1Predictive clinical features of cardioembolic infarction in patients aged 85 years and older显示文摘Objective To assess predictive clinical factors of cardioembolic infarction in very old patients(85 years of age and older).Methods Prospective hospital-based stroke registry('The Sagrat Cor Hospital of Barcelona Stroke Registry')is an acute-care teaching hospital in Barcelona,Catalonia,Spain.From 956 first-ever cardioembolic stroke patients included in the stroke registry over a 24-year period,639 were younger than 85 years of age and 317 were 85 years or older(mean age:88.9 years).Demographics,clinical characteristics,risk factors and early outcome were compared.Predictors of cardioembolic infarction in the oldest age group were assessed by multivariate analyses.Results In a logistic regression model based on demographics,risk factors,clinical features and complications,female gender(odds ratio[OR]=1.74,95%confidence interval[CI]:1.27–2.39),heart failure(OR=2.27,95%CI:1.46–3.56),altered consciousness(OR=1.76,95%CI:1.28–2.42),and infectious complications(OR=2.01,95%CI:1.39–2.91)were predictors of cardioembolic stroke in the oldest age group.By contrast,heavy smoking,heart valve disease,hypertension,headache,early seizures,sensory deficit,and involvement of the posterior cerebral artery were independently associated with cardioembolic stroke in the younger group.Conclusions Identification of a differential clinical profile of cardioembolic stroke between patients aged 85 years or more and those younger than 85 years helps clinicians to the optimal management of ischemic infarction in the oldest segment of the population.Ana Maria Carbajo-García Jonatan Cortés AdriàArboix Joan Massons Laura Díez Enric Vergés Jordi Arboix-Alió Luís García-Eroles 2019Journal of Geriatric Cardiology2019,16,11:3

4Revisiting Emergency Food Reserve Policy and Practiceunder Disaster and Extreme Climate Events显示文摘All food systems will continue to be affected by disasters and extreme climate events. Triggered by recent food crises around the world and climate change concerns,some governments have been trying to develop more robust and resilient food systems. One of the oldest options for many governments is to stockpile emergency food reserves for the purpose of food security and disaster preparedness. In the aftermath of the world food price crises in2007–2008 and 2011, some governments in Asia have been maintaining emergency food reserves to ensure greater supply and price stability. Disasters and extreme climate events help governments to justify emergency food reserves. This research examined emergency food reserve policies in Indonesia, the Philippines, and Malaysia.Emergency food reserves emerged as a practice where the shared objectives of development, disaster risk reduction,and climate change adaptation have been demonstrated by governments. The findings suggest that most governments maintain the strong view that adequate emergency food reserves can buffer national food price shocks and shocks from disasters and climate change, and soften disruptions in trade due to export bans during times of disasters and climate emergencies.Under global climate change scenarios, food security is at risk and volatile(Porter et al. 2014). The expected increase in climate extremes has generated anticipatory actions from governments, including a new push for EFR policy adoption. Triggered by recent disasters and climate change concerns, some governments have been trying to develop more robust and resilient food systems(Fan and Brzeska 2014; Porter et al. 2014). For many countries in Asia, this means the renewed adoption of EFR. Unfortunately, we argue that this is not well understood in climate change adaptation studies as well as contemporary disaster studies.The Association of Southeast Asian Nations(ASEAN)first initiated a Food Security Reserve Agreement in 1979,with the purpose of meeting emergency requirements(ASEAN 1979). Policymakers have been aware of the susceptibility of the region to natural hazards and the possibility of food shortages. But it took 30 years, until soon after the world food crisis in 2007–2008(Hadley and Fan 2010), for the association plus three additional East Asian nations(China, Japan, South Korea) to establish the ASEAN Plus Three Emergency Rice Reserve(APTERR)in 2009, as a mechanism to address potential food shortages in the region in the light of climate and market uncertainties. The final formal agreement was signed in October 2011 in Jakarta(APTERR 2017).The Association of Southeast Asian Nations also established the ASEAN Food Security Information System(AFSIS) that functions as a central information repository for five commodities—rice, maize, soybeans, sugar, and cassava. AFSIS not only monitors and analyzes production,import, export, inventory stock, price, food security ratio,and self-sufficiency ratio for these commodities but also provides data on losses from both floods and droughts in every member state(Lassa et al. 2016). In theory, AFSIS serves as an early warning mechanism for ASEAN to trigger the activation of APTERR's response mechanism(Saengbangka 2014, personal interview; AFSIS 2017).This article argues that EFR can function as a means of disaster risk reduction, including climate change adaptation, and aims to understand why governments in Asia are readopting emergency food reserves as national policies,with a focus on Indonesia, the Philippines, and Malaysia.We also explore how disasters and climate change strengthen or weaken government narratives in support of emergency food reserves.Jonatan A.Lassa Paul Teng Mely Caballero-Anthony Maxim Shrestha 2019International Journal of Disaster Risk Science2019,10,1:2
5Cardiometabolic risk through an integrative classification combining physical activity and sedentary behavior in European adolescents:HELENA study显示文摘Purpose: This study aims to compare adolescents' cardiometabolic risk score through an integrative classification of physical activity(PA),which involves the combination of moderate-to-vigorous physical activity(MVPA) and sedentary behavior(SB).Methods: A cross-sectional study derived from the Healthy Lifestyle in Europe by Nutrition in Adolescence Cross-Sectional Study database(2006-2008) was conducted in adolescents(n = 548; boys, 47.3%; 14.7 ± 1.2 years) from 10 European cities. MVPA and SB were objectively measured using accelerometry. Adolescents were divided into 4 categories according to MVPA(meeting or not meeting the international recommendations) and the median of SB time(above or below sex-and age-specific median) as follows: High-SB & Inactive, Low-SB & Inactive,High-SB & Active, and Low-SB & Active. A clustered cardiometabolic risk score was computed using the homeostatic model assessment, systolic blood pressure, triglycerides, total cholesterol/high-density lipoprotein cholesterol, sum 4 skinfolds, and cardiorespiratory fitness(CRF).Analyses of covariance were performed to discern differences on cardiometabolic risk scores among PA categories and each health component.Results: The cardiometabolic risk score was lower in adolescents meeting the MVPA recommendation and with less time spent in SB in comparison to the high-SB & Inactive group(p < 0.05). However, no difference in cardiometabolic risk score was established between High-SB or Low-SB groups in inactive adolescents. It is important to note that CRF was the only variable that showed a significant modification(higher)when children were compared from the category of physically inactive with 'active' but not from high-to low-SB.Conclusion: Being physically active is the most significant and protective outcome in adolescents to reduce cardiometabolic risk. 6由农业残余物可持续生产活性炭来吸附去除抗生素(英文)显示文摘The objectives of this study are to convert at laboratory scale agricultural residues into activated carbons(AC) with specific properties,to characterize them and to test them in adsorption reactor for tetracycline removal,a common antibiotic.Two new ACs were produced by direct activation with steam from beet pulp(BP-H2O) and peanut hulls(PH-H2O) in environmental friendly conditions.BP-H2O and PH-H2O present carbon content ranged between 78% and 91%,similar BET surface areas(821 and 829 m2·g-1 respectively) and pHPZC values(9.8).Their porosities are different:PH-H2O is mainly microporous(84%) with 0.403 cm3·g-1 of total porous volume,whereas BP-H2O develops a mesoporous volume of 0.361 cm3·g-1 representing 50% of the total porous volume.Two other commercial granular AC carbons(GAC1 and GAC2) were also characterised and used for comparison.The adsorption study is conducted in batch reactors.Two parts can be observed from kinetic decay curves:a very fast concentration decrease during the first 12 h,followed by a slow adsorption.An optimal contact time of 120 h is also deduced from these curves.It is shown as well that adsorption decreases with an increase of pH,indicating that the form preferentially adsorbed is probably the zwitterion form of the tetracycline.From equilibrium isotherms data,two adsorption models have been used:Langmuir and Freundlich.Both of them lead to a very good description of the experimental data.Maximum adsorption capacities deduced from the Langmuir equation follow the sequence:GAC2(817 mg·g-1)>BP-H2O(288 mg·g-1)>GAC1(133 mg·g-1)>PH-H2O(28 mg·g-1).In real spring waters spiked with TC(tetracyclines),adsorption isotherms show that the maximum adsorption capacity of BP-H2O is slightly increased to 309 mg·g-1 while it is decreased by one third to 550 mg·g-1 in the case of GAC2.This study demonstrates that the production of AC from agricultural residues,at lab-scale,is feasible and leads to genuine activated carbons with different intrinsic properties.Jonatan Torres-Pérez Claire Gérente Yves Andrès 2012Chinese Journal of Chemical Engineering2012,20,3:2
