1Role of adipokines and peroxisome proliferator-activated receptors in nonalcoholic fatty liver disease显示文摘Intrahepatic fat deposition has been demonstrated in patients with nonalcoholic fatty liver disease(NAFLD). Genetic and environmental factors are important for the development of NAFLD. Diseases such as obesity, diabetes, and hypertension have been found to be closely associated with the incidence of NAFLD. Evi-dence suggests that obesity and insulin resistance are the major factors that contribute to the development of NAFLD. In comparing the factors that contribute to the buildup of excess calories in obesity, an imbalance of energy homeostasis can be considered as the basis. Among the peripheral signals that are generated to regulate the uptake of food, signals from adipose tissue are of major relevance and involve the maintenance of energy homeostasis through processes such as lipo-genesis, lipolysis, and oxidation of fatty acids. Advances in research on adipose tissue suggest an integral role played by adipokines in NAFLD. Cytokines secreted by adipocytes, such as tumor necrosis factor-α, transform-ing growth factor-β, and interleukin-6, are implicated in NAFLD. Other adipokines, such as leptin and adiponectin and, to a lesser extent, resistin and retinol binding protein-4 are also involved. Leptin and adiponectin can augment the oxidation of fatty acid in liver by activating the nuclear receptor super-family of transcription fac-tors, namely peroxisome proliferator-activated receptor(PPAR)-α. Recent studies have proposed downregula-tion of PPAR-α in cases of hepatic steatosis. 2Mitochondria-targeted agents: Future perspectives of mitochondrial pharmaceutics in cardiovascular diseases显示文摘Mitochondria are one of the major sites for the genera-tion of reactive oxygen species(ROS) as an undesirable side product of oxidative energy metabolism. Damaged mitochondria can augment the generation of ROS. Dys-function of mitochondria increase the risk for a large number of human diseases, including cardiovascular diseases(CVDs). Heart failure(HF) following ischemic heart disease, infantile cardiomyopathy and cardiac hypertrophy associated with left ventricular dilations are some of the CVDs in which the role of mitochon-drial oxidative stress has been reported. Advances in mitochondrial research during the last decade focused on the preservation of its function in the myocardium, which is vital for the cellular energy production. Expe-rimental and clinical trials have been conducted using mitochondria-targeted molecules like: MnSOD mimetics, such as EUK-8, EUK-134 and MitoSOD; choline esters of glutathione and N-acetyl-L-cysteine; triphenylphospho-nium ligated vitamin E, lipoic acid, plastoquinone andmitoCoQ10; and Szeto-Schiller(SS)- peptides(SS-02 and SS-31). Although many results are inconclusive, some of the findings, especially on CoQ10, are worthwhile. 4Association of Satellite Sign with Postoperative Rebleeding in Patients Undergoing Stereotactic Minimally Invasive Surgery for Hypertensive Intracerebral Haemorrhage显示文摘There are few studies regarding imaging markers for predicting postoperative rebleeding after stereotactic minimally invasive surgery(MIS)for hypertensive intracerebral haemorrhage(ICH),and little is known about the relationship between satellite sign on computed tomography(CT)scans and postoperative rebleeding after MIS.This study aimed to determine the value of the CT satellite sign in predicting postoperative rebleeding in patients with hypertensive ICH who undergo stereotactic MIS.We retrospectively examined and analysed 105 patients with hypertensive ICH who underwent standard stereotactic MIS for hematoma evacuation within 72 h following admission.Postoperative rebleeding occurred in 14 of 65(21.5%)patients with the satellite sign on baseline CT,and in 5 of the 40(12.5%)patients without the satellite sign.This diiTerence was statistically significant.Positive and negative values of the satellite sign for predicting postoperative rebleeding were 21.5%and 87.5%,respectively.Multivariate logistic regression analysis verified that baseline ICH volume and intraventricular rupture were independent predictors of postoperative rebleeding.In conclusion,the satellite sign on baseline CT scans may not predict postoperative rebleeding following stereotactic MIS for hypertensive ICH.Ajith Bemardin Raj Li-fei LIAN Feng XU Guo LI Shan-shan HUANG Qi-ming LIANG Kai LU Jian-ling ZHAO Fu-rong WANG 2021Current Medical Science2021,41,3:6
5More efficient ground truth ROI image coding technique: implementation and wavelet based application analysis显示文摘In this paper, more efficient, low-complexity and reliable region of interest (ROI) image codec for compressing smooth low texture remote sensing images is proposed. We explore the efficiency of the modified ROI codec with respect to the selected set of convenient wavelet filters, which is a novel method. Such ROI coding experiment analysis representing low bit rate lossy to high quality lossless reconstruction with timing analysis is useful for improving remote sensing ground truth sur- veillance efficiency in terms of time and quality. The subjective [i.e. fair, five observer (HVS) evaluations using enhanced 3D picture view Hyper memory display technology] and the objective results revealed that for faster ground truth ROI coding applications, the Symlet-4 adaptation performs better than Biorthogonal 4.4 and Biorthogonal 6.8. 6Quality of life assessment in patients with chronic pancreatitis receiving antioxidant therapy显示文摘AIM:To undertake a baseline study comparing quality of life(QoL) in patients with chronic pancreatitis(CP) on Antox to those with CP,matched for disease duration,who were not on this medication.METHODS:CP was defined according to the Zurich classification.Sixty eight consecutive patients with CP who were taking Antox(antioxidants) were compared with 69 consecutive control CP patients not on Antox.European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core questions 30 and Pancreatic Modification(28 questions) were used to assess QoL.Out of a total of 137 patients 28 in each group were matched for disease duration(within 12 mo).Median disease duration was 8(1-22) years in the Antox group and 7(1-23) years in the Non-Antox cohort(P = NS,Mann-Whitney U-test).Other parameters(age,gender,etiology,endocrine and exocrine insufficiency) were similar between groups.RESULTS:Median visual analogue pain score in the Antox group was 3(0-8) compared with 6(0-8) in the Non-Antox group(P < 0.01).Perceptions of cognitive,emotional,social,physical and role function were impaired in the Non-Antox group compared to Antox patients(P < 0.0001,P = 0.0007,P = 0.0032 and P < 0.005 and P < 0.001,respectively).Analgesics and opiate usage was significantly lower in the Antox group(P < 0.01).Overall physical health and global QoL was better in the Antox group(P < 0.0001,95% CI:1.5-3).CONCLUSION:Contemporary quality of life assessments show that after correction for disease duration and cigarette smoking,patients with CP taking antox had better scores than non-antox controls.Nehal S Shah Alistair J Makin Aali J Sheen Ajith K Siriwardena 2010World Journal of Gastroenterology2010,16,32:4

8Dengue fever may mislead the surgeons when it presents as an acute abdomen显示文摘Objective: To review the management experience of a consecutive series of patients presenting as acute surgical abdomen whom were ultimately diagnosed to have DF(Dengue fever)/ DHF(Dengue heamorrhagic fever),Methods: Clinical data of all cases of apparent acute abdomen(AA) which were later confirmed as having DF/DHF reviewed by two surgical units from December 2012 to December 2013 were analyzed,Initially confirmed patients with DF/DHF who developed abdominal symptoms were not considered,Results: Out of the seventeen cases(7 males,age range 10-71 years) presented with fever and AA; appendicitis,cholecystitis,pancreatitis and non-specific peritonitis were suspected initially in 8,5,1 and 3 cases,respectively,Neutropenia or thrombocytopenia signifying DF/DHF occurred only in 11 patients at first evaluation thus six remained as surgical candidates beyond 24 h,One patient underwent appendicectomy with a prolonged hospital stay,DF was confirmed by serology in all patients,latest by fourth day of admission,One required blood product transfusion,4 needed critical care treatment and there was 1 death,Conclusions: DF/DHF misleads the clinicians when it presents as AA,Initial heamatological and ultrasonographic findings may be equivocal creating a diagnostic and management dilemma,Vigilant clinical suspicion and early dengue serological assessment is advisable in equivocal cases of AAs with fever in dengue endemic areas,to confirm/exclude the infection in order to avoid unnecessary surgical morbidity in the presence of DF.Bingumal Jayasundara Lalith Perera Ajith de Silva 2017Asian Pacific Journal of Tropical Medicine2017,10,1:3
9Experimental Investigation on Friction Welding of UNS S32205 Duplex Stainless Steel显示文摘UNS S 32205 duplex stainless steel specimens were joined by continuous drive friction welding process. The experiments were conducted as per the Taguchi(L16 orthogonal array) method. The friction welding process parameters such as heating pressure, heating time, upsetting pressure, upsetting time, and speed of rotation were fixed with low,medium, and high levels of range based on the machine capacity, and the required knowledge was acquired from the preliminary experiments. The joint characterization studies included micro structural examination and evaluation of mechanical properties of the joints. Microhardness variation, impact toughness, and tensile strength of the joints were evaluated. Neither a crack nor an incomplete bonding zone was observed. The tensile strength of the joints was higher than the strength of the base material, and the friction and upsetting pressures were found to influence the joint strength. The tensile strength of all the welds was observed to be increasing with an increase in the rotational speed. The toughness of the friction welds was evaluated at room temperature and also at subzero(cryo) temperature conditions. The toughness for friction welds was found to be superior to the fusion welds of duplex stainless steel at room temperature and cryo conditions. 10Pathological abnormalities in splenic vasculature in non-cirrhotic portal hypertension:Its relevance in the management of portal hypertension显示文摘BACKGROUND Portal hypertension(PH)is associated with changes in vascular structure and function of the portosplenomesenteric system(PSMS).This is referred to as portal hypertensive vasculopathy.Pathological abnormalities of PSMS has been described in the literature for cirrhotic patients.Raised portal pressure and hyperdynamic circulation are thought to be the underlying cause of this vasculopathy.In view of this,it is expected that pathological changes in splenic and portal vein similar to those reported in cirrhotic patients with PH may also be present in patients with non-cirrhotic PH(NCPH).AIM To investigate pathological abnormalities of splenic vein in patients with NCPH,and suggest its possible implications in the management of PH.METHODS A prospective observational study was performed on 116 patients with NCPH[Extrahepatic portal vein obstruction(EHPVO):53 and non-cirrhotic portal fibrosis(NCPF):63]who underwent proximal splenorenal shunt(PSRS),interposition shunt or splenectomy with devascularization in JIPMER,Pondicherry,India,a tertiary level referral center,between 2011-2016.All patients were evaluated by Doppler study of PSMS,computed tomography portovenogram and upper gastrointestinal endoscopy.An acoustic resonance forced impulse(ARFI)scan and abdomen ultrasound were done for all cases to exclude cirrhosis.Intraoperative and histopathological assessment of the harvested splenic vein was performed in all.The study group was divided into delayed and early presentation based on the median duration of symptoms(i.e.108 mo).RESULTS The study group comprising of 116 patients[77(66%)females and 39(34%)males]with NCPH had a median age of 22 years.Median duration of symptoms was 108 mo.The most common presentation in both EHPVO and NCPF patients was upper gastrointestinal bleeding(hematemesis and melena).The ARFI scan revealed a median score of 1.2(1.0-1.8)m/s for EHPVO and 1.5(0.9-2.8)m/s for NCPF.PSRS was performed in 84 patients(two of whom underwent interposition PSRS using a 10 mm Dacron graft);splenoadrenal shunt in 9;interposition mesocaval shunt in 5;interposition 1st jejunal to caval shunt in 1 patient and devascularization with splenectomy in 17 patients.Median presplenectomy portal pressure was 25(range:15-51)mm Hg.In 77%cases,the splenic vein was abnormal upon intraoperative assessment.Under macroscopic examination,wall thickening was observed in 108(93%),venous thrombosis in 32(28%)and vein wall calcification in 27(23%)cases.Upon examination under a surgical magnification loupe,21(18%)patients had intimal defects in the splenic vein.Histopathological examination of veins was abnormal in all cases.Medial hypertrophy was noted in nearly all patients(107/116),while intimal fibrosis was seen in 30%.Ninety one percent of patients with intimal fibrosis also had venous thrombosis.Vein wall calcification was found in 22%,all of whom had intimal fibrosis and venous thrombosis.The proportion of patients with pathological abnormalities in the splenic vein were significantly greater in the delayed presentation group as compared to the early presentation group.CONCLUSION Pathological changes in the splenic vein similar to those in cirrhotic patients with PH are noted in NCPH.We recommend that PH in NCPH be treated as systemic and pulmonary hypertension equivalent in the gastrointestinal tract,and that early aggressive therapy be initiated to reduce portal pressure and hemodynamic stress to avoid potential lethal effects.Shahana Gupta Biju Pottakkat Surendra Kumar Verma Raja Kalayarasan Sandip Chandrasekar A Ajith Ananthakrishna Pillai 2020World Journal of Gastrointestinal Surgery2020,12,1:3
11Neoadjuvant chemoradiotherapy before resection of perihilar cholangiocarcinoma: A systematic review显示文摘Background: Treatment with neoadjuvant chemoradiotherapy followed by liver transplantation yields promising results in perihilar cholangiocarcinoma (PH-CCA). This study reviews the literature to assess whether there is evidence to justify modern phase Ⅱ studies of neoadjuvant chemoradiotherapy prior to resection of PH-CCA.Data sources: A systematic review of the literature for reports of patients undergoing resection of PH- CCA after neoadjuvant chemoradiotherapy was performed using MEDLINE and EMBASE databases for the period between 1990 and 2019. The keywords and MeSH headings 'hilar cholangiocarcinoma', 'Klatskin', 'chemoradiotherapy' and 'chemotherapy' were used. Data were extracted on demographic profile, dis- ease staging, chemoradiotherapy protocols, complications and outcome. Risks of bias were assessed using Cochrane methodology. Results: There were seven reports on this topic, with median recruitment period of 14 (range 4–31) years. The total number of patients in these studies was 87. Interval from completion of neoadjuvant treatment to surgery varied from 3 days to 6 months. Resection was by hepatectomy with three studies reporting an R0 rate of 100%, 24% and 63%, respectively. Three studies reported histopathological evidence of prior treatment response. There were two treatment related deaths at 90 days. Median survival was 19 (95% CI: 9.9–28) months and 5-year survival 20%. Conclusions: There are potential benefits of treatment on both R0 rate and complete response in resected specimens. 13Efficient Gene Targeting in Maize Using Inducible CRISPR-Cas9 and Marker-free Donor Template显示文摘CRISPR-Cas9 is a powerful tool for generating targeted mutations and genomic deletions.However,precise gene insertion or sequence replacement remains a major hurdle before application of CRISPR-Cas9 technology is fully realized in plant breeding.Here,we report high-frequency,selectable marker-free intra-genomic gene targeting(GT)in maize.Heat shock-inducible Cas9 was used for generating targeted double-strand breaks and simultaneous mobilization of the donor template from pre-integratad T-DNA.The construct was designed such that release of the donor template and subsequent DNA repair activated expression of the selectable marker gene within the donor locus.This approach generated up to 4.7%targeted insertion of the donor sequence into the target locus in TO plants,with up to 86%detected donor template release and 99%mutation rate being observed at the donor loci and the genomic target site,respectively.Unlike previous in planta or intra-genomic homologous recombination reports in which the original chimeric GT plants required extensive progeny screening in the next generation to identify non-chimeric GT individuals,our method provides non-chimeric heritable GT in one generation.Pierluigi Barone Emily Wu Brian Lenderts Ajith Anand William Gordon-Kamm Sergei Svitashev Sandeep Kumar 2020Molecular Plant2020,13,8:3

15Reappraisal of xenobiotic-induced,oxidative stress-mediated cellular injury in chronic pancreatitis:A systematic review显示文摘AIM:To reappraise the hypothesis of xenobiotic induced,cytochrome P450-mediated,micronutrient-deficient oxidative injury in chronic pancreatitis.METHODS:Individual searches of the Medline and Embase databases were conducted for each component of the theory of oxidative-stress mediated cellular injury for the period from 1st January 1990 to 31st December2012 using appropriate medical subject headings.Boolean operators were used.The individual components were drawn from a recent update on theory of oxidative stress-mediated cellular injury in chronic pancreatitis.RESULTS:In relation to the association between exposure to volatile hydrocarbons and chronic pancreatitis the studies fail to adequately control for alcohol intake.Cytochrome P450(CYP)induction occurs as a diffuse hepatic and extra-hepatic response to xenobiotic exposure rather than an acinar cell-specific process.GSH depletion is not consistently confirmed.There is good evidence of superoxide dismutase depletion in acute phases of injury but less to support a chronic intraacinar depletion.Although the liver is the principal siteof CYP induction there is no evidence to suggest that oxidative by-products are carried in bile and reflux into the pancreatic duct to cause injury.CONCLUSION:Pancreatic acinar cell injury due to short-lived oxygen free radicals(generated by injury mediated by prematurely activated intra-acinar trypsin)is an important mechanism of cell damage in chronic pancreatitis.However,in contemporary paradigms of chronic pancreatitis this should be seen as one of a series of cell-injury mechanisms rather than a sole mediator.Ajith K Siriwardena 2014World Journal of Gastroenterology2014,20,11:2
16Efficacy of dietary phytase supplementation on laying performance and expression of osteopontin and calbindin genes in eggshell gland显示文摘This study was conducted to evaluate the effects of different levels of dietary phytase supplementation in the layer feed on egg production performance,egg shell quality and expression of osteopontin(OPN) and calbindin(CALB1) genes.Seventy-five White Leghorn layers at 23 weeks of age were randomly divided into 5 groups consisting of a control diet with 0.33% non-phytate phosphorus(NPP) and 4 low phosphorus(P) diets: 2 diets(T1 and T2) with 0.24% NPP t 250 FTU/kg laboratory produced phytase or commercial phytase and another 2 diets(T3 and T4) with 0.16% NPP t 500 FTU/kg laboratory produced phytase or commercial phytase with complete replacement of inorganic P.The results indicated that there were no significant differences(P > 0.05) in egg production performance and quality of egg during the first 2 months of trial.However,in next 2 months,a significant drop in egg production and feed intake was observed in birds fed diets with low P and 500 FTU/kg supplementation of laboratory produced phytase.Osteopontin gene was up-regulated whereas the CALB1 gene was down regulated in all phytase treatment groups irrespective of the source of phytase.The current data demonstrated that250 FTU/kg supplementation of laboratory produced phytase with 50% less NPP supplementation and500 FTU/kg supplementation of commercial phytase even without NPP in diet can maintain the egg production.The up-regulation of OPN and down regulation of CALB1 in egg shell gland in the entire phytase treated group birds irrespective of the source of enzymes is indicative of the changes in P bioavailability at this site.Divya Shet Jyotirmoy Ghosh Sreeja Ajith Vaibhav B.Awachat Arumbackam V.Elangovan 2018Animal Nutrition2018,,1:2
17Characterization of microstructure, toughness, and chemical composition of friction-welded joints of UNS S32205 duplex stainless steel显示文摘Friction welding is a solid-state joining process which is applied extensively because of its advantages such as low heat input,efficient application,ease in manufacturing,and environmental friendliness.The present study investigates the mechanical and metallurgical properties of UNS S32205 duplex stainless steel friction-welded joints.The process parameters,namely friction pressure,upsetting pressure,and rotational speed are individually varied from low level to high level(within the range of the machine setup)and their effects on the joint properties are analyzed.The partial-deformation zone had higher hardness than the weld and base metal.The toughness of the joints was evaluated at room temperature and at subzero temperature conditions.The impact toughness of the friction-welded joints was found to be superior to fusion-joined duplex stainless steel in room and cryogenic conditions.Puthuparambil Madhavan AJITH Paulraj SATHIYA Sivanandam ARAVINDAN 2014Friction2014,2,1:2

19Antibacterial potential of sponge endosymbiont marine Enterobacter sp at Kavaratti Island,Lakshadweep archipelago显示文摘Objective:To isolate antibacterial potential of sponge endosymbiotic bacteria from marine sponges at Lakshadweep archipelago.Also to identify the potent bacteria by 16s rDNA sequencing and determine the antibacterial activity against clinical pathogens by MIC.Methods: Sponge samples was collected from sub-tidal habitats at Kavaratti Island and identified.The endosymbiotic bacteria were isolated and selected potential bacteria which show antibacterial activity in preliminary screening against clinical pathogens Staphylococcus aureus(S.aureus), Escherichia coli(E.coli),Salmonella typhi(S.typhi),Klebsiella pneumoniea(K.pneumoniea) and Streptococcus sp.by disc diffusion assay.The crude extracts of potential bacteria LB3 was tested against clinical pathogens by MIC.The LB3 strain was identified by 16s rDNA sequencing, 1111 bp was submitted in NCBI(HQ589912) and constructed phylogenetic tree.Results:Sponge sample was identified as Dysidea granulosa(D.granulosa) and potential bacteria LB3 identified as Enterobacter sp TTAG.Preliminary screening of sponge isolates against clinical pathogens,LB3 strain was selected as potential producer of secondary metabolites and crude extract was implies on MIC of LB3 have confirmed with lowest concentration of 5.0 mg/mL in broth medium influence of crude extract on growth inhibitory activity after 5 h of incubation period and completed the inhibitory activity at 15 h.Conclusions:The present study concluded that phylogenetic analysis of endosymbiotic bacteria Enterobacter sp from sponge D.granulosa of Lakshadweep islands showed significant antibacterial activity against clinical bacterial pathogens.Mohan Gopi Subramanian Kumaran Thipparamalai Thangappanpillai Ajith Kumar Balaraman Deivasigamani Kumarappan Alagappan Srinivasan Guru Prasad 2012Asian Pacific Journal of Tropical Medicine2012,5,2:2
20利用柔性神经树的实时肌电信号手势识别模型显示文摘传统的手势识别通常采用数字信号处理(Digital Signal Processing,DSP)芯片或者集合方法(Ensem-ble Methods)研究实时识别问题。这些方法易导致数学模型参数繁多、硬件连接复杂和实时识别率较低。提出一种基于表面肌电信号与柔性神经树(Flexible Neural Trees,FNT)模型的实时手势识别模型。表面肌电信号(surface Electromyography,sEMG)具有非入侵式、易于采集特点,故被广泛应用于行为识别和诊断等领域。柔性神经树模型通过简单的预定义来构建,能够解决人工神经网络(Artificial Neural Network,ANN)的结构依赖性高的问题。柔性神经树模型不仅能够避免复杂的计算和电路连接,还具有较高的实时识别率和较低的方均根误差(Root Mean Square Error,RMSE)。实验针对六名参与者的六种手势进行测试,结果表明该模型实时识别率较高,实际应用也证明该算法可行。郭一娜 王清华 Abraham Ajith 2012计算机工程与应用2012,48,17:2
