| 2 | Foveal avascular zone area and parafoveal vessel density measurements in different stages of diabetic retinopathy by optical coherence tomography angiography显示文摘AIM:To investigate foveal avascular zone(FAZ) and parafoveal vessel densities(PRVD) by means of optical coherence tomography angiography(OCTA) in diabetic patients with or without diabetic retinopathy(DR) and to assess the reproducibility of FAZ and PRVD measurements.METHODS:Sixty diabetic patients(60 eyes) with different stage of DR(graded according to the International Clinical Severity Scale for DR) and 20 healthy subjects underwent FAZ area and PRVD measurements using OCTA by two experienced examiners.FAZ area in all patients was also assessed using fluorescein angiography(FA).RESULTS:In subject with proliferative DR and with moderate-severe non proliferative DR,FAZ area was significantly increased compared to healthy controls(P=0.025 and P=0.050 respectively measured with OCTA and P=0.025 and P=0.048 respectively measured with FA).OCTA showed significantly less inter-observer variability compared to FA.Concordance correlation coefficient(CCC) for FAZ area measurements was 0.829(95%CI:0.736-0.891) P<0.001 with FA and 1.000(95%CI:0.999-1.000) P<0.001 with OCTA.CCC was 0.834(95%CI:0.746-0.893) P<0.001 and 0.890(95%CI:0.828-0.930) P<0.001 for parafoveal superficial and deep vessel density measurements,respectively.CONCLUSION:OCTA shows progressive increase of FAZ area and reduction of PRVD in both superficial and deep plexus at increasing DR severity.FAZ area and PRVD measurements using OCTA are highly reproducible. | Rodolfo Mastropasqua Lisa Toto Alessandra Mastropasqua Raffaella Aloia Chiara De Nicola Peter A Mattei Guido Di Marzio Marta Di Nicola Luca Di Antonio | 2017 | International Journal of Ophthalmology(English edition)2017，10，10： | 26 |

| 5 | Successful photodynamic therapy for biliary papillomatosis:A case report显示文摘Papillomatosis of the bile duct is a rare disease with a high risk of malignant transformation. Therapeutical options include partial hepatectomy and liver transplantation. A previously healthy 65-years old male developed jaundice and right upper abdominal quadrant pain in 1996. A villous adenoma of the distal bile duct was diagnosed. A Whipple procedure was performed. In 2002 the patient turned symptomatic again. Another adenoma was found in the right hepatic duct resulting in a right hepatectomy. Two years later the patient again developed cholestasis. After drainage of the left hepatic duct with a percutaneous transhepatic cholangial drainage (PTCD) catheter, a recurrent biliary adenomatosis was diagnosed by cholangioscopy. As there was no surgical option left, the patient received photodynamic therapy (PDT) for the recurrent biliary papillomatosis. Three mo after he received further photodynamic therapies, the bile duct epithelium appeared normal and the patient had no signs of adenomatosis, both macroscopically and histologically. The follow-up cholangioscopy in late 2005 revealed only a small papilloma without the need for intervention. In early 2006, the patient died of multi organ failure without signs of extrahepatic cholestasis or cholangitis at the age of 75, 10 years after the diagnosis of biliary papillomatosis was established. The patient exceeded the average life expectancy of patients with biliary papillomatosis by far. | 8 | Prospective evaluation of the short access cholangioscopy for stone clearance and evaluation of indeterminate strictures显示文摘BACKGROUND: Peroral cholangioscopy facilitates diagnosis and therapy of biliary disorders. This study prospectively evaluated a new short access cholangioscopy. METHODS: Consecutive patients were included as follows: difficult stones(group 1) underwent cholangioscopy with electrohydraulic lithotripsy and indeterminate biliary strictures(group 2) were evaluated with macroscopic assessment and cholangioscopy guided biopsy sampling. We evaluated the complete stone clearance rate(group 1) and diagnostic accuracy(group 2). Follow-up was performed over a median of 13 and 16 months, respectively.RESULTS: Group 1(n=21): complete stone clearance defined as lack of stones in cholangiography and stone removal during cholangioscopy was achieved in 15(71.4%) patients. Clinical stone clearance defined as lack of symptoms, laboratory abnormalities and hospital visits during follow-up, irrespective of stone clearance was evident in 17(81.0%) patients. One serious adverse event occurred(bile duct perforation). Group 2(n=28): malignancy was confirmed in 15 patients. Sensitivity, specificity and diagnostic accuracy of cholangioscopy were 85.7%, 75.0% and 80.7%, respectively. Sensitivity, specificity and diagnostic accuracy of biopsies were 54.5%, 100.0% and 72.2%, respectively. No serious adverse events occurred, and one patient was lost to follow-up.CONCLUSIONS: The novel system enabled complex stone treatment and biliary stricture diagnosis.
