| 3 | Upper-gastrointestinal bleeding secondary to peptic ulcer disease:Incidence and outcomes显示文摘AIM:To evaluate the incidence,surgery,mortality,and readmission of upper gastrointestinal bleeding(UGIB)secondary to peptic ulcer disease(PUD).METHODS:Administrative databases identified all hospitalizations for UGIB secondary to PUD in Alberta,Canada from 2004 to 2010(n=7079)using the International Classification of Diseases Codes(ICD-10).A subset of the data was validated using endoscopy reports.Positive predictive value and sensitivity with 95%confidence intervals(CI)were calculated.Incidence of UGIB secondary to PUD was calculated.Logistic regression was used to evaluate surgery,in-hospital mortality,and 30-d readmission to hospital with recurrent UGIB secondary to PUD.Co-variants accounted for in our logistic regression model included:age,sex,area of residence(i.e.,urban vs rural),number of Charlson comorbidities,presence of perforated PUD,undergoing upper endoscopy,year of admission,and interventional radiological attempt at controlling bleeding.A subgroup analysis(n=6356)compared outcomes of patients with gastric ulcers to those with duodenal ulcers.Adjusted estimates are presented as odds ratios(OR)with95%CI.RESULTS:The positive predictive value and sensitivity of ICD-10 coding for UGIB secondary to PUD were85.2%(95%CI:80.2%-90.2%)and 77.1%(95%CI:69.1%-85.2%),respectively.The annual incidence between 2004 and 2010 ranged from 35.4 to 41.2 per100000.Overall risk of surgery,in-hospital mortality,and 30-d readmission to hospital for UGIB secondary to PUD were 4.3%,8.5%,and 4.7%,respectively.Interventional radiology to control bleeding was performed in 0.6%of patients and 76%of these patients avoided surgical intervention.Thirty-day readmission significantly increased from 3.1%in 2004 to 5.2%in 2010(OR=1.07;95%CI:1.01-1.14).Rural residents(OR rural vs urban:2.35;95%CI:1.83-3.01)and older individuals(OR≥65 vs<65:1.57;95%CI:1.21-2.04)were at higher odds of being readmitted to hospital.Patients with duodenal ulcers had higher odds of dying(OR=1.27;95%CI:1.05-1.53),requiring surgery(OR=1.73;95%CI:1.34-2.23),and being readmitted to hospital(OR=1.54;95%CI:1.19-1.99)when compared to gastric ulcers.CONCLUSION:UGIB secondary to PUD,particularly duodenal ulcers,was associated with significant morbidity and mortality.Early readmissions increased over time and occurred more commonly in rural areas. | Samuel Quan Alexandra Frolkis Kaylee Milne Natalie Molodecky Hong Yang Elijah Dixon Chad G Ball Robert P Myers Subrata Ghosh Robert Hilsden Sander Veldhuyzen van Zanten Gilaad G Kaplan | 2014 | World Journal of Gastroenterology2014，20，46： | 21 |
| 4 | Surgical vs percutaneous radiofrequency ablation for hepatocellular carcinoma in dangerous locations显示文摘AIM:To compare the long-term outcome of percutaneous vs surgical radiofrequency ablation(RFA) for hepatocellular carcinoma(HCC) in dangerous locations.METHODS:One hundred and sixty-two patients with HCC in dangerous locations treated with percutaneous or surgical RFA were enrolled in this study.The patients were divided into percutaneous RFA group and surgical RFA group.After the patients were regularly followed up for a long time,their curative rate,hospital stay time,postoperative complications and 5-year local tumor progression were compared and analyzed.RESULTS:No significant difference was observed in curative rate between the two groups(91.3% vs 96.8%,P = 0.841).The hospital stay time was longer and more analgesics were required while the incidence of bile duct injury and RFA-related hemorrhage was lower in surgical RFA group than in percutaneous RFA group(P < 0.05).The local progression rate of HCC in dangerous locations was significantly lower in surgical RFA group than in percutaneous RFA group(P = 0.05).The relative risk of local tumor progression was 14.315 in percutaneous RFA group.CONCLUSION:The incidence of severe postoperative complications and local tumor progression is lower after surgical RFA than after percutaneous RFA. | Ji-Wei Huang Roberto Hernandez-Alejandro Kristopher P Croome Lu-Nan Yan Hong Wu Zhe-Yu Chen Pankaj Prasoon Yong Zeng | 2011 | World Journal of Gastroenterology2011，17，1： | 14 |

| 6 | 肾包膜下种植建立人前列腺癌异种移植动物模型显示文摘目的 采用肾包膜下种植方案建立人前列腺癌异种移植动物模型.方法 15只SCID雄鼠按随机表分组法随机分为3组,每组5只.应用组织重组技术,将人前列腺癌新鲜组织和新出生BALB/c雄鼠精囊间质(SVM)在体外重组,显微外科条件下种植于SCID鼠肾包膜下,4周后观察肿瘤种植率并称重、计算体积.检测血清前列腺特异性抗原(PSA)值.用细胞角蛋白(CK)8/18、波形蛋白(vimentin)单克隆抗体特异标记人前列腺上皮和基质成纤维细胞,用P63单克隆抗体标记上皮基底膜证实其是否为前列腺癌组织.另将人前列腺癌新鲜组织单独种植于SCID鼠肾包膜下作为对照.结果 15只SCID鼠78只次组织种植中无一只死亡.4周后,重组种植组成瘤率为100%(39/39),单独种植组为94.1%(37/39),差异无统计学意义(P>0.05).新生瘤体为实体状、不规则、黄/白色,隆出肾脏表面和(或)嵌入肾实质.重组种植组瘤体生长旺盛,单独种植组瘤体较为平坦,肿瘤大小及重量在两组间差异有统计学意义[(9.7±3.1)mm^3比(6.8±2.0)mm^3;(12.1±3.6)μg比(8.2±2.2)μg,均P<0.01].重组种植组血清PSA值高于单独种植组,但两组间差异无统计学意义(P>0.05).CK8/18、vimentin单抗标记证实新生瘤体为人源性前列腺组织,P63单抗标记显示上皮基底膜消失证实为前列腺癌.结论 肾包膜下种植可高效率建立人前列腺癌异种移植动物模型.本模型含有肿瘤基质成分,可为体内研究前列腺癌发病和病程演变中间质-上皮细胞相互作用提供技术平台. | 孙宏斌 WANG Hong Renea A Taylor Gall P Risbridger | 2010 | 中华医学杂志2010，90，30： | 4 |

But the Keff can not be measured directly for each sample and only the external radiative efficiency K3.7 can be measured. In order to obtain the Keff a special study for the conversion factor of Keff to K3.t has been made using the ultrathin TLD. | 8 | Ultrafast photonic PCR显示文摘Nucleic acid amplification and quantification via polymerase chain reaction(PCR)is one of the most sensitive and powerful tools for clinical laboratories,precision medicine,personalized medicine,agricultural science,forensic science and environmental science.Ultrafast multiplex PCR,characterized by low power consumption,compact size and simple operation,is ideal for timely diagnosis at the point-of-care(POC).Although several fast/ultrafast PCR methods have been proposed,the use of a simple and robust PCR thermal cycler remains challenging for POC testing.Here,we present an ultrafast photonic PCR method using plasmonic photothermal light-to-heat conversion via photon–electron–phonon coupling.We demonstrate an efficient photonic heat converter using a thin gold(Au)film due to its plasmon-assisted high optical absorption(approximately 65%at 450 nm,the peak wavelength of heat source light-emitting diodes(LEDs)).The plasmon-excited Au film is capable of rapidly heating the surrounding solution to over 150℃ within 3 min.Using this method,ultrafast thermal cycling(30 cycles;heating and cooling rate of 12.7960.93℃ s^(-1) and 6.660.29℃ s^(-1),respectively)from 55℃(temperature of annealing)to 95℃(temperature of denaturation)is accomplished within 5 min.Using photonic PCR thermal cycles,we demonstrate here successful nucleic acid(λ-DNA)amplification.Our simple,robust and low cost approach to ultrafast PCR using an efficient photonic-based heating procedure could be generally integrated into a variety of devices or procedures,including on-chip thermal lysis and heating for isothermal amplifications. | Jun Ho Son Byungrae Cho SoonGweon Hong Sang Hun Lee Ori Hoxha Amanda J Haack Luke P Lee | 2015 | Light(Science & Applications)2015，4，1： | 2 |

| 10 | Direct observation of localized surface plasmon field enhancement by Kelvin probe force microscopy显示文摘A surface plasmon(SP)is a fundamental excitation state that exists in metal nanostructures.Over the past several years,the performance of optoelectronic devices has been improved greatly via the SP enhancement effect.In our previous work,the responsivity of GaN ultraviolet detectors was increased by over 30 times when using Ag nanoparticles.However,the physics of the SP enhancement effect has not been established definitely because of the lack of experimental evidence.To reveal the physical origin of this enhancement,Kelvin probe force microscopy(KPFM)was used to observe the SP-induced surface potential reduction in the vicinity of Ag nanoparticles on a GaN epilayer.Under ultraviolet illumination,the localized field enhancement induced by the SP forces the photogenerated electrons to drift close to the Ag nanoparticles,leading to a reduction of the surface potential around the Ag nanoparticles on the GaN epilayer.For an isolated Ag nanoparticle with a diameter of~200 nm,the distribution of the SP localized field is located within 60 nm of the boundary of the Ag nanoparticle.For a dimer of Ag nanoparticles,the localized field enhancement between the nanoparticles was the strongest.The results presented here provide direct experimental proof of the localized field enhancement.These results not only explain the high performance of GaN detectors observed with the use of Ag nanoparticles but also reveal the physical mechanism of SP enhancement in optoelectronic devices,which will help us further understand and improve the performance of SP-based optoelectronic devices in the future. | Da-Bing Li Xiao-Juan Sun Yu-Ping Jia Mark I Stockman Hari P Paudel Hang Song Hong Jiang Zhi-Ming Li | 2017 | Light(Science & Applications)2017，6，1： | 2 |
