2低剂量罗哌卡因程控硬膜外间歇脉冲注入与连续硬膜外输注用于分娩镇痛的临床研究显示文摘目的对低剂量罗哌卡因程控硬膜外间歇脉冲注入(programmed intermittent epidural bolus,PIEB)及连续硬膜外输注(continuous epidural infusion,CEI)两种镇痛方式进行比较。方法2018年08月-2019年08月在我院阴道分娩,有分娩镇痛需求,无麻醉禁忌症的初产妇84例,按随机数字表分为PIEB组(n=44)和CEI组(n=40)。PIEB组运用低剂量罗哌卡因程控硬膜外间歇脉冲注入,CEI组运用连续硬膜外输注法。观察比较两组研究对象镇痛效果、分娩动力、产妇血流动力学改变、产后出血量及新生儿Apgar评分。结果两组之间的孕产妇血压和心率变化、产程进展、新生儿Apgar评分及产后出血量等方面,差异均无显著意义(P>0.05)。PIEB组相对于CEI组而言,麻醉诱导开始至第一次额外推注的时间间隔更长(P=2.3×10-23),整个产程中,需额外推注的次数更少(P=5.4×10-8),且经过统计分析,PIEB组在整个麻醉过程中罗哌卡因的消耗量低于CEI组(P=0.005)。PIEB组的VAS评分在宫颈扩张5 cm(P=0.001)和第二产程阶段(P=0.003)均低于CEI组。结论当运用0.08%低浓度罗哌卡因施行分娩镇痛时,与连续硬膜外输注技术相比,使用程控硬膜外间歇脉冲技术可提供更好,更长的镇痛效果,并且减少了局部麻醉剂的消耗。此外,两组技术对产程进展、产妇血流动力学状态、新生儿的状况及产后出血的影响并无明显差异。Pappu Kumar shah 夏佳 刘珏 2020西南军医2020,22,3:2
