1中国103个地区吸烟与食管癌风险研究:死因调查中的病例对照方法学研究显示文摘目的探讨吸烟与食管癌死亡危险的关系,验证在死因调查中同时进行病例对照研究方法的可行性与科学性。方法分别采用非随机和随机方式在中国选择24个大中城市以及79个农村县进行死因调查,并同时开展以人群为基础的病例对照方法学研究。病例为1986至1988年调查地区内的死于食管癌、年龄≥35岁的男性患者,同时设置两组对照,对照组Ⅰ为同期死于其他消化道疾病、年龄≥35岁的男性患者;对照组Ⅱ为同期死亡的女性患者(全死因)的存活配偶,并且他们在其妻死亡时的年龄≥35岁。主要测定指标为按地区分别计算吸烟与不吸烟人群中食管癌绝对死亡率以及吸烟与食管癌死亡风险的关系。结果(1)在病例与对照组Ⅰ中:非吸烟者/吸烟者食管癌绝对死亡率(1/千人)城市为0.39/0.63,内陆农村为1.06/1.24,沿海农村为1.18/1.55。对照组Ⅱ与对照组Ⅰ的趋势相同;每日吸烟支数与食管癌的死亡风险存在显著的剂量反应关系。每日吸烟<10、10~、20~支的相对危险度值在对照组Ⅰ中为1.42、1.82、2.22,在对照组Ⅱ中为1.57、1.95、3.18。吸烟年限与食管癌的死亡风险同样存在显著的剂量反应关系。(2)采用不同的对照,所得出的结果具有较好的一致性。结论吸烟是食管癌重要的致病因素,在死因调查中进行病例对照研究是一种创新的、科学而易行的研究方法。刘伯齐 姜晶梅 陈铮鸣 陈君石 张孔来 曾宪嘉 赵平 Jillian Boreham 武燕平 黎均耀 Richard Peto 2006中华医学杂志2006,86,6:24
2Targeting matrix metalloproteinases in cancer:Bringing new life to old ideas显示文摘Since the identification of matrix metalloproteinases(MMPs),a family of zincdependent endopeptidases,as being a driving factor for cancer progression and patient prognosis,MMPs have been studied extensively.Although early programs targeting MMPs were largely unsuccessful in clinical trials,they remain a viable and highly desirable therapeutic target based on preclinical studies and their role in disease progression.As information regarding the structure and function of these proteinases is compiled and biotechnology evolves,tools to develop better inhibitors are within our grasp.Improved methods for high throughput screening and in silico drug design programs have identified compounds which are highly potent,have high binding affinities,and exhibit favorable pharmacokinetic profiles.More recently,advances in drug delivery methods or compounds which bind outside the active site have brought new light to the field.In this review,we highlight the role of MMPs in cancer,clinical trials for MMP inhibitors,and novel approaches to targeting MMPs in cancer.Jillian Cathcart Ashleigh Pulkoski-Gross Jian Cao 2015Genes & Diseases2015,2,1:18

4Lubiprostone vs Senna in postoperative orthopedic surgery patients with opioid-induced constipation:A double-blind,active-comparator trial显示文摘AIM:To investigate the efficacy of lubiprostone compared to Senna on bowel symptoms and constipation in post-operative orthopedic patients treated with opioids.METHODS:In this double blind,randomized,active comparator trial,adults who required opioids for analgesia following orthopedic procedures and who were admitted in inpatient rehabilitation were randomized following baseline assessments to lubiprostone(Amitza),orally twice aday or Senna(generic)two capsules administered daily for six days.Subjects were assessed using the patient assessment of constipation(PAC)-symptoms(PAC-SYM)and the PAC-quality of life(PAC-QOL)scales measured at baseline and Day 7;Subjects were assessed daily for secondary measures included the Bristol stool scale bowel consistency,specific bowel symptom score(Nausea,cramping,straining,completeness,abdominal pain,time per lavatory attempt,assistance needed),adverse events and rescue medications required.Function was measured using the functional independence measure(FIM)at admission and discharge;length of stay(LOS)and missed treatments due to gastrointestinal symptoms were also assessed.RESULTS:64 adults were enrolled;56 participants(28 in each group)had baseline and follow up measures and were included in the intention to treat(ITT)analyses.43 participants completed the study,21 in the active lubiprostone and 22 in the active Senna group.The mean age of the participants was 71.5years(SD=11.4 years,range:28-96 years).In the ITT analyses,participants showed significant improvement in bowel symptoms as measured by the PACSYM(mean±SD,-0.28±0.60,range:-1-2.33)and PAC-QOL(mean±SD,0.33±0.81,range:-1.5-2.0)over time,but there were no significant differences between the lubiprostone and Senna groups in mean change in the PAC-SYM(-0.20±0.60 vs-0.36±0.61,P=0.61 respectively)or the PAC-QOL(0.29±0.76 vs0.37±0.87,P=0.61 respectively).The mean change in each bowel symptom also did not significantly differ between treatment groups on ITT analyses,except for completeness of bowel movement,with the Senna group showing greater negative mean change in bowel movement completeness(-0.56±1.01 vs-2.00±1.41,P=0.03)and for reduction of abdominal pain,favoring Senna(-0.14±0.73 vs-0.73±1.08,P=0.04).Fifteen(75%)participants in the lubiprostone and in the Senna group requested rescue treatments.Participants made significant functional improvement from admission to discharge over a median LOS of 12 d,with a mean FIM change of 29.13±13.58 and no significant between group differences(27.0±9.2 vs 31.5±16.6,P=0.27).CONCLUSION:Both lubiprostone and Senna improved constipation-related symptoms and QOL in opioid-induced constipation,with no significant between-group differences.Christina M Marciniak Santiago Toledo Jungwhalia Lee Michael Jesselson Jillian Bateman Benjamin Grover Joy Tierny 2014World Journal of Gastroenterology2014,20,43:7
5Sieving characteristics of cytokine-and peroxide-induced epithelial barrier leak:Inhibition by berberine显示文摘AIM:To study whether the inflammatory bowel disease(IBD)colon which exhibits varying severity and cytokine levels across its mucosa create varying types of transepithelial leak.METHODS:We examined the effects of tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),interleukin-1-β(IL1β)and hydrogen peroxide(H_2O_2)-singly and in combinations-on barrier function of CACO-2 cell layers.Our focus was on the type(not simply the magnitude)of transepithelial leak generated by these agents as measured by transepithelial electrical resistance(TER)and transepithelial flux of ^(14)C-D-mannitol,3H-Lactulose and ^(14)C-Polyethylene glycol as radiolabeled probe molecules.The isoquinoline alkaloid,berberine,was then examined for its ability to reduce specific types of transepithelial leak.RESULTS:Exposure to TNF-α alone(200 ng/mL;48 h)induced a 50% decrease in TER,i.e.,increased leak of Na^+ and Cl--with only a marginal but statistically significant increase in transepithelial leak of ^(14)C-mannitol(Jm).Exposure to TNF-α + IFN-γ(200 ng/mL;48 h)+ IL1β(50 ng/mL;48 h)did not increase the TER change(from TNF-α alone),but there was now a 100% increase in J_m.There however was no increase in transepithelial leak of two larger probe molecules,~3H-lactulose and ^(14)C-polyethylene glycol(PEG).However,exposure to TNF-α + IFN-γ + IL1β followed by a 5 h exposure to2 mmol/L H_2O_2 resulted in a 500% increase in ^(14)C-PEG leak as well as leak to the luminal mitogen,epidermal growth factor.CONCLUSION:This model of graded transepithelial leak is useful in evaluating therapeutic agents reducing IBD morbidity by reducing barrier leak to various luminal substances.Katherine M DiGuilio Christina M Mercogliano Jillian Born Brendan Ferraro Julie To Brittany Mixson Allison Smith Mary Carmen Valenzano James M Mullin 2016World Journal of Gastrointestinal Pathophysiology2016,7,2:6

10Geochemistry of the Batang Group in the Zhaokalong Area, Yushu, Qinghai: Implications for the Late Triassic Tectonism in the Northern Sanjiang Region, China显示文摘The Batang Group in the Zhaokalong area not only hosts an important Fe-Cu polymetallic deposit but also reveal important insights for the Late Triassic tectonism in the northern Sanjiang region. In order to delineate the tectonic setting and evolutionary process of the paleo arc-basin system, geochemical studies on the Batang Group strata have been carried out. The results suggest that andesite in the Zhaokalong area mainly belongs to the tholeiite series and is characterized by obvious fractionation of LREE and HREE, enrichment in LILE and depletion in HFSE, and a distinctly elevated δ^(34)SCDT average of 10.5‰. The sandstone is classified as lithic sandstone, which is also characterized by obvious fractionation of LREE and HREE, as well as weak negative Ce and Eu anomalies. The limestone displays positive Eu anomalies, with δ^(13)CPDB ranging from-1.3‰ to 4.4‰ and δ^(18) OSMOW ranging from 14.6‰ to 22.5‰. These results indicate that the andesite has a dual signature of both arc andesite and rift volcanic rocks, whereas the sandstone may be formed in an active continental margin, and the limestone could be deposited in a weak oxidizing shallow sea. The sandstone in the Zhaokalong area represents sedimentation in a platform slope facies, corresponding to the main stage of the Jinshajing oceanic basin subduction during the middle period of Late Triassic. The continental arc volcanic activity resulted from subsequent strengthened subduction, forming the andesite in the North Qiangtang backarc basin. Afterwards, the limestone was formed after the cessation of magmatic activity. 11Safe acupuncture and dry needling during pregnancy:New Zealand physiotherapists' opinion and practice显示文摘Objective: Acupuncture guidelines have advised caution when treating women during pregnancy,because historical ‘‘forbidden' acupuncture points are believed to stimulate miscarriage or early labor.Despite recent research demonstrating that acupuncture is a useful and safe treatment tool for pregnancy-related low-back pain(LBP) and pelvic girdle pain(PGP), it is postulated that fear of miscarriage and subsequent blame by association, restricts its provision. More recently, an increase in dry needling(DN) courses for physiotherapists has potentiated the rapid growth in DN practice in New Zealand(NZ). Many dry needlers do not consider DN to be a form of acupuncture; it is unknown if they have similar safety concerns.Methods: NZ registered physiotherapists practicing acupuncture and/or DN were invited to participate in an electronic survey to examine their practice and level of understanding in regard to safe needling during pregnancy.Results: Of 124 respondents, only 60(48%) would needle pregnant women, with a further 66% of those still expressing safety concerns. NZ physiotherapists practicing DN only, were more likely to needle areas related to ‘‘forbidden' points in all trimesters. However, overall, NZ physiotherapists were less likely to needle ‘‘forbidden' points than their UK peers.Conclusion: Conflicting literature and a ‘‘fear of blame' influences NZ physiotherapists' decisions to offer needling(both acupuncture and DN) during pregnancy. Further training in this field is recommended to ensure safe practice and adequate provision of acupuncture treatment options for pregnant women suffering musculoskeletal pain, such as LBP and PGP. 14Quantitative analysis of opioids and cannabinoids in wastewater samples显示文摘Wastewater-based epidemiology is an innovative approach that uses the analysis of human excretion products in wastewater to obtain information about exposure to drugs in defined population groups.We developed and validated an analytical method for the simultaneous determination of opioids(morphine,oxycodone,hydrocodone,oxymorphone and hydromorphone),and cannabinoids(△9-tetrahydrocannabinol,11-nor-9-carboxy-tetrahydrocannabinol(THCCOOH)and THCCOOH-glucuronide)in raw-influent wastewater samples by ultra-high performance liquid chromatography-tandem mass spectrometry.Method validation included linearity(5-1000 ng/L for opioids,10-1000 ng/L for cannabinoids),imprecision(<21.2%),accuracy(83%-131%),matrix effect(from-35.1%to-14.7%)and extraction efficiency(25%-84%),limit of detection(1-5 ng/L)and quantification(5-10 ng/L)and auto-sampler stability(no loss detected).River and wastewater samples were collected in triplicate from different locations in New York City and stored at-20℃until analysis.Water from sewage overflow location tested positive for morphine(10.7 ng/L),oxycodone(4.2-23.5 ng/L),oxymorphone(4.8 ng/L)and hydromorphone(4.2 ng/L).Raw influent wastewater samples tested positive for morphine(133.0-258.3 ng/L),oxycodone(31.1-63.6 ng/L),oxymorphone(16.0-56.8 ng/L),hydromorphone(6.8-18.0 ng/L),hydrocodone(4.0-12.8 ng/L)and THCCOOH(168.2-772.0 ng/L).This method is sensitive and specific for opioids and marijuana determination in wastewater samples.Alethea Jacox Jillian Wetzel Shu-Yuan Cheng Marta Concheiro 2017Forensic Sciences Research2017,2,1:3

17Characterization of the proto-Philippine Sea Plate:Evidence from the emplaced oceanic lithospheric fragments along eastern Philippines显示文摘The proto-Philippine Sea Plate(pPSP)has been proposed by several authors to account for the origin of the Mesozoic supra-subduction ophiolites along the Philippine archipelago.In this paper,a comprehensive review of the ophiolites in the eastern portion of the Philippines is undertaken.Available data on the geology,ages and geochemical signatures of the oceanic lithospheric fragments in Luzon(Isabela,Lagonoy in Camarines Norte,and Rapu-Rapu island),Central Philippines(Samar,Tacloban,Malitbog and Southeast Bohol),and eastern Mindanao(Dinagat and Pujada)are presented.Characteristics of the Halmahera Ophiolite to the south of the Philippines are also reviewed for comparison.Nearly all of the crust-mantle sequences preserved along the eastern Philippines share Early to Late Cretaceous ages.The geochemical signatures of mantle and crustal sections reflect both mid-oceanic ridge and suprasubduction signatures.Although paleomagnetic information is currently limited to the Samar Ophiolite,results indicate a near-equatorial Mesozoic supra-subduction zone origin.In general,correlation of the crust-mantle sequences along the eastern edge of the Philippines reveal that they likely are fragments of the Mesozoic pPSP.Carla B.Dimalanta Decibel V.Faustino-Eslava Jillian Aira S.Gabo-Ratio Edanjarlo J.Marquez Jenielyn T.Padrones Betchaida D.Payot Karlo L.Queano Noelynna T.Ramos Graciano P.Yumul Jr. 2020Geoscience Frontiers2020,11,1:2
