1Therapeutic potential of flavonoids in inflammatory bowel disease: A comprehensive review显示文摘The inflammatory process plays a central role in the development and progression of numerous pathological situations,such as inflammatory bowel disease(IBD),autoimmune and neurodegenerative diseases,metabolic syndrome,and cardiovascular disorders. IBDs involve inflammation of the gastrointestinal area and mainly comprise Crohn's disease(CD) and ulcerative colitis(UC). Both pathological situations usually involve recurring or bloody diarrhea,pain,fatigue and weight loss. There is at present no pharmacological cure for CD or UC. However,surgery may be curative for UC patients. The prescribed treatment aims to ameliorate the symptoms and prevent and/or delay new painful episodes. Flavonoid compounds are a large family of hydroxylated polyphenolic molecules abundant in plants,including vegetables and fruits which are the major dietary sources of these compounds for humans,together with wine and tea. Flavonoids are becoming very popular because they have many health-promoting and disease-preventive effects. Most interest has been directed towards the antioxidant activity of flavonoids,evidencing a remarkable free-radical scavenging capacity. However,accumulating evidence suggests that flavonoids have many other biological properties,including anti-inflammatory,antiviral,anticancer,and neuroprotective activities through different mechanisms of action. 2Probiotics for the treatment of Clostridium difficile associated disease显示文摘The purpose of this review paper is to update the current and potential future role of probiotics for Clostridium difficile-associated disease (CDAD). Included in this review, is an update on the testing of newer probiotics (e.g. , Bacillus coagulans GBI-30, 6086) in animal models of CDAD. There is a focus on the modulation of signal transduction pathways (i.e. , transcription factors like cAMP response element-binding, activator protein 1, and nuclear factor kappa B), as well as the inhibition of certain kinases (e.g. , p38 mitogen activated protein kinases) by probiotics. Inhibition of signal transduction by probiotics, such as Saccharomyces boulardii , result in multiple effects on intestinal fluid secretion, neutrophil influx into the colon, inflammation, and colonocyte apoptosis that may positively impact CDAD. Recent clinical approaches with probiotics, for the prevention of primary and recurrent CDAD, are also summarized in this review paper. Future directions for the treatment of CDAD by probiotics are also mentioned in this review. In particular, the use of multi-strain probiotic formulations such as Ecologic AAD and VSL #3 may represent a rationale pharmacological approach, particularly as adjunctive therapies for CDAD. 3Noninvasive biomarkers in non-alcoholic fatty liver disease:Current status and a glimpse of the future显示文摘The development of non invasive biomarkers of disease has become a major focus of interest in nonalcoholic fatty liver disease(NAFLD). The large prevalence of the disease and the invasive nature of the investigation means that screening with liver biopsy is impractical.In addition to screening, the differentiation of those with simple steatosis vs steatohepatitis and fibrosis is clinically important as the prognosis of each differs.Serum biomarkers may be a combination of simple markers derived from large data sets or direct markers of disease activity. Serum markers of inflammation,apoptosis and oxidative stress in addition to fibrosis have been extensively studied in patients with NAFLD.Other techniques such as transient elastography,magnetic resonance elastography and acoustic radiation force imaging are becoming more established as noninvasive methods of detecting fibrosis in a variety of chronic liver conditions in addition to NAFLD. Newer high throughput methods such as proteomics and glycomics allow the nonhypothesis-driven identification of novel markers and may also potentially contribute to our understanding of the pathogenesis of the condition.This review addresses some of the methodological issues which need to be considered in the search for the ideal biomarker. 4Have patients with esophagitis got an increased risk of adenocarcinoma? Results from a population-based study显示文摘AIM: To examine an increased risk of esophageal adenocarcinoma is restricted to patients who develop Barrett's esophagus or whether esophagitis per se is a risk factor for adenocarcinoma.METHODS: A population-based cohort of patients with histological evidence of esophagitis without Barrett's esophagus was constructed using electronic pathology reports relating to all esophageal biopsies in Northern Ireland between 1993 and 1996. Person-years of followup and incident cases of esophageal cancer were calculated by linking the cohort to death files and the Northern Ireland Cancer Registry records. Standardized incidence ratios (SIR) were calculated for esophageal cancers (adenocarcinoma, squamous cell carcinoma (SCC), and histologically unspecified cancers).RESULTS: A total of 2 013 patients in the cohort provided 13 559 patient-years of follow-up (mean follow-up 6.7 years). None of the patients developed adenocarcinoma. Three patients developed SCC, and six developed histologically unspecified cancers. The SIR for all esophageal cancers and for SCC were 2.73 (95%CI 1.25-5.19) and 2.93 (95%CI 0.61-8.59), respectively. In a sensitivity analysis in which all unspecified esophageal cancers were treated as adenocarcinomas, the SIR for adenocarcinoma was 2.64 (0.97-5.75).CONCLUSION: The risk of adenocarcinoma is not elevated in patients with histological evidence of esophagitis without Barrett's esophagus; however, these patients may have a moderately increased risk of SCC.Further studies are required to confirm these findings,which suggest that Barrett's esophagus, not esophagitis,is the key precursor lesion in the development of adenocarcinoma.Seamus J Murphy Lesley A Anderson Brian T Johnston Deirdre A Fitzpatrick Peter RG Watson Pauline Monaghan Liam J Murray 2005World Journal of Gastroenterology2005,11,46:4
5The cross-cutting contribution of the end of neglected tropical diseases to the sustainable development goals显示文摘The Sustainable Development Goals(SDGs)call for an integrated response,the kind that has defined Neglected Tropical Diseases(NTDs)efforts in the past decade.NTD interventions have the greatest relevance for SDG3,the health goal,where the focus on equity,and its commitment to reaching people in need of health services,wherever they may live and whatever their circumstances,is fundamentally aligned with the target of Universal Health Coverage.NTD interventions,however,also affect and are affected by many of the other development areas covered under the 2030 Agenda.Strategies such as mass drug administration or the programmatic integration of NTD and WASH activities(SDG6)are driven by effective global partnerships(SDG17).Intervention against the NTDs can also have an impact on poverty(SDG1)and hunger(SDG2),can improve education(SDG4),work and economic growth(SDG8),thereby reducing inequalities(SDG10).The community-led distribution of donated medicines to more than 1 billion people reinforces women’s empowerment(SDG5),logistics infrastructure(SDG9)and non-discrimination against disability(SDG16).Interventions to curb mosquito-borne NTDs contribute to the goals of urban sustainability(SDG11)and resilience to climate change(SDG13),while the safe use of insecticides supports the goal of sustainable ecosystems(SDG15).Although indirectly,interventions to control waterand animal-related NTDs can facilitate the goals of small-scale fishing(SDG14)and sustainable hydroelectricity and biofuels(SDG7).NTDs proliferate in less developed areas in countries across the income spectrum,areas where large numbers of people have little or no access to adequate health care,clean water,sanitation,housing,education,transport and information.This scoping review assesses how in this context,ending the epidemic of the NTDs can impact and improve our prospects of attaining the SDGs.Mathieu Bangert David H.Molyneux Steve W.Lindsay Christopher Fitzpatrick Dirk Engels 2017Infectious Diseases of Poverty2017,6,1:3
