| 1 | Relationship between LYVE-1, VEGFR-3 and CD44 gene expressions and lymphatic metastasis in gastric cancer显示文摘AIM: To investigate the expression levels of lymphatic vessel endothelial hyaluronan receptor-1 (LYVE-1), vascular endothelial growth factor receptor-3 (VEGFR-3) and CD44 genes and the relationship between their lev- els and clinicopathological parameters in gastric cancer.METHODS: Tissue samples were obtained from 33 patients (8 females) with gastric cancer. mRNA levels of LYVE-1, VEGFR-3 and CD44 in normal and tumor tissues were quantitatively measured using real time polymerase chain reaction. The results were correlated with lymph node metastasis, histological type and differentiation of the tumor, T-stage, and presence of vascular, perineural and lymphatic invasions. The distribution of molecules in the tissue was evaluated using immunohistochemistry. RESULTS: LYVE-1, CD44 and VEGFR-3 gene expression levels were significantly higher in gastric cancer than in normal tissue. While there was no correlation between gene expressions and clinicopathologic fea- tures such as histologic type, differentiation and stage, gene expression levels were found to be increased in conjunction with positive lymph node/total lymph node ratio and the presence of perineural invasion. A significant correlation was also found between LYVE-1 and CD44 over-expressions and perineural invasion and lymph node positivity in gastric cancers. When the dis- tribution of LYVE-1 antibody-stained lymphatic vessels in tissue was evaluated, lymphatic vessels were located intra-tumorally in 13% and peri-tumorally in 27% of the patients. Moreover, lymph node metastases were also positive in all patients with LYVE-1-staining. CONCLUSION: LYVE-1, VEGFR-3 and CD44 all play an important role in lymphangiogenesis, invasion and metastasis. | 2 | Clinical probability and risk analysis of patients with suspected pulmonary embolism显示文摘BACKGROUND: Pulmonary embolism(PE) is one of the most frequent diseases that could be missed in overcrowded emergency departments as in Turkey. Early and accurate diagnosis could decrease the mortality rate and this standard algorithm should be defi ned. This study is to fi nd the accurate, fast, non-invasive, cost-effective, easy-to-access diagnostic tests, clinical scoring systems and the patients who should be tested for clinical diagnosis of PE in emergency department.METHODS: One hundred and forty patients admitted to the emergency department with the fi nal diagnosis of PE regarding to anamnesis, physical examination and risk factors, were included in this prospective, cross-sectional study. The patients with a diagnosis of pulmonary embolism, acute coronary syndrome or infection and chronic obstructive pulmonary disease(COPD) were excluded from the study. The demographics, risk factors, radiological fi ndings, vital signs, symptoms, physicallaboratory fi ndings, diagnostic tests and clinical scoring systems of patients(Wells and Geneva) were noted. The diagnostic criteria for pulmonary emboli were: fi lling defect in the pulmonary artery lumen on spiral computed tomographic angiography and perfusion defect on perfusion scintigraphy.RESULTS: Totally, 90(64%) of the patients had PE. | 3 | Association of serum lipid levels with diabetic retinopathy显示文摘AIM: To assess the association between serum lipids and diabetic retinopathy (DR). · METHODS: Sixty -one diabetic patients without retinopathy (NDR), 55 diabetic patients with non - proliferative retinopathy (NPDR) and 75 diabetic patients with proliferative retinopathy (PDR) according to ETDRS grading scale were enrolled in this study. Total cholesterol, high density lipoprotein (HDL), low density lipoprotein (LDL), very low density lipoprotein (VLDL) and triglyceride values were compared between the groups. ·RESULTS: The groups were well-balanced in terms of age and gender (P =0.071, P =0.265 respectively). The mean HbA1c values were significantly lower in NDR group than the NPDR and PDR groups(P =0.004, P =0.009 respectively). Mean total cholesterol, triglyceride, LDL, HDL and VLDL levels were not significantly different between the groups (P =0.693, P =0.774, P =0.644, P = 0.910 and P =0.967 respectively, one way ANOVA). Mean total cholesterol, triglyceride, LDL, HDL and VLDL levels were not significantly different between the patients with ME and patients without ME(P =0.622, P =0.113, P =0.955, P =0.735 and P =0.490 respectively, t -test). The mean blood glucose significantly correlated with total cholesterol (r =0.173, P =0.017) and LDL (r =0.190, P = 0.008). | 5 | Effects of Helicobacter pylori infection on gastric epithelial cell kinetics in patients with chronic renal failure显示文摘AIM: To evaluate the effects of Helicobacter pylori infection on gastric epithelial cell kinetics in patients with chronic renal failure (CRF).METHODS: Forty-four patients were enrolled in this study and divided into four groups with respect to their Helicobacter pylori (H pylori) and CRF status. Groups were labeled as follows: 1a: normal renal function, H pylori negative (n = 12), 1b: normal renal function,H pylori positive (n = 11), 2a: CRF, H pylori negative (n = 10), 2b: CRF, H pylori positive (n = 11). Upper gastrointestinal endoscopy was done in all the patients involved in the study. During endoscopical investigation,antral biopsy specimens were taken from each patient.In order to evaluate the cell apoptosis and proliferation in gastric epithelial cells, Bax and proliferating cell nuclear antigen (PCNA) labeling indexes (LI) were assessed with immunohistochemical staining method.RESULTS: For groups 1a, 1b, 2a, and 2b, mean Bax LI was identified as 34.4±13.7, 44.1±16.5, 46.3±20.5,60.7±13.8, respectively and mean PCNA LI was identified as 36.2±17.2, 53.6±25.6, 59.5±25.6, 67.2±22,respectively. When the one-way ANOVA test was applied,statistically significant differences were detected between the groups for both Bax LI (P = 0.004 <0.01) and PCNA LI (P = 0.009 <0.01). When groups were compared further in terms of Bax LI and PCNA LI with Tukey's HSD test for multiple pairwise comparisons, statistically significant difference was observed only between groups 1a and 2b (P = 0.006 <0.01).CONCLUSION: In gastric epithelial cells, expression of both the pre-apoptotic protein Bax and the proliferation marker PCNA increase with H pylori infection. This increase is more evident in patients with uremia. | 6 | Targeted therapies in gastric cancer and future perspectives显示文摘Advanced gastric cancer(AGC) is associated with a high mortality rate and, despite multiple new chemotherapy options, the survival rates of patients with AGC remains poor. After the discovery of targeted therapies, research has focused on the new treatment options for AGC. In the last two decades, many targeted molecules were developed against AGC. Currently, two targeted therapy molecules have been approved for patients with AGC. In 2010, trastuzumab was the first molecule shown to improve survival in patients with HER2-positive AGC as part of a first-line combination regimen. In 2014, ramucirumab was the second targeted molecule to improve survival rates and was suggested as treatment for patients with AGC who had progressed after firstline platinum plus fluoropyrimidine with or without anthracycline chemotherapy. Ramucirumab was the first targeted therapy acting as a single agent in patients with advanced gastroesophageal cancers. Although these two molecules were introduced into clinical use, many other promising molecules have been tested in phase Ⅰ-Ⅱ trials. It is obvious that in the near future many different targeted therapies will be in use for treatment of AGC. | 9 | Surface-enhanced Raman scattering on dielectric microspheres with whispering gallery mode resonance显示文摘Conventionally, metallic nanostructures are used for surface-enhanced Raman spectroscopy(SERS), but recently there has been increasing interest in the enhancement of Raman scattering from dielectric substrates due to their improved stability and biocompatibility compared with metallic substrates. Here, we report the observation of enhanced Raman scattering from rhodamine 6 G molecules coated on silica microspheres. We excite the whispering gallery modes(WGMs) supported in the microspheres with a tapered fiber coupler for efficient WGM excitation, and the Raman enhancement can be attributed to the WGM mechanism. Strong resonance enhancement in pump laser intensity and modified Raman emission from the Purcell effect in the microsphere resonator are observed from the experiment and compared with theoretical results. A total Raman enhancement factor of 1.4 × 10~4 is observed, with contribution mostly from the enhancement in pump laser intensity. | 10 | Comparison of non-cycloplegic photorefraction,cycloplegic photorefraction and cycloplegic retinoscopy in children显示文摘AIM: To compare the results of noncycloplegic photorefraction, cycloplegic photorefraction and cycloplegic refraction in preschool and non-verbal children.METHODS: One hundred and ninety-six eyes of 98children(50 females, 48 males) were included in the study. Firstly, non-cycloplegic photorefraction was achieved with Plusoptix A09; secondly, cycloplegic photorefraction was carried out with Plusoptix A09 after10 min cyclopentolate. Finally, 30 min after instillation of twice cyclopentolate, cycloplegic refraction was obtained with autorefraction and/or standard retinoscopy. Spheric equivalent, spheric power, cylindric power and cylindrical axis measurements were statistically compared.RESULTS: The mean age was 28.8±18.5mo(range12-72mo). The differences in spherical equivalent, spheric power and cylindrical power measured by the three methods were found statistically significant(P <0.05).The spherical equivalent and spheric power measured by cycloplegic photorefraction were statistically higher than the measurements of the other methods(P <0.05). The cylindrical power measured by cycloplegic refraction was statistically lower than the measurements of the photorefraction methods(P <0.05). There was no significant difference in cylindrical axis measurements between three methods(P >0.05).CONCLUSION: For the determination of refractive errors in children, the Plusoptix A09 measurements give incorrect results after instillation of cyclopentolate.Additionally, the cylindrical power measured by Plusoptix A09 with or without cycloplegia is higher. | 14 | General characteristics of patients with electrolyte imbalance admitted to emergency department显示文摘BACKGROUND:Fluid and electrolyte balance is a key concept to understand for maintaining homeostasis,and for a successful treatment of many metabolic disorders.There are various regulating mechanisms for the equilibrium of electrolytes in organisms.Disorders of these mechanisms result in electrolyte imbalances that may be life-threatening clinical conditions.In this study we defined the electrolyte imbalance characteristics of patients admitted to our emergency department.METHODS:This study was conducted in the Emergency Department(ED) of Uludag University Faculty of Medicine,and included 996 patients over 18 years of age.All patients had electrolyte imbalance,with various etiologies other than traumatic origin.Demographic and clinical parameters were collected after obtaining informed consent from the patients.The ethical committee of the university approved this study.RESULTS:The mean age of the patients was 59.28±16.79,and 55%of the patients were male.The common symptoms of the patients were dyspnea(14.7%),fever(13.7%),and systemic deterioration(11.9%);but the most and least frequent electrolyte imbalances were hyponatremia and hypermagnesemia,respectively.Most frequent findings in physical examination were confusion(14%),edema(10%) and rales(9%);and most frequent pathological findings in ECG were tachycardia in24%,and atrial fibrillation in 7%of the patients.Most frequent comorbidity was malignancy(39%).Most frequent diagnoses in the patients were sepsis(11%),pneumonia(9%),and acute renal failure(7%).CONCLUSIONS:Electrolyte imbalances are of particular importance in the treatment of ED patients.Therefore,ED physicians must be acknowledged of their fluid-electrolyte balance dynamics and general characteristics. | Arif Kadri Balci Ozlem Koksal Ataman Kose Erol Armagan Fatma Ozdemir Taylan Inal Nuran Oner | 2013 | World Journal of Emergency Medicine2013,4,2: | 3 |

| 16 | PPARγ配体罗格列酮及其激动剂GW9662对脂肪细胞因子表达的影响(英文)显示文摘目的:脂肪组织是一个内分泌器官已逐渐得到了肯定,它能分泌多种信号分子如:脂联素和抵抗素。过氧化物酶体增殖物激活受体(peroxisome pro-liferator activated receptorγ,PPARγ)在脂肪组织高水平表达,胰岛素增敏剂-噻唑烷二酮类药物是它的选择性激动剂,噻唑烷二酮类药物如罗格列酮的胰岛素增敏作用部分是通过激活PPARγ调节脂联素(胰岛素增敏分子)和抵抗素(涉及胰岛素抵抗)表达介导的。但现在不同研究发现PPARγ激动剂对抵抗素的表达调控方向存在矛盾,我们的问题是当抵抗素表达增加的情况下脂联素的表达还能否上调。方法:用3T3-L1细胞株作为研究模型,分别用溶媒对照、罗格列酮(10μmol/L)、GW9662(5μmol/L)或罗格列酮+GW9662作用细胞,然后检测脂联素和抵抗素 mRNA表达变化情况。结果:与对照组相比,罗格列酮分别增加脂联素和抵抗素 mRNA水平1.77和1.66倍,其差异具有统计学意义(P<0.05) ;重要的是GW9662也增加脂联素水平(1.57倍,P<0.05)但对抵抗素无影响。罗格列酮和GW9662两者合用时,仍上调adiponectin mRNA水平(对照组的1.87倍,P<0.05) ,抵抗素的增加与罗格列酮单用比弱下降(对照组的1.31倍,P<0.05)。结论:本研究为PPARγ激动剂(罗格列酮)和拮抗剂(GW9662)都上调脂联素的转录提供了新的证据,两者合用时GW9662不阻断罗格列酮诱导的脂联素上调作用。综合这些数据提示噻唑烷二酮类药上调脂联素的机制可能不依赖于PPARγ。并且, GW9662在增加脂联素水平的同时不上调抵抗素水平的特性进一步支持PPARγ拮抗剂用于临床治疗胰岛素抵抗的可能性。降低抵抗素表达可能不是罗格列酮胰岛素增敏作用的重要机制。我们的结果为将来研究噻唑烷二酮类药物对人脂肪细胞因子表达在剂量和时间上提供了一定的基础。 | LIU Ying-zi Vural Ozdemir OUYANG Dong-shengI LIU Zhao-qian LIU Jie LI Zhi WANG Dan ZENG Fei-yue TAN Zhi-rong HU Dong-li ZHOU Hong-hao | 2007 | 中国临床药理学与治疗学2007,12,10: | 3 |

| 18 | Parity-time-symmetric whispering-gallery mode nanoparticle sensor[Invited]显示文摘We present a study of single nanoparticle detection using parity-time(PT) symmetric whispering-gallery mode(WGM) resonators. Our theoretical model and numerical simulations show that, with balanced gain and loss, the PT-symmetric WGM nanoparticle sensor, tailored to operate at PT phase transition points(also called exceptional points), exhibits significant enhancement in frequency splitting when compared with a single WGM nanoparticle sensor subject to the same perturbation. The presence of gain in the PT-symmetric system leads to narrower linewidth, which helps to resolve smaller changes in frequency splitting and improve the detection limit of nanoparticle sensing. Furthermore, we also provide a general method for detecting multiple nanoparticles entering the mode volume of a PT-symmetric WGM sensor one by one.
