2Prevention of esophageal strictures after endoscopic submucosal dissection显示文摘Endoscopic mucosal resection(EMR) and endoscopic submucosal dissection(ESD) have recently been accepted as less invasive methods for treating patients with early esophageal cancers such as squamous cell carcinoma and dysplasia of Barrett's esophagus.However,the large defects in the esophageal mucosa often cause severe esophageal strictures,which dramatically reduce the patient's quality of life.Although preventive endoscopic balloon dilatation can reduce dysphagia and the frequency of dilatation,other approaches are necessary to prevent esophageal strictures after ESD.This review describes several strategies for preventing esophageal strictures after ESD,with a particular focus on anti-inflammatory and tissue engineering approaches.The local injection of triamcinolone acetonide and other systemic steroid therapies are frequently used to prevent esophageal strictures after ESD.Tissue engineering approaches for preventing esophageal strictures have recently been applied in basic research studies.Scaffolds with temporary stents have been applied in five cases,and this technique has been shown to be safe and is anticipated to prevent esophageal strictures.Fabricated autologous oral mucosal epithelial cell sheets to cover the defective mucosa similarly to how commercially available skin products fabricated from epidermal cells are used for skin defects or in cases of intractable ulcers.Fabricated autologous oralmucosal-epithelial cell sheets have already been shown to be safe.Shinichiro Kobayashi Nobuo Kanai Takeshi Ohki Ryo Takagi Naoyuki Yamaguchi Hajime Isomoto Yoshiyuki Kasai Takahiro Hosoi Kazuhiko Nakao Susumu Eguchi Masakazu Yamamoto Masayuki Yamato Teruo Okano 2014World Journal of Gastroenterology2014,20,41:12
3Cell sheet technology for regeneration of esophageal mucosa显示文摘The progress of tissue-engineering technology has realized development of new therapies to treat various disorders by using cultured cells. Cell-and tissue-based therapies have been successfully applied to human patients, and several tissue-engineered products have been approved by the regulatory agencies and are commercially available. In the review article, we describe our experience of development and clinical application of cell sheet-based regenerative medicine.Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) have been shown to be useful for removal of gastrointestinal neoplasms with less invasiveness compared with open surgery, especially in esophageal surgery. However, postoperative inflammation and stenosis are major complications observed after intensive mucosal resection. Therefore, we have developed novel regenerative medicine to prevent such complications and promote wound healing of esophageal mucosa after EMR or ESD. Transplantable oral mucosal epithelial cell sheets were fabricated from patients' own oral mucosa. Immediately after EMR or ESD, fabricated autologous cell sheets were endoscopically transplanted to the ulcer sites. We performed a preclinical study with a canine model. In human clinical settings, cell culture and cell sheet fabrication were performed in clean rooms according to good manufacturing practice guidelines, and pharmaceutical drugs were used as supplements to culture medium in place of research regents used in animal study. 4Recent development of temperature-responsive surfaces and their application for cell sheet engineering显示文摘Cell sheet engineering,which fabricates sheet-like tissues without biodegradable scaffolds,has been proposed as a novel approach for tissue engineering.Cells have been cultured and proliferate to confluence on a temperature-responsive cell culture surface at 37℃.By decreasing temperature to 20℃,an intact cell sheet can be harvested from the culture surface without enzymatic treatment.This new approach enables cells to keep their cell–cell junction,cell surface proteins and extracellular matrix.Therefore,recovered cell sheet can be easily not only transplanted to host tissue,but also constructed a three-dimensional(3D)tissue by layering cell sheets.Moreover,cell sheet manipulation technology and bioreactor have been combined with the cell sheet technology to fabricate a complex and functional 3D tissue in vitro.So far,cell sheet technology has been applied in regenerative medicine for several tissues,and a number of clinical studies have been performed.In this review,recent advances in the preparation of temperature-responsive cell culture surface,the fabrication of organ-like tissue and the clinical application of cell sheet engineering are summarized and discussed.Zhonglan Tang Teruo Okano 2014Regenerative Biomaterials2014,1,1:8
5Inhibitory effects of carbon dioxide insufflation on pneumoperitoneum and bowel distension after percutaneous endoscopic gastrostomy显示文摘AIM:To evaluate the inhibitory effects of carbon dioxide (CO2) insufflation on pneumoperitoneum and bowel distension after percutaneous endoscopic gastrostomy (PEG).METHODS:A total of 73 consecutive patients who were undergoing PEG were enrolled in our study.After eliminating 13 patients who fitted our exclusion criteria,60 patients were randomly assigned to either CO2 (30 patients) or air insufflation (30 patients) groups.PEG was performed by pull-through technique after threepoint fixation of the gastric wall to the abdominal wall using a gastropexy device.Arterial blood gas analysis was performed immediately before and after the procedure.Abdominal X-ray was performed at 10 min and at 24 h after PEG to assess the extent of bowel distension.Abdominal computed tomography was performed at 24 h after the procedure to detect the presence of pneumoperitoneum.The outcomes of PEG for 7 d postprocedure were also investigated.RESULTS:Among 30 patients each for the air and the CO2 groups,PEG could not be conducted in 2 patients of the CO2 group,thus they were excluded.Analyses of the remaining 58 patients showed that the patients' backgrounds were not significantly different between the two groups.The elevation values of arterial partial pressure of CO2 in the air group and the CO2 group were 2.67 mmHg and 3.32 mmHg,respectively (P = 0.408).The evaluation of bowel distension on abdominal X ray revealed a significant decrease of small bowel distension in the CO2 group compared to the air group (P < 0.001) at 10 min and 24 h after PEG,whereas there was no significant difference in large bowel distension between the two groups.Pneumoperitoneum was observed only in the air group but not in the CO2 group (P = 0.003).There were no obvious differences in the laboratory data and clinical outcomes after PEG between the two groups.CONCLUSION:There was no adverse event associated with CO2 insufflation.CO2 insufflation is considered to be safer and more comfortable for PEG patients because of the lower incidence of pneumoperitoneum and less distension of the small bowel.Shinji Nishiwaki Hiroshi Araki Motoshi Hayashi Jun Takada Masahide Iwashita Atsushi Tagami Hiroo Hatakeyama Takao Hayashi Teruo Maeda Koshiro Saito 2012World Journal of Gastroenterology2012,18,27:7
6Functional outcomes of traumatic and non-traumatic rotator cuff tears after arthroscopic repair显示文摘AIM To compare the functional outcomes of traumatic and non-traumatic rotator cuff tears after arthroscopic repair. METHODS Eighty-seven patients with rotator cuff tears following arthroscopic treatment were divided into traumatic and non-traumatic tear groups. Postoperative muscle strength and outcomes using the modified University of California, Los Angeles score were evaluated. Sex, age, affected limb and dominant limb were correlated between groups. Muscle strength of the repaired and unaffected shoulders was compared. Rotator cuff injury size was measured. RESULTS Of the 87 patients who underwent rotator cuff repairs, 35 had traumatic tears and 52 had non-traumatic tears. In patients with non-traumatic tears, the average age was 59 years, 74.5% were female, 96.1% were righthand dominant and 92.3% had their dominant shoulder affected. Patients with traumatic tears were 59.5 yearsold on average, 51.4% were female, 91.4% were righthand dominant and 88.5% had their dominant shoulder affected. No difference existed in the mean modified University of California, Los Angeles score between patients with traumatic tears(33.7) compared with those with non-traumatic tears(32.8). No strength differences were observed between groups: The strength difference between the non-affected and affected sides was 1.21 kg in the non-traumatic group and 1.39 kg in the traumatic group(P = 0.576), while the strength ratio between the non-affected/affected sides was 0.805 in the nontraumatic group and 0.729 in the traumatic group(P = 0.224). CONCLUSION The functional results of traumatic rotator cuff repairs are similar to non-traumatic tears. 8Effects on coagulation factor production following primary hepatomitogen-induced direct hyperplasia显示文摘AIM:To investigate the molecular mechanisms involved in coagulation factor expression and/or function during direct hyperplasia(DH) -mediated liver regeneration.METHODS:Direct hyperplasia-mediated liver regeneration was induced in female C57BL/6 mice by administering 1,4-bis[2-(3,5-dichloropyridyloxy) ] benzene(TCPOBOP) ,a representative hepatomitogen.Mice were weighed and sacrificed at various time points [Day 0(D0:prior to injection) ,3 h,D1,D2,D3,and D10] after TCPOBOP administration to obtain liver and blood samples.Using the RNA samples extracted from the liver,a comprehensive analysis was performed on the hepatic gene expression profiling of coagulation-related factors by real-time RT-PCR(fibrinogen,prothrombin,factors Ⅴ,Ⅶ,Ⅷ,Ⅸ,Ⅹ,Ⅺ,Ⅻ,XⅢβ,plasminogen,antithrombin,protein C,protein S,ADAMTS13,and VWF) .The corresponding plasma levels of coagulation factors(fibrinogen,prothrombin,factors Ⅴ,Ⅶ,Ⅷ,Ⅸ,Ⅹ,Ⅺ,Ⅻ,XⅢ,and VWF) were also analyzed and compared with their mRNA levels.RESULTS:Gavage administration of TCPOBOP(3 mg/kg body weight) resulted in a marked and gradual increase in the weight of the mouse livers relative to the total body weight to 220% by D10 relative to the D0(control) ratios.At the peak of liver regeneration(D1 and D2) ,the gene expression levels for most of the coagulationrelated factors(fibrinogen,prothrombin,factors Ⅴ,Ⅶ,Ⅷ,Ⅸ,Ⅺ,Ⅻ,XⅢβ,plasminogen,antithrombin,protein C,ADAMTS13,VWF) were found to be downregulated in a time-dependent manner,and gradually recovered by D10 to the basal levels.Only mRNA levels of factor Ⅹ and protein S failed to show any decrease during the regenerative phase.As for the plasma levels,5 clotting factors(prothrombin,factors Ⅷ,Ⅸ,Ⅺ,and Ⅻ) demonstrated a significant decrease(P < 0.05) during the regeneration phase compared with D0.Among these 5 factors,factor Ⅸ and factor Ⅺ showed the most dramatic decline in their activities by about 50% at D2 compared to the basal levels,and these reductions in plasma activity for both factors were consistent with our RT-PCR findings.In contrast,the plasma activities of the other coagulation factors(fibrinogen,factors Ⅴ,Ⅶ,XⅢ,and VWF) were not significantly reduced,despite the reduction in the liver mRNA levels.Unlike the other factors,FX showed a temporal increase in its plasma activity,with significant increases(P < 0.05) detected at D1.CONCLUSION:Investigating the coagulation cascade protein profiles during liver regeneration by DH may help to better understand the basic biology of the liver under normal and pathological conditions.Kohei Tatsumi Kazuo Ohashi Sanae Taminishi Soichi Takagi Rie Utoh Akira Yoshioka Midori Shima Teruo Okano 2009World Journal of Gastroenterology2009,15,42:5

11Biomimetic artificial cartilage:fibre-reinforcement of PVA hydrogel to promote biphasic lubrication mechanism显示文摘The fibre-reinforced structure in synovial articular cartilage plays an important role in enhancing the fluid load support in the biphasic lubrication mechanism.Poly(vinyl alcohol)(PVA)hydrogel is a potential biomimetic articular cartilage material.In this study,PVA hydrogel was reinforced with PVA fibres to improve its frictional properties.By computational analysis,the position of the PVA fibre layer was examined with a migrating contact condition to reduce the friction coefficient.To improve the fibre reinforcement,a method for fibre placement was developed to retain the initial strain of the fibre in the hydrogel matrix.The experimental results showed that the fibre-reinforced PVA hydrogel with a surface fibre layer had a low friction coefficient of 0.031 in pure water.The fibre-reinforced PVA hydrogel successfully reduced friction coefficient.Nobuo Sakai Seido Yarimitsu Yoshinori Sawae Mochimitsu Komori Teruo Murakami 2019Biosurface and Biotribology2019,5,1:3
