1Comparative effectiveness of traditional chemoembolization with or without sorafenib for hepatocellular carcinoma显示文摘AIM: To compare the overall survival (OS) and progression-free survival (PFS) with associated adverse events (AE) in patients with unresectable hepatocellular carcinoma (HCC) treated with transarterial chemoembolization (TACE) + sorafenib vs TACE alone. METHODS: In this retrospective cohort study we collected data on all consecutive patients with a diagnosis of unresectable HCC between 2007 and 2011 who had been treated with TACE + sorafenib or TACE alone. We hypothesized that the combination therapy is superior to TACE alone in improving the survival in these patients. Data extracted included patient's demographics, etiology of liver disease, histology of HCC, stage of liver disease with respect to model of end stage liverdisease score and Child-Turcotte-Pugh (CTP) classification and Barcelona Clinic Liver Cancer (BCLC) staging for HCC. Computed tomography scan findings, alpha fetoprotein levels, number of treatments and related AE were also recorded and analyzed. RESULTS: Of the 43 patients who met inclusion criteria, 13 were treated with TACE + sorafenib and 30 with TACE alone. There was no significant difference in median survival: 20.6 mo (95%CI: 13.4-38.4) for the TACE + sorafenib and 18.3 mo (95%CI: 11.8-32.9) for the TACE alone (P = 0.72). There were also no statistically significant differences between groups in OS (HR = 0.82, 95%CI: 0.38-1.77; P = 0.61), PFS (HR = 0.93, 95%CI: 0.45-1.89; P = 0.83), and treatment-related toxicities (P = 0.554). CTP classification and BCLC staging for HCC were statistically significant (P = 0.001, P = 0.04 respectively) in predicting the survival in patients with HCC. The common AE observed were abdominal pain, nausea, vomiting and mild elevation of liver enzymes. CONCLUSION: Combination therapy with TACE + sorafenib is safe and equally effective as TACE alone in patients with unresectable HCC. CTP classification and BCLC staging were the significant predictors of survival. 2Preoperative therapy in locally advanced esophageal cancer显示文摘Esophageal cancer is an aggressive malignancy associated with dismal treatment outcomes. Presence of two distinct histopathological types distinguishes it from other gastrointestinal tract malignancies. Surgery is the cornerstone of treatment in locally advanced esophageal cancer(T2 or greater or node positive); however, a high rate of disease recurrence(systemic and loco-regional) and poor survival justifies a continued search for optimal therapy. Various combinations of multimodality treatment(preoperative/perioperative, or postoperative; radiotherapy, chemotherapy, or chemoradiotherapy) are being explored to lower disease recurrence and improve survival. Preoperative therapy followed by surgery is presently considered the standard of care in resectable locally advanced esophageal cancer as postoperative treatment may not be feasible for all the patients due to the morbidity of esophagectomy and prolonged recovery time limiting the tolerance of patient. There are wide variations in the preoperative therapy practiced across the centres depending upon the institutional practices, availability of facilities and personal experiences. There is paucity of literature to standardize the preoperative therapy. Broadly, chemoradiotherapy is the preferred neo-adjuvant modality in western countries whereas chemotherapy alone is considered optimal in the far East. 3Biodiversity,current developments and potential biotechnological applications of phosphorus-solubilizing and-mobilizing microbes:A review显示文摘As one of the most important and essential macronutrients next to nitrogen,phosphorus(P)is important for plant development,but it is the least mobile nutrient element in plant and soil.Globally,P is mined from geological sediments and added to agricultural soils so as to meet the critical requirements of crop plants for agronomic productivity.Phosphorus exists in soil in both organic and inorganic forms.The various inorganic forms of the element in soil are salts with calcium,iron,and aluminum,whereas the organic forms come from decaying vegetation and microbial residue.There is a huge diversity of plant microbiomes(epiphytic,endophytic,and rhizospheric)and soil microbiomes that have the capability to solubilize the insoluble P and make it available to plant.The main mechanism for the solubilization of inorganic P is by the production of organic acids,which lowers soil pH,or by the production of acid and alkaline phosphatases,which causes the mineralization of organic P.The P-solubilizing and-mobilizing microorganisms belong to all three domains,comprising archaea,bacteria,and eukarya.The strains belonging to the genera Arthrobacter,Bacillus,Burkholderia,Natrinema,Pseudomonas,Rhizobium,and Serratia have been reported as efficient and potential P solubilizers.The use of P solubilizers,alone or in combination with other plant growth-promoting microbes as an eco-friendly microbial consortium,could increase the P uptake of crops,increasing their yields for agricultural and environmental sustainability.Divjot KOUR Kusam Lata RANA Tanvir KAUR Neelam YADAV Ajar Nath YADAV Manish KUMAR Vinod KUMAR Harcharan Singh DHALIWAL Anil Kumar SAXENA 2021Pedosphere2021,31,1:10
4Anti-HBc screening in Indian blood donors:Still an unresolved issue显示文摘AIM:To study the seroprevalence of antibody to hepatitis B core antigen (anti-HBc) in healthy blood donors negative for HBsAg and to evaluate whether anti-HBc detection could be adopted in India as a screening assay for HBV in addition to HBsAg. METHODS: A total of 1700 serum samples collected from HBsAg-negative healthy blood donors were tested for the presence of anti-HBc antibody (IgM + IgG). All samples reactive for anti-HBc antibody were then investigated for presence of anti-HBs and for liver function tests (LFTs). One hundred serum samples reactive for anti-HBc were tested for HBV DNA by PCR method. RESULTS: Out of 1700 samples tested, 142 (8.4%) blood samples were found to be reactive for anti-HBc. It was signif icantly lower in voluntary (6.9%) as compared to replacement donors (10.4%, P = 0.011). Seventy- two (50.7%) anti-HBc reactive samples were also reactive for anti-HBs with levels > 10 mIU/mL and 70 (49.3%) samples were non-reactive for anti-HBs, these units were labeled as anti-HBc-only. These 142 anti-HBc reactive units were also tested for liver function test. HBV DNA was detected in only 1 of 100 samples tested. CONCLUSION: Keeping in view that 8%-18% of donor population in India is anti-HBc reactive, inclusion of anti- HBc testing will lead to high discard rate. Anti-HBs as proposed previously does not seem to predict clearance of the virus. 5Is prophylactic placement of drains necessary after subtotal gastrectomy?显示文摘AIM: To determine the evidence-based values of prophylactic drainage in gastric cancer surgery. METHODS: One hundred and eight patients, who underwent subtotal gastrectomy with D1 or D2 lymph node dissection for gastric cancer between January 2001 and December 2005, were divided into drain group or no-drain group. Surgical outcome and post-operative complications within four weeks were compared between the two groups. RESULTS: No significant differences were observed between the drain group and no-drain group in terms of operating time (171 ± 42 min vs 156 ± 39 min), number of post-operative days until passage of flatus (3.7 ± 0.5 d vs 3.5 ± 1.0 d), number of post-operative days until initiation of soft diet (4.9 ± 0.7 d vs 4.8 ± 0.8 d), length of post-operative hospital stay (9.3 ± 2.2 d vs 8.4 ± 2.4 d), mortality rate (5.4% vs 3.8%), and overall post- operative complication rate (21.4% vs 19.2%). 7Leishmaniasis:Current status of available drugs and new potential drug targets显示文摘The control of Leishmania infection relies primarily on chemotherapy till date.Resistance to pentavalent antimonials,which have been the recommended drugs to treat cutaneous and visceral leishmaniasis,is now widespread in Indian subcontinents.New drug formulations like amphotericin B,its lipid formulations,and miltefosine have shown great efGcacy to treat leishmaniasis but their high cost and therapeutic complications limit their usefulness.In addition, irregular and inappropriate uses of these second line drugs in endemic regions like state of Bihar, India threaten resistance development in the parasite.In context to the limited drug options and unavailability of either preventive or prophylactic candidates,there is a pressing need to develop true antileishmanial drugs to reduce the disease burden of this debilitating endemic disease.Notwithstanding significant progress of leishmanial research during last few decades, identification and characterization of novel drugs and drug targets are far from satisfactory.This review will initially describe current drug regimens and later will provide an overview on few important biochemical and enzymatic machineries that could be utilized as putative drug targets for generation of true antileishmanial drugs.Nisha Singh Manish Kumar Rakesh Kumar Singh 2012Asian Pacific Journal of Tropical Medicine2012,5,6:5

9Protective effect of diosmin against diabetic neuropathy in experimental rats显示文摘OBJECTIVE:The present study was undertaken to evaluate the effect of diosmin in diabetic neuropathy in type 2 diabetic rats.METHODS:Type 2 diabetes was induced in male Sprague-Dawley rats by single intraperitoneal injection of streptozotocin(35 mg/kg) and high-fat diet.Four weeks after the confirmation of diabetes,diabetic rats were treated with diosmin(50 and 100 mg/kg,p.o.) for next 4 weeks.Rats were evaluated for biochemical,behavioral and oxidative stress parameters.Eddy's hot plate and tail immersion test were performed on 6th,7th,8th,9th and 10th weeks of experiment to assess thermal hyperalgesia and cold allodynia respectively.Further,the walking function test was performed for assessing the motor responses at the end of the treatment schedule.RESULTS:Rats were fed with high-fat diet throughout the experiment schedule and administration of low-dose streptozotocin induced significant elevation in blood glucose level and insulin resistance which was confirmed by oral glucose tolerance test.Treatment with diosmin at doses of 50 and 100 mg/kg significantly restored the reduced body weight,elevated blood sugar and lipid profiles.Further the dose-dependent improvement was observed in thermal hyperalgesia,cold allodynia and walking function in diabetic rats treated with diosmin.Elevated levels of malondialdehyde,and nitric oxide and decreased glutathione levels and superoxide dismutase activity in diabetic rats were restored significantly after the 4 weeks of diosmin treatment.CONCLUSION:Diosmin has shown beneficial effect in preventing the progression of early diabetic neuropathy in rats.Dilpesh Jain Manish Kumar Bansal Rahul Dalvi Aman Upganlawar Rahul Somani 2014Journal of Integrative Medicine2014,12,1:4
10Investigation of Structural, Rheological and Thermal Properties of PMMA/ONi-Al LDH Nanocomposites Synthesized via Solvent Blending Method: Effect of LDH Loading显示文摘This article addresses the synthesis of organically tailored Ni-Al layered double hydroxide(ONi-Al LDH) and its use in the fabrication of exfoliated poly(methyl methacrylate)(PMMA) nanocomposites. The pristine Ni-Al LDH was initially synthesized by co-precipitation method and subsequently modified using sodium dodecyl sulfate to obtain ONi-Al LDH. Nanocomposites of PMMA containing various amounts of modified Ni-Al LDH(3 wt%?7 wt%) were synthesized via solvent blending method to investigate the influence of LDH content on the properties of PMMA matrix. Several characterization methods such as X-ray diffraction(XRD), transmission electron microscopy(TEM), Fourier transform infrared spectroscopy(FTIR), rheological analysis, differential scanning calorimetry(DSC) and thermo gravimetric analysis(TGA), were employed to examine the structural, viscoelastic and thermal properties of PMMA/OLDH nanocomposites. The results of XRD and TEM examination confirm the formation of partially exfoliated PMMA/OLDH nanocomposites. The FTIR results elucidate that the characteristic bands for both pure PMMA and modified LDH are present in the spectra of PMMA/OLDH nanocomposites. Rheological analyses were carried out to examine the adhesion between polymer matrix and fillers present in the nanocomposite sample. The TGA data indicate that the PMMA nanocomposites exhibit higher thermal stability when compared to pure PMMA. The thermal decomposition temperature of PMMA/OLDH nanocomposites increases by 28 K compared to that of pure PMMA at 15% weight loss as a point of reference. In comparison with pure PMMA, the PMMA nanocomposite containing 7 wt% LDH demonstrates improved glass transition temperature(Tg) of around 3 K. The activation energy(Ea), reaction orders(n) and reaction mechanism of thermal degradation of PMMA/OLDH nanocomposites were evaluated using different kinetic models. 11Prevalence of excessive internet use and its correlation with associated psychopathology in 11th and 12th grade students显示文摘Background Globally, the number of internet users has crossed the three-billion mark, while in India users grew over 17% in the first 6 months of 2015 to 354 million. This study presented a background on internet use and the existence of excessive internet use. Aim To study the extent of internet use in 11th and 12th grade students and the psychopathology, if any, associated with excessive internet use. Methods 426 students who met the inclusion criteria were recruited from 11 th and 12th grade classes from Kendriya Vidyalaya, New Delhi, India, and were assessed by Young's Internet Addiction Test and the Strength and Difficulties Questionnaire. Results Among the 426 students, the mean internet addiction total score was 36.63 (20.78), which indicated mild level of internet addiction. 1.41%(six students) was diagnosed as excessive internet users, while 30.28% and 23.94% were classified as moderate and mild internet users, respectively. The prevalence of internet addiction between gender was 58.22% in males and 41.78% in females. While both positive (prosocial) and negative (hyperactivity, emotional, conduct and peer problem) impacts of internet use were reported by students, in the current study excessive use of internet had a negative impact on students' lives as compared with positive impact, which was statistically significant (p<0.0001). Conclusion Excessive internet use led to abnormal behaviours which cause negative consequences to users. 12Synergism of co-delivered nanosized antioxidants displayed enhanced anticancer efficacy in human colon cancer cell lines显示文摘Combination of chemopreventive and/or therapeutic agents is the imminent smart approach to cope upwith cancer because it may act on multiple targets through different pathways. In the present study, wehave synthesized multiple chemopreventive and/or therapeutic agents (Curcumin, Quercetin andAspirin) loaded nanoparticles by simple cation-anion interaction among the amine groups of chitosan(CS) and phosphate groups of sodium hexametaphosphate (SHMP). These nanosized bioactive materials(CS-SHMP-CQA-NPs) were well characterized and found most effective in colon cancer cell line (HCT-116)compared to other cancer cell lines. Triplex chemopreventive and/or therapeutic agents-loaded NPs weresynergistically inducing apoptosis in HCT-116 cells compared to two-chemopreventive agents-loadedNPs as evident by an increase in sub-G1 cells (percent), and chromatin condensation along with thedecrease in mitochondrial membrane potential (MMP). Interestingly, ChoueTalalay analysis revealedthat CS-SHMP-CQA-NPs showed strong synergistic effect in its all doses. 14Arbutin effectively ameliorates the symptoms of Parkinson’s disease:the role of adenosine receptors and cyclic adenosine monophosphate显示文摘An antagonistic communication exists between adenosinergic and dopaminergic signaling in the basal ganglia,which suggests that the suppression of adenosine A2A receptors-cyclic adenosine monophosphate pathway may be able to restore the disrupted dopamine transmission that results in motor symptoms in Parkinson’s disease(PD).Arbutin is a natural glycoside that possesses antioxidant,antiinflammatory,and neuroprotective properties.The purpose of this study was to investigate whether arbutin could ameliorate the symptoms of PD and to examine the underlying mechanism.In this study,Swiss albino mouse models of PD were established by the intraperitoneal injection of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine for 4 successive days,with the concurrent intraperitoneal administration of arbutin(50 and 100 mg/kg)for 7 days.The results showed that arbutin significantly reduced lipid peroxidation,total nitrite levels,and inflammation in the substantia nigra and striatum of PD mouse models.In addition,arbutin decreased the activity of endogenous antioxidants,reduced the levels of dopamine,3,4-dihydroxyphenylacetic acid,homovanillic acid,andγ-aminobutyric acid,and minimized neurodegeneration in the striatum.Arbutin also reduced the abnormal performance of PD mouse models in the open field test,bar test,pole test,and rotarod test.The therapeutic efficacy of arbutin was similar to that of madopar.The intraperitoneal injection of the A2AR agonist CGS21680(0.5 mg/kg)attenuated the therapeutic effects of arbutin,whereas the intraperitoneal injection of forskolin(3 mg/kg)enhanced arbutin-mediated improvements.These findings suggest that arbutin can improve the performance of PD mouse models by inhibiting the function of the A2AR and enhancing the effects of cyclic adenosine monophosphate.This study was approved by the Institutional Animal Ethics Committee(1616/PO/Re/S/12/CPCSEA)on November 17,2019(approval No.IAEC/2019/010).Jie Zhao Manish Kumar Jeevan Sharma Zhihai Yuan 2021Neural Regeneration Research2021,16,10:2

17PDE solution to UAV/UGV trajectory planning problem by spatio-temporal estimation during wildfires显示文摘Unmanned Aerial Vehicles(UAVs)and Unmanned Ground Vehicles(UGVs)have been used in research and development community due to their strong potential in high-risk missions.One of the most important civilian implementations of UAV/UGV cooperative path planning is delivering medical or emergency supplies during disasters such as wildfires,the focus of this paper.However,wildfires themselves pose risk to the UAVs/UGVs and their paths should be planned to avert the risk as well as complete the mission.In this paper,wildfire growth is simulated using a coupled Partial Differential Equation(PDE)model,widely used in literature for modeling wildfires,in a grid environment with added process and measurement noise.Using principles of Proper Orthogonal Decomposition(POD),and with an appropriate choice of decomposition modes,a low-dimensional equivalent fire growth model is obtained for the deployment of the space-time Kalman Filtering(KF)paradigm for estimation of wildfires using simulated data.The KF paradigm is then used to estimate and predict the propagation of wildfire based on local data obtained from a camera mounted on the UAV.This information is then used to obtain a safe path for the UGV that needs to travel from an initial location to the final position while the UAV’s path is planned to gather information on wildfire.Path planning of both UAV and UGV is carried out using a PDE based method that allows incorporation of threats due to wildfire and other obstacles in the form of risk function.The results from numerical simulation are presented to validate the proposed estimation and path planning methods.Mohammadreza RADM ANESH Balaji SHARMA Manish KUMAR Donald FRENCH 2021Chinese Journal of Aeronautics2021,34,5:1
