| 2 | Management of proximal humerus fractures in adults显示文摘The majority of proximal humerus fractures are lowenergy osteoporotic injuries in the elderly and their incidence is increasing in the light of an ageing population. The diversity of fracture patterns encountered renders objective classification of prognostic value challenging. Non-operative management has been associated with good functional outcomes in stable, minimally displaced and certain types of displaced fractures.Absolute indications for surgery are infrequent and comprise compound, pathological, multi-fragmentary head-splitting fractures and fracture dislocations, as well as those associated with neurovascular injury. A constantly expanding range of reconstructive and replacement options however has been extending the indications for surgical management of complex proximal humerus fractures. As a result, management decisions are becoming increasingly complicated, in an attempt to provide the best possible treatment for each indi-vidual patient, that will successfully address their specific fracture configuration, comorbidities and functional expectations. | 3 | Genetic association of interleukin-6 polymorphism (-174 G/C) with chronic liver diseases and hepatocellular carcinoma显示文摘Interleukin-6 (IL-6) is a pleiotropic cytokine which is expressed in many inflammatory cells in response to different types of stimuli, regulating a number of biological processes. The IL-6 gene is polymorphic in both the 5' and 3' flanking regions and more than 150 single nucleotide polymorphisms have been identified so far. Genetic polymorphisms of IL-6 may affect the outcomes of several diseases, where the presence of high levels of circulating IL-6 have been correlated to the stage and/or the progression of the disease itself. The -174 G/C polymorphism is a frequent polymorphism, that is located in the upstream regulatory region of the IL-6 gene and affects IL-6 production. However, the data in the literature on the genetic association between the -174 G/C polymorphism and some specific liver diseases characterized by different etiologies are still controversial. In particular, most of the studies are quite unanimous in describing a correlation between the presence of the high-producer genotype and a worse evolution of the chronic liver disease. This is valid for patients with hepatitis C virus (HCV)-related chronic hepatitis and liver cirrhosis and hepatocellu-lar carcinoma (HCC) whatever the etiology. Studies in hepatitis B virus-related chronic liver diseases are not conclusive, while specific populations like non alcoholic fatty liver disease/non-alcoholic steatohepatitis, autoimmune and human immunodeficiency virus/HCV coinfected patients show a higher prevalence of the lowproducer genotype, probably due to the complexity of these clinical pictures. | 4 | Comparative cytotoxicity of fourteen trivalent and pentavalent arsenic species determined using real-time cell sensing显示文摘The occurrence of a large number of diverse arsenic species in the environment and in biological systems makes it important to compare their relative toxicity. The toxicity of arsenic species has been examined in various cell lines using different assays, making comparison difficult. We report real-time cell sensing of two human cell lines to examine the cytotoxicity of fourteen arsenic species: arsenite(As~Ⅲ), monomethylarsonous acid(MMA~Ⅲ) originating from the oxide and iodide forms, dimethylarsinous acid(DMA~Ⅲ), dimethylarsinic glutathione(DMAG~Ⅲ), phenylarsine oxide(PAO~Ⅲ), arsenate(AsV), monomethylarsonic acid(MMA~Ⅴ), dimethylarsinic acid(DMA~Ⅴ),monomethyltrithioarsonate(MMTTA~Ⅴ), dimethylmonothioarsinate(DMMTA~Ⅴ),dimethyldithioarsinate(DMDTA~Ⅴ), 3-nitro-4-hydroxyphenylarsonic acid(Roxarsone, Rox),and 4-aminobenzenearsenic acid(p-arsanilic acid, p-ASA). Cellular responses were measured in real time for 72 hr in human lung(A549) and bladder(T24) cells. IC50 values for the arsenicals were determined continuously over the exposure time, giving rise to IC50 histograms and unique cell response profiles. Arsenic accumulation and speciation were analyzed using inductively coupled plasma-mass spectrometry(ICP-MS). On the basis of the 24-hr IC50 values, the relative cytotoxicity of the tested arsenicals was in the following decreasing order: PAO~Ⅲ? MMA~Ⅲ≥ DMA~Ⅲ≥ DMAG~Ⅲ≈ DMMTA~Ⅴ≥ As~Ⅲ? | 7 | Prevention and management of hepatitis B virus reactivation in patients with hematological malignancies treated with anticancer therapy显示文摘Hepatitis due to hepatitis B virus(HBV) reactivation can be severe and potentially fatal, but is preventable. HBV reactivation is most commonly reported in patients receiving cancer chemotherapy, especially rituximabcontaining therapy for hematological malignancies and those receiving stem cell transplantation. All patients with hematological malignancies receiving anticancer therapy should be screened for active or resolved HBV infection by blood tests for hepatitis B surface antigen(HBs Ag) and antibody to hepatitis B core antigen(antiHBc). Patients found to be positive for HBs Ag should be given prophylactic antiviral therapy to prevent HBV reactivation. For patients with resolved HBV infection, no standard strategy has yet been established to prevent HBV reactivation. There are usually two options. One is pre-emptive therapy guided by serial HBV DNA monitoring, whereby antiviral therapy is given as soon as HBV DNA becomes detectable. However, there is little evidence regarding the optimal interval and period of monitoring. An alternative approach is prophylactic antiviral therapy, especially for patients receiving highrisk therapy such as rituximab, newer generation of anti-CD20 monoclonal antibody, obinutuzumab or hematopoietic stem cell transplantation. This strategy may effectively prevent HBV reactivation and avoid the inconvenience of repeated HBV DNA monitoring. Entecavir or tenofovir are preferred over lamivudine as prophylactic therapy. Although there is no well-defined guideline on the optimal duration of prophylactic therapy, there is growing evidence to recommend continuing prophylactic antiviral therapy for at least 12 mo after cessation of chemotherapy, and even longer for those who receive rituximab or who had high serum HBV DNA levels before the start of immunosuppressive therapy. Many novel agents have recently become available for the treatment of hematological malignancies, and these agents may be associated with HBV reactivation. Although there is currently limited evidence to guide the optimal preventive measures, we recommend antiviral prophylaxis in HBs Ag-positive patients receiving novel treatments, especially the Bruton tyrosine kinase inhibitors and the phosphatidylinositol 3-kinase inhibitors, which are B-cell receptor signaling modulators and reduce proliferation of malignant B-cells. | 11 | Conversion from calcineurin inhibitors to mTOR inhibitors stabilizes diabetic and hypertensive nephropathy after liver transplant显示文摘AIM: To investigate if conversion to the mammalian target of rapamycin inhibitors(mTORi) improves renal function in diabetic and/or hypertensive liver transplant patients immunosuppressed with tacrolimus or cyclosporine.METHODS: The study included 86 liver graft recipients immunosuppressed with mTORi treatment after orthotopic liver transplantation(OLT), including all liver recipients with worsening renal function before conversion to mTORi(n = 55 patients) and recipients with normal renal function who converted to m TORi for other reasons(n = 31 patients). We identified patients with diabetes mellitus(n = 28), arterial hypertension(n = 27), proteinuria(n = 27) and all three factors(n = 8)(some patients have hypertension and diabetes and no proteinuria). The primary endpoint was evolution in renal function defined as the development in plasma creatinine as a function of diabetes mellitus(DM), hypertension(HT) or proteinuria. We required elevated serum creatinine for at least two weeks to define renal dysfunction.RESULTS: Only patients that converted because of renal failure with plasma creatinine levels > 1.5 mg/dL showed an improvement of renal function(2.14 to 1.77 mg/dL)(P = 0.02). Patients with DM showed no improvement of serum creatinine levels(1.31 mg/dL to 1.37 mg/dL) compared with non DM patients(1.31 mg/dL to 1.15 mg/dL)(P = 0.01), HT patients(1.48 mg/dL to 1.5 mg/dL) with non HT patients(1.21mg/d L to 1.08 mg/dL) and patients with proteinuria(1.44 mg/dL to 1.41 mg/dL) and no proteinuria(1.31 mg/dL to 1.11 mg/dL). CONCLUSION: In OLT recipients with diabetes or hypertensive nephropathy, conversion to m TORi does not improve renal function but stabilizes plasma levels of creatinine. Proteinuria is not a contraindication to conversion to m TORi; it also stabilizes renal function. | 12 | Management of primary achalasia: The role of endoscopy显示文摘Achalasia is an oesophageal motor disorder which leads to the functional obstruction of the lower oesophageal sphincter(LES) and is currently incurable. The main objective of all existing therapies is to achieve a reduction in the obstruction of the distal oesophagus in order to improve oesophageal transit, relieve the symptomatology, and prevent long-term complications. The most common treatments used are pneumatic dilation(PD) and laparoscopic Heller myotomy, which involves partial fundoplication with comparable shortterm success rates. The most economic non-surgical therapy is PD, with botulinum toxin injections reserved for patients with a higher surgical risk for whom the former treatment option is unsuitable. A new technology is peroral endoscopic myotomy, postulated as a possible non-invasive alternative to surgical myotomy. Other endoluminal treatments subject to research more recently include injecting ethanolamine into the LES and using a temporary self-expanding metallic stent. At present, there is not enough evidence permitting a routine recommendation of any of these three novel methods. Patients must undergo follow-up after treatment to guarantee that their symptoms are under control and to prevent complications. Most experts are in favour of some form of endoscopic follow-up, however no established guidelines exist in this respect. | 13 | Application of Ordinary Kriging in Mapping Soil Organic Carbon in Zambia显示文摘The quantification of the pattern and spatial distribution of soil organic carbon(SOC) is fundamental to understand many ecosystem processes. This study aimed to apply ordinary kriging(OK) to model the spatial distribution of SOC in a selected part of Zambia. A total of 100 soil samples were collected from the study area and analyzed for SOC by determining soil oxidizable carbon using the Walkley-Black method. An automated fitting procedure was followed when modeling the spatial structure of the SOC data with the exponential semivariogram. The results indicated that the short range spatial dependence of SOC was strong with a nugget close to zero. The spatial autocorrelation was high to medium with a nugget to sill ratio of 0.25. The root mean square error of the predictions was 0.64, which represented 58.18% of the mean observed data for SOC. | 14 | Youth Participation at the Third UN World Conference on Disaster Risk Reduction显示文摘This article summarizes and analyzes the Children & Youth Forum and youth participation in the process during and leading up to the Third UN World Conference on Disaster Risk Reduction(WCDRR) in2015. An organizing committee consisting of international students and young professionals brought together around200 young professionals and students from around the globe to exchange ideas and knowledge on reducing disaster risk, building resilient communities, and advocating for the inclusion of youth priorities within the Sendai Framework for Disaster Risk Reduction 2015–2030(SFDRR). The knowledge exchange during the Forum was structured around a Toolbox for Resilience that connected to the SFDRR section on Priorities for Action. This article presents the outcomes of these young people’s participation in the disaster risk reduction capacity building eventsand policy-making, as well as the follow-up actions envisioned by the young participants of the Forum. The voices of the younger generation were heard in the SFDRR and young people are ready to expand their actions for the framework’s effective implementation. | 15 | 揭示2021年中国北方特大沙尘暴的粉尘传输过程显示文摘大气粉尘是地球系统的重要组成部分,沙尘暴是诸多国家尤其是中国面临的严峻环境问题.亚洲东部地区2021年3月15~16日发生的特大沙尘暴事件是过去十年中最大的一次,为从多学科角度理解同一天气过程下粉尘传输机制也提供了独特机遇.本研究在同一特大沙尘暴事件中不同地点系统采集了新鲜粉尘样品,采样地点涉及西起嘉峪关东至济南等受沙尘暴影响的9个不同城市.这种系统的采样策略使我们能够通过分析不同样品稀土元素、Sr-Nd同位素组成并结合高时间分辨率遥感数据和实时气象数据,从粉尘物质成分变化的角度探讨粉尘释放和传输过程.结果显示,外部物源是本次沙尘暴物质的主要来源(>70%),携带粉尘的气团运动轨迹途经的土地覆盖类型决定了粉尘系统的发育过程及强度.利用地球化学端元模型及新鲜降尘的Sr-Nd同位素组成并结合背光散射雷达监测数据(CALIPSO)及MODIS影像,定量评估了'3·15'特大沙尘暴事件期间中国北方不同地点降尘的潜在物源贡献.结果表明,最初起源于蒙古戈壁的细粒沉积物贡献显著,但各个城市附近的沙漠/沙地仍然是沙尘暴物质的主要补给来源.本研究为亚洲特大沙尘暴期间的粉尘传输机制提供了基于遥感观测和地球化学分析的直接证据,揭示了粉尘系统从源到汇是一个复杂且随时进行物质交换的开放系统,大规模沙尘暴的粉尘物质高度混合,物质成分接近上地壳平均值,远源输送和沿途补给是维持粉尘系统发育的重要机制. | 梁鹏 陈波 杨小平 刘倩倩 李艾芮 Lydia Mackenzie 张德国 | 2022 | Science Bulletin2022,67,1: | 5 |

| 17 | Infant intestinal Enterococcus faecalis down-regulates inflammatory responses in human intestinal cell lines显示文摘AIM: To investigate the ability of Lactic acid bacteria (LAB) to modulate inflammatory reaction in human intestinal cell lines (Caco-2, HT-29 and HCT116). Different strains of LAB isolated from new born infants and fermented milk, together with the strains obtained from culture collections were tested.METHODS: LABs were treated with human intestinal cell lines. ELISA was used to detect IL-8 and TGF-β protein secretion. Cytokines and Toll like receptors (TLRs) gene expression were assessed using RT-PCR. Conditional medium, sonicated bacteria and UV killed bacteria were used to find the effecter molecules on the bacteria. Carbohydrate oxidation and protein digestion were applied to figure out the molecules' residues. Adhesion assays were further carried out.RESULTS: It was found that Enterococcus faecalis is the main immune modulator among the LABs by downregulation of IL-8 secretion and upregulation of TGF-β. Strikingly, the effect was only observed in four strains of E. faecalis out of the 27 isolated and tested. This implies strain dependent immunomodulation in the host. In addition, E. faecalis may regulate inflammatory responses through TLR3, TLR4, TLR9 and TRAF6. Carbohydrates on the bacterial cell surface are involved in both its adhesion to intestinal cells and regulation of inflammatory responses in the host. | 19 | Degradation of direct azo dye by Cucurbita pepo free and immobilized peroxidase显示文摘Enzymatic decolourization of the azo dye,Direct Yellow(DY106) by Cucurbita pepo(courgette) peroxidase(CP) is a complex process,which is greatly affected by pH,temperature,enzyme activity and the concentrations of H2O2 and dye.Courgette peroxidase was extracted and its performance was evaluated by using the free-CP(FCP) and immobilized-CP(ICP) forms in the decolourization of DY106.Immobilization of peroxidase in calcium alginate beads was performed according to a strategy aiming to minimize enzyme leakage and keep its activity at a maximum value by optimizing sodium alginate content,enzyme loading and calcium chloride concentration.The initial conditions at which the highest DY106 decolourization yield was obtained were found at pH 2,temperature 20℃,H2O2 dose 1 mmol/L(FCP) and 100 mmol/L(ICP).The highest decolourization rates were obtained for dye concentrations 50 mg/L(FCP) and 80 mg/L(ICP).Under optimal conditions,the FCP was able to decolorize more than 87% of the dye within 2 min.While with ICP,the decolourization yield was 75% within 15 min.The decolourization and removal of DY106 was proved by UV-Vis analysis.Fourier transform infrared(FT-IR) spectroscopy analysis was also performed on DY106 and enzymatic treatment precipitated byproduct. | Nabila Boucherit Mahmoud Abouseoud Lydia Adour | 2013 | Journal of Environmental Sciences2013,25,6: | 3 |
