| 1 | Endoscopic management of esophageal varices显示文摘The rupture of gastric varices results in variceal hemorrhage, which is one the most lethal complications of cirrhosis. Endoscopic therapies for varices aim to reduce variceal wall tension by obliteration of the varix. The two principal methods available for esophageal varices are endoscopic sclerotherapy (EST) and band ligation (EBL). The advantages of EST are that it is cheap and easy to use, and the injection catheter fits through the working channel of a diagnostic gastroscope. Endoscopic variceal ligation obliterates varices by causing mechanical strangulation with rubber bands. | 2 | Laparoscopic approach in gastrointestinal emergencies显示文摘This review focuses on the laparoscopic approach to gastrointestinal emergencies and its more recent indications. Laparoscopic surgery has a specific place in elective procedures, but that does not apply in emergency situations. In specific emergencies, there is a huge range of indications and different techniques to apply, and not all of them are equally settle. We consider that the most controversial points in minimally invasive procedures are indications in emergency situations due to technical difficulties. Some pathologies, such as oesophageal emergencies, obstruction due to colon cancer, abdominal hernias or incarcerated postsurgical hernias, are nearly always resolved by conventional surgery, that is, an open approach due to limited intraabdominal cavity space or due to the vulnerability of the bowel. These technical problems have been solved in many diseases, such as for perforated peptic ulcer or acute appendectomy for which a laparoscopic approach has become a wellknown and globally supported procedure. On the other hand, endoscopic procedures have acquired further indications, relegating surgical solutions to a second place; this happens in cholangitis or pancreatic abscess drainage. This endoluminal approach avoids the need for laparoscopic development in these diseases. Nevertheless, new instruments and new technologies could extend the laparoscopic approach to a broader array of potentials procedures. | 3 | Endoscopic and anesthetic feasibility of EUS and ERCP combined in a single session versus two different sessions显示文摘AIM:To discuss the feasibility of single session endoscopic ultrasonography(EUS) to discuss and endoscopic retrograde cholangiopancreatography(ERCP) execution.METHODS:Retrospective endoscopic and anesthetic outcome comparison of performing both EUS and ERCP in a single endoscopic session(Group Ⅰ) versus performing each procedure in two different sessions(Group Ⅱ) was made.The following variables were evaluated:epidemiological variables,American Society of Anesthesiologists Physical Status Classification(ASA) level,procedural time,propofol dose,anesthetic complications,endoscopic complications and diagnostic yield,and therapeutic procedures on both groups.T-student,ChiSquare and Fisher test were used for comparison.RESULTS:We included 39 patients in Group Ⅰ(mean age:69.85 ± 9.25;27 men) and 46 in Group Ⅱ(mean age:67.46 ± 12.57;25 men).Procedural time did not differ significantly between both groups(Group Ⅰvs Group Ⅱ:93 ± 32.78 vs 98.98 ± 38.17;P >0.05) but the dose of propofol differed(Group Ⅰ vs Group Ⅱ:322.28 ± 250.54 mg vs 516.96 ± 289.06 mg;P = 0.001).Three patients had normal findings on both explorations.Three anesthetic complications [O2 desaturation(2),broncoaspiration(1)] and 9 endoscopic complications [pancreatitis(6),bleeding(1),perforation(1),cholangitis(1)] occurred without significant differences between both groups(P > 0.05).We did not find any significant difference regarding age,sex,ASA scale level,diagnostic yield or therapeutic maneuvers between both groups.CONCLUSION:The performance of EUS and ERCP in a single session offers a similar diagnostic and therapeutic yield,does not entail a higher complication risk and requires a significantly smaller dose of propofol for sedation compared with performing each exploration in a different session. | Juan J Vila Marcos Kutz Silvia Goi Miriam Ostiz Edurne Amorena Carlos Prieto Cristina Rodriguez Ignacio Fernández-Urien Francisco J Jiménez | 2011 | World Journal of Gastrointestinal Endoscopy2011，3，3： | 3 |

| 5 | Development and test verification of air temperature model for Chinese solar and Spainish Almeria-type greenhouses显示文摘Growth can be defined as an increment in biomass or an increment in weight or height of the organs of the plant influenced by physiological processes.Many of these processes have their limits genetically determined,but climate and irrigation play an important role.Because of its importance,microclimate has been extensively studied in the modeling as a surrounding condition which is imposed by the exterior climate.The main objective of this work was to develop a temperature model based on the energy balance dynamics at two different greenhouse locations-South-eastern Spain and Northern China,and the traditional structures of Chinese solar greenhouse and Almería-type multi-span greenhouse were taken into account.The final model was developed by combining the external conditions,the actuator influence and the crop growth,where the temperature is influenced by soil,crop,cover,actuators,back wall and greenhouse geometry.The model took into account the energy lost by convective and conductive fluxes,as well as the energy supplied by solar radiation and heating systems.The soil and the back wall are the main media for energy storage.The temperature dynamic was determined by a physical model,which considered the energy balance from a holistic point of view-as a sub-model for a customizable interface among the external climate,the plant and the greenhouse system.The influences of different subsystems included in the temperature model were analyzed and evaluated.The results showed a high R^(2)value of 0.94 for Beijing and 0.95 for Almeria,and the average error was low,of which the MAE and RMSE were 0.71 and 1.365 for Almeria and 0.62 and 1.102 for Beijing,respectively.Thus,the model can be considered as a powerful tool for control design purposes in microclimate systems. | Jorge Antonio Sanchez-Molina Li Ming Francisco Rodriguez Jose Luis Guzman Wang Hui Yang Xinting | 2017 | International Journal of Agricultural and Biological Engineering2017，10，4： | 2 |
