| 1 | Preoperative selection of patients with colorectal cancerliver metastasis for hepatic resection显示文摘Surgical resection of colorectal liver metastases(CRLM) has a well-documented improvement in survival. To benefit from this intervention, proper selection of patients who would be adequate surgical candidates becomes vital. A combination of imaging techniques may be utilized in the detection of the lesions. The criteria for resection are continuously evolving; currently, the requirements that need be met to undergo resection of CRLM are: the anticipation of attaining a negative margin(R0 resection), whilst maintaining an adequate functioning future liver remnant. The timing of hepatectomy in regards to resection of the primary remains controversial; before, after, or simultaneously. This depends mainly on the tumor burden and symptoms from the primary tumor. The role of chemotherapy differs according to the resectability of the liver lesion(s); no evidence of improved survival was shown in patients with resectable disease who received preoperative chemotherapy. Presence of extrahepatic disease in itself is no longer considered a reason to preclude patients from resection of their CRLM, providing limited extra-hepatic disease, although this currently is an area of active investigations. | 2 | 灌溉施肥对尼罗河三角洲玉米产量和水分利用率的影响显示文摘针对埃及尼罗河三角洲地区传统灌溉方式存在的水分利用效率低、产量低和农户增收困难等问题,通过田间试验探索该地区玉米种植适宜的灌溉施肥模式。大田试验于2012年和2013年在埃及苏伊士运河大学试验基地进行,共设置滴灌(D)、涌泉细流沟灌(B)、传统沟灌(F)3个灌溉方式和施无机肥(C)、液态有机肥(O)2个施肥种类,共6个处理。试验研究了水肥互作对玉米叶面积指数(LAI)、地上生物量(ADM)、产量构成要素、经济产量、水分利用效率(WUE)和经济效益的影响。结果表明:抽雄期以后,灌溉方式和施肥种类对玉米LAI和ADM的影响均有显著差异(P〈0.05),其中OD处理的LAI和ADM均处最高水平。两种施肥条件下,D、B和F 3种灌溉方式对玉米产量和WUE的影响均表现为:B〉D〉F。不同灌水条件下,O处理比C处理玉米产量提高18.6%~24.4%,WUE提高18.7%~23.4%。CB处理玉米的年均净效益最高,达7 373元/hm2,但与CF处理差异不显著。CB处理(即涌泉细流沟灌技术与施无机肥结合)是埃及尼罗河三角洲玉米最佳种植模式。 | 向友珍 张富仓 Mohamed A Rashad 强生才 邹海洋 Ahmed Ali Hassanain | 2015 | 农业机械学报2015，46，10： | 7 |
| 3 | Portal vein embolization effect on colorectal cancer liver metastasis progression:Lessons learned显示文摘Colorectal liver metastasis(CRLM) is the major cause of death in patients diagnosed with colorectal cancer. The gold standard treatment of CRLM is surgical rese-ction. Yet, in the past, more than half of these patients were deemed unresectable due to the inadequate future liver remnant(FLR). The introduction of efficient portal vein embolization(PVE) preoperatively allowed more resections of metastasis in CRLM patients by stimulating adequate liver hypertrophy. However, several exp-erimental and clinical studies reported tumor progression after PVE which critically influences the subsequent management of these patients. The underlying path-ophysiological mechanism of tumor progression post-PVE is still not fully understood. In spite of the adverse effects of PVE, it remains a potentially curative procedure in patients who would remain otherwise unresectable because of the insufficient FLR. Currently, the challenge is to halt tumor proliferation following PVE in patients who require this technique. This could potentially be achieved by either attempting to suppress the underlying oncologic stimulus or by inhibiting tumor growth once observed after PVE, without jeopardizing liver regeneration. | 4 | Effectiveness of hepatitis B virus vaccination program in Egypt:Multicenter national project显示文摘AIM:To assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated children.METHODS:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULTS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. Multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to < 10,10 to < 15 and ≥ 15 years respectively compared to those < 5 years and 1.1 among girls compared to boys with P < 0.01. HBs Ag was positive in 0.11% and breakthrough infection was 0.36% and 0.39% depending on positivity of anti-HBc and DNA detection respectively. The prevalence of HBV infection was significantly higher among children aged ≥ 7 years(0.59%) compared to 0.07% among younger children with odds ratio equal to 8.4(95%CI:1.1-64.2) and P < 0.01.The prevalence was higher among girls(0.48%) than boys(0.29%) with P > 0.05.
