| 1 | Role of targeted therapy in metastatic colorectal cancer显示文摘Colorectal cancer(CRC) is a significant cause of cancer-related morbidity and mortality all over the world.Improvements of cytotoxic and biologic agents have prolonged the survival in metastatic CRC(mC RC),with a median overall survival of approximately 2 years and more in the past two decades.The biologic agents that have proven clinical benefits in m CRC mainly target vascular endothelial growth factor(VEGF) and epidermal growth factor receptor(EGFR).In particular,bevacizumab targeting VEGF and cetuximab and panitumumab targeting EGFR have demonstrated sig-nificant survival benefits in combination with cytotoxic chemotherapy in the first-line,second-line,or salvage setting.Aflibercept,ramucirumab,and regorafenib are also used in second-line or salvage therapy.Recent retrospective analyses have shown that KRAS or NRAS mutations were negative predictive markers for anti-EGFR therapy.Based on the evidence from large rand-omized clinical trials,personalized therapy is necessary for patients with m CRC according to their tumor biology and characteristics.The aim of this paper was to summarize the results of the major randomized clinical trials and highlight the benefits of the molecular targeted agents in patients with mC RC. | Yoshihito Ohhara Naoki Fukuda Satoshi Takeuchi Rio Honma Yasushi Shimizu Ichiro Kinoshita Hirotoshi Dosaka-Akita | 2016 | World Journal of Gastrointestinal Oncology2016，8，9： | 8 |
| 2 | Repeat endoscopic submucosal dissection for recurrent gastric cancers after endoscopic submucosal dissection显示文摘AIM:To clarify the safety and efficacy of repeat endoscopic submucosal dissection(re-ESD)for locally recurrent gastric cancers after ESD.METHODS:A retrospective evaluation was performed of the therapeutic efficacy,complications and followup results from ESD treatment for early gastric cancers in 521 consecutive patients with 616 lesions at St.Luke`s International Hospital between April 2004 and November 2012.In addition,tumor size,the size of resected specimens and the operation time were compared between re-ESD and initial ESD procedures.A flex knife was used as the primary surgical device and a hook knife was used in cases with severe fibrosis in the submucosal layer.Continuous variables were analyzed using the non-parametric Mann-Whitney U test and are expressed as medians(range).Categorical variables were analyzed using a Fisher’s exact test and are reported as proportions.Statistical significance was defined as a P-value less than 0.05.RESULTS:The number of cases in the re-ESD group and the initial ESD group were 5 and 611,respectively.The median time interval from the initial ESD to re-ESD was 14(range,4-44 mo).En bloc resection with free lateral and vertical margins was successfully performed in all re-ESD cases without any complications.No local or distant recurrence was observed during the median follow-up period of 48(range,11-56 mo).Tumor size was not significantly different between the re-ESD group and the initial ESD group(median 22 mm vs 11mm,P=0.09),although the size of resected specimens was significantly larger in the re-ESD group(median 47 mm vs 34 mm,P<0.05).There was a nonsignificant increase observed in re-ESD operation time compared to initial ESD(median 202 min vs 67 min,respectively,P=0.06).CONCLUSION:Despite the low patient number and short follow-up,the results suggest that re-ESD is a safe and effective endoscopic treatment for recurrent gastric cancer after ESD. | Yuto Shimamura Naoki Ishii Kaoru Nakano Takashi Ikeya Kenji Nakamura Koichi Takagi Katsuyuki Fukuda Koyu Suzuki Yoshiyuki Fujita | 2013 | World Journal of Gastrointestinal Endoscopy2013，5，12： | 5 |

| 4 | Surgery for gastric cancer patients of age 85 and older: Multicenter survey显示文摘AIM To investigate the surgical therapies for gastric cancer(GC) patients of age 85 or older in a multicenter survey.METHODS Therapeutic opportunities for elderly GC patients have expanded in conjunction with extended life expectancy. However, the number of cases encountered in a single institution is usually very small and surgical therapies for elderly GC patients have not yet been standardized completely. In the present study, a total of 134 GC patients of age 85 or older who underwent surgery in 9 related facilities were retrospectively investigated. The relationships between surgical therapies and clinicopathological or prognostic features were analyzed.RESULTS Eighty-nine of the patients(66%) presented with a comorbidity, and 26(19% overall) presented with more than two comorbidities. Radical lymphadenectomy was performed in 59 patients(44%), and no patient received pre- or post-operative chemotherapy. Forty of the patients(30%) experienced perioperative complications, but no surgical or perioperative mortality occurred. Laparoscopic surgery was performed in only 12 of the patients(9.0%). Univariate and multivariate analyses of the 113 patients who underwent R0 or R1 resection identified the factors of p T3/4 and limited lymphadenectomy as predictive of worse prognosis(HR = 4.68, P = 0.02 and HR =2.19, P = 0.05, respectively). Non-cancer-specific death was more common in c Stage Ⅰ patients than in c Stage Ⅱ or Ⅲ patients. Limited lymphadenectomy correlated with worse cancer-specific survival(P = 0.01), particularly in c Stage Ⅱ patients(P < 0.01). There were no relationships between limited lymphadenectomy and any comorbidities, except for cerebrovascular disease(P = 0.07).
