| 1 | Neoadjuvant chemotherapy for locally advanced cervical cancer显示文摘Neoadjuvant chemotherapy followed by surgery(NCS) has not been fully evaluated clinically. Currently, the main regimen of neoadjuvant chemotherapy(NAC) used in NCS includes cisplatin. The antitumor effects of NAC reduce lymph node metastasis and the tumor diameter in patients prior to surgery, and this can reduce the number of high risk patients who require postoperative radiation therapy. Many randomized controlled trials(RCTs) have examined the long-term prognosis of NCS compared to primary surgery, but the utility of NCS remains uncertain. The advent of concurrent chemoradiotherapy(CCRT) has markedly improved the outcome of radiotherapy(RT), and CCRT is now used as a standard method in many cases of advanced bulky cervical cancer. | 2 | Recent Advances in Researches on Physiologically Active Substances in Holothurians显示文摘In this report, we reviewed recent literature on physiologically active substances from sea cucumbers (SCs) andtheir activities together with results obtained from our study. Preventive properties against lipid metabolism were reported inrats using a whole SC preparation with no particular constituent specified. Administration of the preparation lowered serumand hepatic cholesterol levels and improved the HDL/LDL ratio. These functions may be attributed to the stimulatory effectof the extract on the secretion of cholesterol in feces. Novel fucosylated chondroitin sulfates (FCSs) from Ludwigothureagrisea significantly induced fibroblast growth factor 2-dependent angiogenesis in human umbilical vein endothelial cells (HU-VECs). The proangiogenetic activity seemed attributable to the action of the sulfated fucose branches on the polysaccharide.SCs contain mycosporine-like amino acids (MAAs) that are capable of absorbing UV. A biogenetic precursor of MAAs wasfirst reported in SCs. The anti-proliferative effects of a branched chain fatty acid from a sea cucumber on prostate cancercells was reported with the activity of 5-lipoxygenase. Glycosphingolipid constituents in SCs have been systematically ana-lyzed over the past ten years. The results showed that the gangliosides in several SCs differed from those of mammals in thata sialic acid of SC gangliosides directly binded to glucose of cerebroside. Neuritogenic activity of the glycosphingolipids wasdemonstrated in vitro experiments and may lead to the development of therapeutic products for neurological disorders. | 4 | 病理性近视患者视乳头周围脱离的特征显示文摘目的:在一组高度近视患者中,评估新近认识的视乳头周围病变。即病理性近视视乳头周围脱离(PD-PM)的患病率和临床特征。
方法:共纳入324位高度近视患者(632只眼)。在这些患者中观察PDPMD的发病率、病变范围、荧光素和吲哚青绿造影检查结果及OCT结果。并对有PDPM的患者进行视野检查(Goldmann动态视野检查法和Humphrey 30-2程序)。结果:632只高度近视眼中有31只眼(4.9%)存在视乳头周围脱离。PDPM病变部位的OCT扫描显示视神经邻近部位视网膜色素上皮层的局限性脱离。PDPM常位于视盘下方。但一些患者的病变几乎环绕整个视盘。邻近PDPM的下方近视弧形斑有陡峭的凹陷。且颞下视网膜静脉在跨过PDPM和凹陷的近视弧形斑时明显弯曲。PDPM眼常被检出存在青光眼性视野缺损(71.0%)。

| 5 | Hemi-hepatectomy in pediatric patients using two-surgeon technique and a liver hanging maneuver显示文摘AIM: To evaluate the efficacy of the two-surgeon technique with the liver hanging maneuver (LHM) for hepatectomies in pediatric patients with hepatoblastoma. METHODS: Three pediatric patients with hepatoblastoma were enrolled in this study. Two underwent right hemi-hepatectomies and one underwent a left hemihepatectomy using the two-surgeon technique by means of saline-linked electric cautery (SLC) and the Cavitron Ultrasonic Surgical Aspirator (CUSA; Valleylab, Boulder, CO) and the LHM. RESULTS: The mean operative time during the parenchymal transections was 50 min and the mean blood loss was 235 g. There was no bile leakage from the cut surface after surgery. No macroscopic or microscopic-positive margins were observed in the hepatic transections. | 6 | Reproducibility and Validity of a Chinese Food Frequency Questionnaire显示文摘Objectives This study was design to develop a semi-quantitative Chinese Food Frequency Questionnaire (FFQ) and to conduct a validation study for the questionnaire. Methods Based on the survey experience in recent years, a new Chinese food frequency questionnaire (FFQ) with 149 items in 17 food categories was developed. A validation study on this new FFQ was conducted in Jiangsu and Beijing of China between 1999 and 2001. The period of study covered 1 year and the FFQ was validated by comparing with data obtained by a six repeated 24-hour recalls for 3 consecutive days, or a totally 18-day 24- hour recall throughout the year. A total of 271 healthy adult subjects were enrolled in the study. Food and nutrient intakes measured by the 18-day dietary recalls and food frequency questionnaires (FFQs) were computed in the National Institute for Nutrition and Food Safety, China CDC using the existing nutrition database. The average daily intake of foods and nutrients over the 18-day recall was used to compare with FFQ1 and FFQ2, which was conducted at the beginning and the end of the year, respectively. | 7 | Expression of the tumor suppressor gene maspin and its significance in intraductal papillary mucinous neoplasms of the pancreas显示文摘BACKGROUND: Maspin is a member of the serpin family of protease inhibitors and is thought to inhibit carcinoma invasion, metastasis, and angiogenesis and induce apoptosis. We examined maspin expression immunohistochemically and assessed its significance in intraductal papillary mucinous neoplasm (IPMN) of the pancreas. METHODS: We examined 39 surgically resected specimens of IPMN that included 17 adenomas (IPMAs), 5 borderline tumors (IPMBs), 4 non-invasive carcinomas (non-invasive IPMCs), and 13 invasive carcinomas (invasive IPMCs). Immunostaining was performed according to the EnVision ChemMate method. The degree of maspin expression was scored and assessed according to the percentage and staining intensity of positive cells. RESULTS: Maspin expression was minimal in normal pancreatic duct epithelium, whereas in IPMNs, maspin was expressed in neoplasms of all stages. Maspin expression increased with increasing grade from IPMAs, IPMBs, to non-invasive IPMCs but decreased significantly in invasive IPMCs. No specific association between maspin expression and mucin type was found. Analysis of maspin expression with respect to clinicopathologic factors in cases of invasive IPMC revealed a greater extent of invasion in cases of low maspin expression and significantly fewer apoptotic cells in the tumor.CONCLUSIONS: Maspin was expressed at high levels in IPMNs at various stages from adenoma to invasive carcinoma, and our results suggest that maspin may be involved in the occurrence and progression of IPMN. | 8 | Promising biological therapies for ulcerative colitis: A review of the literature显示文摘Ulcerative colitis(UC) is a chronic lifelong condition characterized by alternating flare-ups and remission. There is no single known unifying cause, and the pathogenesis is multifactorial, with genetics, environmental factors, microbiota, and the immune system all playing roles. Current treatment modalities for UC include 5-aminosalicylates, corticosteroids, immunosuppressants(including purine antimetabolites, cyclosporine, and tacrolimus), and surgery. Therapeutic goals for UC are evolving. Medical treatment aims to induce remission and prevent relapse of disease activity. Infliximab, an anti-tumor necrosis factor(TNF)-α monoclonal antibody, is the first biological agent for the treatment of UC. Over the last decade, infliximab and adalimumab(anti-TNF-α agents) have been used for moderate to severe UC, and have been shown to be effective in inducing and maintaining remission. Recent studies have indicated that golimumab(another anti-TNF-α agent), tofacitinib(a Janus kinase inhibitor), and vedolizumab and etrolizumab(integrin antagonists), achieved good clinical remission and response rates in UC. Recently, golimumab and vedolizumab have been approved for UC by the United States Food and Drug Administration. Vedolizumab may be used as a first-line alternative to anti-TNF-α therapy in patients with an inadequate response to corticosteroids and/or immunosuppressants. | 9 | Diabetes mellitus and metformin in hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC) is the leading cause of cancer-related death worldwide. Diabetes mellitus, a risk factor for cancer, is also globally endemic. The clinical link between these two diseases has been the subject of investigation for a century, and diabetes mellitus has been established as a risk factor for HCC. Accordingly, metformin, a first-line oral anti-diabetic, was first proposed as a candidate anti-cancer agent in 2005 in a cohort study in Scotland. Several subsequent large cohort studies and randomized controlled trials have not demonstrated significant efficacy for metformin in suppressing HCC incidence and mortality in diabetic patients; however, two recent randomized controlled trials have reported positive data for the tumor-preventive potential of metformin in non-diabetic subjects. The search for biological links between cancer and diabetes has revealed intracellular pathways that are shared by cancer and diabetes. The signal transduction mechanisms by which metformin suppresses carcinogenesis in cell lines or xenograft tissues and improves chemoresistance in cancer stem cells have also been elucidated. | 12 | Selective decrease in colonic CD56^+ T and CD161^+ T cells in the inflamed mucosa of patients with ulcerative colitis显示文摘AIM: To investigate the role of local colonic mucosal NK receptor-positive T (NKR+ T) cells in the regulation of intestinal inflammation, we analyzed the population and function of these cells in ulcerative colitis (UC). METHODS: Colonic mucosal tissues were obtained from colonoscopic biopsies of the descending colon from 96 patients with UC (51 endoscopically uninfl amed, 45 inflamed) and 18 normal controls. Endoscopic appearance and histologic score at the biopsied site were determined by Matts’ classification. A single cell suspension was prepared from each biopsy by collagenase digestion. Two NKR+ T cell subsets, CD56+ (CD56+CD3+) T cells and CD161+ (CD161+CD3+) T cells, were detected by flow cytometric analysis. Intracellular cytokine analysis for anti-in? ammatory cytokine interleukin-10 (IL-10) was performed by in vitro stimulation with phorbol-myristate- acetate (PMA) and ionomycin. RESULTS: CD56+ T cells and CD161+ T cells are present in the normal human colon and account for 6.7% and 21.3% of all mononuclear cells, respectively. The populations of both CD56+ T cells and CD161+ T cells were decreased significantly in the inflamed mucosa of UC. In contrast, the frequency of conventional T cells (CD56-CD3+ cells and CD161-CD3+ cells) was similar among the patient and control groups. The populations of NKR+ T cells were correlated inversely with the severity of infl ammation, which was classif ied according to the endoscopic and histologic Matts’ criteria. | 13 | FTY720,Sphingosine 1-Phosphate Receptor Modulator,Ameliorates Experimental Autoimmune Encephalomyelitis by Inhibition of T Cell Infiltration显示文摘FTY720, a sphingosine 1-phosphate receptor modulator, induces a marked decrease in the number of peripheral blood lymphocytes and exerts immunomodulating activity in various experimental allograft and autoimmune disease models. In this study, we evaluated the effect of FTY720 and its active metabolite, (S)-enantiomer of FTY720-phosphate [(S)-FTY720-P] on experimental autoimmune encephalomyelitis (EAE) in rats and mice.Prophylactic administration of FTY720 at 0.1 to 1 mg/kg almost completely prevented the development of EAE,and therapeutic treatment with FTY720 significantly inhibited the progression of EAE and EAE-associated histological change in the spinal cords of LEW rats induced by immunization with myelin basic protein. Consistent with rat EAE, the development of proteolipid protein-induced EAE in SJL/J mice was almost completely prevented and infiltration of CD4+ T cells into spinal cord was decreased by prophylactic treatment with FTY720 and (S)-FTY720-P. When FTY720 or (S)-FTY720-P was given after establishment of EAE in SJL/J mice, the relapse of EAE was markedly inhibited as compared with interferon-β, and the area of demyelination and the infiltration of CD4+ T cells were decreased in spinal cords of EAE mice. Similar therapeutic effect by FTY720 was obtained in myelin oligodendrocyte glycoprotein-induced EAE in C57BL/6 mice. | 14 | 在有类型 1 自体免疫的肝炎的日本病人的尖锐表示的 Centrilobular 坏死显示文摘 AIM:To compare clinicopathological features of acute presentation of type 1 autoimmune hepatitis(AIH) with or without centrilobular necrosis(CN).METHODS:Our study comprised 41 patients with biopsy-proven acute presentation(acute exacerbation phase 36,acute hepatitis phase 5) of type 1 AIH at our hospital from 1975 to 2009.Elevated serum alanine aminotransferase(ALT)(> 5x upper limit of normal) identified acute presentation of the disease.We compared clinicopathological features of these AIH patients with or without CN.The data used for analysis included patient background(age,sex,type of disease,presence of complications with other autoimmune diseases,human leukocyte antigen,and International Autoimmune Hepatitis Group score),clinical parameters at presentation(ALT,alkaline phosphatase,IgG,anti-nuclear antibodies,and anti-smooth muscle antibodies),histology and therapy.RESULTS:CN was found in 13(31.7%) patients with acute presentation(acute exacerbation phase 10,acute hepatitis phase 3) of AIH.Serum IgG levels of patients with CN were significantly lower than those of patients without CN(mean:2307 mg/dL vs 3126 mg/dL,P < 0.05),while antinuclear antibody-negative rates were significantly higher(30.7%vs 3.5%,P < 0.05).However,other clinical features were similar between the two groups.The frequency of advanced fibrosis in patients with CN was significantly lower than in patients without CN(F0-2:84.6% vs 35.7%,F3-4:15.4% vs 64.3%,P < 0.05).Other histological features were similar between the two groups.Although there was no significant difference between groups when evaluated using the revised original score(12 vs 14),the simplified AIH score of patients with CN was significantly lower(6 vs 7,P < 0.05).Frequency of DR4 was similar between patients with and without CN.CONCLUSION:CN is observed in both Japanese patients with acute hepatitis phase and acute exacerbation phase of type 1 AIH,although AIH with CN often shows clinical features of the genuine acute form. | Kazumichi Abe Yukiko Kanno Ken Okai Fumiko Katsushima Kyoko Monoe Hironobu Saito Atsushi Takahashi Junko Yokokawa Hiromasa Ohira | 2012 | World Journal of Hepatology2012,4,9: | 5 |
| 15 | Toll-like receptor 9 gene mutations and polymorphisms in Japanese ulcerative colitis patients显示文摘Abnormal innate immune responses toward luminal bacteria play an important role in the pathogenesis of inflammatory bowel disease.It has been demonstrated that bacteria having CpG DNA ameliorate experimental colitis in mice,and Toll-like receptor 9 (TLR9) signaling mediates the anti-inflammatory effects in mouse colonic inflammation.A gene variation in NOD2/CARD15 has been reported in Crohn's disease (CD) patients in Western countries,but this variation has not been identified in Japanese CD patients.Therefore,we hypothesized that TLR9 is a key factor in the development of ulcerative colitis (UC),and we investigated gene mutations and polymorphisms of TLR9 in Japanese UC patients.Three single nucleotide polymorphisms (SNPs) in TLR9 were identified in healthy controls,and were assessed in 48 UC patients and 47 healthy controls.Control subjects were matched for age,sex and date of blood sampling from among a subgroup of participants.We found that TLR9-1486CC,1174GG and 2848AA increase the risk of UC [odds ratio (OR) 2.64,95% confidence interval (95% CI):1.73-6.53,P=0.042],and TLR9-1486TT,1174AA and 2848GG decrease the risk of UC (OR 0.30,95% CI:0.10-0.94,P=0.039),although there were no correlations between SNPs and disease phenotype or TLR9 mRNA expression.These findings suggest that TLR9 polymorphisms are associated with increased susceptibility to UC. | Kaori Fuse Kyoko Katakura Natsumi Sakamoto Hiromasa Ohira | 2010 | World Journal of Gastroenterology2010,16,46: | 4 |
