1急性硬脊膜外血肿的MRI研究显示文摘目的 探讨急性硬脊膜外血肿 (ASEH)的发病机制、MRI表现特征及鉴别诊断。方法15例ASEH病人 ,男 8例 ,女 7例 ,平均 37.8岁。 5例有脊柱损伤史 ,1例有脊柱手术史 ,1例有腰硬脊膜穿刺史 ,其余 8例无特殊病史。 11例经手术证实 ,2例经CT引导穿刺抽吸治疗并证实 ,2例经临床相应检查及随访证实。所有病人均行矢状面SET1WI和快速SE(FSE)或SET2 WI。 12例行横轴面FSET2 WI ,8例行SET1WI。 4例行SET1WI增强扫描。结果 15例血肿共发生于 18个脊柱节段 ,其中 6例次 (6 /18)位于颈段、9例次 (9/18)位于胸段、3例次 (3/18)位于腰段。 7例次 (7/18)血肿位于硬膜囊前方 ,11例次 (11/18)位于硬膜囊后方。血肿累及 1~ 13个椎体高度 ,平均 4 .87个椎体高度。T1WI上 ,所有血肿与脊髓之间均显示有线样低信号区。T2 WI矢状面和横轴面上 ,分别有 4例(4/15 )和 8例 (8/12 )在血肿与蛛网膜下腔之间显示有低信号线。矢状面上 13例 (13/15 )血肿呈长梭形 ,横轴面上均呈双凸镜形或半圆形。SET1WI上 ,5例呈等T1信号 ,6例呈短T1信号 ,4例呈等、短T1混杂信号 ;T2 WI上 ,5例呈短T2 信号 ,10例呈短、长T2 混杂信号。 4例增强扫描无特异性。结论 ASEH的MRI表现具有特征性 。徐文坚 徐爱德 Sebastian Lange 2003中华放射学杂志2003,37,10:19

3Development of an internally braced prosthesis for total talus replacement显示文摘Total loss of talus due to trauma or avascular necrosis,for example,still remains to be a major challenge in foot and ankle surgery with severely limited treatment options.Implantation of a custom made total talar prosthesis has shown promising results so far.Most important factors for long time success are degree of congruence of articular surfaces and ligamentous stability of the ankle.Therefore,our aim was to develop an optimized custom made prosthesis for total talus replacement providing a high level of primary stability.A custom made hemiprosthesis was developed using computed tomography and magnetic resonance imaging data of the affected and contralateral talus considering the principles and technology for the development of the S.T.A.R.prosthesis(Stryker).Additionally,four eyelets for fixation of artificial ligaments were added at the correspondent footprints of the most important ligaments.Two modifications can be provided according to the clinical requirements:A tri-articular hemiprosthesis or a bi-articular hemiprosthesis combined with the tibial component of the S.T.A.R.total ankle replacement system.A feasibility study was performed using a fresh frozen human cadaver.Maximum range of motion of the ankle was measured and ligamentous stability was evaluated by use of standard X-rays after application of varus,valgus or sagittal stress with 150 N.Correct implantation of the prosthesis was technically possible via an anterior approach to the ankle and using standard instruments.Malleolar osteotomies were not required.Maximum ankle dorsiflexion and plantarflexion were measured as 22-0-28 degrees.Maximum anterior displacement of the talus was 6 mm,maximum varus tilt 3 degrees and maximum valgus tilt 2 degrees.Application of an internally braced prosthesis for total talus replacement in humans is technically feasible and might be a reasonable procedure in carefully selected cases with no better alternatives left.Markus Regauer Mirjam Lange Kevin Soldan Steffen Peyerl Sebastian Baumbach Wolfgang Bocker Hans Polzer 2017World Journal of Orthopedics2017,8,3:4

8Adolescents' personal beliefs about sufficient physical activity are more closely related to sleep and psychological functioning than self-reported physical activity: A prospective study显示文摘Background: Preliminary evidence among adults suggests that the ways in which individuals think about their physical activity(PA) behavior is more closely associated with their well-being than self-reported PA. This study therefore aimed to examine whether and how self-reported PA and personal beliefs about suffbcient PA are associated with sleep and psychological functioning in a sample of Swiss adolescents, using both cross-sectional and prospective data.Methods: An overall sample of 864 vocational students(368 girls, 17.98± 1.36 years, mean ± SD) was followed prospectively over a 10-month period. At each measurement occasion, participants filled in a series of self-report questiocnaires to assess their PA levels, their personal beliefs about whether or not they engage in sufficient PA, sleep(insomnia symptoms, sleep quality, sleep-onset latency, and number of awakenings),and psychological functioning(depressive symptoms, quality of life, perceived stress, and mental toughness).Results: Adolescents who believe that they are sufficiently physically active to maintain good health reported more restoring sleep. No differen?ces in sleep were found between adolescents who meet PA recommendationsvs. those who do not. Additionally, adolescents who believe that they were sufficiently physically active also reported better psychological functioning.
