2Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients显示文摘AIM To evaluate the reversibility of minimal hepatic encephalopathy(MHE) following liver transplantation(LT) in Egyptian cirrhotic patients. METHODS This prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty ageand sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A(TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score(PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTS Before LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls(P < 0.001). There was a statistically significant improvement in test values in the patient group after LT; however, their values were still significantly worse than those of the controls(P < 0.001). The PHES detected MHE in 16 patients(80%) before LT with a median value of -7 ± 3.5. The median PHES value was significantly improved following LT, reaching-4.5 ± 5(P < 0.001), and the number of patients with MHE decreased to 11(55%). The pre-transplant model for end-stage liver disease(MELD) score ≥ 15 was significantly related to the presence of post-transplant MHE(P = 0.005). 3Microsecond scale vibrational spectroscopic imaging by multiplex stimulated Raman scattering microscopy显示文摘Real-time vibrational spectroscopic imaging is desired for monitoring cellular states and cellular processes in a label-free manner.Raman spectroscopic imaging of highly dynamic systems is inhibited by relatively slow spectral acquisition on millisecond to second scale.Here,we report microsecond scale vibrational spectroscopic imaging by lock-in free parallel detection of spectrally dispersed stimulated Raman scattering signal.Using a homebuilt tuned amplifier array,our method enables Raman spectral acquisition,within the window defined by the broadband pulse,at the speed of 32 μs and with close to shot-noise limited detection sensitivity.Incorporated with multivariate curve resolution analysis,our platform allows compositional mapping of lipid droplets in single live cells,observation of intracellular retinoid metabolism,discrimination of fat droplets from protein-rich organelles in Caenorhabditis elegans,spectral detection of fast flowing tumor cells and monitoring drug diffusion through skin tissue in vivo.The reported technique opens new opportunities for compositional analysis of cellular compartment in a microscope setting and high-throughput spectral profiling of single cells in a flow cytometer setting.Chien-Sheng Liao Mikhail N Slipchenko Ping Wang Junjie Li Seung-Young Lee Robert A Oglesbee Ji-Xin Cheng 2015Light(Science & Applications)2015,4,1:8
4Bound states in the continuum and Fano resonances in the strong mode coupling regime显示文摘The study of resonant dielectric nanostructures with a high refractive index is a new research direction in the nanoscale optics and metamaterial-inspired nanophotonics.Because of the unique optically induced electric and magnetic Mie resonances,high-index nanoscale structures are expected to complement or even replace different plasmonic components in a range of potential applications.We study a strong coupling between modes of a single subwavelength high-index dielectric resonator and analyze the mode transformation and Fano resonances when the resonator’s aspect ratio varies.We demonstrate that strong mode coupling results in resonances with high-quality factors,which are related to the physics of bound states in the continuum when the radiative losses are almost suppressed due to the Friedrich–Wintgen scenario of destructive interference.We explain the physics of these states in terms of multipole decomposition,and show that their appearance is accompanied by a drastic change in the far-field radiation pattern.We reveal a fundamental link between the formation of the high-quality resonances and peculiarities of the Fano parameter in the scattering cross-section spectra.Our theoretical findings are confirmed by microwave experiments for the scattering of high-index cylindrical resonators with a tunable aspect ratio.The proposed mechanism of the strong mode coupling in single subwavelength high-index resonators accompanied by resonances with high-quality factors helps to extend substantially functionalities of all-dielectric nanophotonics,which opens horizons for active and passive nanoscale metadevices.Andrey A.Bogdanov Kirill LKoshelev Polina VKapitanova Mikhail VRybin Sergey AGladyshev Zarina FSadrieva Kirill BSamusev Yuri SKivshar a Mikhail FLimonov 2019Advanced Photonics2019,1,1:6
5Neurohumoral,cardiac and inflammatory markers in the evaluation of heart failure severity and progression显示文摘Heart failure is common in adult population,accounting for substantial morbidity and mortality worldwide.The main risk factors for heart failure are coronary artery disease,hypertension,obesity,diabetes mellitus,chronic pulmonary diseases,family history of cardiovascular diseases,cardiotoxic therapy.The main factor associated with poor outcome of these patients is constant progression of heart failure.In the current review we present evidence on the role of established and candidate neurohumoral biomarkers for heart failure progression management and diagnostics.A growing number of biomarkers have been proposed as potentially useful in heart failure patients,but not one of them still resembles the characteristics of the'ideal biomarker.'A single marker will hardly perform well for screening,diagnostic,prognostic,and therapeutic management purposes.Moreover,the pathophysiological and clinical significance of biomarkers may depend on the presentation,stage,and severity of the disease.The authors cover main classification of heart failure phenotypes,based on the measurement of left ventricular ejection fraction,including heart failure with preserved ejection fraction,heart failure with reduced ejection fraction,and the recently proposed category heart failure with mid-range ejection fraction.One could envisage specific sets of biomarker with different performances in heart failure progression with different left ventricular ejection fraction especially as concerns prediction of the future course of the disease and of left ventricular adverse/reverse remodeling.This article is intended to provide an overview of basic and additional mechanisms of heart failure progression will contribute to a more comprehensive knowledge of the disease pathogenesis.Ekaterina A Polyakova Evgeny N Mikhaylov Dmitry L Sonin Yuri V Cheburkin Mikhail M Galagudza 2021Journal of Geriatric Cardiology2021,18,1:4
6High-sensitivity infrared vibrational nanospectroscopy in water显示文摘Mid-infrared vibrational spectroscopy is a universal label-free tool for identifying molecular compounds in chemical and biological samples on the basis of their‘fingerprint’vibrational absorption lines.Vibrational spectroscopy with nanometer spatial resolution can reveal the chemical composition of samples at the nanoscale,and several scanning-probe techniques have been developed to address this need1–22.It is also highly desirable to study biological and chemical samples in their native aqueous environments rather than in air.In aqueous environments,however,the sensitivity of the current vibrational nanospectroscopy techniques deteriorates dramatically7,23.Here,we report the first mid-infrared nanospectroscopy technique that retains nanoscale sensitivity and spatial resolution when the sample and the scanning probe are completely immersed in water.This method overcomes challenges including water absorption and scattering and the mechanical damping of cantilever vibrations.We further demonstrate spectroscopy and imaging of 20-to 50-nmthick polymer samples with a 25-nm spatial resolution in the biologically relevant Amide I and II spectral regions.Mingzhou Jin Feng Lu Mikhail A Belkin 2017Light(Science & Applications)2017,6,1:3
7Differing profiles of people diagnosed with acute and chronic hepatitis B virus infection in British Columbia,Canada显示文摘AIM To describe the characteristics of people diagnosed with acute and chronic hepatitis B virus(HBV) infection in British Columbia(BC).METHODS We used data from the BC Hepatitis Testers Cohort(BCHTC),which includes all individuals tested for hepatitis C virus(HCV) or human immunodeficiency virus(HIV) or those diagnosed with HBV or active tuberculosis in BC since 1990.These data were integrated with prescription drug,medical visit,hospitalization and mortality data.HBV cases were classified as acute or chronic according to provincial guidelines.We compared characteristics of individuals by HBV infection group(acute,chronic and negative).Factors associated with acute or chronic HBV infection were assessed with multinomial logistic regression models in comparison to the HBV negative group.RESULTS46498 of the 1058056 eligible BC-HTC participants were diagnosed with HBV infection.4.3% of HBV positive individuals were diagnosed with acute HBV infections while 95.7% had chronic infections.Problematic alcohol use,injection drug use,and HIV or HCV co-infection were more common among individuals diagnosed with acute HBV compared to those with chronic infections and HBV negative individuals.In multivariable multinomial logistic regression models,we observed significant associations between acute or chronic HBV diagnosis and being male,age at HBV diagnosis or birth cohort,South and East Asian ethnicity,HCV or HIV infection,and injection drug use.The odds of acute HBV decreased with increasing age among people who inject drugs,while the opposite was true for chronic HBV.Persons with acute HBV were predominantly White(78%) while those with chronic HBV were mostly East Asian(60%).Relative to Whites,East Asians had 12 times greater odds of being diagnosed with chronic HBV infection.These odds increased with increasing socioeconomic deprivation.CONCLUSION Differences in the profiles of people diagnosed with acute and chronic HBV infection necessitate differentiated screening,prevention,care and treatment programs.Mawuena Binka Zahid A Butt Stanley Wong Mei Chong Jane A Buxton Nuria Chapinal Amanda Yu Maria Alvarez Maryam Darvishian Jason Wong Gina McGowan Mikhail Torban Mark Gilbert Mark Tyndall Mel Krajden Naveed Z Janjua 2018World Journal of Gastroenterology2018,24,11:3
8Effect of intra-abdominal pressure on respiratory function in patients undergoing ventral hernia repair显示文摘AIM: To determine the influence of intra-abdominal pressure(IAP) on respiratory function after surgical repair of ventral hernia and to compare two different methods of IAP measurement during the perioperative period. METHODS: Thirty adult patients after elective repair of ventral hernia were enrolled into this prospective study.IAP monitoring was performed via both a balloontipped nasogastric probe [intragastric pressure(IGP), Ci MON, Pulsion Medical Systems, Munich, Germany] and a urinary catheter [intrabladder pressure(IBP), Uno Meter Abdo-Pressure Kit, Uno Medical, Denmark] on five consecutive stages:(1) after tracheal intubation(AI);(2) after ventral hernia repair;(3) at the end of surgery;(4) during spontaneous breathing trial through the endotracheal tube; and(5) at 1 h after tracheal extubation. The patients were in the complete supine position during all study stages.RESULTS: The IAP(measured via both techniques) increased on average by 12% during surgery compared to AI(P < 0.02) and by 43% during spontaneous breathing through the endotracheal tube(P < 0.01). In parallel, the gradient between РаСО2 and Et CO2 [Р(а-et)CO2] rose significantly, reaching a maximum during the spontaneous breathing trial. The PаO2/Fi O2 decreased by 30% one hour after tracheal extubation(P = 0.02). The dynamic compliance of respiratory system reduced intraoperatively by 15%-20%(P < 0.025). At all stages, we observed a significant correlation between IGP and IBP(r = 0.65-0.81, P < 0.01) with a mean bias varying from-0.19 mm Hg(2SD 7.25 mm Hg) to-1.06 mm Hg(2SD 8.04 mm Hg) depending on the study stage. Taking all paired measurements together(n = 133), the median IGP was 8.0(5.5-11.0) mm Hg and the median IBP was 8.8(5.8-13.1) mm Hg. The overall r2 value( n = 30) was 0.76(P < 0.0001). Bland and Altman analysis showed an overall bias for the mean values per patient of 0.6 mm Hg(2SD 4.2 mm Hg) with percentage error of 45.6%. Looking at changes in IAP between the different study stages, we found an excellent concordance coefficient of 94.9% comparing IBP and IGP( n = 117).CONCLUSION: During ventral hernia repair, the IAP rise is accompanied by changes in Р(а-et)CO2 and PаO2/Fi O2-ratio.
