| 7 | High-flow nasal oxygen availability for sedation decreases the use of general anesthesia during endoscopic retrograde cholangiopancreatography and endoscopic ultrasound显示文摘AIM To examine whether high-flow nasal oxygen(HFNO) availability influences the use of general anesthesia(GA) in patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) and endoscopic ultrasound(EUS) and associated outcomes.METHODS In this retrospective study, patients were stratified into 3 eras between October 1, 2013 and June 30, 2014 based on HFNO availability for deep sedation at the time of their endoscopy. During the first and last 3-mo eras(era 1 and 3), no HFNO was available, whereas it was an option during the second 3-mo era(era 2). The primary outcome was the percent utilization of GA vs deep sedation in each period. Secondary outcomes included oxygen saturation nadir during sedation between periods, as well as procedure duration, and anesthesia-only time between periods and for GA vs sedation cases respectively.RESULTS During the study period 238 ERCP or EUS cases were identified for analysis. Statistical testing was employed and a P < 0.050 was significant unless the Bonferroni correction for multiple comparisons was used. General anesthesia use was significantly lower in era 2 compared to era 1 with the same trend between era 2 and 3(P = 0.012 and 0.045 respectively). The oxygen saturation nadir during sedation was significantly higher in era 2 compared to era 3(P < 0.001) but not between eras 1 and 2(P = 0.028) or 1 and 3(P = 0.069). | 8 | To mask or not to mask:Modeling the potential for face mask use by the general public to curtail the COVID-19 pandemic显示文摘Face mask use by the general public for limiting the spread of the COVID-19 pandemic is controversial,though increasingly recommended,and the potential of this intervention is not well understood.We develop a compartmental model for assessing the communitywide impact of mask use by the general,asymptomatic public,a portion of which may be asymptomatically infectious.Model simulations,using data relevant to COVID-19 dynamics in the US states of New York and Washington,suggest that broad adoption of even relatively ineffective face masks may meaningfully reduce community transmission of COVID-19 and decrease peak hospitalizations and deaths.Moreover,mask use decreases the effective transmission rate in nearly linear proportion to the product of mask effectiveness(as a fraction of potentially infectious contacts blocked)and coverage rate(as a fraction of the general population),while the impact on epidemiologic outcomes(death,hospitalizations)is highly nonlinear,indicating masks could synergize with other nonpharmaceutical measures.Notably,masks are found to be useful with respect to both preventing illness in healthy persons and preventing asymptomatic transmission.Hypothetical mask adoption scenarios,for Washington and New York state,suggest that immediate near universal(80%)adoption of moderately(50%)effective masks could prevent on the order of 17e45%of projected deaths over two months in New York,while decreasing the peak daily death rate by 34e58%,absent other changes in epidemic dynamics.Even very weak masks(20%effective)can still be useful if the underlying transmission rate is relatively low or decreasing:InWashington,where baseline transmission is much less intense,80%adoption of such masks could reduce mortality by 24e65%(and peak deaths 15e69%),compared to 2e9%mortality reduction in New York(peak death reduction 9e18%).Our results suggest use of face masks by the general public is potentially of high value in curtailing community transmission and the burden of the pandemic.The community-wide benefits are likely to be greatest when face masks are used in conjunction with other non-pharmaceutical practices(such as social-distancing),and when adoption is nearly universal(nation-wide)and compliance is high. | Steffen E.Eikenberry Marina Mancuso Enahoro Iboi Tin Phan Keenan Eikenberry Yang Kuang Eric Kostelich Abba B.Gumel | 2020 | Infectious Disease Modelling2020,5,1: | 8 |

| 13 | Predictors of re-hospitalization in patients with chronic heart failure显示文摘Heart failure (HF) is a chronic, progressive illness that is highly prevalent in the United States and worldwide. This morbid illness carries a very poor prognosis, and leads to frequent hospitalizations. Repeat hospitalization in HF is both largely burdensome to the patient and the healthcare system, as it is one of the most costly medical diagnoses among Medicare recipients. For years, investigators have strived to determine methods to reduce hospitalization rates of HF patients. Despite such efforts, recent reports indicate that rehospitalization rates remain persistently high, without any improvement over the past several years and thus, this topic clearly needs aggressive attention. We performed a key-word search of the literature for relevant citations. Published articles, limited to English abstracts indexed primarily in the PubMed database through the year 2011, were reviewed. This article discusses various clinical parameters, serum biomarkers, hemodynamic parameters, and psychosocial factors that have been reviewed in the literature as predictors of re-hospitalization of HF patients. | 14 | Numerical investigation of aerodynamic flow actuation produced by surface plasma actuator on 2D oscillating airfoil显示文摘Numerical simulation of unsteady flow control over an oscillating NACA0012 airfoil is investigated. Flow actuation of a turbulent flow over the airfoil is provided by low current DC surface glow discharge plasma actuator which is analytically modeled as an ion pressure force produced in the cathode sheath region. The modeled plasma actuator has an induced pressure force of about 2 k Pa under a typical experiment condition and is placed on the airfoil surface at 0% chord length and/or at 10% chord length. The plasma actuator at deep-stall angles(from 5° to 25°) is able to slightly delay a dynamic stall and to weaken a pressure fluctuation in down-stroke motion. As a result, the wake region is reduced. The actuation effect varies with different plasma pulse frequencies, actuator locations and reduced frequencies. A lift coefficient can increase up to 70% by a selective operation of the plasma actuator with various plasma frequencies and locations as the angle of attack changes. | 15 | Can tranexamic acid change preoperative anemia management during total joint arthroplasty?显示文摘AIM: To investigate the postoperative transfusion and complication rates of anemic and nonanemic total joint arthroplasty patients given tranexamic acid(TXA).METHODS: A cross-sectional prospective study was conducted of primary hip and knee arthroplasty cases performed from 11/2012 to 6/2014. Exclusion criteria included revision arthroplasty, bilateral arthroplasty, acute arthroplasty after fracture, and contraindication to TXA. Patients were screened prior to surgery, with anemia was defined as hemoglobin of less than 12 g/d L for females and of less than 13 g/d L for males. Patients were divided into four different groups, based on the type of arthroplasty(total hip or total knee) and hemoglobin status(anemic or nonanemic). Intraoperatively, all patients received 2 g of intravenous TXA during surgery. Postoperatively, allogeneic blood transfusion(ABT) was directed by both clinical symptomsand relative hemoglobin change. Complications were recorded within the first two weeks after surgery and included thromboembolism, infection, and wound breakdown. The differences in transfusion and complication rates, as well as the relative hemoglobin change, were compared between anemic and nonanemic groups.RESULTS: A total of 232 patients undergoing primary joint arthroplasty were included in the study. For the total hip arthroplasty cohort, 21%(18/84) of patients presented with preoperative anemia. Two patients in the anemic group and two patients in the nonanemic group needed ABTs; this was not significantly different(P = 0.20). One patient in the anemic group presented with a deep venous thromboembolism while no patients in the nonanemic group had an acute complication; this was not significantly different(P = 0.21). For nonanemic patients, the average change in hemoglobin was 2.73 ± 1.17 g/d L. For anemic patients, the average change in hemoglobin was 2.28 ± 0.96 g/d L. Between the two groups, the hemoglobin difference of 0.45 g/d L was not significant(P = 0.13). For the total knee arthroplasty cohort, 18%(26/148) of patients presented with preoperative anemia. No patients in either group required a blood transfusion or had an acute postoperative complication. For nonanemic patients, the average change in hemoglobin was 1.85 ± 0.79 g/d L. For anemic patients, the average change in hemoglobin was 1.09 ± 0.58 g/d L. Between the two groups, the hemoglobin difference of 0.76 g/d L was significant(P < 0.001).
