abdominal pain and melaena.Colonoscopy revealed a friable mass filling the entire lumen of the cecum;histologically,it was classified as perivascular epithelioid cell tumor(PEComa).An magnetic resonance imaging scan showed,in addition to the primary tumor,two large mesenteric lymph node metastases and four metastatic lesions in the liver.The patient underwent right hemicolectomy and left hemihepatectomy combined with wedge resections of metastases in the right lobe of the liver,the resection status was R0.Subsequently,the patient was treated with sirolimus.After 4 mo of adjuvant mammalian target of rapamycin inhibition he developed two new liver metastases and a local pelvic recurrence.The visible tumor formations were again excised surgically,this time the resection status was R2 with regard to the pelvic recurrence.The patient was treated with 12 cycles of doxorubicin and ifosfamide under which the disease was stable for 9 mo.The clinical course was then determined by rapid tumor growth in the pelvic cavity.Second line chemotherapy with gemcitabine and docetaxel was ineffective,and the patient died 23 mo after the onset of disease.This case report adds evidence that,in malignant PEComa,the mainstay of treatment is curative surgery.If not achievable,the effects of adjuvant or palliative chemotherapy are unpredictable. | Wolfgang Scheppach Nikolaus Reissmann Thomas Sprinz Ekkehard Schippers Bjoern Schoettker Justus G Mueller | 2013 | World Journal of Gastroenterology2013,19,10: | 8 |

| 4 | The role of sphingosine-1-phosphate in bone remodeling and osteoporosis显示文摘Osteoporosis is a systemic bone disease that affects more than 200 million people worldwide and is caused by the disruption of the equilibrium between osteoclastic bone resorption and osteoblastic bone formation. Sphingosine-1-phosphate(S1 P) is a natural,bioactive sphingolipid that has been shown to play a major role in cardiovascular and immunological pathologies by regulating biological and cellular processes, including migration, differentiation, proliferation and survival. Recent studies also suggest a central role for S1 P in bone diseases, including osteoporosis;however, the effects of S1 P, particularly in bone metabolism, remain to be further elucidated. In this review, we summarize the available literature on the role of S1 P in bone metabolism with a focus on osteoporosis. On the cellular level, S1 P acts as an osteoclast-osteoblast coupling factor to promote osteoblast proliferation and bone formation. Moreover, the recruitment of osteoclast precursors to resorption sites is regulated by the interplay of S1 P gradients and S1 P receptor expression. From a clinical perspective, increasing evidence suggests that systemically elevated S1 P blood levels may serve as an independent risk factor for osteoporosis-related fractures. | 5 | 提高金属有机骨架材料在二氧化碳热催化转化中的性能:策略及前景展望显示文摘近年来,大气中CO_(2)的浓度不断增加,带来全球变暖等一系列严重后果,成为国际社会共同关注的环境问题.将CO_(2)催化转化为高附加值化学品可有效降低其向大气中的排放,同时可实现其资源化利用,符合低碳社会的发展目标.目前,已有多种催化体系实现了CO_(2)向不同化学品的转化.然而,由于CO_(2)自身的热力学稳定性和动力学惰性,这些转化通常需要在苛刻的反应条件和较高能耗下进行.设计开发高效催化体系、实现温和条件下CO_(2)的转化利用引起了工业界和学术界的广泛兴趣.金属有机骨架材料(MOFs)是一类由有机配体和金属中心通过配位键组装而成的有机-无机杂化材料,在很多方面展现出良好的应用性能.由于其结构的多样性、可设计性、高比表面积和多孔性等独特性质,MOFs在催化领域吸引了很多研究者的关注.其中,MOFs作为非均相催化剂在CO_(2)热催化转化中表现出良好的应用前景,已实现多种CO_(2)向高值化学品的转化路径.但这些催化体系也存在一些缺点,如有些MOFs材料在催化反应中稳定性差以及其微孔性对反应中的传质造成限制等.因此,设计稳定的MOFs和MOF-基材料并对其结构进行优化改性,从而在温和条件下实现高效的CO_(2)转化具有重要意义.本文综述了提高MOFs在CO_(2)热催化转化反应中性能的几种策略:(1)对MOFs结构中的配体进行设计,包括具有活性官能团的配体、活性配合物作为配体和引入混合配体设计多元MOF;(2)调节MOFs结构中的金属中心,设计混合金属中心和包含活性金属团簇的金属中心;(3)构筑多级孔MOFs;(4)设计MOF-基的复合材料,包括MOFs作为载体与金属纳米颗粒、活性配合物和聚合物构建复合材料;(5)利用MOFs作为前驱体制备MOF-基衍生物材料,重点阐述了如何增加MOFs作为非均相催化剂的催化活性位点以及在CO_(2)转化反应中各位点之间的协同作用.此外,介绍了原位表征技术在MOF-基材料用于CO_(2)固定和转化中的应用.最后,分析了MOF-基非均相催化材料在CO_(2)热催化转化领域目前面临的问题和挑战,包括MOFs材料结构优化、催化机理研究和规模化制备等方面,并对未来的发展趋势进行了展望. | 郝磊端 夏启能 张强 Justus Masa 孙振宇 | 2021 | Chinese Journal of Catalysis2021,42,11: | 4 |
| 6 | Enhanced microfracture techniques in cartilage knee surgery: Fact or fiction?显示文摘The limited intrinsic healing potential of human articular cartilage is a well-known problem in orthopedic surgery. Thus a variety of surgical techniques have been developed to reduce joint pain, improve joint function and delay the onset of osteoarthritis. Microfractures as a bone marrow stimulation technique present the most common applied articular cartilage repair procedure today. Unfortunately the deficiencies of fibrocartilaginous repair tissue inevitably lead to breakdown under normal joint loading and clinical results deteriorate with time. To overcome the shortcomings of microfracture, an enhanced microfracture technique was developed with an additional collagen Ⅰ/Ⅲ membrane(Autologous, Matrix-Induced Chondrogenesis, AMIC). This article reviews the pre-clinical rationale of microfractures and AMIC, presents clinical studies and shows the advantages and disadvantages of these widely usedtechniques. PubM ed and the Cochrane database were searched to identify relevant studies. We used a comprehensive search strategy with no date or language restrictions to locate studies that examined the AMIC technique and microfracture. Search keywords included cartilage, microfracture, AMIC, knee, ChondroGide. Besides this, we included our own experiences and study authors were contacted if more and non published data were needed. Both cartilage repair techniques represent an effective and safe method of treating full-thickness chondral defects of the knee in selected cases. While results after microfracture deteriorate with time, mid-term results after AMIC seem to be enduring. | 12 | Extracorporeal membrane oxygenation for acute respiratory distress syndrome in burn patients:a case series and literature update显示文摘Background:Acute respiratory distress syndrome(ARDS)has a reported incidence of 34–43%in ventilated burn patients and is associated with a mortality of 59%in the severe form.The use and experience with extracorporeal membrane oxygenation(ECMO)in burn patients developing ARDS are still limited.We present our results and discuss the significance of ECMO in treating burn patients.Methods:A retrospective analysis of burn patients treated with ECMO for ARDS between January 2017 and January 2019 was performed.Demographic,clinical,and outcome data were collected and analyzed.Results:Eight burn patients were treated at our institution with ECMO in the designated time period.Of these,all but one patient had inhalation injury,burn percentage of TBSA was 37±23%,ABSI score was 8.4±2,and R-Bauxscore was 98±21.Seven patients developed severe ARDS and one patient moderate ARDS according to the Berlin classification with a PaO_(2)/FiO_(2) ratio upon initiation of ECMO therapy of 62±22 mmHg.ECMO duration was 388±283 h.Three patients died from severe sepsis while five patients survived to hospital discharge.Conclusions:ECMO is a viable therapy option in burn patients developing severe ARDS and can contribute to survival rates similar to ECMO therapy in non-burn-associated severe ARDS.Consequently,patients with severe respiratory insufficiency with unsuccessful conventional treatment and suspected worsening should be transferred to burn units with the possibility of ECMO treatment to improve outcome. | Mehran Dadras Johannes M.Wagner Christoph Wallner Julika Huber Dirk Buchwald Justus Strauch Kamran Harati Nicolai Kapalschinski Björn Behr Marcus Lehnhardt | 2019 | Burns & Trauma2019,7,1: | 2 |
