| 2 | Inclusion Modification with Mg Treatment for 35CrNi3MoV Steel显示文摘The effects of Mg on the inclusion characteristics of 35CrNi3MoV steel have been studied. The results show that Mg treatment reduces obviously the amount of inclusion in the steel.Magnesiurn and oxygen in liquid steel form dispersed fine particles of MgO, to which other oxides or sulphides nucleated at heterogeneity and grown during solidification. In this way the inclusions are modified. The small spherical inclusions containing MgO have been observed in the Mg treated steel in contrast to the large angular inclusions with Al2O3 in the conventional steel. Especially the elongated MnS inclusion is replaced by small MgS·MgO or MgS. MnS·MgO complex inclusion. | 4 | Current concepts in total femoral replacement显示文摘Total femoral replacement(TFR) is a salvage arthroplasty procedure used as an alternative to lower limb amputation. Since its initial description in the mid-20^(th) century, this procedure has been used in a variety of oncologic and non-oncologic indications. The most compelling advantage of TFR is the achievement of immediate fixation which permits early mobilization. It is anticipated that TFR will be increasingly performed as the rate of revision arthroplasty rises worldwide. The existing literature is mainly composed of a rather heterogeneous mix of retrospective case series and a wide assortment of case reports. Numerous TFR prostheses are currently available and the surgeon must understand the unique implications of each implant design. Long-term functional outcomes are dependent on adherence to proper technique and an appropriate physical therapy program for postoperative rehabilitation. Revision TFR is mainly performed for periprosthetic infection and the severe femoral bone loss associated with aseptic revisions. Depending on the likelihood of attaining infection clearance, it may sometimes be advisable to proceed directly to hip disarticulation without attempting salvage of the TFR. Other reported complications of TFR include hip joint instability, limb length discrepancy, device failure, component loosening, patellar maltracking and delayed wound healing. | 5 | Role of negative pressure wound therapy in total hip and knee arthroplasty显示文摘Negative-pressure wound therapy(NPWT) has been a successful modality of wound management which is in widespread use in several surgical fields. The main mechanisms of action thought to play a role in enhancing wound healing and preventing surgical site infection are macrodeformation and microdeformation of the wound bed, fluid removal, and stabilization of the wound environment. Due to the devastating consequences of infection in the setting of joint arthroplasty, there has been some interest in the use of NPWT following total hip arthroplasty and total knee arthroplasty. However, there is still a scarcity of data reporting on the use of NPWT within this field and most studies are limited by small sample sizes, high variability of clinical settings and end-points. There is little evidence to support the use of NPWT as an adjunctive treatment for surgical wound drainage, and for this reason surgical intervention should not be delayed when indicated. The prophylactic use of NPWT after arthroplasty in patients that are at high risk for postoperative wound drainage appears to have the strongest clinical evidence. | 7 | 脉压对颈动脉壁重构的影响——探讨Laplace定律对大血管重构的作用显示文摘目的:血管壁的增厚和功能指数正越来越多的被用作评价心血管疾病的重要指标。然而大血管的结构、功能、压力的相互关系在活体上还没有很好的确定。我们研究组对一组正常和高血压病人进行了大血管的结构-功能-压力的相互关系的交叉研究。方法:307例正常和高血压的人群被选择到此研究,其中男110例,女197例。年龄49~75岁。颈动脉内膜和中层厚度(Intima-media wall thickness,IMT)和内腔直径(Lumen diameter,LD)用Diasonic(DRF400)高分辨率超声来确定。血管壁环状压(Circumferential stress,Cs)是依据LAPLACE定律(δ=P×r/h)计算。其中压力(Pressure,P)用平均压和脉压两种方法计算。此动脉的结构和功能指数用年龄、身高和心率来校正。颈动脉压用无创压力感受器测定。结果:共有70例高血压(BP>140/90)患者。用年龄、身高和心率校正后的内中膜厚度和内腔直径在高血压组显著增加。而且IMT随着脉压的增大而增大(r=0.39,P<0.001)。内腔半径与内中膜厚度比值(R/IMT,R=LD/2)和环状压在高血压组明显增加。但是不管高血压或正常人群的R/IMT比值与脉压呈负相关关系(r=0.38,P<0.001),而与平均压无关。结论:在高血压状态下颈动脉壁重构是服从LAPLACE定律的,但颈动脉壁重构不足以抵销高血压状态下的血管壁环状压的增加。 | 梁雨露 Cameron J McGrath BP | 2002 | 中国医药导刊2002,4,1: | 4 |
