1Immune checkpoint inhibitor-induced colitis:A comprehensive review显示文摘Immune checkpoint inhibitors(ICIs) are monoclonal antibodies that target downregulators of the anti-cancer immune response: Cytotoxic T-lymphocyte antigen-4, programmed cell death protein-1, and its ligand programmed death-ligand 1.ICIs have revolutionized the treatment of a variety of malignancies. However,many immune-related adverse events have also been described which mainly occurs as the immune system becomes less suppressed, affecting various organs including the gastrointestinal tract and causing diarrhea and colitis. The incidence of immune-mediated colitis(IMC) ranges from 1%-25% depending on the type of ICI and if used in combination. Endoscopically and histologically there is a significant overlap between IMC and inflammatory bowel disease,however more neutrophilic inflammation without chronic inflammation is usually present in IMC. Corticosteroids are recommended for grade 2 or more severe colitis while holding the immunotherapy. About one third to two thirds of patients are steroid refractory and benefit from infliximab. Recently vedolizumab has been found to be efficacious in steroid and infliximab refractory cases. 3天然水蛭素联合高压氧治疗对大鼠随意皮瓣成活的影响研究显示文摘目的探讨天然水蛭素联合高压氧治疗对大鼠随意皮瓣成活的影响。方法取72只SD大鼠,于背部制备面积为10.0 cm×2.5 cm的随意皮瓣移植模型后,随机分为4组(n=18)。对照组术后即刻及之后4 d内注射生理盐水,高压氧组注射生理盐水同时行高压氧治疗,水蛭素组仅注射天然水蛭素,联合组注射天然水蛭素同时行高压氧治疗。术后大体观察皮瓣成活情况,第6天计算皮瓣成活率;第2、4天取材,HE染色观察皮瓣组织学变化;免疫组织化学染色检测皮瓣微血管密度(microvessel density,MVD)以及TNF-α表达水平。结果术后各组皮瓣均出现部分坏死,其中联合组皮瓣成活最佳;术后第6天高压氧组、水蛭素组及联合组皮瓣成活率明显高于对照组,联合组高于水蛭素组、高压氧组,比较差异有统计学意义(P<0.05);水蛭素组与高压氧组比较,差异无统计学意义(P>0.05)。组织学观察示,术后第2天,水蛭素组、高压氧组及联合组微血管结构较对照组多,各组均见炎性细胞浸润;第4天,高压氧组、水蛭素组及联合组中仍可见较多微血管形成,对照组中见大量炎性细胞浸润,其余各组炎性细胞均较术后第2天时明显减少。免疫组织化学染色观察示,术后第2天,高压氧组、水蛭素组及联合组MVD均显著高于对照组、TNF-α蛋白表达量显著低于对照组(P<0.05);高压氧组、水蛭素组及联合组以上指标比较,差异无统计学意义(P>0.05)。术后第4天,高压氧组、水蛭素组及联合组MVD均显著高于对照组,联合组及水蛭素组高于高压氧组,比较差异有统计学意义(P<0.05),联合组及水蛭素组间比较差异无统计学意义(P>0.05);高压氧组、水蛭素组及联合组TNF-α蛋白表达量显著低于对照组,联合组低于水蛭素组及高压氧组,比较差异有统计学意义(P<0.05),水蛭素组及高压氧组间比较差异无统计学意义(P>0.05)。结论高压氧和天然水蛭素干预均能提高随意皮瓣移植后成活率,且两者联合发挥协同效应,可能与促进血管生成和减轻炎性反应有关。蔡洁云 林博杰 潘新元 崔佳 Pradhan Rohan 殷国前 2018中国修复重建外科杂志2018,32,4:11
4Outcomes of liver transplantation in patients with hepatorenal syndrome显示文摘Hepatorenal syndrome(HRS) plays an important role in patients with liver cirrhosis on the wait list for liver transplantation(LT). The 1 and 5-year probability of developing HRS in cirrhotic with ascites is 20% and 40%, respectively. In this article, we reviewed current concepts in HRS pathophysiology, guidelines for HRS diagnosis, effective treatment options presently available, and controversies surrounding liver alone vs simultaneous liver kidney transplant(SLKT) in transplant candidates. Many treatment options including albumin, vasoconstrictors, renal replacement therapy, and eventual LT have remained a mainstay in the treatment of HRS. Unfortunately, even after aggressive measures such as terlipressin use, the rate of recovery is less than 50% of patients. Moreover, current SLKT guidelines include:(1) estimation of glomerular filtration rate of 30 m L/min or less for 4-8 wk;(2) proteinuria > 2 g/d; or(3) biopsy proven interstitial fibrosis or glomerulosclerosis. Even with these updated criteria there is a lack of consistency regarding longterm benefits for SLKT vs LT alone. Finally, in regards to kidney dysfunction in the post-transplant setting, an estimation of glomerular filtration rate < 60 mL /min per 1.73 m2 may be associated with an increased risk of patients having long-term end stage renal disease. HRS is common in patients with cirrhosis and those on liver transplant waitlist. Prompt identification and therapy initiation in transplant candidates with HRS may improve post-transplantation outcomes. Future studies identifying optimal vasoconstrictor regimens, alternative therapies, and factors predictive of response to therapy are needed. 5Autoheated thermophilic aerobic sludge digestion and metal bioleaching in a two-stage reactor system显示文摘A two-stage process has been developed for stabilization of sludge and removal of heavy metals from the secondary activated sludge with high rate of energy and time conservation.The first stage of the process involves autoheated thermophilic aerobic digestion at 55-60°C inoculated with less-acidophilic thermophilic sulfur-oxidizing microorganisms(ATAD).The results show that it is possible to maintain the autoheated conditions(55-60°C) in the ATAD reactor up to 24 hr,leading to reduction of 21% total solids(TS),27% volatile solids(VS),27% suspended solids(SS) and 33% volatile suspended solids(VSS) from the sludge.The sludge pH also decreased from 7 to 4.6 due to the activity of less-acidophilic thermophilic microorganisms.In the second stage operation,the digested sludge(pH 4.6,TS 31.6 g/L) from stage one was subjected to bioleaching in a continuous stirred tank reactor,operated at mean hydraulic retention times(HRTs) of 12,24 and 36 hr at 30°C.An HRT of 24 hr was found to be sufficient for removal of 70% Cu,70% Mn,75% Ni,and 80% Zn from the sludge.In all,39% VSS,76% Cu,78.2% Mn,79.5% Ni and 84.2% Zn were removed from the sludge in both the stages.Rohan Jain Ashish Pathak T.R.Sreekrishnan M.G.Dastidar 2010Journal of Environmental Sciences2010,22,2:7
6Motility-based label-free detection of parasites in bodily fluids using holographic speckle analysis and deep learning显示文摘Parasitic infections constitute a major global public health issue.Existing screening methods that are based on manual microscopic examination often struggle to provide sufficient volumetric throughput and sensitivity to facilitate early diagnosis.Here,we demonstrate a motility-based label-free computational imaging platform to rapidly detect motile parasites in optically dense bodily fluids by utilizing the locomotion of the parasites as a specific biomarker and endogenous contrast mechanism.Based on this principle,a cost-effective and mobile instrument,which rapidly screens~3.2 mL of fluid sample in three dimensions,was built to automatically detect and count motile microorganisms using their holographic time-lapse speckle patterns.We demonstrate the capabilities of our platform by detecting trypanosomes,which are motile protozoan parasites,with various species that cause deadly diseases affecting millions of people worldwide.Using a holographic speckle analysis algorithm combined with deep learningbased classification,we demonstrate sensitive and label-free detection of trypanosomes within spiked whole blood and artificial cerebrospinal fluid(CSF)samples,achieving a limit of detection of ten trypanosomes per mL of whole blood(~five-fold better than the current state-of-the-art parasitological method)and three trypanosomes per mL of CSF.We further demonstrate that this platform can be applied to detect other motile parasites by imaging Trichomonas vaginalis,the causative agent of trichomoniasis,which affects 275 million people worldwide.With its costeffective,portable design and rapid screening time,this unique platform has the potential to be applied for sensitive and timely diagnosis of neglected tropical diseases caused by motile parasites and other parasitic infections in resource-limited regions.Yibo Zhang Hatice Ceylan Koydemir Michelle M.Shimogawa Sener Yalcin Alexander Guziak Tairan Liu Ilker Oguz Yujia Huang Bijie Bai Yilin Luo Yi Luo Zhensong Wei Hongda Wang Vittorio Bianco Bohan Zhang Rohan Nadkarni Kent Hill Aydogan Ozcan 2018Light(Science & Applications)2018,7,1:6

8Clinical characteristics and outcome of hepatocellular carcinoma in alcohol related and cryptogenic cirrhosis: a prospective study显示文摘BACKGROUND: Non-alcoholic fatty liver disease(NAFLD) is becoming a leading cause of chronic liver disease. Hepatocellular carcinoma(HCC) is one of its complications. Although the pathophysiology is unclear, it is reasonable to expect that cryptogenic cirrhosis related HCC(cryptogenic HCC) behaves differently to other types of HCC. This study prospectively compared patients with cryptogenic HCC and those with HCC related to alcoholic cirrhosis. METHODS: A total of 150 consecutive patients with HCC(89 cryptogenic HCC and 61 alcohol related HCC) referred to our unit over a 23-month period were studied. Their demographic data, liver function, tumor characteristics and outcomes were compared.RESULTS: Alcohol related HCC was seen only in males. Compared with cryptogenic HCC, alcohol related HCC had significantly higher aspartate aminotransferase/alanine aminotransferase(AST/ALT) ratio(1.7 vs 1.4, P=0.002), model for endstage liver disease score(13 vs 11, P=0.018) and Child's score(7 vs 6, P=0.037). No significant difference was seen in platelet counts, serum sodium and AST to platelet ratio index. Single nodular tumors were more common in cryptogenic HCC,while diffuse type tumors and macroscopic vascular invasion were common in alcohol related HCC. In patients who could not be offered any treatment because of advanced tumors or poor liver function, alcohol related HCC had a significantly lower median survival(5.3 months) compared with cryptogenic HCC(9.3 months, P=0.034).CONCLUSIONS: Compared with cryptogenic HCC, alcohol related HCC had worse liver function and aggressive tumor morphology at presentation, and a higher proportion was untreatable. 9Clinically unrecognized mitral regurgitation is prevalent in lone atrial fibrillation显示文摘AIM:To investigate the prevalence of clinically unrecognized mitral regurgitation(MR) in lone atrial fibrillation(AF).METHODS:We studied the prevalence and severity of MR by transesophageal echocardiography(TEE) in patients with 'lone' AF as compared to a matched cohort of patients in normal sinus rhythm(NSR) undergoing TEE for other indications besides recognized valvular heart disease.RESULTS:A total of 157 subjects(57 in the AF group and 100 in the NSR group) with structurally normal cardiac valves were included in the study.In the AF group,moderate MR or more was noted in 66% of thepatients,mild MR in 18%,trace or no MR in 16%.In the control group,moderate MR was noted in 6% of patients,mild MR 31%,trace or no MR in 63 % of patients.Moderate MR or greater was significantly more prevalent in the AF group compared to the NSR group(66% vs 6%,P < 0.0001).CONCLUSION:Clinically unrecognized moderate MR is prevalent in 'lone' AF-either as an etiologic factor leading to 'lone' AF or developing after onset of AF.Sanjiv Sharma Joel Lardizabal Mark Monterroso Neil Bhambi Rohan Sharma Rasham Sandhu Sarabjeet Singh 2012World Journal of Cardiology2012,4,5:5

11Optimized pre-treatment of high strength food waste digestate by high content aluminum-nanocluster based magnetic coagulation显示文摘Coagulation-based pre-treatment efficiency of high strength digestate of food waste(HSDFW) anaerobic digestion is negated by organic ligand-catalyzed decomposition of coagulants. In this study, an efficient HSDFW pre-treatment method, magnetic seeds(MS) coagulation, was employed by using highly stable Keggin Al_(30) nanocluster(PAC_(30)), MS and polyacrylamide(PAM), and its operation was optimized by evaluating the performance of removing turbidity, total suspended solids(TSS), chemical oxygen demand(COD), and total phosphorous(TP) phosphate. Results showed that at the optimum dosage of 4.82 g/L, PAC_(30) demonstrated excellent removals as high as 98.93% ± 0.1% of turbidity, 98.04% ± 0.1% of TSS, 58.28% ± 0.3% of total COD, 99.98% ± 0.01% of TP and 99.50% ± 0.01% of dissolved phosphate, respectively. Apparent molecular weight(AMW) and three-dimensional excitation-emission matrix(3 D-EEM) fluorescence spectroscopy analyses demonstrated more efficient removal of dissolved organic matter(DOM), particularly non-biodegradable and hydrophobic components by PAC_(30) than commercial coagulant. The sedimentation was much improved from 40 min by coagulation/flocculation to about 5 min settling by MS coagulation. 12Performance Comparison of DCT and VQ Based Techniques for Iris Recognition显示文摘Iris recognition enjoys universality,high degree of uniqueness and moderate user co-operation.This makes iris recognition systems unavoidable in emerging security & authentication mechanisms.An iris recognition system based on vector quantization (VQ) techniques is proposed and its performance is compared with the discrete cosine transform (DCT).The proposed system does not need any pre-processing and segmentation of the iris.We have tested Linde-BuzoGray (LBG),Kekre's proportionate error (KPE) algorithm and Kekre's fast codebook generation (KFCG) algorithm for the clustering purpose.Proposed vector quantization based method using KFCG requires 99.99% less computations as that of full 2-dimensional DCT.Further,the KFCG method gives better performance with the accuracy of 89.10% outperforming DCT that gives accuracy around 66.10%.H.B.Kekre Tanuja K. 16Improving sessile serrated adenoma detection rates with high definition colonoscopy:A retrospective study显示文摘BACKGROUND Sessile serrated adenomas(SSAs)are important premalignant lesions that are difficult to detect during colonoscopy due to poor definition,concealment by mucous caps,and flat appearance.High definition(HD)colonoscopy may uniquely aid in the detection of these inconspicuous lesions compared to standard definition(SD)colonoscopes.In the absence of existing clinical guidelines to obligate the use of HD colonoscopy for colorectal cancer screening in average-risk patients,demonstrating the benefit of HD colonoscopy on SSA detection rate(SSADR)may help strengthen the evidence to recommend its use in all settings.AIM To evaluate the benefit of HD colonoscopy compared to SD colonoscopy on SSADR in average-risk patients undergoing screening colonoscopy.METHODS Data from screening colonoscopies for patients aged 50-76 years two years before and two years after the transition from SD colonoscopy to HD colonoscopy at our large,academic teaching center were collected.Patients with symptoms of colorectal disease,positive occult blood test,history of colon polyps,cancer,polyposis syndrome,inflammatory bowel disease or family history of colon cancer or polyps were excluded.Patients whose endoscopists did not perform colonoscopies both before and after scope definition change were also excluded.Differences in individual endoscopist SSADR,average SSADR,and overall SSADR with SD colonoscopy vs HD colonoscopy were also evaluated for significance.RESULTS A total of 3657 colonoscopies met eligibility criteria with 2012 colonoscopies from the SD colonoscopy period and 1645 colonoscopies from the HD colonoscopy period from a pool of 11 endoscopists.Statistically significant improvements of 2.30%in mean SSADR and 2.53%in overall SSADR were noted with HD colonoscopy(P=0.00028 and P=0.00849,respectively).On the individual level,three endoscopists experienced statistically significant benefit with HD colonoscopy(+5.74%,P=0.0056;+4.50%,P=0.0278;+4.84%,P=0.03486).CONCLUSION Our study suggests that HD colonoscopy statistically significantly improves sessile serrated adenoma detection rate in the screening of average risk patients during screening colonoscopy.By improving the detection and removal of these lesions,adoption of HD colonoscopy may reduce the significant premalignant burden of sessile serrated adenomas.Abhinav Sehgal Soorya Aggarwal Rohan Mandaliya Thomas Loughney Mark C Mattar 2022World Journal of Gastrointestinal Endoscopy2022,14,4:2

18Trends and outcomes of transarterial chemoembolization in hepatocellular carcinoma:a national survey显示文摘BACKGROUND:Transarterial chemoembolization(TACE) is a palliative procedure frequently used in patients with advanced hepatocellular carcinoma(HCC). We examined the national inpatient trends of TACE and related outcomes in the United States over the last decade.METHODS:We utilized the National Inpatient Sample(2002 to 2012) and performed trend analyses of TACE for HCC in all adult patients(age >18 years). Multivariate analyses for the outcomes of in-hospital 'procedure-related complications'(PRCs) and 'post-procedure complications'(PPCs) were performed. We also compared early(2002 to 2006) and late(2007 to 2012) eras by multivariate analyses to identify predictors of complications, healthcare resource utilization and mortality.RESULTS:Overall, 19058 patients underwent TACE for HCC where PRCs and PPCs were seen in 24.2% and 17.6% of patients, respectively. The overall trends in the use of TACE(P<0.001) and associated PRCs(P=0.006) were observed to be increasing. There was less mortality [adjusted Odds ratio(a OR):0.58; 95% CI:0.41, 0.82], reduced length of hospital stay(-1.87 days; 95% CI:-2.77,-0.97) and increased hospital charges($19232; 95% CI:11013, 27451) in the late era. Additionally, there was increased mortality(a OR:4.07; 95% CI:2.96, 5.59), PRCs(a OR:3.21; 95% CI:2.56, 4.02), and PPCs(a OR:2.70; 95% CI:2.11, 3.46) among patients with coagulopathy.CONCLUSIONS:There is an increasing trend of TACE utilization in HCC. However, the outcomes are worse in patients with coagulopathy. Although PRCs have increased, mortality has decreased in recent years.
