| 1 | Gut microbiota profile in healthy Indonesians显示文摘BACKGROUND Recently, gut microbiota has been associated with various diseases other than intestinal disease. Thus, there has been rapid growth in the study of gut microbiota. Considering the numerous factors influencing gut microbiota such as age, diet, etc., area-based research is required. Indonesia has numerous different tribes and each of these tribes have different lifestyles. Hence, it is expected that each tribe has a specific gut microbiota. A deeper insight into the composition of gut microbiota can be used to determine the condition of gut microbiota in Indonesians and to consider which treatment may be suitable and effective to improve health status.AIM To investigate the gut microbiota of Indonesian subjects represented by Javanese and Balinese tribes by analyzing fecal samples.METHODS Fecal samples were collected from a total of 80 individuals with 20 in each of the young groups ranging from 25-45 years and the elderly group aged 70 years or more from two different regions, Yogyakarta and Bali. Fecal sample collection was performed at the end of the assessment period(day 14 ± 1 d) during which time the subjects were not allowed to consume probiotic or antibiotic products.The quantification of various Clostridium subgroups, Lactobacillus subgroups,Enterococcus, Streptococcus, Staphylococcus, Bacteroides fragilis group and Prevotella,Bifidobacterium and Atopobium cluster, Enterobacteriaceae and Pseudomonas was performed using the Yakult intestinal flora-scan(YIF-SCAN).RESULTS The bacterial population in younger subjects' feces was higher than that in the elderly population, with a total of approximately 10.0 – 10.6 log10 bacterial cells/g feces. The most abundant bacteria in all groups were Clostridium, followed by Prevotella, Atopobium, Bifidobacterium and Bacteroides. In the elderly, an increase in Enterobacteriaceae, Coliform and Escherichia coli was found. In terms of bacterial counts in Yogyakarta, total bacteria, Clostridium coccoides(C. coccoides) group,Bifidobacterium, Prevotella, Lactobacillus plantarum subgroup, and Streptococcus were significantly higher(P < 0.05) in younger than elderly subjects, while the Lactobacillus gasseri subgroup, Lactobacillus casei subgroup, and Lactobacillus reuteri subgroup counts were significantly lower(P < 0.05) in younger subjects. In Balinese subjects, total bacteria, C. coccoides group, Clostridium leptum subgroup,Bacteroides fragilis group, and Prevotella were significantly higher(P < 0.05) in younger compared to elderly individuals, while the Lactobacillus ruminis subgroup, and Enterobacteriaceae were significantly lower(P < 0.05) in younger subjects. The results also revealed that, besides the C. coccoides group and Clostridium leptum group being the most abundant gut microbiota in both Yogyakarta and Balinese people, the latter was indicated by a higher Clostridium perfringens count, which was almost 10 times that of Yogyakarta subjects. This may be a response to different lifestyles in the different tribes; however, this phenomenon requires further extensive study.CONCLUSION Bacterial populations were higher in younger than in elderly subjects. Most abundant bacterial groups were Clostridium, Prevotella, Atopobium, Bifidobacterium,and Bacteroides. | 2 | e-antigen-negative chronic hepatitis B in Bangladesh显示文摘BACKGROUND:Bangladesh is a densely populated country where about 10 million people are chronically infected with hepatitis B virus(HBV).The aim of the present study was to evaluate the biochemical,virological and histological characteristics of HBeAg-negative chronic hepatitis B(CHB). METHODS:Patients were included in this study if they were chronically infected with HBV with detectable DNA.The patients who were co-infected with human immunodeficiency virus,hepatitis delta virus or hepatitis C virus,and previously subjected to antiviral treatment,and those with hepatocellular carcinoma were excluded.The study was conducted during the period of January 2001 to December 2007.During this period 2617 patients with CHB were studied.HBeAg-positive cases were included to compare the characteristics.Among them,237 cases underwent liver biopsy. RESULTS:2296 patients(87.7%)were male,with a mean age of 28.9±13.7 years.2375 patients(90.8%)had CHB,and 242(9.2%)were cirrhotic.HBV DNA levels were 7.6±1.5 copies/ml,ALT was 111.3±212.5 U/L,and AST was 91.5± 148.9 U/L.The number of HBeAg-negative CHB cases was 1039(39.7%).HBeAg-negative patients with a lower DNA load were older,and they had more fibrotic changes in the liver than HBeAg-positive patients.The two groups did not differ in necroinflammatory activity,but the former had lower ALT and AST values.Cirrhosis was more common in e-antigen-negative patients.CONCLUSIONS:e-antigen-negative CHB patients are older and have more hepatic fibrosis patients than HBeAg-positive patients,although they have similar necroinflammatory activity. | Nooruddin Ahmad Shahinul Alam Golam Mustafa Abul Barkat Muhammad Adnan Rahat Hasan Baig Mobin Khan | 2008 | Hepatobiliary & Pancreatic Diseases International2008,7,4: | 6 |

| 4 | Splenic injuries secondary to colonoscopy:Rare but serious complication显示文摘BACKGROUND Colonoscopy is a safe and routine diagnostic and therapeutic procedure for evaluation of large bowel diseases.Most common procedure related complications include bleeding and perforation but rarely a splenic Injury.AIM To investigate the reason for colonoscopy,presentation of patient with spleen injury,types of injury,diagnosis,management and outcomes of patients METHODS A structured search on four databases was done and 45 articles with 68 patients were selected.The reason for colonoscopy,presentation of patient with spleen injury,types of injury,diagnosis,management and outcomes of patients were identified and analyzed using SPSS.RESULTS The mean age of the patients was 62.7 years with 64%females.Twenty two percent had a complete splenic rupture with colonoscopy while 63%had subcapsular hematoma,spleen laceration and spleen avulsion.The most common reason for colonoscopy was screening(46%)followed by diagnostic colonoscopy(28%).Eighty seven percent of patients presented with abdominal pain.Patients with spleen rupture mostly required splenectomy(47%),while minor spleen hematomas and lacerations were managed conservatively(38%).Six percent of the patients were managed with proximal splenic artery splenic embolization and 4%were managed with laparoscopic repair.The overall mortality was 10%while 77%had complete recovery.The reason of colonoscopy against presentation specifically,abdominal pain showed no statistical significance P=0.69.The indication of colonoscopy had no significant impact on incidence of splenic injury(P=0.89).Majority of the patients(47%)were managed with splenectomy while the rest were managed conservatively(P=0.04).This association was moderately strong at a cramer’s V test(0.34).The Fisher exact test showed a higher mortality with spleen rupture(P=0.028).CONCLUSION Spleen rupture due to colonoscopy is a significant concern and is associated with high mortality.The management of the patients can be individualized based on clinical presentation. | Waqas Ullah Mamoon Ur Rashid Asif Mehmood Yousaf Zafar Ishtiaq Hussain Deepika Sarvepalli Muhammad Khalid Hasan | 2020 | World Journal of Gastrointestinal Surgery2020,12,2: | 4 |

| 6 | Colon mucosal neoplasia referred for endoscopic mucosal resection:Recurrence of adenomas and prediction of submucosal invasion显示文摘BACKGROUND Endoscopic mucosal resection(EMR)is an effective and minimally invasive alternative to surgery for large polyps and laterally spreading lesions.Gross morphology and surface characteristics may help predict submucosal invasion of the lesion(SMIL)during endoscopic evaluation.This is one of the largest singlecenter studies reporting endoscopic mucosal resection for larger(≥20 mm)colorectal lesions in the United States.AIM To determine the recurrence rate of adenomas and endoscopic features that may predict submucosal invasion of colonic mucosal neoplasia.METHODS This is a retrospective cohort study of all the patients referred for endoscopic mucosal resection for lesions≥20 mm,spanning a period from January 2013 to February 2017.The main outcome measure was identifying features that may predict submucosal invasion of mucosal lesions and predict recurrence of adenomas on follow-up surveillance colonoscopy performed at 4-6 mo.RESULTS A total of 480 patients with 500 lesions were included in the study.The median age was 68(Inter quantile range:14)with 52%males.The most common lesion location was ascending colon(161;32%).Paris classification 0-IIa(Flat elevation of mucosa-316;63.2%);Kudo Pit Pattern IIIs(192;38%)and Granular surface morphology(260;52%)were most prevalent.Submucosal invasion was present in 23(4.6%)out of 500 lesions.The independent risk factors for SMIL were Kudo Pit Pattern IIIL+IV and V(Odds ratio:4.5;P value<0.004)and Paris classification 0-IIc(Odds ratio:18.2;P value<0.01).Out of 500,354 post-endoscopic mucosal resection scars were examined at surveillance colonoscopy.Recurrence was noted in 21.8%(77 cases).CONCLUSION There was overall low prevalence of SMIL in our study.Kudo pit pattern(IIIL+IV and V)and Paris classification 0-IIc were the only factors identified as an independent risk factor for submucosal invasion.The independent risk factor for recurrence was adenoma size(>40 mm).Almost all recurrences(98.8%)were treated endoscopically. | Mamoon Ur Rashid Neelam Khetpal Hammad Zafar Saeed Ali Evgeny Idrisov Yuan Du Assaf Stein Deepanshu Jain Muhammad Khalid Hasan | 2020 | World Journal of Gastrointestinal Endoscopy2020,12,7: | 3 |
| 7 | 高密度电法电性结构揭示天山天池堆积坝体厚度及其阻渗稳水功能分析显示文摘天山天池北堆积坝体结构受其成因和演化控制,影响其含水和渗流特性。对该坝体稳定至关重要的下伏基岩埋深、坝体结构阻渗水结构特征等需现场探测并给出科学判断。在地质调查基础上采用高密度电法对天池堆积坝体结构进行探测,首次获得深达下伏基岩的堆积坝体纵向和横向电性剖面。结合光释光测年、水化学成分对比、水量平衡计算等手段,分析坝体堆积物形成时代以及坝体水文地质结构特征。结果显示:天池坝堆积体厚度大于100 m并可分为3层,上部浅表层滑坡形成的巨大块石层厚30~40 m且基本不含水,其下松散含水层厚度为30~50 m;下部为较低电阻率弱透水的冰碛物,主要分布在坝体下游和东端;底部为高电阻率不透水的石炭系火山岩。坝体中间部位地下水集中下渗补给了西小天池。同时靠坝体东侧发育排泄通道,呈现多处溢水点,控制了飞龙涧左岸下降泉季节性发育,同河谷冲刷作用一起影响并导致下游左岸冰碛物中发育4处滑坡体。电法剖面揭示出与坝体轴向近平行的2条隐伏断层,其中堆积坝高堤处气象台所在山丘上游一侧发育一条近东西走向并倾向上游的断层F1,构成地下水阻隔带而使天池堆积坝上游潜水水位呈现一定雍高,同坝体下伏基岩和弱透水冰碛物联合组成的坝体结构致使旱季天池水不会大量溢出,夏季丰水时浅表泉水渗漏,保证了天池水满而不溢、结构松而不垮。由此构建该堆积体内断层和地层结构组成的浅表导水模型,计算得出天池堆积体地下水排泄量约为1200 m^(3)/d,天池总下泄水量为52×10^(4)~113×10^(4)m^(3)/d(从冬季到夏季),维持了天池湖水动态稳定。 | 尚彦军 金维浚 伊学涛 姜东廷 HASAN Muhammad | 2023 | 现代地质2023,37,1: | 3 |
| 8 | Investigation of fractured rock aquifer in South China using electrical resistivity tomography and self-potential methods显示文摘Assessment of fractured rock aquifers in many parts of the world is complicated given their strong heterogeneity. Delineation of the subsurface geological formation in the weathered terrain is essential for groundwater exploration. To achieve this goal, 2D electrical resistivity tomography(ERT) and self-potential(SP) in combination with joint profile method(JPM) and boreholes have been carried out to delineate the subsurface geological units, detect the fracture/fault zones in hard rock, monitor the groundwater flow, and estimate the groundwater reserves contained within the weathered terrain at a complex heterogeneous site of Huangbu, South Guangdong of China. The integration of resistivity images with the borehole lithology along three profiles delineates three subsurface distinct layers namely topsoil cover, weathered and unweathered layers. The incorporation of ERT and SP with JPM reveal five fractures/faults, i.e., F_1, F_2, F_3, F_4 and F_5. 2D ERT models interpret the less resistive anomalies as the fractures/faults zones, and high resistive anomalies as the fresh bedrock. The inversion program based on the smoothness-constraint is used on the resistivity field data to get more realistic three layered model. SP measurements are obtained along the same electrical profiles which provide the negative anomalies clearly indicating the groundwater preferential flow pathways along the fracture/fault zones. Hydraulic parameters namely hydraulic conductivity and transmissivity were determined to estimate the groundwater resources contained within the fractures/faults. The integrated results suggest that the fractures/faults zones are most appropriate places of drilling for groundwater exploration in the investigated area. Geophysical methods coupled with the upfront borehole data provides better understanding about the conceptual model of the subsurface geological formations.
